Background: Schizophrenia (SCZ) is a severe, chronic illness characterized by psychotic symptoms and impairments in many cognitive domains. Dysregulation of brain derived neurotrophic factor (BDNF) is associated with the cognitive impairments seen in patients with SCZ. Given the growing literature supporting a positive effect of aerobic exercise on cognition in other populations, we hypothesized that a structured aerobic exercise program would improve cognitive and functional outcomes in subjects with SCZ, potentially mediated by increases in BDNF. Methods: The study was a small randomized parallel group clinical trial of subjects with SCZ comparing 12 weeks of aerobic exercise (AE) against control (CON) stretching and balance training. At Baseline, Week 12, and Week 20 we collected serum samples for analysis of brain derived neurotrophic factor (BDNF), and assessed functional, physical, and cognitive outcomes. Linear regression models were used to compare change scores between timepoints. Results: We randomized 21 subjects to AE and 17 to CON; however, only 9 AE and 6 CON completed their programs. Subjects in both groups were slower at the 400 m walk in Week 12 compared to Baseline, but the AE group had significantly less slowing than the CON group (B = –28.32, p = 0.011). Between Week 12 and Week 20, the AE group had a significantly greater change score on the Composite and Visual Learning Domain of the MATRICS Consensus Cognitive Battery (B = 5.11, p = 0.03; B = 13.96, p = 0.006). Conclusion: These results indicate that participation in a structured aerobic exercise paradigm may modestly blunt physical function decline and enhance cognitive function in individuals with SCZ.

The benefit of exercise in inducing brain-derived neurotrophic factor (BDNF) functions in relation to cognition had been reported. Nevertheless, the ambiguity remains with regards to the types of exercise and the duration of exercise required for one to have beneficial effects. In this study, we aimed to analyse the effects of varying modes of exercises and the duration required to improve BDNF functions, specifically in the young adults. The types of exercises evaluated in the meta-analysis include (1) single bout of acute aerobic exercise, (2) repeated and frequent sessions of aerobic exercise (program exercise) over a course of several weeks, and (3) resistance training. Only a single bout of acute aerobic exercise (z=4.92, p=0.00001) is sufficient to cause an increase in BDNF following exercise intervention, while program exercise (z=1.02, p=0.31) and resistance training (z=0.92, p=0.36) demonstrated inconsistencies, some exhibited significant increase in BDNF levels while others exhibited similar results with the control groups.

AbstractResearch suggests strong inter-relationships between physical exercise, levels of brain-derived neurotrophic factor (BDNF), levels of estrogen, and the menstrual cycle, and yet no single study has examined these factors collectively in humans. The current study assessed the effect of an acute bout of vigorous aerobic exercise (20 minutes of stationary cycling at 80% of heart rate reserve) on serum BDNF and estradiol in healthy, eumenorrheic women, ages 18-28. In addition, this study determined whether basal BDNF or the exercise-induced increase in BDNF varies throughout the menstrual cycle. Thirty-four subjects were assigned to an experimental (n = 27) or control condition (n = 7). Exercise transiently increased both estradiol (51.2%) and BDNF (23.6%), and basal levels of BDNF and estradiol predicted the magnitude of the exercise-induced increases. Basal BDNF did not vary significantly throughout the menstrual cycle. Exercise-induced changes in BDNF did not correlate with menstrual cycle day or basal estradiol. Basal estradiol and basal BDNF showed a marginally significant positive correlation. Taken together, these results indicate that brief, vigorous aerobic exercise is sufficient to elevate both BDNF and estradiol in healthy women and that the menstrual cycle dramatically influences the magnitude of exercise-induced changes in estradiol, but not BDNF

To test the hypothesis that brain-derived neurotrophic factor-(BDNF-) mediated neuroprotection is reduced by high-mobility group box-1 (HMGB1) in diabetic retina, paired vitreous and serum samples from 46 proliferative diabetic retinopathy and 34 nondiabetic patients were assayed for BDNF, HMGB1, soluble receptor for advanced glycation end products (sRAGE), soluble intercellular adhesion molecule-1 (sICAM-1), monocyte chemoattractant protein-1 (MCP-1), and TBARS. We also examined retinas of diabetic and HMGB1 intravitreally injected rats. The effect of the HMGB1 inhibitor glycyrrhizin on diabetes-induced changes in retinal BDNF expressions was studied. Western blot, ELISA, and TBARS assays were used. BDNF was not detected in vitreous samples. BDNF levels were significantly lower in serum samples from diabetic patients compared with nondiabetics, whereas HMGB1, sRAGE, sICAM-1, and TBARS levels were significantly higher in diabetic serum samples. MCP-1 levels did not differ significantly. There was significant inverse correlation between serum levels of BDNF and HMGB1. Diabetes and intravitreal administration of HMGB1 induced significant upregulation of the expression of HMGB1, TBARS, and cleaved caspase-3, whereas the expression of BDNF and synaptophysin was significantly downregulated in rat retinas. Glycyrrhizin significantly attenuated diabetes-induced downregulation of BDNF. Our results suggest that HMGB1-induced downregulation of BDNF might be involved in pathogenesis of diabetic retinal neurodegeneration.

A randomized clinical trial was utilized to compare the improvement of depression and brain-derived neurotrophic factor (BDNF) levels between community women with and without music aerobic exercise (MAE) for 12 weeks. The MAE group involved 47 eligible participants, whereas the comparison group had 59 participants. No significant differences were recorded in the demographic characteristics between the participants in the MAE group and the comparison group. Forty-one participants in the MAE group and 26 in the comparison group completed a pre- and posttest. The MAE group displayed significant improvement in depression scores (p= 0.016), decreased depression symptoms in crying (p= 0.03), appetite (p= 0.006), and fatigue (p= 0.011). The BDNF levels of the participants significantly increased after the 12-week MAE (p= 0.042). The parallel comparison group revealed no significant changes in depression scores or BDNF levels. In summary, the 12-week MAE had a significant impact on the enhancement of BDNF levels and improvement of depression symptoms. Middle-aged community women are encouraged to exercise moderately to improve their depression symptoms and BDNF levels.

Ramadan fasting (RF) is a type of diurnal intermittent fasting. Previous studies reported the benefits of RF in healthy subjects on mood and health related to quality of life (QoL). Cortisol and brain-derived neurotrophic factor (BDNF) have been shown to play a role in mood, body composition parameters, and health-related QoL. This study aimed at elucidating the mechanism of the benefit of RF, particularly cortisol and BNDF and their association with mood and QoL. Insulin growth factor-1 (IGF-1), interleukin (IL)-8, matrix metalloproteinase (MMP)-9, and myoglobin were determined. Thirty-four healthy men and women were recruited. Serum from peripheral venous blood samples was collected at five time points: 1 week before RF (T1); mid of RF (T2), last days of RF (T3), 1 week after RF (T4), and 1 month after RF (T5). The amounts of biological mediators in the serum samples were determined by enzyme-linked immunosorbent assay (ELISA) and Luminex assays. BDNF and cortisol significantly decreased at T3 (p < 0.05) and T4 (p < 0.001) compared to T1, respectively. It seems the benefits of RF for mood-related symptoms are mediated by different biological mediators, particularly cortisol and BDNF.
