The purpose of the paper was the assessment of the success of an artificial intelligence (AI) algorithm formed on a deep-convolutional neural network (D-CNN) model for the segmentation of apical lesions on dental panoramic radiographs. A total of 470 anonymized panoramic radiographs were used to progress the D-CNN AI model based on the U-Net algorithm (CranioCatch, Eskisehir, Turkey) for the segmentation of apical lesions. The radiographs were obtained from the Radiology Archive of the Department of Oral and Maxillofacial Radiology of the Faculty of Dentistry of Eskisehir Osmangazi University. A U-Net implemented with PyTorch model (version 1.4.0) was used for the segmentation of apical lesions. In the test data set, the AI model segmented 63 periapical lesions on 47 panoramic radiographs. The sensitivity, precision, and F1-score for segmentation of periapical lesions at 70% IoU values were 0.92, 0.84, and 0.88, respectively. AI systems have the potential to overcome clinical problems. AI may facilitate the assessment of periapical pathology based on panoramic radiographs.

Abstract Objective: The aim of the present study was to predict osteoporosis on panoramic radiographs of women over 50 years of age through deep learning algorithms.Method: Panoramic radiographs of 744 female patients over 50 years of age were labeled as C1, C2, and C3 depending on mandibular cortical index (MCI). According to this index; C1: presence of a smooth and sharp mandibular cortex (normal); C2: resorption cavities at endosteal margin and 1 to 3-layer stratification (osteopenia); C3: completely porotic cortex (osteoporosis). The data of the present study were reviewed in different categories including C1-C2-C3, C1-C2, C1-C3 and C1-(C2+C3) as two-class and three-class prediction. The data were separated as 20% random test data; and the remaining data were used for training and validation with 5-fold cross-validation. AlexNET, GoogleNET, ResNET-50, SqueezeNET, and ShuffleNET deep learning models are trained through the transfer learning method. The results were evaluated by performance criteria including accuracy, sensitivity, specificity, F1-score, AUC and training duration. Findings: The dataset C1-C2-C3 has an accuracy rate of 81.14% with AlexNET; the dataset C1-C2 has an accuracy rate of 88.94% with GoogleNET; the dataset C1-C3 has an accuracy rate of 98.56% with AlexNET; and the dataset C1-(C2+C3) has an accuracy rate of 92.79% with GoogleNET. Conclusion: The highest accuracy was obtained in differentiation of C3 and C1 where osseous structure characteristics change significantly. Since the C2 score represent the intermediate stage (osteopenia), structural characteristics of the bone present behaviors closer to C1 and C3 scores. Therefore, the data set including the C2 score provided relatively lower accuracy results.

Objectives: Maxillary impacted canines have an important place in orthodontics for the reason of being a common anomaly and its treatment is time consuming and difficult. The aim of this study was to investigate the mandibular indexes and cephalometric parameters in adolescents with impacted maxillary canines. Materials and Methods: The radiographs of 4026 patients aged 12–19 years old were scanned and a total of 155 patients were included in the study. Subjects were divided into three groups according to the permanent maxillary canines: Bilateral impacted canine (BIC) (52 subjects; mean age 15.02 ± 1.74 years), unilaterally impacted canine (UIC) (51 subjects; mean age 14.89 ± 1.39 years), and the control (52 subjects; mean age 14.84 ± 1.65 years) groups. Skeletal, soft tissue and dental measurements were performed on cephalometric radiographs. Mandibular index analyzes including the panoramic mandibular index (PMI), mental index (MI), antegonial index (AI), and gonial index (GI) measurements and also angular measurements of impacted maxillary canines were performed on panoramic radiographs. Data were analyzed statistically and P < 0.05 was considered significant. Results: Maxillary skeletal and dental measurements showed retrusive positioned maxillary dental arch in the sagittal direction in the impacted canine groups (P < 0.05). PMI and MI values were greater in the BIC and UIC groups than in control group (P < 0.05). There was no statistically significant difference in AI value among groups (P < 0.05). GI value of BIC group was found statistically lower than other groups (P < 0.05). Conclusion: Individuals with impacted maxillary canines have more retrusive maxillary dental arch and soft-tissue components. Furthermore, mandibular cortex was thicker in subjects with impacted maxillary canine than individuals with not and clinicians should keep in mind this when examine the panoramic radiographs in terms of early diagnosis of impacted maxillary canine teeth.

Objectives: The purpose of this study was to determine the sensitivity of using this panoramic radiograph in diagnosing maxillary sinusitis. Review: Scoping Review with article searches was carried out through NCBI's PubMed database, Science Direct, and EbscoHost from January 2020 to March 2021, with the inclusion criteria being articles published in 2010-2020.   Conclusion: Panoramic radiographs were found to be less sensitive for diagnosing maxillary sinusitis, but due to the low availability and accessibility of CBCT and CT, and related to their expensive cost and high radiation, panoramic radiographs are considered to be quite optimal.

Purpose: The purpose of this study was to determine the correlation of panoramic radiographic findings and clinical findings among dental patients at Moi Teaching and Referral Hospital (MTRH), Kenya. Materials and Methods: This was a hospital based cross sectional study conducted at the Radiology and Imaging department and dental department at Moi Teaching and Referral Hospital from September, 2019 to June, 2020. A total of 93 consenting patients were enrolled using systematic random sampling. Standard chart review forms were administered, and clinical examination findings documented. Panoramic radiograph findings were then discussed with a consultant radiologists and findings recorded. Descriptive statistics were carried out. Categorical variables were summarized as frequencies and proportions, and reported in tables. Numerical variables were summarized as median and interquartile ranges. Cross tabulation was done to compare clinical examination and panoramic radiograph findings where total raw agreement was reported as proportions. Results: 93 patients whose ages ranged from 5-73 years with a mean of 29 years were included into the study. Radiographic features of dental caries were present in 54% of panoramic radiographs compared to clinical examination (50.5%) while 23.7% of radiographs revealed impacted teeth compared to clinical examination (19.4%).  Radiographic features of periodontitis were observed in 14% compared to clinical examination (16.1%). Fractures (12.9%) were observed radiographically compared to (10.7%) clinically. Periapical lesions (8.6%) were observed radiographically compared to clinical examination (6.4%). Temporo-mandibular disorder (6.5%) were observed both on radiographs and clinical examination. Radiographic features of missing teeth (3.2%) were observed compared to clinical examination (1.1%) while only 1.1% of malpositioned teeth were observed radiographically compared to clinical examination (2.2%). Notably, mandibular lesions (3.2%) and nasal congestion (14%) were only discovered radiographically. The overall raw agreement between panoramic radiograph and clinical examination was 75.3% (70/93) with a p value of less than 0.001. Unique contribution to theory, practice and policy: The study recommends that all patients presenting with dental conditions should be done panoramic radiograph to improve diagnosis. Moi Teaching and Referral Hospital and the Ministry of Health to consider routine panoramic radiographs in the guidelines for the assessment of dental patients in all the hospitals offering, dental health services

Objective: The purpose of this study was to analyse the prevalence of tonsilloliths and to determine radiographically whether there is a relationship between tonsilloliths and dental plaque-related pathologies in a series of digital panoramic radiographs. Materials and Methods: This retrospective study included digital panoramic radiographs of 859 patients admitted for a routine dental examination. The panoramic images were examined for both the presence of tonsilloliths and the number of decayed, missing, restored tooth and apical periodontitis. Periodontal bone loss was also measured in thirds of optimal bone height according to the root length and finally a percentage of bone loss was obtained for each panoramic radiograph evaluated. Results: Tonsilloliths were observed in 141 (16.4%) of all individuals. While there was no significant difference regarding the number of decayed teeth and restored teeth between tonsillolith cases (TT) and cases without tonsillolith (TC), the number of missing teeth and apical periodontitis in TT was significantly higher than TC (p: 0.004, p: 0.030, respectively). There was a significant difference between the groups in terms of the mean percentage of bone loss (p: 0.001; p< 0.05). In addition, cases showing bone loss between one-third and two-thirds of the optimal bone height in the TT group (52.5%) were significantly higher than those in TC (45.5%) (p: 0.035; p< 0.05). Conclusion: The relationship between dental plaque-related pathologies and tonsilloliths, observed in this retrospective study, should be confirmed by Computed Tomography studies and randomized, prospective, clinical trials conducted in a multidisciplinary manner.

Routine panoramic radiographs often reveal hidden lesions other than those related to the patient's chief complaint [1]. The study aims to determine the prevalence of incidental radiopacities in panoramic views. There are several conditions where calcifications can occur in soft tissue, including major and minor salivary glands, lymph nodes, and blood vessels, such as sialolith, phlebolith, exostosis, dystrophic calcifications, etc. [2]. Interpretation of panoramic radiograph should consider the possibilities of soft tissue calcification super-imposed in bone. Three hundred and sixty (360) panoramic radiographs are included in the study. The prevalence and distribution of radiopacities were documented along with the probable diagnosis. In addition, statistical analysis was done by SPSS software to evaluate the age and gender-related distribution of radiopacities in panoramic radiographs.

Occlusal displacement often occurs after surgery for condylar process fractures because it is difficult to reduce these fractures precisely. However, performing semi-rigid fixation using a sliding plate may overcome this limitation. A retrospective clinical comparison between semi-rigid and rigid fixations was performed. Among 34 patients who had unilateral condylar process fractures, 17 were treated with rigid fixation and the remaining with semi-rigid fixation using a sliding plate. For all patients, panoramic radiographs were collected 1 day and 6 months after surgery. In these radiographs, ramus height and condylar process inclination were measured, and the differences between the fractured and normal sides were assessed. Additionally, the radiographic density of the fracture area was measured. Differences in surgical outcomes and operative times between the two groups and changes in postoperative deviations within each group were analyzed. There was no statistically significant difference in ramus height and condylar process inclination between the two groups at postoperative day 1 and 6 months. Radio-density was observed to be higher in the rigid fixation group, and it increased with time in both groups. The semi-rigid fixation group had a significantly shorter operative time than the other group did. Semi-rigid and rigid fixations showed no differences in terms of effectiveness and outcomes of surgery. In terms of operative time, semi-rigid fixation was superior to rigid fixation.

Styloid process elongation associated with symptoms such as cervicofacial pain, tinnitus and otalgia is called Eagle Syndrome. The objective of this study is to assess the presence of elongated styloid process through panoramic radiographs, in a subpopulation of the State of Para, Brazil, as to age and sex. Panoramic radiographs were selected and the apparent size of the styloid process was measured from the point where the styloid leaves the tympanic plate to the lower tip of the process. Data were analyzed by means of chi-square tests and the “t” test with a significance level of 5%. Forty-three percent of all elongated styloid processes were observed in patients aged between 18 and 35 years old. Of these styloid processes measuring more than 30 mm, there were 815 that showed a Type I elongation pattern, being 98 that showed a Type II calcification pattern and 92 showed a Type III calcification pattern. It is concluded that the Type I elongated styloid process was the most frequent, but no statistically significant correlation was found between the presence of the elongated styloid process and the studied variables.
