PLANTAR LOADING IN THE YOUTH SOCCER PLAYER DURING COMMON SOCCER MOVEMENTS AND RISK FOR FOOT INJURY

Background: Soccer players are at high risk of stress injuries in the foot. While most research addresses this issue in professional athletes, there is little information concerning young athletes. Objective: As soccer is practiced around the world since early infancy, we set out to determine whether young soccer athletes are susceptible to increased foot loading that increase risk factors for foot injuries. Design: Case control study;Setting: LaboratoryParticipants: twenty-six male adolescents (mean age 16 years old) were organized into two groups: soccer players (n=13) and controls (n=13). Interventions: Participants completed a circuit of soccer relevant motor tasks while plantar pressure data were recorded with different footwear conditions. Additionally, foot sensitivity, ankle range of motion and Q-angle were determined for each group. Data were compared between the soccer and control groups within the different footwear conditions.Main Outcome Measures: Groups were compared regarding foot sensitivity, ankle range of motion, Q-angle, and plantar pressure determined during running and cutting movements performed at maximal speed and using different shoes. Results: Foot sensitivity, ankle range of motion and Q-angle did not differ between the groups. During performance of soccer actions, young players showed higher peak pressure in the lateral region of the foot including the fifth metatarsal. These higher peaks were minimized by manipulation of the footwear. Conclusion: In summary, young soccer athletes show dynamic plantar pressure patterns that are related to foot injury in the adult athlete, and this condition can be minimized by the manipulation of the footwear.

Context:Risk of anterior cruciate ligament (ACL) injuries in young female athletes increases with age, appearing to peak during maturation. Changes in hip muscle strength and range of motion (ROM) during this time may contribute to altered dynamic movement patterns that are known to increase risk of ACL injuries. Understanding the longitudinal changes in hip strength and ROM is needed to develop appropriate interventions to reduce the risk of ACL injuries.Objective:To examine the longitudinal changes in hip strength and ROM in female youth soccer players.Design:Longitudinal descriptive study.Setting:Field setting.Participants:14 female youth soccer athletes (14.1 ± 1.1 y, 165.8 ± 5.3 cm, 57.5 ± 9.9 kg) volunteered as part of a multiyear risk factor screening project.Main Outcome Measures:Clinical measures of hip strength and ROM were collected annually over 3 consecutive years. Passive hip internal rotation (IR), external rotation (ER), abduction (ABD), and adduction (ADD) ROM were measured with a digital inclinometer. Isometric hip ABD and extension (EXT) strength were evaluated using a hand-held dynamometer. Separate repeated-measures ANOVAs compared hip strength and ROM values across 3 consecutive years (P < .05).Results:As youth female soccer players increased in age, there were no changes in normalized hip ABD (P = .830) or EXT strength (P = .062) across 3 consecutive years. Longitudinal changes in hip ROM were observed with increases in hip IR (P = .001) and ABD (P < .001), while hip ADD (P = .009) and ER (P < .001) decreased.Conclusions:Anatomical changes at the hip occur as youth female soccer players increase in age. While there are no changes in hip strength, there is an increase in hip IR and ABD ROM with a concomitant decrease in hip ER and ADD ROM. The resulting asymmetries in hip ROM may decrease the activation and force producing capabilities of the hip muscles during dynamic activities, contributing to altered lower extremity mechanics known to increase the risk of ACL injuries.

Context:After years of focusing on the management of anterior cruciate ligament (ACL) injuries, the most common soccer-related injuries, the orthopedic community has concluded that soccer players have a wide range of variation in joint biomechanics and has thus started to focus research efforts on the morphological factors that might contribute to ACL trauma. One such factor is decreased hip-rotation range of motion (ROM), which may be due to compensatory musculoskeletal changes occurring in response to longstanding soccer practice since childhood.Objective:This study sought to assess decreased hip rotation and the influence of stretching exercises on the behavior of the hip joint in players of the youth soccer categories of a Brazilian soccer team.Design:Randomized clinical trial.Setting:University hospital.Patients:262 male soccer players.Interventions:Subjects were randomly allocated into 2 groups—control or a stretching program.Main Outcome Measures:Subjects were reassessed after 12 wk.Results:The findings suggest that hip-rotation ROM decreases over the years in soccer players. In the study sample, adherence to a stretching program improved only external hip-rotation ROM in the nondominant limb.Conclusion:Playing soccer can restrict rotation ROM of the hip, and adherence to stretching exercises may decrease the harmful effects on the hip joints.

Background: Previous studies have examined the relationship between external training load and creatine kinase (CK) response after soccer matches in adults. This study aimed to build training- and match-specific CK prediction models for elite youth national team soccer players. Hypothesis: Training and match load will have different effects on the CK response of elite youth soccer players, and there will be position-specific differences in the most influential external and internal load parameters on the CK response. Study Design: Prospective cohort study. Level of Evidence: Level 4. Methods: Forty-one U16-U17 youth national team soccer players were measured over an 18-month period. Training and match load were monitored with global positioning system devices. Individual CK values were measured from whole blood every morning in training camps. The dataset consisted of 1563 data points. Clustered prediction models were used to examine the relationship between external/internal load and consecutive CK changes. Clusters were built based on the playing position and activity type. The performance of the linear regression models was described by the R2 and the root-mean-square error (RMSE, U/L for CK values). Results: The prediction models fitted similarly during games and training sessions ( R2 = 0.38-0.88 vs 0.6-0.77), but there were large differences based on playing positions. In contrast, the accuracy of the models was better during training sessions (RMSE = 81-135 vs 79-209 U/L). Position-specific differences were also found in the external and internal load parameters, which best explained the CK changes. Conclusion: The relationship between external/internal load parameters and CK changes are position specific and might depend on the type of session (training or match). Morning CK values also contributed to the next day’s CK values. Clinical Relevance: The relationship between position-specific external/internal load and CK changes can be used to individualize postmatch recovery strategies and weekly training periodization with a view to optimize match performance.

Abstract Purpose Sled towing has been shown to be an effective method to enhance the physical qualities in youth athletes. The aim of this study was to evaluate the impact of a 6-week sled towing intervention on muscular strength, speed and power in elite youth soccer players of differing maturity status. Method Seventy-three male elite youth soccer players aged 12–18 years (Pre-Peak Height Velocity [PHV] n = 25; Circa-PHV n = 24; Post-PHV n = 24) from one professional soccer academy participated in this study. Sprint assessments (10 m and 30 m), countermovement jump and isometric mid-thigh pull were undertaken before (T1) and after (T2) a 6-week intervention. The training intervention consisted of 6 weeks (2 × per week, 10 sprints over 20 m distance) of resisted sled towing (linear progression 10%–30% of body mass) during the competitive season. Bayesian regression models analysed differences between T1 and T2 within each maturity group. Results There were minimal changes in strength, speed and power (P = 0.35–0.80) for each maturity group across the 6-week intervention. Where there were changes with greater certainty, they are unlikely to represent real effect due to higher regression to the mean (RTM). Conclusion It appears that a 6-week sled towing training programme with loadings of 10%–30% body mass only maintains physical qualities in elite youth soccer players pre-, circa-, and post-PHV. Further research is required to determine the effectiveness of this training method in long-term athletic development programmes.

Although asymmetries in lower limbs have been linked with players’ performance in male soccer players, literature that has been published addressing female soccer is scarce. Thus, the aim of this study was twofold: (i) describe the asymmetries of women soccer players during jumping, change-of-direction and range-of-motion tests; and (ii) test possible relationships between asymmetries and injury risk in female soccer players. Sixteen female players (15.5 ± 1.5 years) performed a battery of fitness tests (i.e., jump ability, change-of-direction ability and passive range-of-motion) and muscle mass analysis via dual-energy X-ray absorptiometry, through which the specific asymmetry index and the related injury risk were calculated. Significant (p < 0.05) lower asymmetries in the change-of-direction test were observed in comparison to those observed in jumping and range-of-motion tests; significant (p < 0.05) lower asymmetries in muscle mass were also reported compared to those found in the change-of-direction and countermovement jump tests. Additionally, increased injury risk for countermovement jump and hip flexion with extended knee range-of-motion (relating to asymmetry values) and for ankle flexion with flexed knee range-of-motion in both legs (relating to reference range-of-motion values), as well as increased individual injury risk values, were observed across all tests. These findings suggest the necessity to implement individual approaches for asymmetry and injury risk analyses.

Abstract Objective Systemic elevations in pro-inflammatory cytokines are a marker of non-functional over reaching, and betaine has been shown to reduce the secretion of pro-inflammatory cytokines in vitro. The aim of this study was to investigate the effects of betaine supplementation on tumor necrosis factor alpha (TNF-α), interleukins-1 beta (IL-1β), − 6 (IL-6) and the complete blood cell (CBC) count in professional youth soccer players during a competitive season. Methods Twenty-nine soccer players (age, 15.5 ± 0.3 years) were randomly divided into two groups based on playing position: betaine group (BG, n = 14, 2 g/day) or placebo group (PG, n = 15). During the 14-week period, training load was matched and well-being indicators were monitored daily. The aforementioned cytokines and CBC were assessed at pre- (P1), mid- (P2), and post- (P3) season. Results Significant (p < 0.05) group x time interactions were found for TNF-α, IL-1β, and IL-6. These variables were lower in the BG at P2 and P3 compared to P1, while IL-1β was greater in the PG at P3 compared to P1 (p = 0.033). The CBC count analysis showed there was significant group by time interactions for white blood cells (WBC), red blood cells (RBC), hemoglobin (Hb), and mean corpuscular hemoglobin concentration (MCHC). WBC demonstrated increases at P3 compared to P2 in PG (p = 0.034); RBC was less at P3 compared to P1 in BG (p = 0.020); Hb was greater at P2 compared to P1, whilst it was less at P3 compared to P3 for both groups. MCHC was greater at P3 and P2 compared to P1 in BG, whereas MCHC was significantly lower at P3 compared to P2 in the PG (p = 0.003). Conclusion The results confirmed that 14 weeks of betaine supplementation prevented an increase in pro-inflammatory cytokines and WBC counts. It seems that betaine supplementation may be a useful nutritional strategy to regulate the immune response during a fatiguing soccer season.
