Introduction: Several factors influence women’s decision to take First Trimester Screening (FTS) tests. These factors are associated with the ambivalence of women toward undergoing screening tests. Objective: This study aimed to investigate the effect of Shared Decision-Making (SDM) about undergoing FTS on Decisional Conflict (DC) immediately after consultation and uptake of FTS. Materials and Methods: This quasi-experimental study was conducted on 200 pregnant women (100 women in the intervention and 100 in the control groups) referred to health centers for prenatal care in 2019. They were selected by the block randomization sampling method. The control group received the routine care and the intervention group, in addition to routine care, attended a 90-min long consultation session based on SDM. The women were contacted via phone at 14 weeks of pregnancy to collect data on their undertaking prenatal screening tests. The demographic characteristics form and O’Conner’s decisional conflict scale were filled out immediately after the consultation session for the intervention group. The obtained data were analyzed by the Chi-square, Fisher exact-test, Mann-Whitney U, and linear regression tests. The P value less than 0.05 was considered statistically significant. Results: There was no significant difference between the two groups regarding women’s demographic characteristics, except for education level, job, and insurance coverage. The Mean±SD DC score was significantly lower in the intervention group (7.35±8.55) compared to the control group (27.32±13.81) (95%CI; 16.80-24.19, P=0.001). In addition, there was a significant difference between the two groups in terms of undergoing the offered FTS (P=0.04). The DC scores ≥25 were associated with a decreased chance of undergoing FTS (P=0.02). Women were less likely to undergo FTS when they were self-employed (OR=0.15, 95%CI; 0.03-0.71, P=0.01). Conclusion: The SDM consultation can help women experience significantly lower levels of DC. Furthermore, factors such as self-employment can prevent women from undergoing FTS despite lower levels of DC.

Abstract Background To determine the effectiveness of integrating spirituality into prenatal care on Improving Sleep Quality and Insomnia Severity among Pregnant Women. Methods This randomized controlled trial was carried out on 40 pregnant women recruited at five health centers of zanjan, Iran, 2020. The eligible women were allocated into two intervention and control groups according to the randomized design. Group counselling with spiritual content was carried out in eight sessions, two times a week at 16 to 20 weeks of gestation. The control group only received routine care. Data were collected using the Pittsburgh Sleep Quality Index (PSQI) and the Insomnia Severity Index (ISI) questionnaires in three stages, before the intervention, at 28 and 36 weeks of gestation. Statistical analysis was performed using the SPSS 16.0 software (SPSS Inc., Chicago, IL, USA). Results The results showed that comparing the mean score of insomnia severity, sleep quality and its components in the pre-intervention phase was not statistically significant between the two groups(p>0.05). Comparison of the insomnia severity, sleep quality, and its components scores decreased statistically in the second and third trimesters compared to the first trimester in the intervention group than the control group(p<0.05). The effect of the intervention (Eta score) was 64%. Conclusion The results showed that counselling with spiritual content could effectively ameliorate sleep quality and reduce insomnia severity in pregnant women. It seems that the approach is an acceptable basis to design intervention programs in this field that can be considered by midwives.

Abstract Introduction Tonsillectomy is one of the most common pediatric surgeries in the world and pain control following tonsillectomy is very important. Objective The aim of this study is to investigate the effect of acupressure on the amount of pain following tonsillectomy in children. Method One hundred and forty-four children aged 5–12 years old were assigned into one of three groups: interventions, control and placebo. In the intervention group, acupressure was applied at three acupoints and in the placebo group, sham acupressure was applied. In the control group routine care only was applied. Results There was a significant difference between an average of changes in pain score before and after the intervention during the 3 time periods after the tonsillectomy operation in the acupressure group (p = 0.002). Conclusion The results showed that acupressure had a positive effect on pain reduction after tonsillectomy in children in the intervention group.

Background: More than one million children throughout the world are born by using fertility techniques. In this process, human intervention and laboratory conditions may have an impact on the growth and development of the fetus. Objectives: The present study aimed to consider the effect of assisted reproductive techniques on the results of embryonic health screening tests. Methods: In this case-control study, among clients who went to the Infertility Center of the Ome Leila Hospital in Bandar Abbas Province, we selected 200 pregnant women who were impregnated via assisted reproductive techniques as the case group and 200 natural pregnant women as the control group by sample random sampling. Checklists were filled out and data analyzed by the SPSS version 21 and chi-square tests by using an interview with pregnant women and investigating the embryonic screening tests. Results: Most of the participants in the study were housewives and had primary infertility. Their BMI was normal. There was no significant difference between the final results of the embryonic screening test in the two groups at the first trimester (P > 0.05). However, in the second trimester, there was a significant difference between them. The positive results were higher in the case group in comparison with the control group (P = 0.001). The mean of all types of screening tests in the first trimester in both groups was not different, significantly (P < 0.05), although in the second trimester, the mean of all the testis, including BHCG (P = 0.006) and AFP (P = 0.018) in both case and control groups, was significantly different. The mean of BHCG and AFP was higher in the case group, while the mean of Estradiol and Inhibin, NB, and NT were not really different (P < 0.05). Conclusions: Our study showed that embryonic screening tests, particularly the BHCG and AFP tests, would be affected by applying the assisted reproductive techniques. For the couples who had a history of infertility and used reproductive methods, screening tests are essential in the first and second trimester.

Abstract Background Mental health experts believe that stigma is the most challenging issue for caregivers of clients with psychiatric disorders. Therefore, monitoring and assessing the affiliate stigma in the caregivers is necessary. This study is aimed to investigate the effect of psycho-education program on the affiliate stigma in the caregivers of clients with bipolar disorder. Materials and Methods This quasi-experimental study with both intervention and control groups was conducted Ghods Hospital in Sanandaj (n = 64). Psycho-education program was performed for the intervention group for 4 weeks (one session per week for 60 to 90 minutes). Data was collected using the Affiliate Stigma Scale before and after intervention (with 4 weeks gap). Results The results of study showed that there was no difference between intervention and control groups in terms of the distribution of contextual variables. The mean and the standard deviation of the affiliate stigma before the conducting the psycho-education program in the control group was (73 ± 14.72), and in the intervention group was (69.63 ± 14.66). After the intervention and administrating the post-test, the results in the control group was (74 ± 14.21) and in the intervention group was (35.06 ± 8.31) which showed a statistically significant difference (P < 0.001). Conclusions According to the obtained results in this study and based on the other studies’ result, the routine care is not able to meet the educational needs of the caregivers of clients with bipolar disorder. Psycho-education is effective on the reduction of the affiliate stigma of caregivers of clients with bipolar disorder and it can be considered as one of the stigmatization strategies.

Objectives: Preterm birth and admission to the neonatal intensive care unit (NICU) would disrupt mother-infant attachment. Neurodevelopmental care training and support of family programs are essential for the family of such infants. The purpose of this study was to investigate the effect of neurodevelopmental care training program for mothers with preterm infants on mother-infant attachment at one month’s age. Materials and Methods: Study population included all the mothers of preterm infants born in the hospitals of Tehran in 2018. The research was designed as a multicenter cluster clinical trial and four hospitals were randomly selected and divided into intervention and control groups. Before the intervention, the mother-infant attachment was measured by the Maternal-Postnatal Attachment Scale (MPAS). The mothers in the intervention group received a 12-session preterm infant neurodevelopmental care training program while the control group only received the routine care in the unit. Finally, the mother-infant attachment was re-measured in both groups at the one month corrected age. Results: No significant difference was observed in the attachment score before and after the intervention in the control group but the mother-infant attachment score was significantly different in the intervention group (57.75±11.09 vs. 78.27±4.54). Conclusions: The neurodevelopmental care training program was effective and increased the mother-infant attachment rate.

Abstract Objective This study determined the effect of skin-to-skin contact (SSC) between mother and infant during the third stage of labor on postpartum levels of oxytocin and pain and placental separation time. Methods This randomized controlled trial was composed of 64 parturients: 32 in the intervention group and 32 in the control group. SSC interventions between infants and their mothers occurred for 30 min after birth, whereas infants in the control group were provided routine care. Data were gathered using an introductory information form, Visual Analog Scale, determination of placental separation degree form, and time and records of oxytocin analyses. Results There was no significant difference regarding placental separation time and 1st and 30th min oxytocin levels of mothers (p>0.05), whereas postpartum 5th, 15th and 30th min pain levels of women in the intervention group were significantly lower than women in the control group (p<0.05). Conclusion SSC is a valuable practice for both mother and infant, which especially reduces a mother’s pain. The study should be reiterated by excluding other variables that are likely to affect oxytocin levels.

Objective: To study the effect of shared decision-making (SDM) on the anxiety of women who were recommended for prenatal screening tests. Materials and methods: This quasi-experimental study was conducted on a total of 200 pregnant women who referred to the health centers of Zahedan, Iran, for prenatal care within April 7 to September 7, 2019. The control group received routine care, and the intervention group attended a session based on SDM. The demographic characteristics form and Spielberger Six-item State-Trait Anxiety Inventory were filled out before and immediately after the counseling, as well as before receiving the results of maternal serum biochemical markers. Results: No statistically significant effect of SDM on anxiety was reported between the control and intervention groups immediately after the counseling session (P=0.46). However, the obtained data showed that the mean value of anxiety scores (16.52±3.06) was higher among the women in the intervention group than that reported for the control group (13.80±3.55) on the day before receiving the results of the blood tests (P<0.001). Nevertheless, logistic regression analysis showed only women with a university level of education were likely to have higher anxiety scores than women with lower educational levels (AOR=10.60; 95% CI: 2.07-54.24; P=0.005). Conclusion: Offering prenatal screening can cause a slight increase in the level of anxiety among women with a university level of education. Therefore, it is required to implement supportive strategies to help high-risk pregnant women in coping with anxiety.

Background: Considering recent economic challenges, the short-term hospitalization of cancer patients for routine chemotherapy, and the lack of routine home visits for cancer patients in Iran, the quality of life of these patients after discharge can be a source of concern. Objectives: This study aimed to determine the effect of a planned discharge program on the quality of life of gastrointestinal cancer patients undergoing chemotherapy in hospitals of Zahedan University of Medical Sciences. Methods: This clinical trial was conducted in 2018 on gastrointestinal cancer patients undergoing chemotherapy in hospitals affiliated to the Zahedan University of Medical Sciences. The sample was selected by convenience sampling, and the participants were randomly assigned into two groups of intervention (n = 45) and control (n = 45). The intervention was performed in three 45 to 60-minute sessions and sending text messages, which were based on the patients’ identified problems and needs for the intervention group. Data collection tools included the QLQ-C30 questionnaire and a demographic characteristics form. Data were analyzed by SPSS V. 21 software using descriptive and analytical statistical tests. Results: The mean scores of the patients’ quality of life increased on most of the functional subscales and global health/quality of life in the intervention group. In the control group (routine care), however, no significant difference was observed in most functional subscales of quality of life. There was a significant change in the mean severity of symptoms in the intervention group so that the severity of symptoms decreased after the intervention. In the control group, there was a significant difference in the mean severity of symptoms before and after the intervention so that the severity of symptoms increased in most cases. Conclusions: Home visits of cancer patients are not in routine care after discharge in Iran, and they are not covered by insurance companies. Thus, home visits are expensive. However, in areas where the economic index is low, and patients have little access to smartphones, the planned discharge program can be a suitable method to increase the quality of life of cancer patients.

Pain in labor is physiological because the pain comes from the cervical dilatation and uterine contractions that needed for labor (Potter & Perry, 2005). Research conducted by Brown, Douglas and Flood (2001) using 10 nonpharmacological methods performed on 46 people found that acupressure technique is the most effective technique to reduce pain during labor through the increased of endorphins and shorten the length of labor. The purpose of this study is to know the influence of acupressure method on endorphin levels and the progress of labor in the first period. This type of research is quantitative research by using quasi experiment design with pretest approach and posttest design with control group by performing acupressure on primipara maternal mother during period I active phase. The study was conducted at Bidan Praktik Mandiri. The population of this study is primipara mother inpartu period I active phase using Non Probability sampling technique with Consecutive Sampling. The results showed a significant difference in endorphin levels before and after receiving acupressure in the intervention group (p = 0.048) and there was a significant difference in the progress of labor before and after receiving acupressure in the intervention group (p = 0.001). Acupressure can be used as a routine care in every care of normal childbirth thus mother's care and baby care can be created well.

ABSTRACT Objectives: to evaluate the effectiveness of educational intervention in the knowledge and attitude of candidates for gastroplasty. Methods: randomized controlled clinical trial with 56 preoperative patients (intervention=28; control=28). The control group received routine care from the health institution and the intervention group participated in a book-mediated educational intervention. Forms were used for clinical-epidemiological characterization and evaluation of knowledge/attitude about bariatric surgery, which were reapplied seven weeks after intervention. RBR-297fzx. Results: in the pre-test there was no significant difference between the groups (p=0.254). In the post-test, the group that received verbal guidance had a mean score of 19.5 (± 6.17) and the group reading the booklet averaged 31.1 (± 2.96), p=0.000. The attitude is adequate, especially for care such as vitamin supplement intake, water intake and physical activity practice. Conclusions: the educational intervention mediated by booklet was effective in improving knowledge and attitude when compared to verbal guidance. Thus, it can be replicated during the preoperative preparation.
