Background: Diabetics Mellitus a chronic metabolic disorder that prevents the body to utilize glucose completely or partially requires low caloric diets. Unfortunately, Nigerian traditional menu is based on starchy foods. Many diabetic individuals had been ignorantly oriented to consume some staple foods that are believed to have lower calorie than others for low blood glucose response.The study is focused on determining the glycemic index of commonly consumed fortified staple foods as well as determining the extent of their effects on the postprandial blood glucose responses of undergraduate students.Methods: Fifteen volunteered non- diabetic undergraduate students aged 20 – 25yrs on which standard oral glucose tolerate test was performed were selected and were asked to report at the study centre each day after overnight fasting (10 – 12hrs) with certain precautions by 8am for a period of 5days for different prepared test meals from the commonly consumed fortified staple food; namely; beans stew, rice stew, yam stew, unripe plantain stew and garri and stew. Fasting blood samples (2ml each) were collected from the prominent arm veins of each student prior to the eating of one of the test meals on each day. Each test meal contains 50g carbohydrate portion per meal. After the meals had been eaten, 2ml venous blood samples were collected at 30minutes interval for 120 minutes each day from each student and put into the specimen bottles for blood glucose estimations. Also the random sugar was determined 2hours later. A glucometer was used for measuring the blood sugar using test strips. The mean values of postprandial glucose blood sugar for each test meals for the selected students were obtained.Results: The glycemic index of the commonly consumed fortified staple test meals was highest in rice stew (107.7± 8.4), followed by yam stew (102.4±1.8), garri and stew (101.4±11.8), beans stew (86.9±6.7), and unripe plantain stew (81.8±8.4) in that order. The carbohydrate content of the commonly consumed fortified staple test meals was highest in garri with soup (68.2±0.2g) and lowest in unripe plantain stew while the fibre content was highest in plantain stew (5.8±0.4g) and lowest in rice stew (2.8±0.12g). The mean blood oral glucose tolerance test of the students was below 100mg/dl but has the mean peak after an hour interval (112.8±7.3mg/dl) the test food meals were ingested.The highest increase on the postprandial blood glucose after ingestion of the test meals was highest for garri and stew test meal (97.2±14.7mg/dl) but lowest for unripe plantain stew meal (85.574.3±11.6mg/dl) after 60 minutes the test meals were ingested.The random blood sugar test was highest in garri and soup (93.1±2.4mg/dl), followed by beans stew (88.4±4.9mg/dl), and lowest in plantain stew (74.3±11.6mg/dl) test meals after 2hrs the test meals were ingested.Conclusion: Unripe plantain stew meal had the lowest glycemic index response, with lowest postprandial blood glucose response after for the period of time the test meal were ingested.. This could be attributed to the relatively higher level of fibre content of the unripe plantain than other test meals.Key Words: glycemic index, postprandial blood glucose concentrations, glucose tolerance test

Aim: Is to find the mean and two standard deviation of the serum blood sugar among pregnant women while running the modified oral glucose tolerance test (MOGTT) as screening for gestational diabetes mellitus (GDM) & to compare the readings with other protocols adopted in diagnosing GDM. Method: A cross sectional study among pregnant women running routine MOGTT at 24-28 weeks’ gestation. A total of 149 women participated in 4 months period. The test included 5 ml of venous blood sample taken after fasting for 8 hours and a second blood sample 2 hours after having 200 ml of 75 g glucose solution within 10 minutes. Results: The mean for the fasting blood sugar is 4.32 mmol/L±0.52 making value of 2SD of 5.36 mmol/l. The mean of the 2 hours glucose level was 6.11mmol/l±1.38 making the 2SD value of 8.87 mmol/l. Conclusion: Our results for the 1st reading in MOGTT is near to the value of the local protocol in diagnosing GDM. The 2 hours postprandial reading in the local protocol is fairly low when compared with our findings & with guidelines of nearby communities.

Background: Corrigiola telephiifolia Pourr, is a perennial species, woody distributed throughout the north of Africa. This plant is used in traditional Mediterranean preparations and has many traditional uses especially treatment of diabetes. Aim/Methods: The current research was carried out to evaluate the antidiabetic effect of Aerial Parts of Aqueous Extract (APAE) of Corrigiola telephiifolia (C. telephiifolia) on both normal and streptozotocin (STZ)-induced diabetic rats treated at a dose of 5 mg/kg for fifteen days. Additionally, the histopathological changes in the liver, morphometric analysis, Oral Glucose Tolerance Test (OGTT) in normal rats and preliminary phytochemical screening for various components were realized. Results: Single oral administration of the APAE of C. telephiifolia (5mg/kg) showed no significant change in glycaemia of normal and STZ-induced diabetic rats. In contrast, repeated oral administration of C. telephiifolia reduced blood glucose levels from 4.11 ± 0.10 mmol/L to 3.16 ± 0.16 mmol/L (p<0.01) 15 days after administration in normal rats. Furthermore, blood glucose levels decreased from 17.84 ± 1.75mmol/L to 1.93 ± 0.33 mmol/L (p<0.0001) in STZ diabetic rats after fifteen days of treatment. According to the oral glucose tolerance test, C. telephiifolia (5 mg/kg) was shown to prevent significantly the increase in blood glucose levels in normal treated rats 30 min after glucose administration when compared to the control group. Also, the liver architecture of diabetic rats treated by C. telephiifolia was improved when compared with the liver architecture of untreated diabetic rats. Concerning the preliminary phytochemical screening of C. telephiifolia, several compounds have been found such as polyphenols, flavonoids, saponins, mucilage and terpenoids. Conclusion: The results show that the aqueous extract of C. telephiifolia possesses significant antihyperglycemic activity.

The purpose of this study is to compare whether or not there is a difference between venous and capillary blood samples in blood glucose measurements and investigate the effects of different aseptic methods used in skin cleaning before collecting blood samples on measurement results. This quasi-experimental study was conducted with 109 patients. The capillary first and second blood drop values taken from the patients after fasting and at 2 hours following 75 g oral glucose tolerance test (OGTT) and capillary and venous blood glucose values were compared. There was no significant difference between the median venous blood glucose value and the capillary second blood drop value taken after wiping the finger with alcohol. There was no significant difference between the first and second blood drop values of capillary blood glucose 2 hours after OGTT.

Background: The anti-hyperglycemic potential of methanol stem bark extract of Anacardium occidentale (MSBEAO) was investigated using an alloxan-induced diabetic rat model. Alloxan administration induces the generation of free radicals which can affect antioxidant status resulting in the disruption of the β-cells of the pancreas. Therefore, this study examines the antioxidant potential of the plant extract and the ameliorating effect on the pancreas of alloxan-induced diabetic rats. Methods: Diabetes was induced by intraperitoneal injection of 150 mg/kg body weight of alloxan monohydrate. MSBEAO, at a concentration of 100 or 200 mg/kg b.w. was orally administered to alloxan-induced diabetic rats and normal rats. The hypoglycemic effect, oral glucose tolerance test, and biochemical assay of alloxan-induced diabetic rats were assayed using standard procedures. Results: Preliminary phytochemical screening of the extract revealed the presence of alkaloids, tannins, saponins, terpenoids, carbohydrates, and phenols at moderate concentrations. The lethality dose (LD50) of the plant extract was found to be equal to or less than 5000 mg/kg b.w. The hypoglycemic effect of the extract on the non-diabetic rats revealed a significant (p<0.05) decrease in the blood glucose concentration of animals administered with 1 g/kg b.w. of the extract, compared to normal control rats administered with normal saline. In the oral glucose tolerance test, the methanol extract exerted the highest response, similar to glibenclamide after 15 and 30 minutes of administration, compared to the control rats. The methanol extract yielded the highest blood glucose lowering effects after 9 days of treatment (p<0.05), compared to diabetic rats administered with normal saline and 0.3 mg/kg b.w. of glibenclamide. Administration of the extract at 200 mg/kg b.w. showed improved pancreas architecture and regeneration of the β-cells, compared with the pancreas of animals in the other groups. Conclusion: The results of this study suggest that MSBEAO is a potentially effective agent for the management of diabetes which might result from the antioxidant-generating capacity of the stem bark.

Objective To test the hypothesis that there is seasonal variation in the rates of gestational diabetes (GDM) diagnosed using a 2 hour oral glucose tolerance test. Design Monthly assessment of the percentage of women screened from 1st April 2016 to the 31st December 2020 who were diagnosed as having gestational diabetes Setting London Teaching Hospital Population 28,128 women receiving antenatal care between April 1st 2016 and 31 December 2020. Methods Retrospective study of prospectively collected data. Main Outcome Measures Proportion of women screened diagnosed as having gestational diabetes. Results The mean (SD) percentage of women diagnosed with GDM was 14.78 (2.24) in summer (June, July, August) compared with 11.23 (1.62) in winter (p < 0.001), 12.13 (1.94) in spring (p = 0.002), and 11.88 (2.67) in autumn (p = 0.003). There was a highly significant positive correlation of the percentage testing positive for GDM with the mean maximum monthly temperature (R2 = 0.248, p < 0.001). There was a statistically significant 33.8% increase in the proportion of GDM diagnoses from June 2020 onwards, possibly related to a reduction in exercise secondary to the Covid-19 pandemic. Conclusions There is a 23.3% higher rate of GDM diagnoses in the warmer summer months. There has been a 33.8% rise in GDM diagnoses associated with the Covid-19 pandemic.

Introduction: Gestational diabetes mellitus is common disorder in pregnancy. It is associated with adverse pregnancy outcome. There is no consensus regarding the optimal approach to screening of gestational diabetes mellitus. The present study has tried toobserve the value of fasting blood glucose in screening of gestational diabetes. Objective: To determine the frequency of patients in whomfasting blood glucose and 100gm glucose tolerance show agreement for screening of gestational diabetes mellitus at 24 -28 wks. Studydesign: Comparative cross sectional study. Settings: The study was conducted at Gynecology and Obstetrics department Shaikh ZayedFederal Post Graduate Institute Lahore. Duration of study with dates: 6 months from 12Nov 2010 to 11 May 2011. Material and method: Thestudy included 135 booked patients with positive family history of diabetes mellitus. All patients underwent fasting blood glucose at 24-28 weeksof gestation, regardless of results of fasting blood glucose on next visit they underwent 100g oral glucose tolerance test (OGTT). The agreementbetween fasting blood glucose and 100g oral glucose tolerance test was calculated in frequency and percentages. Results: The mean age ofwomen in studied population was 27.15±3.70.Out of 135 patients 86.7 %( 117) showed agreement between results of fasting blood glucose and100g OGTT while 13.31 %( 18) showed no agreement between both of the tests. Conclusions: Fasting blood glucose is a good screeningoption for gestational diabetes mellitus along with positive history. It provides a simple, cheap and more practical test for screening of gestationaldiabetes mellitus. However diagnostic confirmation with 100g OGTT should be done.

Madam F. Kayes Bitters® is an herbal formulation commonly used in Nigeria and some African countries in the management of diabetes mellitus and other diseases conditions. This study evaluated the in-vivo hypoglycaemic activity, as well as acute toxicity of the polyherbal formulation to provide its efficacy and safety. Healthy albino mice (20-30 g) and Sprague Dawley female rats (90-130 g) were used for this study. Acute toxicity study (LD50) of the herbal formulation was determined by methods originally described by Miller and Tainter in 1994. Following oral dosing with glucose (2 g/kg) in normal fasted animals, herbal formulation (HF) at various doses was administered and blood glucose levels at 30 minutes, 60 minutes, 90 minutes, and 120 minutes were taken and recorded. Diabetes was induced using alloxan 150 mg/kg and diabetic rats were given the HF at doses of 50, 100, and 200 mg/kg with glibenclamide 2.5 mg/kg used as standard drug treatment. Blood glucose level was determined on 1st day, 7th day, 14th and 21st day. The LD50 was greater than 5g/kg with oral administration. The oral glucose tolerance test showed that the group that received 100 mg/kg HF showed a significant reduction (p<0.05) in glucose level after 120 minutes when compared to the basal level of glucose recorded. All treated diabetic groups showed a significant decrease in glucose level on the 21st day. The herbal formulation of Hydrastis canadesis Aloe capensis, Echinacea angustifolia and honey exhibited a significant glucose-lowering activity in alloxan-induced diabetic rats.

Abstract Background: Diabetes mellitus (DM) is concomitant with significant morbidity and mortality and its prevalence is accumulative worldwide. The conventional antidiabetic agents are known to mitigate the symptoms of diabetes; however, they may also cause adverse effects. This study explores the efficacy of polyherbal dietary supplement cinnamon, purple onion, and tea on the mediation of postprandial hyperglycemia for in the search of combinations with a maximal response. Materials and methods: A starch solution (3 g/kg Bwt) of oral starch tolerance test (OSTT) and glucose solution (4 g/kg Bwt) of oral glucose tolerance test (OGTT) with and without cinnamon, purple onion, tea extract (15 mg/kg Bwt), and mixture (each 5 mg/kg Bwt, 1:1:1), metformin (14 mg/kg Bwt), or acarbose (50 mg/kg Bwt) was administered to high fat plus high fructose-induced diabetic mice after an overnight fast. Postprandial plasma glucose levels were measured and incremental areas under the response curve were calculated. Results: Compared with acarbose, the mixture of extracts (purple onion, cinnamon, and tea) indicated decreasing blood glucose in OSTT. In OGTT, the mixture of extracts showed greater efficacy for hypoglycemia when compared with metformin. The molecular docking of α-Amylase, α-Glucosidase, and AMPK confirmed the putatively acting molecules from the extracts of purple onion, cinnamon, and tea. Conclusions: Overall, this investigation evidenced a beneficial mediation for the progression of lowering blood glucose with a combinatory extract of cinnamon, dietary onion, and tea, implicating their prospective as nutraceuticals that might ameliorate hyperglycemia in diabetes.
