Comparative analysis of the effectiveness of simultaneous laparoscopic operations in acute appendicitis accompanied by various gynecological pathologies

Introduction. According to the World Health Organization 20 to 30% of gynecological pathologies in women are assosiatedwith surgical pathology and these challenge surgeons and gynecologists to correct these pathologiessimultaneously. The study aimed to evaluation of effectiveness of simultaneous laparoscopic operations in acute appendicitis accompanied by various gynecological pathologies Material and Methods. Total 88 patients were involved in the study. Of these, 55 patients underwent laparoscopic surgery - appendectomy (A control subgroup) and 33 simultan surgeries (core B subgroups). Diagnosis of acute abdomenal in gynecological patients, including diagnostic laparoscopy, was performed on the basis of algorithm. Comparison of arterial pressure, heart rate, intensity of pain and patients' activation period was studied in control and main subgroups during surgery and postoperative period. Results. Comparative analysis has showed that the statistical difference between these parameters in patients with isolated appendectomy and simultaneous surgery is almost inaccurate and simultaneous operation may be adequate, despite the complex effects of some negative factors of laparoscopy.

The purpose of this study is to conduct a comparative analysis of the consumption of antibiotics for systemic use reimbursed by the state in Kazakhstan for 2017–2019 with the Access, Watch, and Reserve classification (AWaRe 2019) of the World Health Organization (WHO). The evaluation of the consumption of antibiotics for systemic use in Kazakhstan for 2017–2019 was carried out using the ATC/DDD methodology in accordance with the WHO AWaRe classification. The study used data on all antibiotics that were centrally purchased by a single purchaser during the study period. To understand how often Access group antibiotics are taken in Kazakhstan, the top-10 most consumed antibiotics were additionally studied. The results of a comparative analysis of the antibiotics for systemic use consumption for 2017–2019 by the Access, Watch, and Reserve groups showed a negative trend of a decrease in the consumption of Access group drugs from 1.17 defined daily dose (DDDs) per 1000 inhabitants per day (DID) (39%) in 2017 to 0.59 DID (30%) in 2019. There is an increase in consumption of Watch group antibiotics from 1.84 DID (61%) in 2017 to 1.37 DID (68%) in 2019, as well as an increase in consumption of Reserve antibiotics from 0.001 DID (0.03%) to 0.4 DID (2.11%). In recent years in Kazakhstan, there has been a decrease in the consumption of Access group antibiotics. In addition, the Watch group antibiotics are widely consumed with a certain upward trend. In 2019, one Reserve antibiotic was included in the top-10 most commonly consumed antibiotics. There is a predominant consumption of parenteral forms of antibiotics for systemic use in the country.

The article describes the results of comparative analysis of two classifiers – the nomenclature of medical services, approved by the order of the Ministry of Health of Russia from October 13 2017 № 804(n), and International Classification of Health Interventions (ICHI), developed by World Health Organization. Similarities and differences are identified in classification of medical interventions in ICHI and medical services in nomenclature. Results of this work are expected to serve as a base for nomenclature’s improvement.

The world is in the midst of a paramount pandemic owing to the rapid dissemination of coronavirus disease (COVID-19) brought about by the spread of the virus ‘SARS-CoV-2’. It is mainly transmitted among persons through airborne diffusion of droplets containing the virus produced by an infected person sneezing or coughing without covering their face. The World Health Organization (WHO) has issued numerous guidelines which state that the spread of this disease can be limited by people shielding their faces with protective face masks when in public or in crowded areas. As a precautionary measure, many nations have implemented obligations for face mask usage in public spaces. But manual monitoring of huge crowds in public spaces for face masks is laborious. Hence, this requires the development of an automated face mask detection system using deep learning models and related technologies. The detection system should be viable and deployable in real-time, predicting the result accurately so as to be used by monitoring bodies to ensure that the face mask guidelines are followed by the public thereby preventing the disease transmission. In this paper we aim to perform a comparative analysis of various sophisticated image classifiers based on deep learning, in terms of vital metrics of performance to identify the effective deep learning based model for face mask detection.

Around the end of December 2019, a new beta-coronavirus from Wuhan City, Hubei Province, China began to spread rapidly. The new virus, called SARS-CoV-2, which could be transmitted through respiratory droplets, had a range of mild to severe symptoms, from simple cold in some cases to death in others. The disease caused by SARS-CoV-2 was named COVID-19 by WHO and has so far killed more people than SARS and MERS. Following the widespread global outbreak of COVID-19, with more than 132758 confirmed cases and 4955 deaths worldwide, the World Health Organization declared COVID-19 a pandemic disease in January 2020. Earlier studies on viral pneumonia epidemics has shown that pregnant women are at greater risk than others. During pregnancy, the pregnant woman is more prone to infectious diseases. Research on both SARS-CoV and MERS-CoV, which are pathologically similar to SARS-CoV-2, has shown that being infected with these viruses during pregnancy increases the risk of maternal death, stillbirth, intrauterine growth retardation and, preterm delivery. With the exponential increase in cases of COVID-19 throughout the world, there is a need to understand the effects of SARS-CoV-2 on the health of pregnant women, through extrapolation of earlier studies that have been conducted on pregnant women infected with SARS-CoV, and MERS-CoV. There is an urgent need to understand the chance of vertical transmission of SARS-CoV-2 from mother to fetus and the possibility of the virus crossing the placental barrier. Additionally, since some viral diseases and antiviral drugs may have a negative impact on the mother and fetus, in which case, pregnant women need special attention for the prevention, diagnosis, and treatment of COVID-19.
