We examine the impact of remedial and developmental courses on college students with varying levels of academic preparedness, thus focusing on a wider range of students than previous studies. Using a regression discontinuity design, we provide causal estimates of the effects of placement in different levels of remedial courses on short-, intermediate-, and long-term outcomes at both 2- and 4-year colleges. Similar to other research, we find that remediation has negative effects for students on the margin of needing one developmental course. However, for students with lower levels of academic preparation, the effects of remediation are estimated to be positive in some subjects. These results suggest that remedial courses can help or hinder students differently depending on their incoming levels of academic preparedness. Moreover, our conclusions are largely driven by positive and negative effects observed for students at 2-year institutions, and we discuss several hypotheses that may explain these findings.

Abstract Aims To investigate the impact of established cardiovascular disease (CVD) on 10-year all-cause death following coronary revascularization in patients with complex coronary artery disease (CAD). Methods The SYNTAXES study assessed vital status out to 10 years of patients with complex CAD enrolled in the SYNTAX trial. The relative efficacy of PCI versus CABG in terms of 10-year all-cause death was assessed according to co-existing CVD. Results Established CVD status was recorded in 1771 (98.3%) patients, of whom 827 (46.7%) had established CVD. Compared to those without CVD, patients with CVD had a significantly higher risk of 10-year all-cause death (31.4% vs. 21.7%; adjusted HR: 1.40; 95% CI 1.08–1.80, p = 0.010). In patients with CVD, PCI had a non-significant numerically higher risk of 10-year all-cause death compared with CABG (35.9% vs. 27.2%; adjusted HR: 1.14; 95% CI 0.83–1.58, p = 0.412). The relative treatment effects of PCI versus CABG on 10-year all-cause death in patients with complex CAD were similar irrespective of the presence of CVD (p-interaction = 0.986). Only those patients with CVD in ≥ 2 territories had a higher risk of 10-year all-cause death (adjusted HR: 2.99, 95% CI 2.11–4.23, p < 0.001) compared to those without CVD. Conclusions The presence of CVD involving more than one territory was associated with a significantly increased risk of 10-year all-cause death, which was non-significantly higher in complex CAD patients treated with PCI compared with CABG. Acceptable long-term outcomes were observed, suggesting that patients with established CVD should not be precluded from undergoing invasive angiography or revascularization. Advances in neuroimaging of the preterm infant have enhanced the ability to detect brain injury. This added information has been a blessing and a curse. Neuroimaging, particularly with magnetic resonance imaging, has provided greater insight into the patterns of injury and specific vulnerabilities. It has also provided a better understanding of the microscopic and functional impacts of subtle and significant injuries. While the ability to detect injury is important and irresistible, the evidence for how these injuries link to specific long-term outcomes is less clear. In addition, the impact on parents can be profound. This narrative summary will review the history and current state of brain imaging, focusing on magnetic resonance imaging in the preterm population and the current state of the evidence for how these patterns relate to long-term outcomes.

e15540 Background: Recent research has demonstrated the impact that body composition parameters can have on the outcomes following cancer surgery. Adipose tissue deposition in muscle, known as myosteatosis, can be detected on pre-operative imaging. This systematic review aims to analyse the impact of pre-operative myosteatosis on long-term outcomes following surgery for gastro-intestinal malignancy. Methods: Using MeSH terms, a systematic search of the databases PubMed MEDLINE, EMBASE, Cochrane, CINAHL and AMED was performed. Studies were included if they reported hazard ratios (HR) analysing the impact of pre-operatively defined myosteatosis, or similar term, on the long-term outcomes following surgery for gastro-intestinal malignancy. A total of 39 full texts articles were reviewed for inclusion, with 19 being included after the inclusion criteria were applied. A sub-group analysis was performed for those studies reporting outcomes for colorectal cancer patients only. Results: The total number of included patients across all studies was 14,481. Patients with myosteatosis had a significantly poorer overall survival, according to univariate (HR 1.82, 95% CI 1.67 – 1.99) and multivariable (HR 1.66, 95% CI 1.49 – 1.86) analysis. This was also demonstrated with regards to cancer-specific survival (univariate HR 1.62, 95% CI 1.18 – 2.22, multivariable HR 1.73, 95% CI 1.48 – 2.03) and recurrence-free survival (univariate HR 1.28, 95% CI 1.10 – 1.48, multivariable HR 1.38, 95% CI 1.07 – 1.77). Conclusions: This review demonstrates that patients with pre-operative myosteatosis have poorer long-term outcomes following surgery for gastro-intestinal malignancy. Therefore, myosteatosis should be used for pre-operative optimisation and as a prognostic tool before surgery. More standardised definitions of myosteatosis and further cohort studies of patients with non-colorectal malignancies are required.

This article represents the first step in filling a large gap in knowledge concerning why Public Assistance (PA) use recently rose so fast in Japan. Specifically, we try to address this problem not only by performing a Blanchard and Quah decomposition on long-term monthly time series data (1960:04-2006:10), but also by estimating prefecturelevel longitudinal data. Two interesting findings emerge from the time series analysis. The first is that permanent shock imposes a continuously positive impact on the PA rate and is the main driving factor behind the recent increase in welfare use. The second finding is that the impact of temporary shock will last for a long time. The rate of the use of welfare is quite rigid because even if the PA rate rises due to temporary shocks, it takes about 8 or 9 years for it to regain its normal level. On the other hand, estimations of prefecture-level longitudinal data indicate that the Financial Capability Index (FCI) of the local government2 and minimum wage both impose negative effects on the PA rate. We also find that the rapid aging of Japan's population presents a permanent shock in practice, which makes it the most prominent contribution to surging welfare use.

Ground-based skidding operations can lead to soil compaction and displacement, which could cause negative effects on forest soil. Hence, some efforts such as forestry best management practices (BMPs) must be implemented in the prone area to mitigate these possible impacts. Several materials and treatments have been adopted to suppress these adverse effects by increasing the ground cover. However, the effects of mulch treatments on runoff and sediment yield are inconclusive with a diverse range of effectiveness. For these reasons, in this research mulch treatments were tested as to determine how the application of organic mulch amendments such as straw and leaf litter and contour-felled logs would alleviate the runoff and sediment yield on machine operating trails and ensure successful hillslope stabilization. The aims of the study were to analyse and compare the effectiveness of leaf litter (LM) and straw mulch (SM) rate and different distances of contour-felled logs (CFL) to mitigate the runoff and sediment yield, and examine the impact of rainfall intensity on effectiveness of litter mulch, straw mulch, and contour-felled logs. Totally, 30 bounded runoff plots in the machine operating trails and four treatments including litter mulch (LMR1: 0.62, LMR2: 1.24, and LMR3: 1.86 kg m-2), straw mulch (SMR1: 0.45, SMR2: 0.92, and SMR3: 1.34 kg m-2), contour-felled logs (CFL10: 10, CFL20: 20, and CFL30: 30 m), and untreated area were established in triplicate with 4 m width and 100 m length. During the study period, the runoff and sediment yield in the untreated trails (U) were 2.36 mm and 11.84 g m-2. Straw (from 41.5 to 60.6%) and litter mulch (from 38.1 to 55.1%), and contour-felled logs treatments (from 70.8 to 88.1%) significantly decreased the runoff, compared to U treatment. Results show that mulch treatments with three different levels of Litter Mulch Rate, LMR1, LMR2, and LMR3 decreased mean sediment by 46.6, 64.0 and 71.8%, in the treatments with three different levels of Straw Mulch Rate, SMR1, SMR2, and SMR3 decreased mean sediment by 42.9, 62.1, and 69.9%, and in the treatments with three different distances of Contour-Felled Logs, CFL10, CFL20, and CFL30 decreased mean sediment by 90.6, 94.7 and 88.3% comparing to U, respectively. The relationships of the runoff and sediment responses to increasing mulching rate of litter and straw followed as negative logarithmic curves, but the decreasing-increasing trends were observed in runoff and sediment yield as the distance between contour-felled logs increased from 10 to 30 m. Polynomial regression equations were developed for predicting the runoff and sediment yield as a function of the application rate of litter and straw mulch and the distance between contour-felled logs, and rainfall intensity. We concluded that contour-felled logs treatment was more effective than both litter and straw mulch to mitigate the runoff, runoff coefficient, and sediment yield on machine operating trails. As a management measure, it could be possible to propose that the contour-felled logs with a distance of 20 m be prescribed to protect the machine operating trails from the negative effects of surface waterflow.

Background and Aim of the Study: Ischemic mitral valve regurgitation (IMR) in patients undergoing coronary artery bypass grafting (CABG) is associated with worse long-term outcomes. The aim of this study was to assess the impact of mitral valve repair with CABG in patients with moderate IMR. Method: This observational study enrolled 3,215 consecutive patients from the Juntendo CABG registry with moderate IMR and multivessel coronary artery disease who underwent CABG between 2002 and 2017. The CABG alone and CABG with mitral valve surgery (MVs) groups were compared. The propensity score was calculated for each patient. Long-term all-cause death, cardiac death, and major adverse cardiac and cerebrovascular events (MACCEs) were compared between the two groups. Results: A total of 101 patients who underwent CABG had moderate IMR in our database. Propensity score matching selected 40 pairs for final analysis. MVs was associated with increased risks of postoperative atrial fibrillation, blood transfusion, and longer hospitalization. There were no differences between the two groups in long-term outcomes, including all-cause mortality, cardiac mortality, and the incidence of MACCEs. Conclusions: Surgical treatment of moderate IMR combined with CABG was as safe as CABG alone, with no differences in long-term outcomes. Further studies are needed to determine the effects of MVs in patients with moderate IMR and severe coronary artery disease.

The aim of this study is to analyze a new social phenomenon that appeared during the SARS-CoV-2 pandemic, i.e. zoom fatigue. Zoom fatigue is the result of fatigue from long-term conferences, which can also be related to long-term school lessons during distance learning. The main considerations will focus on teenagers, namely on the impact of zoom fatigue on teenagers in Polish schools and on observation of students from technical secondary schools by a school psychologist. This paper, based on the results of research conducted at Stanford University and by Social Changes, is also an attempt to answer the questions: what risks may arise from the phenomenon of zoom fatigue and the threat posed by distance learning for teenagers in the era of Covid-19. It will also attempt to answer the question of how to help students cope with the fatigue of long distance school activities and find ways to help teens deal with isolation and the negative effects of zoom fatigue and find ways to overcome zoom fatigue.
