Traditional Chinese and Iranian medicine are ancient medical and health care systems that still forms an important part of diagnosis and treatment systems in Iran, China and other parts of the world. The integration of these two systems and modern biomedicine is also an important issue. Iranian and Chinese traditional medicines recommended for health promotion and well-being. Chinese herbal drugs have been described by medieval Muslim medical scholars such as Tabari, Rhazes, Haly Abbas, Avicenna and Jurjani (1137 CE). Traditional Chinese and Iranian medicine play an important role in sustainable agriculture and food systems, it also offers a holistic approach to prevent diseases while making appropriate use of organic and herbal products especially growth by small-scale family farmers. Both Iranian and Chinese traditional medicines may help to balance temperature, and both can be health prevention and preservation. Traditional healing practices consist of Islamic religious text based practices, Islamic worship practices and folk healing practices, Islamic religious text based practices are included spiritual healing, medicinal herbs, applied therapy and dietary prescriptions. At present, herbal medications or related products in the global market are derived from Chinese herbs, Indian herbs, Iranian herbs, Arabic herbs, and Western herbs. Herbal remedies may also be classified into three categories, namely, modern herbs, theoretical herbs, and empirical herbs, in accordance with their nature/characteristics and the nature of current usage. Herbal remedies containing aristolochic acid (AA) have been designated to be a strong carcinogen. This review summarizes major epidemiologic evidence to argue for the causal association between AA exposure and urothelial carcinoma as well as nephropathy. The exposure scenarios include the following: Belgian women taking slimming pills containing single material Guang Fang Ji, consumptions of mixtures of Chinese herbal products in the general population and patients with chronic renal failure in Taiwan, occupational exposure in Chinese herbalists, and food contamination in farming villages in valleys of the Danube River. Such an association is corroborated by detecting specific DNA adducts in the tumor tissue removed from affected patients. Preventive actions of banning such use and education to the healthcare professionals and public are necessary for the safety of herbal remedies.

Herbal medicines have been used by mankind from time immemorial. Moreover, many modern medicines are originated from plant sources. In earlier days, patients were dependent on herbs for treatment and well-being. However, due to the advent of the industrial revolution and modern science, the scenario of treating diseases has changed over a period of time. Majority of patients started preferring allopathy medicines due to their several advantages over herbal medicines. However, due to long term treatment by allopathic medicines for chronic diseases led to side effects, patients are now drifting back to the traditional medicines. Herbal medicines have their own drawbacks, viz., lack of safety and efficacy data, standardization difficulties, not well established legislative controls and a few issues with adverse drug reactions. Drug regulations per se were always the prime focus and they are said to be dynamic. There are a few differences in regulations of herbal drugs among various countries. Regulatory authorities of countries are working to evolve the regulations to govern herbal medicines more effectively. A brief overview of the regulations related to a few developing and developed countries have been dealt here.

The use of the medicinal plant Ginkgo biloba has increased worldwide. However, G. biloba is capable of assimilating both essential and toxic metals, and the ingestion of contaminated products can cause damage to health. The aim of this study was to investigate the safety of manganese (Mn), copper (Cu), lead (Pb), arsenic (As), and cadmium (Cd) in 26 items containing Ginkgo biloba (pharmaceutical herbal products, dietary supplements, and traditional herbal remedies) purchased in the metropolitan area of Mexico City. Metal analysis was performed using a graphite furnace atomic absorption spectrometer. All of the products were contaminated with Pb, 54% of them with As, and 81% with Cd. The lowest values of Pb, As, and Cd were detected in pharmaceutical herbal products > dietary supplements > traditional herbal remedies. The daily intake dose (DID) of pharmaceutical herbal products was within the established limits for the five metals. Dietary supplements and traditional herbal remedies exceeded the DID limits for Pb. The hazard quotients estimation and non-carcinogenic cumulative hazard estimation index for Mn, As, and Cd indicated no human health risk. Our results suggest that products containing G. biloba for sale in Mexico are not a health risk.

Sleep is an essential component of physical and emotional well-being, and lack, or disruption, of sleep due to insomnia is a highly prevalent problem. The interest in complementary and alternative medicines for treating or preventing insomnia has increased recently. Centuries-old herbal treatments, popular for their safety and effectiveness, include valerian, passionflower, lemon balm, lavender, and Californian poppy. These herbal medicines have been shown to reduce sleep latency and increase subjective and objective measures of sleep quality. Research into their molecular components revealed that their sedative and sleep-promoting properties rely on interactions with various neurotransmitter systems in the brain. Gamma-aminobutyric acid (GABA) is an inhibitory neurotransmitter that plays a major role in controlling different vigilance states. GABA receptors are the targets of many pharmacological treatments for insomnia, such as benzodiazepines. Here, we perform a systematic analysis of studies assessing the mechanisms of action of various herbal medicines on different subtypes of GABA receptors in the context of sleep control. Currently available evidence suggests that herbal extracts may exert some of their hypnotic and anxiolytic activity through interacting with GABA receptors and modulating GABAergic signaling in the brain, but their mechanism of action in the treatment of insomnia is not completely understood.

Abstract Artisanal and small-scale mining (ASM) has long been a vital source of livelihoods for rural populations in the global South. Yet, it has also been linked to a host of social, political and environmental adversities, including violent conflict. As environmental peacebuilding increasingly stresses the importance of livelihood improvement as a means of fostering peace in conflict-affected extractive societies, ASM formalization has been identified as a solution to mitigate the sector's challenges, thereby addressing underlying causes of conflict. This article critically investigates the contribution of ASM formalization to sustainable peace by focusing on its impact on the livelihood dimension of peacebuilding. It analyses the livelihood impact of three formalization interventions in the diamond sectors of two countries: cooperatives in Liberia, and, in Sierra Leone, ethical sourcing schemes and a community-based natural resource management initiative. In line with calls for a paradigm shift from a narrow legalization-centred understanding of formalization to a broader approach that accounts for livelihood quality, the analysis presented here focuses on interventions that were informed by the ideal of improving the well-being of ASM workers and communities. We propose three pathways through which ASM formalization could potentially contribute to livelihood enhancement: income security, working conditions and community benefits. Based on fieldwork, this article highlights the challenges of generating livelihood improvements through formalization. Even when specifically designed to address the needs of ASM communities, during implementation, they risk prioritizing a narrow conceptualization of formalization and thus failing to become a conductor of transformative change.

Abstract Aquaponics technology has recently been offered as a good option for sustainable food systems among small-scale farmers, particularly those seeking an organic production or dealing with land quality constraints, such as urban farmers. However, there is a lack of empirical evidence for the capacity of small farmers to adopt the technology. The unique requirements of aquaponics may create technical, economic and even cultural constraints and opportunities. This paper uses empirical evidence gathered with small-scale farmers in São Carlos, State of São Paulo, Brazil, to present the capacity of adoption for the technology, including possible limiting factors and incentives for farmers. The study conducted interviews with owners of ‘agriculturas familiares’ (Portuguese for small family owned farms) within 30 km of São Carlos. The interviews revealed that there is widespread interest in the potential profitability of aquaponics systems, significant interest in environmentally friendly practices, familiarity with organic production and hydroponics and a large base of agricultural knowledge in the community that can drive adoption. However, lack of initial financing, limited human power and concerns about product placement were significant barriers to adoption. For settlement farmers (those working on land formerly abandoned) poor soil quality and water scarcity are key issues that could be alleviated by the technology. The city of Sao Carlos present program for purchasing specific types of products from these farms could be used as a model for increasing aquaponics adoption and relieving success concerns.

More parents are considering the use of herbal remedies to maintain their children’s good health and to treat their illnesses. They look to pediatricians and other primary care clinicians for advice concerning the safety and efficacy of herbal products for children. This article reviews principles for the clinician to keep in mind while investigating the literature on herbal medicine and addressing the use of herbal medicines with parents.

In this descriptive case series, we detail the theoretical basis, methodology, and impact of a small-scale pilot implementation of graphic medicine workshops as an innovative approach to well-being and resilience in the age of COVID-19 and increasing awareness of racial injustice. The data provided in this article are anecdotal and based on participation in the workshops. Images created during the workshops are also shared as examples of the types of reflection that graphic medicine can enable. The workshops themselves were designed collaboratively and are based on the theoretical principles of graphic medicine, narrative medicine, and racial and social justice. They were conducted as part of a larger wellness initiative and were offered to health care-focused faculty at our academic medical institution. Our findings suggest that this was a beneficial activity which helped participants to reflect and reconsider their experiences with the COVID-19 pandemic and surging awareness of racial injustice. Reflections also showed that drawings were correlated with ProQOL scores and may, in larger numbers, also help to mitigate or bring attention to issues of burnout in frontline providers. Drawings shared show the tremendous impact of COVID-19 and the simultaneous chaos and emptiness of practicing during dual pandemics. Our workshops engaged about 20 frontline health care providers and other health care faculty and highlight the utility of graphic medicine as a tool for building resilience and encouraging self-reflection. Further study is necessary, as is more rigorous analysis of the relationship between the graphics created and the ability to recognize and mitigate burnout.
