The Value of Aqueous Assessment in Diagnosing Dry Eye

Introduction: The clinical evaluation of the three layers of tear film is still poorly described. The purpose of this study is to evaluate the value of aqueous assessment in diagnosing dry eye. Materials and method: Schirmer test with anaesthesia (STA) and tear meniscus height (TMH) measurement were conducted on non-dry eye (NDE) and dry eye (DE) subjects in this cross-sectional study. The NDE and DE subjects were classified using two types of classification; classification 1 and classification 2. Results: 321 subjects with 642 eyes were recruited in the study. STA was significantly correlated with TMH (r = 0.24, p < 0.001) in all 642 eyes. The comparison between nondry and dry eye subjects in STA and TMH were not significantly different (p > 0.05) if the Classification 1 was used to define dry eye. In Classification 2, there were significantly different between NDE (12.5 ± 8.2 mm) and DE (3.4 ± 0.8 mm) subjects in STA (p < 0.001). Similar trend was also depicted in TMH based on the definition of dry eye stated in Classification 2 (NDE = 0.45 ± 0.20 mm, DE = 0.39 ± 0.14 mm; p < 0.05). Conclusion: The value of STA and TMH were lower significantly in dry eye subjects. However, the significant outcomes were only demonstrated if the clinical signs of dryness were used in the definition of dry eye.

Permukaan okular rentan terhadap efek iritatif dari debu semen tersebut karena epitel konyungtiva dan kornea hanya dilapisi oleh lapisan tipis tear film. Paparan debu semen jangka panjang menyebabkan terjadinya inflamasi subklinis kronik yang dapat mempengaruhi transdiferensiasi epitel konyungtiva dan densitas sel goblet yang kemudian dapat menyebabkan timbulnya gejala dry eye syndrome. Tujuan penelitian ini adalah menilai status tear film pada masyarakat  yang  terpapar  emisi  debu  semen  dibandingkan  dengan  masyarakat  yang  tidak  terpapar.  Penelitian ini berupa analytic cross sectional study pada dua populasi yaitu kelompok terpapar dan tidak terpapar emisi debu pabrik semen. Semua sampel penelitian dari dua populasi menjalani pemeriksaan pH tear film, Schirmer, Ferning dan Tear Break Up Time (TBUT). Hasil studi mendapatkan peningkatan nilai pH tear film yang bermakna pada penduduk di kelompok terpapar dengan p=0.001. Terdapat juga penurunan kualitas Ferning yang bermakna pada penduduk di kelompok terpapar yang didominasi tipe III dengan p=0.005 dan 0.029. Sedangkan hasil pemeriksaan Schirmer dan TBUT masih dalam batas normal dan tidak terdapat perbedaan yang bermakna antara kedua kelompok. Simpulan studi ini ialah erdapat peningkatan rerata nilai pH tear film dan penurunan kualitas lapisan musin tear film yang bermakna pada masyarakat yang terpapar emisi debu semen namun tidak didapatkan peningkatan kejadian dry eye yang bermakna.

Pterygium is a common external eye disease in general population which progresses slow but has a little effect on vision.   Purpose: To know the tear film and ocular surface changes with pterygium.   Study Design:A descriptive cross sectional study     Place and duration of study: Federal Government Services Hospital Islamabad during Jun.2013 to Dec.   Material and methods:Dry eye questionnaire (DEQ-6) was administered by a trained researcher and DE tests were performed in all 256 willing subjects (136 with pterygium+120control) age 30-76years,by a single surgeon under same physical conditions after taking the consent and approval from Hospital Ethical committee.  Diagnosis was made on presence of both symptoms and tear film parameters. Statistical analysis was by simple percentages.   Results:DE was found in 73(53.7%) of the pterygium cases and 28(23.5%) of the normal patients. In this study, 55(40.5%) were symptomatic, defined as reporting 1 or more DE symptoms often or all of the time. 53(39%) showed corneal fluorescein staining CFS and 69(51%) showed lid plugging/mucous threads in both groups Of 136 eyes with pterygium, [male 91 (67%) female 45(33%)], 50(37.5%) showed normal tear film and 86(62.5%) showed deranged functions whereas among 120 control eyes,[Male 73(61%) female 47(39%)],86(72.3%) were normal and 34(27.7%)had abnormal functions.These values were reduced indicating changed tear film in these patients. Conclusion:Pterygium disturbs tear functions causing dry eye like symptoms. Key-words: Dry eye, pterygium, tear film instability, ocular surface

Industri semen diketahui menghasilkan polutan udara yang berbahaya bagi kesehatan yang dapat berupa debu dan memiliki tingkat toksisitas yang paling tinggi dibanding polutan udara lainnya. Paparan debu semen jangka panjang menyebabkan terjadinya inflamasi subklinis kronik yang dapat mempengaruhi transdiferensiasi epitel konyungtiva dan densitas sel goblet yang kemudian dapat menyebabkan timbulnya gejala dry eye syndrome. Penelitian ini berupa analytic cross sectional study pada dua populasi yaitu kelompok terpapar dan tidak terpapar emisi debu pabrik semen. Semua sampel penelitian dari dua populasi menjalani pemeriksaan pH tear film, Schirmer, Ferning dan tear break up time.Terdapat peningkatan nilai pH tear film yang bermakna pada penduduk di kelompok terpapar Terdapat juga penurunan kualitas Ferning yang bermakna pada penduduk di kelompok terpapar yang didominasi tipe III. Sedangkan hasil pemeriksaan Schirmer dan TBUT masih dalam batas normal dan tidak terdapat perbedaan yang bermakna antara kedua kelompok. Pada pemeriksaan sitologi impresi pada kelompok terpapar didapatkan densitas sel goblet yang masih dalam batas normal dan juga tidak didapatkan metaplasia pada sel epitel konjungtiva. Terdapat peningkatan rerata nilai pH tear film dan penurunan kualitas lapisan musin tear film yang bermakna pada masyarakat yang terpapar emisi debu semen namun tidak disertai kerusakan permukaan okular dan peningkatan kejadian dry eye yang bermakna.

Moreover, among the 120 control eyes there were 73 (61%) males and 47 (39%)] females. Out of these 86 (72.3%) patients were normal and 34 (27.7%) had abnormal functions. These values were reduced indicating altered tear film in these patients. Conclusion: Pterygium disturbs tear functions causing dry eye like symptoms. Introduction: There was no previous study to assess relationship between dry eye symptoms and signs in the local population. The purpose of this study is to evaluate the relationship of symptomatic assessment and clinical signs of dryness in dry eye subjects. Materials and method: A Cross-sectional study with convenient sampling involving 321 subjects with 642 eyes was done. Subjects were divided into non-dry eye (NDE) and dry eye (DE) groups. The division of NDE and DE groups were done using two types of classification. In Classification 1(symptoms-based), DE was defined if the score of Ocular Surface Disease Index (OSDI) score was more than 33. The value of tear film break-up time (TFBUT) less than 5 seconds and Schirmer test with anaesthesia (STA) less than 5mm/5 minutes were considered as DE for Classification 2 (signs based). Results: There were poor correlations between OSDI and clinical signs of dryness in all subjects involved (p > 0.05). Similar findings were also noted within NDE subjects in Classification 1 and Classification 2. OSDI score was also not significantly correlated with the signs of dryness within DE subjects except with ocular protection index (OPI) (r = - 0.14, p = 0.047) in Classification 1 and with conjunctival lissamine green staining (CLGS) (r = 0.23, p = 0.040) in Classification 2. Conclusion: There were no significant correlation between the symptoms of dry eye and the clinical signs of dry eye. Therefore, the clinical diagnosis of dry eye can be made based on either just on the symptoms or the signs.

The aim of this study was to determine healthcare providers’ knowledge and practices about dysphagia. A descriptive cross-sectional study was carried out based on a self-administered and anonymous questionnaire addressed to healthcare providers in Spain. A total of 396 healthcare providers participated in the study. Of these, 62.3% knew the definition of dysphagia as a swallowing disorder. In addition, up to 39.2% of the participants reported that they did not know whether the EatingAssessmentTool (EAT-10) dysphagia screening test was usedin their own clinical settings. Similarly, up to 49.1% of them did not know the ClinicalExaminationVolume-Viscosity (MECV-V) method. Nearly all participants (98.8%) reported that thickeners must be used forall liquids administered to patients. A higher percentage of respondents based the choice of texture on patient’s tolerance (78.2%) rather than on the MECV-V result (17.3%). In addition,76.4% of the professionals had witnessed a bronchoaspiration; after it, 44.4% (n = 175) of them reported the appearance of pneumonia, and 14.5% (n = 57) the death of the patient (p = 0.005). The participants revealeda moderate/low knowledge ofthe definition, diagnosis, and clinical management of liquid dysphagia, which indicates some room for improvements.
