The effects of dyslipidemia on noise-induced hearing loss in a petrochemical workers in the Southwest of Iran

BACKGROUND: Research has shown that the decrease in the inner diameter of vessels caused by hyperlipidemia lowers the capacity for blood oxygen delivery to the cochlea. This leads to impaired cochlear metabolism and causes hearing problems. OBJECTIVE: The effects of dyslipidemia on noise-induced hearing loss in workers were examined. METHODS: This descriptive cross-sectional study was performed on 692 male employees in a petrochemical industry in the southwest of Iran exposed to 85 dB noise. Clinical audiometry and blood sample tests were used to evaluate the hearing and prevalence indices of dyslipidemia (cholesterol, triglyceride, HDL and LDL). The data were analyzed using SPSS software version 25 (p = 0.05). RESULTS: The results showed that the prevalence of dyslipidemia was 24.5%with abnormal relative triglyceride frequency of 49.5%, HDL of 28%, LDL of 33%, and total blood cholesterol level of 37.8%. There was no significant relationship between NIHL and dyslipidemia (p >  0.09). However, the major NIHL drops at different frequencies were in the individuals with dyslipidemia. The parameters age and dyslipidemia increased NIHL odds ratio (95%C.I.). by 1.130 (1.160–1.100) and 1.618 (2.418–1.082) respectively CONCLUSION: The rate of hearing loss in individuals with dyslipidemia increases at different frequencies and it leads to an increase of the OR of NIHL in individuals with dyslipidemia. We can control dyslipidemia and its effective factors. The NIHL is more common in people exposed to noise.

Introduction: Ideal total blood cholesterol (TC) levels are associated with lower cardiovascular disease (CVD) morbidity and mortality. In the U.S. TC increases up to middle age, but declines at older ages. Few studies have characterized the transition from ideal to intermediate and poor TC levels in different life epochs and in minorities. Methods: Cross-sectional 2007-2012 NHANES data (N = 11,140) were used to estimate the age-, race-, and sex- specific prevalence of ideal (≥20 years: <200 mg/dL untreated, 16-19 years: <170 mg/dL), intermediate (≥20 years: 200-239 mg/dL or treated to goal, 16-19 years: 170-199 mg/dL), and poor (≥20 years: ≥240 mg/dL, 16-19 years: ≥200 mg/dL) TC, defined per American Heart Association criteria. We then used these data and novel Markov-type models to estimate net transition probabilities between ideal, intermediate and poor TC. Results: Between the ages of 16 and 18, the prevalence of ideal TC among European American (EA) and African American (AA) men was approximately 68%, notably higher than the prevalence in EA women (63%) and AA women (61%). Variation in the loss of ideal TC was also observed by race and sex. Between 16-50 years of age, the proportion of AA men, EA men and EA women with ideal levels of TC declined approximately 2.0% (95% CI: 1.8%, 2.2%) per year. In AA women by contrast, the age-specific decline in ideal TC was not uniform between 16-50 years of age. The proportion of AA women with ideal levels of TC declined 0.7% (95% CI: 0.2%, 1.2%) per year from 16-20 years of age but increased to 2.8% (95% CI: 2.4%, 3.3%) per year by age 50. Among populations with intermediate TC levels, estimated 1-year net transitions to poor TC peaked at age 16, the earliest age under investigation, for EA men, EA women, and AA men but remained stable for AA women through 70 years of age, where a net 0.6% (95% CI: 0.1%, 1.3%) of the population with intermediate TC levels transitioned to poor TC levels one year later. In all demographic groups and life epochs, greater proportions of the population transitioned from intermediate to poor TC than from poor to intermediate TC. Conclusions: Loss of ideal TC begins early in life and shows divergent patters by gender and race. Difficulties re-attaining ideal TC once classified as intermediate or poor support interventions that promote ideal TC levels in younger ages, especially among AA women.

Introduction: Noise Induced Hearing Loss (NIHL) is an occupational health hazard linked with noise exposure of more than 85 dB. Aircraft noise and other occupational noise exposure cause military aviators more susceptible to NIHL than commercial jet pilots. Aim: To find out the prevalence of noise induced hearing loss among different military aircrew population with special emphasis on identifying the associated risk factors. Materials and Methods: This cross sectional study was conducted on Kuwait Air force pilots those who came for annual medical checkup from July 2010 to July 2012. Audiometry of all subjects was done by ENT specialist and necessary data were collected by structured questionnaires. All data were analyzed by SPSS (Statistical Package for the Social Sciences) version 17.0 Results: Out of 221 aircrew 32 (14.48%) developed NIHL. Hearing loss was mild to moderate and predominately at high frequencies. Most affected group was helicopter pilots. Pilots of more than 40 years of age and those who had more than 1000 flying hours had mean hearing threshold more than other groups. The right ear was affected in majority of cases than left ear. Conclusion: Noise exposure to aircrew is inevitable so hearing loss prevention strategies might play a pivotal role to save aircrew hearing. Active surveillance of hearing protection practices might have beneficiary effect. Steps can be taken for early detection of NIHL and hearing conservation program. A cross sectional study was done in the department of otolaryngology of Central Police Hospital. Rajarbagh, Dhaka., during the period of January 2005 to December 2007 to find out noise induced hearing loss of traffic polices and to raise awareness for early diagnosis and treatment and also preventive measures of noise induced hearing loss. 'Thirty patients who were diagnosed as a case of hearing impairment by detailed history, clinical examination and related investigations were included in the study who were exposed to noise 0-20 years. Common chief complaints of the patients were impairment of hearing (100%), Minims (50%), dizziness (20%), aural discharge (6.66)%. Medical history of 11w patients were diabetes (6.66%) and hypertension (26.66 %). 40 % patients were smoker and 60 % patients were non smoker. A ssoc jar ed other disease like chronic supporative otitis media (CSOM) t vas present in 13.33 %. Pure tone audiometry revealed normal hearing 3.3 %, conductive hearing loss 10 %, sensorineural hearing loss 73.33 %, mixed hearing loss 13.33 % and normal hearing 6.66 %, mild hearing loss 50 To, moderate type of hearing loss 36.66 % and moderately severe 3.33 %, severe hearing loss were 3.33 %. Treatment of the patients were given by vitamin 13,, Lli, B6, cochlear vasodilators such as vinpocetine and hearing aids. Patients were advised to Hume to other department of police service. those traffic police persons are vulnerable of noise induced hearing loss determined by patients complain, clinical examination and investigations like PTA, were advised to be changed to other department of police service.

Background: After otorrhea and hearing loss, Tinnitus is the most common symptom in pa­tients with chronic otitis media (COM). The aim of this study was to evaluate the improvement of tinnitus in COM patients after tympanoplasty and tympanomastoidectomy surgeries. Mate­rials and Methods: This cross-sectional study was conducted on COM patients suffering from Tinnitus referred to Baqiyatallah hospital, Tehran, Iran undergoing tympanoplasty or tympano­mastoidectomy surgeries between March 2013 and August 2014. Tinnitus Handicap Inventory (THI) and Visual Analogue Scale (VAS) questionnaires were filled before and two months after surgery by each patient, and considered subjects were compared with each other. Audiometry test was taken from each patient before and two months after surgery. Audiometry results as well as Air-Bone gap were measured and evaluated prior and after surgery. Data were analyzed using SPSS software by ANOVA, sample t-test and Chi-square tests. Results: Eventually, 26 male and 24 female patients with a mean age of 38.62±11.88 years were enrolled. Air conduc­tion at all frequencies was 49.99±17.37 before and 36.98±22.06 after surgery (P<0.001). Sever­ity of tinnitus was 62.92±30.54 before and 30.54±20.08 after surgery based on THI (P<0.001). Also, it was 7.46±1.66 before and 3.5±2.06 after surgery based on VAS evaluations (P<0.001). Tinnitus severity reduction was significantly associated with the improvement of hearing loss and decrement of air-Bone gap (P<0.001). Tinnitus symptoms such as loudness, annoyance, impact on life and perception of Tinnitus significantly reduced after surgery. Moreover, Tym­panomastoidectomy was more effective on the improvement of Tinnitus in comparison with Tympanoplasty (P=0.019).Conclusion: It seems that, both tympanomastoidectomy and tympa­noplasty surgeries are effective on the improvement of tinnitus in patients with COM; however, Tympanomastoidectom surgery was shown to be more effective.[GMJ.2016;5(2):63-69]

AbstractNoise can have an impact on hearing loss. Changes in hearing thresholds due to noise are some temporary but others potentially permanent. The impact of hearing loss must be controlled so the quality of life will not decrease. A cross-sectional study to assess the quality and increasing knowledge about noise-induced hearing loss was implemented with lectures, discussions and a pre-test before the activity and a post-test after the activity to assess the knowledge development. The polling was conducted to assess the participants' attitudes and psychomotor skills. The study was followed by 77 participants, most of whom were aged 20-30 years, namely 23 (30%). It was found that 16 people (20%) had complaints of hearing loss, 18 people (24%) often used headsets and earphones, 18 (23%) people worked in noisy places. The average pre-test result was 35.06 (+13.72) and the post-test was 77.9 (+20.81). Comparison of pre-test and post-test results of hearing health education obtained different results (p < 0.0001) significantly. Noise-induced hearing loss is permanent deafness and cannot be treated with medicine or surgery. Prevention is the most important thing from noise-induced hearing loss. The counseling education about noise-induced hearing loss on community must always be performed as an prevention and early detection.Keywords: deafness, noise induce hearing loss, prevention, education AbstrakBising dapat berdampak terhadap gangguan pendengaran. Perubahan batas pendengaran akibat bising yang bersifat sementara, dan perubahan batas pendengaran akibat bising yang bersifat menetap. Dampak gangguan pendengaran harus dikendalikan agar tidak menimbulkan penuruna kualitas kehidupan. Studi cross sectional untuk menilai kualitas dan upaya peningkatan pengetahuan tentang gangguan pednegaran akibat bising dilaksanakan dengan ceramah, diskusi dan dilakukan pre test sebelum kegiatan dan post test setelah kegiatan untuk menilai peningkatan pengetahuan. Survei jajak pendapat dilakukan untuk menilai sikap dan psikomotor peserta. Studi diikuti 77 peserta, usia terbanyak di usia 20 – 30 tahun yaitu 23 orang (30%). Didapatkan 16 orang (20%) mengalami keluhan gangguan pendengaran, 18 orang (24%) sering menggunakan headset dan earphone,18 (23%) orang bekerja di tempat bising.  Hasil rata-rata pre test 35,06  (+13,72) dan rata-rata post test 77,9 (+20,81). Perbandingan hasil pre test dan  post  test  penyuluhan  kesehatan  pendengaran  didapatkan  hasil  yang  berbeda signifikan (p < 0,0001). Tuli akibat bising merupakan tuli yang bersifat menetap dan tidak dapat diobati dengan obat maupun pembedahan, pencegahan merupakan hal yang terpenting dari tuli akibat bising atau noise induced hearing loss.Kegiatan edukasi penyuluhan gangguan pendengaran akibat bising harus senantiasa dilakukan di masyarakat sebagai upaya pencegahan dan deteksi dini.Kata kunci: Tuli, gangguan pendengaran karena bising, pencegahan, edukasi

Abstract: The noise resulted from the advancement of transportation is an issue that cannot be avoided. The Noise Induced Hearing Loss is a decreases of auditory type sensorineural, which is initially not be aware, because there has not disturbed of daily conversation yet. Risk factors that affect the degree of severity of deafness are the intensity of the noise, frequency, long exposure per day, long working period, individual sensitivity, age and other factors that could influence. The aim of this research is to find out whether there is a relationship between noises by function of hearing at the officers of PT. Gapura Angkasa at Sam Ratulangi Airport, Manado. The Method of this research namely the analytic method is a design cross sectional study, and the samples were 20. The Measurement of noise levels is based on a unit of work the Operation which was obtained at the level of 75 dB and 85 dB Loading. From the results obtained from 20 staffs there are only 8 people who work exceed the Threshold Value (NAB) of noise which has been set. Conclusion: The result showed that there is hearing loss about 20 % in all the officers. The result Analysis Bivariat indicates that there is the absence of meaningful relations between noise-induced hearing disorder with the level of intensity noisy (p = 0.591). From the results of this research it can be concluded that the officers who work in on high intensity noise are more at risk of experiencing hearing loss compared to the officers who work on a low intensity noise level. In addition, the factors such as long hours of work, long exposure noise, the use of Ear Protectors Tools (EPT) was also very influential of the onset hearing loss in workers. Gapura Angkasa.    Abstrak: Kebisingan merupakan suatu masalah yang tidak dapat dihindari akibat kemajuan sarana transportasi. Gangguan pendengaran akibat bising adalah penurunan pendengaran tipe sensorineural, yang pada awalnya tidak disadari, karena belum mengganggu percakapan sehari-hari. Faktor risiko yang berpengaruh pada derajat parahnya ketulian ialah intensitas bising, frekwensi, lama pajanan perhari, lama masa kerja, kepekaan individu, umur dan faktor lain yang dapat berpengaruh. Tujuan penelitian ini  untuk mengetahui  apakah terdapat hubungan antara kebisingan dengan fungsi pendengaran pada petugas PT.Gapura Angkasa di Bandara Sam Ratulangi. Metode penelitian yang digunakan yaitu metode analitik dengan menggunakan rancangan studi cross sectional. Sampel berjumlah 20 orang. Pengukuran tingkat bising di lakukan berdasarkan unit kerja yaitu pada bagian Operation didapatkan 75 dB dan bagian Loading 85 dB. Dari hasil yang didapatkan dari 20 orang petugas hanya 8 orang saja yang bekerja melebihi NAB kebisingan yang telah ditetapkan. Simpulan : Hasil penelitian menunjukkan bahwa terdapat gangguan pendengaran sebesar 20% pada seluruh petugas. Hasil analisis bivariat menunjukkan tidak adanya hubungan yang bermakna antara gangguan pendengaran dengan tingkat intesitas bising (p=0.591). Dari hasil penelitian ini dapat disimpulkan bahwa petugas yang bekerja pada intensitas bising yang tinggi lebih beresiko mengalami gangguan pendengaran dibandingkan dengan petugas yang bekerja pada tingkat intesitas bising yang rendah. Selain itu faktor-faktor seperti lama bekerja, lama pemaparan bising, penggunaan Alat Pelindung Telinga (APT) juga sangat berpengaruh terhadap terjadinya gangguan pendengaran pada petugas.
