Children's dental health has always been and remains an extremely important issue in modern society. According to the WHO, the incidence of dental caries in different countries and among different contingents ranges from 80% to 98%. In recent decades, there has been a growing trend among children. Of particular concern is the significant incidence of early dental caries in children, which is up to two years 62%, and at three years 70.3%. Effective prevention programs and early treatment of caries can improve the quality of children's dental health, and consequently the quality of their general physical health. The aim of our study was to study the dynamics of caries of deciduous teeth (prevalence, intensity, treatment necessity of deciduous teeth, the level of dental aid) in 3-5 years old aged children (observation during the year). All children were examined for the prevalence (%) and intensity of caries of deciduous teeth by the DEF-index and their increase was found in all age groups. The increase in caries for each age group is from 0.2 to 0.3 teeth. The prevalence of caries was studied at each age. 3-year-old children had a caries prevalence of 36.0 ± 6.85%. One year after the first examination, the prevalence of caries in 4-year-old children was now 42.0 ± 7.05%, which is 6% more than a year ago. The same trend is observed in children in the period from 4 to 5 years. The increase in the prevalence of caries in this group for the year is 6.7%. And in the group 5-year-old children the increase in the prevalence of caries per year was 3.1%. Comparison of children 4-year-old, but different groups did not reveal a significant difference. And at the age of 5 a probable difference in the prevalence of caries was revealed. Children with preventive work had a prevalence of 44.0 ± 5.77% against a group of children who were not provided with recommendations for maintaining dental health (65.3 ± 4.91%). In general, in children 3-5-years-old during first examination, the prevalence of caries was 49.1 ± 7.21%. During the year, the increase in the prevalence of caries in the same group of children, but 4-6-years-old, is only 4.9%. The intensity of caries indicates a slight increase in the number of teeth with caries in each group of children. But a comparison of this indicator in children of the same age of different periods of the survey revealed a probable difference at the age of 5 years (1.9 ± 0.35 teeth) (children who were recommended) against 2.7 ± 0.33 teeth (examined 1 year ago).The treatment necessity of deciduous teeth is 54% and the level of dental aid is 13%. Carrying out preventive work with children and their parents on selection of subjects and means of hygiene, regular care of an oral cavity, observance of a healthy food, necessity of sanitation of an oral cavity gave the chance to increase quantity of fillings at children of 4-6 years in comparison with 3-5 years and reducing the number of removed teeth due to complicated caries. Effective prevention programs and early treatment of caries can improve the quality of children's dental health, and consequently the quality of their general physical health. Timely and planned oral hygiene is a common and effective method of prevention of caries and periodontal disease in young patients. That is why a significant place in the prevention of caries in children is occupied by hygienic education and upbringing of children and their parents.

Today, the issues of studying the dental health of children and adolescents living in various regions of the Republic of Kazakhstan remain relevant. The health of the younger generation, including dental health, is an important priority area that contributes to improving the quality of life. Aim. To study dental morbidity in children and adolescents in various regions of the Republic of Kazakhstan. Material and methods. A survey of 608 children and adolescents living in educational institutions was carried out. The results of the preventive dental examination were recorded in an electronic dental card. Passport data, subjective, objective data and data of intraoral examination were included in the electronic map for studying the prevalence. Particular importance was attached to the state of oral hygiene, the indicator of sanitized patients and the need for sanitation of the oral cavity. Results and discussion. The rationale for conducting the survey in various regions of the Republic of Kazakhstan was the study of the level of health in children and adolescents to establish risk factors for dental morbidity. Dental examination of 608 children and adolescents testifies to the high intensity and prevalence of dental caries, periodontal tissue diseases, dentoalveolar anomalies. Most children and adolescents need oral cavity sanitation, this figure was 51,9%. Undoubtedly, in educational institutions it is necessary to carry out sanitary and educational work and timely sanitation of the oral cavity. The obtained statistical data will allow us to draw up a "dental health map" of children and adolescents living in various regions of the Republic of Kazakhstan and develop a comprehensive program for the prevention of dental diseases. Conclusions. One of the main issues in dental practice is the study of the prevalence of caries in early childhood, including among children under 14 years of age, in order to prevent the development of complications, to carry out preventive measures to improve the quality of life. Dental caries is the most common among children living in educational institutions. Also, 28% of children have poor-quality fillings, decayed teeth, and due to non-observance of oral hygiene, inflammatory diseases of the periodontal tissues develop. About 35% of children need orthodontic care due to the development of dentoalveolar anomalies. Orthodontic care, which is a paid service, is not fully provided to this category of children and adolescents. Therefore, there remain a number of organizational-preventive and organizational-therapeutic measures that need to be addressed. Keywords: examination, dental health, analysis, project.

Aim: This study presents the prevalence of dental caries and its relation to the quality of life of adolescents according to the access to dental health services. Methods: Two hundred and fifty-six adolescents between 15 and 19 years of age participated in the study; they were all enrolled in public schools in a countryside municipality of the São Paulo State. Data related to dental caries were evaluated by the DMFT Index, and OHIP-14 was used for evaluating the quality of life. Mann Whitney and Spearmann correlation tests were also used (p<0.05). Results: A DMFT of 3.09 (±3.30) was found with a higher prevalence among the adolescents who used public dental services (3.43±3.34) compared with those who used private services (2.94±3.28). A statistically significant relationship between the decay component of DMFT with physical pain (0.020), physical disability (0.002) and quality of life (0.017) was verified. Conclusions: A low prevalence of dental caries was observed, and it was higher in adolescents who used public oral health services rather than private ones, evidencing the low influence of oral health on the quality of life of the participants.

Abstract: Differences in the incidence rate of dental caries (DMF-T) between men and women aged 12-14 years. Dental and oral health is the well-being of the oral cavity which enables a person to communicate effectively, enjoy various foods, be confident, and improve a better quality of life. Poor dental and oral health can result in disruption of masticatory function due to tooth malfunction. One form of tooth decay is dental caries. The purpose of this study was to determine the incidence of dental caries (DMF-T) between men and women in children aged 12-14 years at SMPN 10 Kota Kupang. This study uses a descriptive study that describes the incidence of dental caries between men and women aged 12-14 years in SMP Negeri 10 Kota Kupang. A sample of 66 people was taken 100% of the population. The results showed as many as 66 children as respondents consisting of 33 male respondents as many as 18 children affected by caries with very low criteria, while 33 female respondents were only 13 children with moderate criteria. It was concluded that the incidence rate of dental caries in boys was very low and the rate of dental caries in girls was moderate. Abstrak: Perbedaan Tingkat Kejadian Karies Gigi (DMF-T) antara Laki-Laki dan Perempuan Usia 12-14 Tahun. Kesehatan gigi dan mulut adalah kesejahteraan rongga mulut yang memungkinkan seseorang untuk berkomunikasi secara efektif, menikmati berbagai makanan, percaya diri dan meningkatkan kualitas hidup yang lebih baik. Kesehatan gigi dan mulut yang buruk dapat mengakibatkan terganggunya fungsi pengunyahan yang disebabkan karena tidak berfungsinya gigi. Salah satu bentuk dari kerusakan gigi adalah karies gigi. Tujuan penelitian ini adalah untuk mengetahui tingkat kejadian karies gigi (DMF-T) antara laki-laki dan perempuan pada anak usia 12-14 tahun di SMPN 10 Kota Kupang. Penelitian ini menggunakan jenis penelitian deskriptif yaitu menggambarkan tingkat kejadian karies gigi antara laki-laki dan perempuan usia 12-14 tahun di SMP Negeri 10 Kota Kupang. Sampel berjumlah 66 orang diambil 100% dari populasi. Hasil penelitian menunjukkan sebanyak 66 anak sebagai responden yang terdiri dari 33 responden laki-laki sebanyak 18 anak terkena karies dengan kriteria sangat rendah, sedangkan 33 responden perempuan hanya 13 anak dengan kriteria sedang. Disimpulkan bahwa tingkat kejadian karies gigi pada anak laki-laki termasuk kriteria sangat rendah dan tingkat kejadian karies gigi pada anak perempuan termasuk kriteria sedang. The problem of treatment of patients with special health needs resulting from the presence of deficits in intellectual, motor or sensory functions, also in dentistry, has not been comprehensively solved. According to the World Health Organization (WHO), about 15% of the world population has some form of disability. Patients with disabilities are considered to have greater dental treatment needs in comparison to healthy individuals. High incidence and intensity of dental caries as well as frequent gum diseases are observed in this environment. Often the situation is related, among other, to the presence of systemic diseases and the treatment used, limitations in shaping proper dental health behaviors, insufficient knowledge of parents about oral health or barriers in access to dental care. The development of recommendations for parents/caregivers as well as medical staff is required to improve the health of these patients and quality of their life. The document contains information on the etiology and epidemiology of dental caries in patients with disabilities but also prophylactic recommendations for this population of patients and their parents/caregivers and dentists.

Modern domestic preventive dentistry has success in the theory of the origin and development of major dental diseases, practice of treatment, including dental caries, and uses these results to prevent children’s diseases in the country. However, the desired effectiveness of preventive measures have not been achieved primarily among the population and individual level, taking into account some specific characteristics of residence region, etiology and pathogenesis of the demineralization phenomena of hard dental tissues in different periods of child’s development. Adverse environmental conditions and technogenic character are not taken into account, when air, soil and groundwater are oversaturated with environmental contaminants and toxicants. They cause fatal breakage in biochemical reactions and metabolic processes in childhood and at a young age, when defective dental health brings a lot of problems in the adult period of life, and, as a result, reduces the life. Investigation of dental health had been conducted in 251 preschool age children (first age group consisted of 3-4 years-old, II group consisted of 5-6 – years-old with 123 and 128 people respectively). In addition, all the observed children were divided into two groups depending on the use of prophylactic anti-caries interventions: the basic group was in compliance with the "Program- maximum" and control the "Program- minimum". Approximately 60 members were in the monitoring teams of different age groups. The "Program-minimum" included measures of general health, balanced diet with the significant restriction of carbohydrate and perform twice a day hygiene actions in the oral cavity. The “Program – maximum” also included the ingestion of calcium citrate and amino acids with SH-group of methionine. Such treatment and prophylactic complex has the adaptive, antioxidant, antidote and immune resolving action. It makes sense for neutralization of eco toxicants – heavy metals in the area of children’s residence. It was found that level of Fe and Zn was in the threatening degree of content, and Cd is moderate in the living environment of the examined children. It provokes and enhances the glycolytic processes in the oral cavity. Harmful effect of toxicants in the oral cavity, and also in a children's organism as a whole, was clearly fixed in the mineralization of the skeleton flat bones of children. It was revealed with densitometry of the calcaneus that 53 individuals had  indicators, particularly BUA. They indicated the weakening of the skeleton mineralization. It indirectly pointed to problems in the hard tissues of the teeth. However, the use of treatment and prophylactic complex improved the bone structures density. The level of calcium and phosphate in the oral fluid of the observed individuals had recovered to optimal after three years of use of treatment and prophylactic complex, contributed to effective secondary mineralization both in temporary and in permanent teeth. Thus, the use of treatment and prophylactic complex in people of the basic group allowed quite convincingly and positively to impact on the incidence of caries of deciduous and permanent teeth. The rate of growth of dental caries intensity after three years of preventive interventions in the basic group was three times less than in the control group. At the same time, the caries preventive efficacy was 59-62%, and this is a very positive indicator. These results are evidence of proven necessity of prophylactic of dental health correction in children years of those people who reside in areas with a very complex technogenic environment due to metallurgical plants located in the Prydniprovsk region.

Невропатическая боль (НБ) возникает вследствие прямого повреждения или болезни соматосенсорной нервной системы. Целью настоящего исследования было проведение анализа базы данных пациентов с НБ для поиска групп пациентов имеющих сходные профили сенсорных симптомов, а также определение взаимосвязи между наличием и выраженностью этих симптомов с интенсивностью боли, тяжестью нарушений сна, степенью ограничений повседневной активности и тяжестью нарушений общего соматического здоровья. Материал и методы. Проведен кластерный и сравнительный анализ базы данных больных с НБ (n = 6506), полученной в скрининговом когортном двухэтапном исследовании пациентов, обратившихся за амбулаторной помощью к врачу неврологу. Результаты и обсуждение. Анализ методом К-средних выявил 3 кластера сенсорных симптомов, не связанных с этиологическими причинами НБ. Фенотипы (кластеры) НБ отличались по интенсивности боли, степени тяжести нарушений сна, степени снижения повседневной активности и тяжести нарушений общего соматического здоровья. Выводы. В реальной клинической практике фенотипическая стратификация НБ важна не только с позиций механизм-обоснованого подхода к её терапии, но и для прогнозирования интенсивности боли и степени ухудшения качества жизни у конкретного больного. Neuropathic pain (NeP) is caused by a direct damage or disease of the somatosensory nervous system. The aim of this study was to conduct an additional analysis of the database of patients with NeP to identify groups of patients with similar profiles of the sensory symptom and to determine the relationship between the presence and severity of these symptoms and the intensity of pain, severity of sleep disorders, degree of limitations in daily activity, and severity of disorders in general physical health. Material and methods. Cluster and comparative analyses were performed of the database of patients with NeP (n = 6506). The database was obtained in a two-stage screening cohort study of patients who had sought outpatient care of a neurologist. Results and discussion. The analysis using the K-means method provided 3 clusters of sensory symptoms not related with etiological causes of NeP. The phenotypes (clusters) differed in pain intensity, severity of sleep disorders, degree of decline in daily activity, and severity of disorders in general somatic health. Conclusions. In real clinical practice, the phenotypic stratification of NeP is important not only from the standpoint of the mechanism-based approach to its treatment but also for predicting the intensity of pain and the degree of deterioration of the quality of life in a particular patient.

Background: Due to little information on Iranian families, this study was conducted to investigate the correlation between the oral-dental health status of primary school children and their Family’s Quality of Life (QoL). Materials and Methods: In this cross-sectional study, the oral-dental health status of 251 primary school children in Torbat Heydariyeh county, Iran, was examined according to decayed, missing, and filled teeth index for primary (dmft) and permanent (DMFT) teeth by a checklist. Their mothers (n=251) responded to the Family Impact Scale (FIS) for the assessment of family’s QoL. Data were analyzed using SPSS version 20 by Poisson and Multiple Linear Regression analyses. Results: Primary dental caries of children at an intermediate level (dmft = 3.4 ± 3.1) and their permanent dental caries at a low level (DMFT = 2.6 ± 2.9) were evaluated. The mean score of families’ QoL was estimated 13±7.5 out of a maximum of 56. Pearson correlation test showed that there was no significant correlation between the FIS and dmft/DMFT index (P > 0.05). Regression models revealed that there was a correlation between mothers' dental visits over the past 8 months with the dmft (p=0.006), the DMFT index (p=0.016) and families’ QoL (p=0.045). Conclusion: Although our study didn't show a correlation between the dental health status of children and family’s QoL, the findings showed that there is still a gap between the goals of the WHO and the dental health status of children in Torbat Heydariyeh county.

AbstractDental biofilm present on the tooth surface is associated with oral diseases, such as dental caries and periodontal disease. Because bacterial numbers rapidly increase in saliva during sleep, oral care before sleeping is recommended for the prevention of chronic oral diseases. However, temporal circadian changes in the quantity and quality of dental biofilms are poorly understood. This study aimed to investigate the impacts of sleeping on dental biofilm amounts and compositions by using an in situ model. The use of this in situ model enabled us to investigate dental biofilm formed in the oral cavity and to perform a quantitative analysis. Subjects began wearing oral splints in the morning or before sleeping, and biofilm samples were collected at 8, 16, and 24 h after the subjects began wearing oral splints; these samples were then used in various experiments. No significant changes in the numbers of biofilm-forming bacteria were caused by sleep. However, the relative abundances of genera related to periodontitis (i.e., Fusobacterium and Prevotella) increased after awakening. In conclusion, the numbers of biofilm-forming bacteria were not affected by sleep, and the abundances of obligate anaerobes increased after sleep. This research may aid in defining efficacious preventive oral care.
