Research on small decisions from experience suggests that people often behave as if they underweight rare events and choose the options that are frequently better. In a pandemic, this tendency implies complacency and reckless behavior. Furthermore, behavioral contagion exacerbates this problem. In two pre-registered experiments (Ntotal = 312), we validate these predictions and highlight a potential solution. Groups of participants played a repeated social game in one of two versions. In the basic version, people clearly preferred the dangerous reckless behavior that was better most of the time over the safer responsible behavior. In the augmented version, we gave participants an additional alternative abstracting the use of an application that frequently saves time but can sometimes have high costs. This alternative was stochastically dominated by the option abstracting the responsible choice and was thus normatively “irrelevant” to the decision participants made. Nevertheless, most people chose the new (“irrelevant”) alternative, providing the first clear demonstration of underweighting of rare events in fully described social games. We discuss public policies that can make the responsible use of health applications better most of the time, thus helping them get traction despite being voluntary. In one field demonstration of this idea amid the COVID-19 pandemic, usage rates of a contact tracing application among nursing home employees more than tripled when using the app also started saving them little time on a daily basis, and the high usage rates sustained over at least four weeks.

Abstract Social relationships play an important role in cognitive health and aging. However, it is unclear how older adult’s cognitive function affects their everyday social interactions, especially for those with mild cognitive impairment (MCI). This study examined whether older adults with intact cognition vs. MCI differed in their daily social interactions. Community-dwelling older adults from the Einstein Aging Study (N=244, 70-91 yrs) reported their social interactions five times daily for 14 consecutive days using smartphones. Compared to those with normal cognitive function, older adults with MCI reported less frequent positive social interactions (p=0.012) and in-person social activities (p=0.006) on a daily basis. These two groups, however, did not show significant differences in their social relationships assessed by a conventional global questionnaire. The results support that, relative to global social relationships, daily social interactions are more sensitive, ecologically valid social markers that can facilitate the early detection of MCI.

A long-standing question in infectious disease dynamics concerns the role of transmission heterogeneities, driven by demography, behavior and interventions. Based on detailed patient and contact tracing data in Hunan, China we find 80% of secondary infections traced back to 15% of SARS-CoV-2 primary infections, indicating substantial transmission heterogeneities. Transmission risk scales positively with the duration of exposure and the closeness of social interactions and is modulated by demographic and clinical factors. The lockdown period increases transmission risk in the family and households, while isolation and quarantine reduce risks across all types of contacts. The reconstructed infectiousness profile of a typical SARS-CoV-2 patient peaks just before symptom presentation. Modeling indicates SARS-CoV-2 control requires the synergistic efforts of case isolation, contact quarantine, and population-level interventions, owing to the specific transmission kinetics of this virus.

AbstractConcerns about the practicality and effectiveness of using Contact Tracing Apps (CTA) to reduce the spread of COVID19 have been well documented and, in the UK, led to the abandonment of the NHS CTA shortly after its release in May 2020. We present a causal probabilistic model (a Bayesian network) that provides the basis for a practical CTA solution that addresses some of the concerns and which has the advantage of minimal infringement of privacy. Users of the model can provide as much or little personal information as they wish about relevant risk factors, symptoms, and recent social interactions. The model then provides them feedback about the likelihood of the presence of asymptotic, mild or severe COVID19 (past, present and projected). When the model is embedded in a smartphone app, it can be used to detect new outbreaks in a monitored population and identify outbreak locations as early as possible. For this purpose, the only data needed to be centrally collected is the probability the user has COVID19 and the GPS location.

The COVID-19 trajectories worldwide have shown several surprising features which are outside the purview of classical epidemiological models. These include (a) almost constant and low daily case rates over extended periods of time, (b) sudden waves emerging from the above solution despite no or minimal change in the level of non-pharmaceutical interventions (NPI), and (c) reduction or flattening of case counts following relaxation of NPI. To explain these phenomena, we add contact tracing to our recently developed cluster seeding and transmission (CST) model. We find no fewer than four effects which make prediction of epidemic trajectories uncertain. These are (a) cryptogenic instability, where a small increase in population-averaged contact rate causes a large increase in cases, (b) critical mass effect, where a wave manifests after weeks of quiescence with no change in parameter values, (c) knife-edge effect, where a small change in parameter across a critical value causes a huge change in the response of the system, and (d) hysteresis effect, where the timing and not just the strength of a particular NPI determines the subsequent behaviour. Despite these effects however, some non-obvious conclusions regarding NPI appear to be robust. In particular, (a) narrowing the circle of one's social interactions can be as effective a measure as reducing interactions altogether, and (b) a good contact tracing program can effectively substitute for much more invasive measures. Finally, we propose the contact tracing capacity ratio - a metric of the load to which the tracers are subject - as a reliable early warning indicator of an imminent epidemic wave.

Abstract BackgroundWe assessed the preferences and trade-offs for social interactions, incentives, and being traced by a digital contact tracing (DCT) tool post lockdown in Singapore.MethodsWe conducted a discrete choice experiment (DCE) among visitors of a large public hospital in Singapore between July 2020 – February 2021. Respondents were sampled proportionately by gender and four age categories (21 – 80 years). The DCE questionnaire had three attributes (1. Social interactions, 2. Being traced by a DCT tool, 3. Incentives to use a DCT tool) and two levels each. The final dataset comprised 3839 respondents after dropping 53 with “irrational” responses. Panel fixed conditional logit model was used to analyze the data.ResultsRespondents were more willing to trade being traced by a DCT tool for social interactions than incentives and unwilling to trade social interactions for incentives. The proportion of respondents preferring no incentives and could only be influenced by their family members increases with age. Among proponents of monetary incentives, the preferred median value for a month’s usage of DCT tools amounted to S$10 (USD7.25) and S$50 (USD36.20) for subsidies and lucky draw.ConclusionsDCE can be used to elicit profile-specific preferences to optimize the uptake of DCT tools during a pandemic. Social interactions are highly valued by the population, who are willing to trade them for being traced by a DCT tool during the COVID-19 pandemic. Although a small amount of incentive is sufficient to increase the satisfaction of using a DCT tool, incentives alone may not increase DCT tool uptake.

Despite the benefits inherent with social interactions, the case of epidemics cum pandemic outbreaks especially the case of the novel corona virus (covid-19) alongside its set protocols employed to contain the spread therein - has continually left the world puzzled as the disease itself has come to stay. The nature of its rapid propagation on exposure alongside its migration spread pattern of this contagion (with retrospect of other epidemics) on daily basis, has also left experts rethinking the set protocols. Our study involved modelling the covid-19 contagion on a social graph, so as to ascertain if its propagation using migration pattern as a threshold parameter can be minimized via the employment of set protocols. We also employed a design that sought to block or minimize targeted spread of the contagion with the introduction of seedset node(s) using the susceptible-infect framework on a time-varying social graph. Study results showed that migration or mobility pattern has become an imperative factors that must be added when modelling the propagation of contagion or epidemics.

Without a vaccine or proven therapeutic options in COVID-19, the World Health Organization (WHO) recommends a combination of measures: rapid diagnosis and immediate isolation of cases; rigorous contact tracing; and precautionary self-isolation of close contacts to curb the spread of COVID-19. During a Nipah outbreak in Kerala, India in 2019, it was confined to a single case. The authors were involved in the in-hospital contact tracing. With a single patient producing a contact list of 98 in a healthcare setting, the implications in a community setting during a pandemic of the scale of COVID-19 are huge but it proves that early and rigorous tracing with quarantining is an effective strategy to limit clusters. We believe that if the public is encouraged to maintain their own contact list on a daily basis, it would help in significantly reducing the time and effort invested into contact tracing in the event of a person contracting COVID-19.

Smartphone applications related to coronavirus disease 2019 (COVID-19) continue to emerge and evolve, but despite a wide variety of different app functions, there has yet to be a comprehensive study of what the most prevalent publicly available apps provide, and there exists no standardized evaluation system for end users to determine the safety and efficacy of an app before they download it. Furthermore, limited oversight means that the rapidly growing space creates challenges for end users trying to find a relevant app. We adapted the M-Health Index and Navigation Database (MIND) from apps.digitalpsych.org that previously has been used to evaluate mental health applications to guide the assessment of COVID apps. Using this framework, we conducted a thorough analysis of the top-100 returned coronavirus apps on two separate dates a month apart to understand the clinical utility and features of COVID-19 apps and how these change in a short period of time. We ultimately identified a significant turnover rate, as well as privacy concerns around lack of privacy policies and disclosure of personal information. Our research offers insight into the current status of COVID-19 apps and provides a comprehensive and adaptable framework to help individuals assess the growing number of such digital tools in the wake of the pandemic.

Abstract Objectives Detecting subtle behavioral changes in everyday life as early signs of cognitive decline and impairment is important for effective early intervention against Alzheimer’s disease. This study examined whether features of daily social interactions captured by ecological momentary assessments could serve as more sensitive behavioral markers to distinguish older adults with mild cognitive impairment (MCI) from those without MCI, as compared to conventional global measures of social relationships. Method Participants were 311 community dwelling older adults (aged 70 to 90 years) who reported their social interactions and socializing activities five times daily for 14 consecutive days using smartphones. Results Compared to those with normal cognitive function, older adults classified as MCI reported less frequent total and positive social interactions and less frequent in-person socializing activities on a daily basis. Older adults with and without MCI, however, did not show differences in most features of social relationships assessed by conventional global measures. Discussion These results suggest that certain features of daily social interactions (quality and quantity) could serve as sensitive and ecologically valid behavioral markers to facilitate the detection of MCI.

From planning daily activities with close others to passively overhearing a stranger’s conversation on the bus, humans spend roughly half of their waking life in the presence of other humans (US Bureau of Labor Statistics, 2003-2016). These social interactions constitute the source of a large proportion of the information that individuals gather on a daily basis. At the same time, people are often limited to only a small portion of information, or samples, in relation to all the possible information made available during a social experience (Ambady & Rosenthal, 1993; Fiedler, 2000). The fundamental question guiding this chapter is how group-motivated sampling of social information facilitates biased evaluations.
