Guidance of eruption and development of the primary, mixed, and permanent dentitions is an integral component of comprehensive oral healthcare for all pediatric dental patients; which should contribute to the development of a permanent dentition that is stable, and functionally and esthetically acceptable. In this case note, we present a case in which various forms of interceptive treatment modalities like; extraction of impacted and erupted supernumerary teeth, correction of anterior crossbite and irregularities using Z-springs and Hawley’s labial bow, and provision of lingual holding arch space maintainer were done to correct malocclusion in a nine-year-old patient. Arch alignment was achieved within seven month of starting of treatment, which was acceptable to the patient.

It is evident that hyperdontia is more common in the permanent dentition than in the primary. There is a considerable difference between males and females in the prevalence of these teeth in permanent dentition; hyperdontia is twice as common in males as in females. However, this approximation varies in terms of location, other associating syndromes that may be present, and the ethnicity of the individual. In terms of ethnicity, it can be seen that hyperdontia is in fact less common in Caucasian than in Asian populations. There is evidence to show that an individual is more likely to have hyperdontia if other members of their family also have the condition. Hence the present study was planned for evaluation of occurrence of hyperdontia in non-syndromic  population from Bihar Region. The present study was planned in Public Health Dentistry, Patna Dental College and Hospital, Patna, Bihar. Total 195 patients referred to Department of Dentistry were evaluated in the present study. Panoramic radiographs and clinical records of patients above the age of 18 years and without any syndromic features were selected for the study.  All the radiographs were examined for the presence of supernumerary teeth, their location, morphology, and number. Morphologically, teeth were classified as conical, tuberculate, supplemental, and odontoma. Early diagnosis of dental anomalies can prevent some esthetic, orthodontic, and periodontal problems, and knowledge of the prevalence and distribution of the anomalies may help clinicians to the detection of these anomalies at early stages. Our study evaluated the prevalence of selected dental anomalies; future studies should investigate the prevalence of dental anomalies of all types. ABSTRACTSupernumerary teeth occur frequently in permanent dentition, but they are rarely found in primary dentition. Supernumerary teeth of orthodox shape and size that resemble normal dentition are called ‘supplemental teeth’. Supplemental teeth are less common than supernumerary teeth and are often overlooked because of their normal shape and size. Supplemental teeth may cause esthetic problems, delayed eruption and crowding, and they require early diagnosis and treatment to prevent complications. The case reported here is one of bilateral supplemental teeth impeding the eruption of permanent maxillary lateral incisors, and it emphasizes the importance of early diagnosis and treatment during early mixed dentition. Objective: To evaluate the dental characteristics of patients subjected to a protocol that included early secondary gingivoalveoloplasty (ESGAP). Design: Panoramic radiographs of 87 patients with unilateral cleft lip and palate (UCLP) and 29 with bilateral cleft lip and palate (BCLP) were evaluated. Missing and supernumerary teeth were also quantified on the cleft and noncleft side and in the maxilla and mandible. Crown and root malformations and tooth rotations were quantified. A subsample in permanent dentition was extrapolated to analyze canine eruption patterns. Results: A total of 48.8% of the UCLP patients presented with missing permanent lateral incisors in the cleft area and 6.1% contralaterally. A total of 4.9% presented with missing second maxillary premolars on the cleft site and 1.2% contralaterally. A total of 7.3% presented with supernumerary lateral incisors, and 45% of the BCLP cleft sites presented with missing lateral incisors, while 25% of the cleft sites presented second maxillary premolars agenesis. Five percent of the cleft sites presented with supernumerary lateral incisors. Evaluation of the subsample in permanent dentition showed that 15.5% had a canine retention and 4.4% of the canines had to be surgically exposed. A significant association was observed between canine inclination and retention but not with absence of the lateral incisor. Conclusions: The frequency of dental anomalies in this sample was similar to other cleft populations. As surgical trauma has been suggested to damage forming teeth, the results of this study indicated that ESGAP has no detrimental influence on subsequent dental development.

Nonsyndromic cleft lip and/or palate (NSCL/P) are the most common congenital anomalies of the face. Dental abnormalities located inside and outside the cleft region are the most frequent dental characteristics found in these individuals. The aim of this paper is to review the literature on dental abnormalities associated with NSCL/P. A search for articles was conducted on electronic databases such as Medline, Lilacs and PubMed. According to the literature, dental abnormalities were more common in permanent dentition. Agenesis and supernumerary teeth showed the highest prevalence. The upper lateral incisor was the most affected teeth by agenesis, supernumerary teeth and microdontia. The teeth located closest to the cleft were more susceptible to defects. The presence of dental anomalies outside the cleft region suggests the presence of genetic events involved in both odontogenesis and craniofacial formation.

Supernumerary teeth occur in approximately 1–2% of the population. Multiple supernumerary teeth are less common and usually associated with certain medical conditions. Most supernumeraries develop at a similar time to the normal series; however, this article describes the presentation and management of three patients who developed multiple supernumerary teeth during active orthodontic treatment in the permanent dentition. CPD/Clinical Relevance: These cases show development of multiple supernumerary teeth in the both the maxilla and mandible demonstrating a variation of types and effects on the dentition. In particular, multiple supernumeraries in the anterior maxilla and supplementals in the mandibular premolar regions highlight that supernumerary teeth may develop a considerable time after the development of the permanent dentition. These can develop during orthodontic treatment. Consideration, therefore, may be given to closer and longer monitoring of dental development in patients with multiple supernumerary teeth.

Supernumerary teeth are considered as one of the most significant dental anomalies during the primary and early mixed dentition stages. They are of great concern to the dentists and parents because of the eruption, occlusal, and esthetic problems they can cause. Supernumerary teeth occur more frequently in the permanent dentition but rarely in primary dentition. Mesiodens is the most common type of supernumerary teeth but rarely seen in lower arch. Early recognition and diagnosis of supernumerary teeth is important to prevent further complications in permanent dentition. Four cases of supernumerary teeth with mesiodens in upper and lower arch in primary dentition and their management have been discussed.

Supernumerary tooth (ST) is a developmental disturbance of number of teeth characterized by teeth present in excess of normal dental formula of deciduous or permanent dentition.1,2,3,4 It may occur in either dental arch, maxillary or mandibular.5 Literature has shown marked predilection in maxilla over mandible.6,7 The first documented report of supernumerary teeth has been revealed in human fossils that are approximately 11,000 years old.8

Supernumerary teeth and odontomas are obstacles for spontaneous tooth eruption and may result in impaction. The aim of the study is to present a conservative treatment approach for impacted teeth following surgical obstruction removal by reviewing three treatment modalities: surgery only, which involves the surgical removal of the obstruction and the spontaneous eruption; surgery with immediate traction, which includes surgery combined with immediate active orthodontic brace cementation and traction; and surgery with delayed traction, which combines a surgical procedure of obstacle removal and orthodontic brace cementation with follow-up for the spontaneous eruption. The first two modalities require orthodontic traction either by an additional surgical procedure for orthodontic brace cementation, or combined with the surgical obstacle removal. With the third approach, clinical follow-up is performed via connected ligature wire elongation applied during the surgical procedure for the spontaneous emergence of the impacted tooth. Active orthodontic traction is only employed if the tooth fails to erupt. The visual follow-up via wire elongation serves as a reference during the emergence of the impacted teeth and reduces the need for radiographic examination. The surgical-orthodontic approach saves both further surgery and orthodontics (spontaneous eruption) or further surgery (in failure to erupt).

This case series describes the different potential approaches to late-developing supernumerary premolars (LDSP). LDSP are supernumerary teeth (ST) formed after the eruption of the permanent dentition; usually they develop in the premolar region of the upper and lower jaw. The choice to extract or to monitor the LDSP depends on many factors and has to be carefully planned due to the several risks that either the monitoring or the extraction could provoke. These four cases of LDSP showed different treatment plan alternatives derived from a scrupulous assessment of the clinical and radiographic information.
