Background: Gilles de la Tourette syndrome is a neuro-psychiatric condition associated with rather bizarre manifestations and generally affects children with normal intelligence. The condition has the potential to make an intelligent child a victim of the disorder, and unfavorably affects his/her school performance and lead to social rejection and isolation. Awareness by the community, educators, and also medical practitioners is useful and help in avoiding the loss of such children who have the potential to be a talented person. The aim of this paper is to present a case and to provide a brief account on the early documentation of syndrome in the literature. Patients and Methods: The case of a ten-year old boy with Gilles de la Tourette syndrome is described and the relevant literatures were reviewed to outline the early documentation of the disorder in the literature. Results: A.Y was first seen at the pediatric psychiatric clinic of the Children Teaching Hospital of Baghdad Medical City at the age of ten years because they were saying at school that the boy is crazy because of his abnormal behaviors. The boy’s abnormal behaviors mentioned by his mother included shrugging his shoulders, shrugging his hips with unusual movements of the legs as if he was dancing, unusual facial grimace and lip licking, rubbing his chin, licking the back of his hand, producing abnormal sounds from his throat, and echolalia. Review of the earliest relevant literature revealed that the condition was first described in a book entitled “Malleus Maleficarum” which means “Witch’s hammer”. Jean Marc Gaspard Itard, a French doctor described the first case of Tourette syndrome in 1825. Conclusion: Awareness of this condition by the community, educators, and also medical practitioners is useful and help in avoiding the loss of such children who have the potential to be a talented person.

Transorbital penetration accounts for one-quarter of the penetrating head injuries (PHIs) in adults and half of those in children. Injuries that traverse (with complete penetration of) the brainstem are often fatal, with survivors rarely seen in clinical practice. Here, the authors describe the case of a 16-year-old male who suffered and recovered from an accidental transorbital PHI traversing the brainstem—the first case of complete neurological recovery following such injury. Neuroimaging captured the trajectory of the initial injury. A delayed-onset carotid cavernous fistula and the subsequent development of internal carotid artery pseudoaneurysms were managed by endovascular embolization.The authors also review the relevant literature. Sixteen cases of imaging-confirmed PHI traversing the brainstem have been reported, 14 involving the pons and 12 penetrating via the transorbital route. Management and outcome of PHI are informed by object velocity, material, entry point, trajectory, relationship to neurovascular structures, and the presence of a retained foreign body. Trauma resuscitation is followed by a careful neurological examination and appropriate neuroimaging. Ophthalmological examination is performed if transorbital penetration is suspected, as injuries may be occult; the potential for neurovascular complications highlights the value of angiography. The featured case shows that complete recovery is possible following injury that traverses the brainstem.

Abstract:: A novel virus, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), appeared and expanded globally by the end of year in 2019 from Wuhan, China, causing severe acute respiratory syndrome. During its initial stage, the disease was called the novel coronavirus (2019-nCoV). It was named COVID-19 by the World Health Organization (WHO) on 11 February 2020. The WHO declared worldwide the SARS-CoV-2 virus a pandemic on March 2020. On 30 January 2020 the first case of Corona Virus Disease 2019 (COVID-19) was reported in India. Now in current situation the virus is floating in almost every part of the province and rest of the globe. -: On the basis of novel published evidences, we efficiently summarized the reported work with reference to COVID-19 epidemiology, pathogen, clinical symptoms, treatment and prevention. Using several worldwide electronic scientific databases such as Pubmed, Medline, Embase, Science direct, Scopus, etc were utilized for extensive investigation of relevant literature. -: This review is written in the hope of encouraging the people successfully with the key learning points from the underway efforts to perceive and manage SARS-CoV-2, suggesting sailent points for expanding future research.

Intraductal carcinomas are rare, malignant tumors that arise from the salivary glands. They commonly grow from the parotid gland and no cases growing from the parapharyngeal space have been reported to date. We report a 76-year-old man who was inadvertently found to have a parapharyngeal lesion by CT scans and MR imaging. The tumor was resected through an upper neck approach and diagnosed histopathologically as intraductal carcinoma. As far as we are aware, this is the first case of intraductal carcinoma arising from the parapharyngeal space. Here, we describe the management of this disease together with a review of the relevant literature.

Whereas osteochondroma is a common benign bone tumor in adolescence, it is rarely observed in elderly patients. It is unknown why osteochondromas, which usually develop during skeletal growth, rarely develop in elderly patients. The authors report 3 cases of symptomatic spinal osteochondroma in elderly patients and discuss the possible reasons for the onset of the enlargement of osteochondromas in elderly patients. Clinical history, radiographs, MR images, and CT myelography studies were obtained in each patient and are described. A review of the relevant literature is also presented. In the first case, the cervical osteochondroma caused spinal canal compression and occipital nerve irritation. It was totally excised, which successfully relieved the pain and allowed the patient to return to normal neurological function. In the second case, total removal of the tumor was effective in alleviating clinical symptoms. In the last case, ablation of the articular facet joint partially relieved the patient's lower-back pain. In the first 2 cases, the patients suffered from psoriasis and associated psoriatic arthritis and in the last case, the patient suffered from HIV-associated psoriatic arthritis. The psoriatic arthritis was characterized as asymmetric chronic multiple-joint arthritis and was HLA B27 positive. The pathology of psoriatic arthritis was the accelerating bone turnover and ankylosis. Symptomatic osteochondroma of the spine in elderly patients is extremely rare since it typically develops during skeletal growth. In this report, the authors show that pathological accelerating bone turnover such as psoriatic arthritis may be a possible mechanism for the onset of the enlargement of osteochondromas in elderly patients. The age of the patients in this report suggests that growth of the osteochondroma continues after skeletal development.

Condylomata acuminata (genital warts) have been identified in transgender women after gender affirmation surgery. We describe a case of a 57-year-old transgender woman with biopsy-proven neovaginal condylomata acuminata following gender-affirmation vaginoplasty using the penile skin inversion technique. The patient was successfully treated with trichloroacetic acid. To our knowledge, this is the first case report demonstrating successful treatment of neovaginal condyloma with trichloroacetic acid. We provide a review of the relevant literature and highlight the evidence for the clinical utility of trichloroacetic acid in transgender women with condyloma of the neovagina.

✓ An intramedullary abscess of the spinal cord (IASC) represents a rare disease associated with a potentially devastating outcome. Few cases involving children suffering from an IASC have been reported in the neurosurgical literature. In the majority of the reported pediatric cases there were either congenital abnormalities, such as a dermal sinus, or signs of local infections leading to a secondary hemopoietic spread. The authors report the case of an 18-month-old girl with an extensive IASC associated with an epidermoid cyst extending from T-11 to S-2 without evidence of a dermal sinus or history of clinically apparent systemic infection. To their knowledge, this is the first case report of an IASC without a condition facilitating either direct contamination via a dermal sinus or hemopoietic spread from an infectious focus outside the central nervous system. Signs and symptoms, the clinical course, and imaging features are discussed and the relevant literature is reviewed.

Background: Allergy is a common ailment affecting more than 20% of the population worldwide. Allergic patients suffer from a debilitating disease with a major impact on their quality of life, work and school performance. Allergic disorders can be debilitating, with a major impact on their quality of life, work and school performance. AIT (Allergen immunotherapy), modify the natural history of allergic disease and prevent the progression of allergic respiratory diseases by maintaining a long-lasting effect. Since the awareness among medical practitioners plays very important role in advising AIT for allergy patients, we attempted to conduct this study.Methods: This was a descriptive study done for a period of three months from December 2016 to February 2017. The 110 study subjects were recruited through purposive sampling. Doctors of various specialties and general practitioners were interviewed using self-administered, semi-structured questionnaire.Results: There were 73 (66.36%) males and 37 (33.64%) females. Mean age of the medical practitioners was 35.73±13.07 (Mean±SD). 98 (89.09%) of the practitioners were aware of skin prick test for the diagnosis of allergy, 70 (63.3%) subjects had heard term AIT, 59 (84.28%) were aware about subcutaneous immunotherapy (SCIT) and 25 (35.71%) of sublingual immunotherapy (SLIT). Forty-four (62.96%) were not advising immunotherapy for their patients in spite of having awareness on immunotherapy. The common reasons for not advising immunotherapy were long duration of treatment, high cost, less effective, not available in India, fear of side effects and others.Conclusions: Though the awareness among medical practitioners on AIT was good, majority are not advising because of long duration of treatment, high cost, less effective, not available in India, fear of side effects and others.

Purpose – The purpose of this paper is to demonstrate how a psychiatric clinic in a general hospital can function in conflict-ridden Iraq through the easing of patient access to services. Design/methodology/approach – The workload of psychiatrists was analyzed for one year (2010) at the psychiatry consultation clinic located in the campus of the Medical City Hospital in Central Baghdad which is also a training center. Findings – A total of 2,997 consultations (both adults and children) occurred in 2010. In total, 96 percent were self or family referrals. Patient services were provided by five consulting psychiatrists for a variety of psychiatric disorders. The main therapeutic intervention was the prescription of psychotropics. Research limitations/implications – Despite the turbulent circumstances and limited mental health resources in Iraq, this clinic was established as a model to attract patients for consultation and triage management to reduce appointment defaults and delayed care. Practical implications – The data can contribute to the planning and development of mental health services in Iraq, contributing to the current body of literature and serving as a model for other conflict areas. Originality/value – To best of the understanding this study is the first in the country.

Small bowel hemorrhage is responsible for approximately 4% of all cases of gastrointestinal bleeding. The etiology of bleeding from the small bowel is a tumour in approximately 10% of cases. Pyogenic granuloma is a common inflammatory vascular tumour of the dermis, which rarely occurs in the gastrointestinal tract. Pyogenic granuloma is a rare cause of overt or obscure small bowel bleeding. The present paper reports the first case of pyogenic granuloma presenting as a massive gastrointestinal bleed, and reviews the relevant literature to date regarding the clinical presentation, diagnosis and management of this rare gastrointestinal lesion.

A child with a complicated medical history that included asplenia acquired an infection withBabesia microtiin the summer of 2013 and had not travelled outside of Manitoba. Although the clinical findings were subtle, astute laboratory work helped to reach a preliminary identification ofBabesiaspecies, while reference laboratory testing confirmed the diagnosis. Blacklegged ticks (Ixodes scapularis) are known to transmitBorrelia burgdorferiandAnaplasma phagocytophilumin the province; however, the present case represents the first known instance of tick-borneB microti, both in Manitoba and in Canada. The expanding territory of the blacklegged tick increases the relevance of this emerging infection. Clinicians, laboratory medical practitioners and public health officials should be aware ofB microtias a potential locally acquired infection in Canada.
