Incidence of Infective Endocarditis among Haemodialysis Patients: Experience at a Single Centre in Bangladesh

Background: Infection in hemodialysis is a very dangerous condition for the patients. Objective: The purpose of the present study was to find out the incidence of infective endocarditis among hemodialysis patients. Methodology: This was a cross sectional study which was conducted in the Department of Nephrology at Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh during the periods of July 2010 and June 2012. Hemodialysis patients who were attending in the Department of Nephrology at BSMMU, Dhaka were taken as study population. Results: A total number of 50 patients presented with hemodialysis. Out of 50 patients 34(68.0%) cases were male and 16(32.0%) cases were female. The mean with SD of age of the patients was 38.44±13.0 years with a range of 18 to 68 years. Out of 50 patients 6(12.0%) cases had positive blood culture of which 3 cases had shown Staphylococcus aureus, 1 case had Escherichia coli, 1 case had Enterococci species and 1 case had Salmonella typhi, Among the 50 patients 5 (10.0%) had vegetation on cardiac valve. The patients frequency of dialysis 2 times per week showed 01 (2.43%) positive blood C/S and 5 positive in 3 times per week showed 05 (55.55%) positive blood C/S (p>0.001). Conclusion: In conclusion infective endocarditis causes infection with high mortality among the patients of CKD undergoing hemodialysis. Background: Different pathology of breast is found among the women. Objective: The purpose of the present study was to see the profiles of breast disease among women seeking for treatment during Covid-19 pandemic at the OPD of medical university of Bangladesh. Methodology: This descriptive cross-sectional study was conducted in the Department of Surgery at Bangabandhu Shiekh Mujib Medical University, Dhaka, Bangladesh from 1 April to 31 October 2020 for a period of six months. All the women who were attended at the OPD of the Department of Surgery with the different complaints of breast at any age were selected as study population. The different disease profiles were recorded in the data sheet. Result: A total number of 1625 women were recruited for this study. The mean age with standard deviation of the study population was 36.4±12.54 years. Among 1492 women the most common breast complaints was mastalgia which was 492(29.9%) cases followed by fibrocystic disease, breast lump, fibroadenoma and carcinoma breast which was 296(18.0%) cases, 202(12.3%) cases, 141(8.6%) cases and 84(5.1%) cases respectively. Similarly duct ectasia, fibroadenoma, fibrocystic disease and lipoma were also most common among the less than or equal to 45 years age group of women which were 47(94.0%) cases, 137(97.2%) cases, 269(90.9%) cases and 75(51.4%) cases respectively. Conclusion: In conclusion breast lump and fibrocystic disease are the most common diseases among the women attending during covid19 era. Background: Major gynaecological operations are frequently performed for the treatment of different surgical causes.Objectives: The purpose of the present study was to see the surgical outcomes of different major gynaecological operation performed at medical university of Bangladesh.Methodology: This descriptive type of cross-sectional study was conducted in the Department of Obstetrics and Gynaecology at Bangabandhu Sheikh Mujib Medical University, Dhaka from August 2002 to January 2003 for a period of six (6) months. All the women who were undergone major gynaecological operation at any were selected as study population. Minor gynaecological operation cases were excluded from this study. The data were collected by face to face interview. The age, weight, socio-economic condition, nutritional status and anaemia were recorded.Result: A total number of 140 patients were undergone major gynaecological surgeries of which majority were abdominal hysterectomy (57.1%), vaginal hysterectomy (24.3%), salpingo-oophorectomy or ovarian cystectomy (9.3%). The mean age with SD of the study population was 43.91± 9.45 years. Majority were presented with mild anaemia (51.4%). Majority were free of wound infection (92.8%).Conclusion: In conclusion abdominal hysterectomy is the most common major gynaecological operation performed with few wound infection rate.Journal of Current and Advance Medical Research, July 2016;3(2):47-50

Background: Senile cataract can be expressed with different clinical presentation. Objective: The purpose of the present study was to find out the clinical profiles of senile cataract patients. Methodology: This cross-sectional study was conducted at National Institute of Ophthalmology, Dhaka, Bangladesh from January 1999 to December 2000 for a period of two (02) years. Patients with senile cataract were selected for study. The details clinical profiles of the study population were recorded like types, grades of cataract and visual acuity. Data were collected on pre-designed data collection sheet, compiled and appropriate statistical analysis was done using computer based software. Result: A total number of 60 eyes of cataract patients were recruited for this study. The mean age with the standard deviation was 58.8±6.055 years. Majority of the patients were suffering from total cataract that is nuclear plus cortical plus posterior subcapsular variety which was 31(51.7%) eyes of cataract patients. Majority of the patients were grade III (amber) type of nuclear sclerosis which was 26(43.3%) eyes of cataract patients. Most of the patients were presented with PL which was 32(53.3%) eyes of cataract patients. Conclusion: In conclusion majority of the patients are suffering from total cataract with grade III (amber) type of nuclear sclerosis. This cross sectional study on oral condition among school going children in Sylhet city was conducted at Osmani Medical High School, Dorgah Gate, Sylhet, Bangladesh. This study was carried out on 8th October, 2019 among 208 students, mostly 12 years aged studying in Class VI. The main objective of the study was to assess the oral health condition of school going children through decayed, missing, and filled teeth (DMFT) status. Data was collected by personal interview and clinical examination of the respondents. Among 208 children, 96 (46.2%) were boys and 112 (53.8%) were girls. Among all, 63 (30.3%) respondents brush their teeth once daily while 137 (65.9 %) respondents brush twice a day, and 8 (3.8 %) of them brush their teeth more than twice daily. Fluoride containing toothpaste is used by 73 (35.1 %) students, 112 (53.8 %) students did not know whether their toothpaste contain fluoride or not, and 23 (11.1 %) students do not use fluoride. Among all respondents, 125(60.09 %) did not have any decayed tooth, while 83(39.90 %) had tooth decay, and 20(9.6 %) had their teeth filled. Normal gingival condition was found in 185 (88.94 %) subjects, but 23(11.06 %) of them had red or swollen gingiva. The mean DMFT of the study population was 1.19 which is considered as “low” according to “WHO quantification for the DMFT index”. Update Dent. Coll. j: 2021; 11 (1): 4-6

Aims: To measure the waist circumference of Sudanese adults in Khartoum Locality and its relationship to blood pressure and lifestyle  during celebration of international day of hypertension in May 2016 . Study Design: It was a descriptive cross-sectional study. Place of the Celebration: Khartoum Locality at Alsahaa Alkhadraa (The Green Park). Methodology: A total of 364 adult participants, 196 men and 168 women were interviewed using structured questionnaire. Blood pressure (BP) was measured considering hypertension as ≥ 140 mmHg and ≥ 90 mmHg for systole and diastole BP respectively. Waist circumference was measured using an anthropometric measuring tape at cut-off point of 94 cm and 80 cm for men and women respectively. Data was managed by SPSS version 20 and Chi-square test at 95% CL was used to test the association between waist circumference, blood pressure and life style characteristics. Results: Age distribution of the study population showed 48.2% females and 45.4% males in the middle age group (38-57 years). Two thirds of the study population were hypertensive, 62.8% of males and 64.3% of females. The mean waist circumference of men was 97.82 cm + 16.7, mean Systolic BP was 127 + 22 and mean Diastolic BP was 85 + 15. The mean waist circumference of women was 99.31 + 16.2, mean Systolic was 128 + 24 and mean Diastolic BP was 84 +17. Abnormal waist circumference was found in 61.2% of males and 86.9% of females. Fifty nine (30.1%) of the males and 86 (51.2%) of the females with abnormal waist circumference were hypertensive. The association between abnormal waist circumference and high blood pressure was significant among both sexes, P value = 0.001. Physical exercise and fat and salt foods were not significantly associated waist circumference in both men and women. Conclusion: Two thirds of women and men in the celebrating areas were hypertensive.  Half of women and one third of men were significantly hypertensive and having abnormal waist circumference. Doing physical exercise, avoiding fat and salt foods was insignificantly associated with normal waist circumference. Large survey with representative sample is needed to estimate the real Sudanese waist circumference.

Objective The focus of this study was to evaluate the visual status of mine workers who were directly involved in mining and/or are exposed to the mining environment. Methods A hospital based cross-sectional study was conducted. Four hundred and six (406) workers were conveniently sampled for the study. Information on their socio-demographic data was also collected. In addition, all participants underwent ophthalmic examination. Results The mean age of the workers was 41.1 years. Visual impairment was found in 114 (28.1%) of the study population of which refractive error (56.8%) was the major cause. Presbyopia was also observed in 45.5% of the workers with only 30.3% of them wearing near correction. The prevalence of eye diseases were pterygium/pingueculae (25.8%), conjunctivitis (26.8%), suspected glaucoma (15.3%), and cataract (7.4%). Conclusions Visual impairment and eye diseases were found among the miners.

SummaryConsanguineous marriage – marriage between relatives – has received a great deal of attention as a potential risk factor for many adverse health outcomes. The present cross-sectional study was done in order to illustrate the prevalence and types of consanguineous marriages among Iranian Georgians living in Frydoonshahr (Isfahan province, central Iran). Data on consanguineous marriages were collected using a simple questionnaire. The total number of couples in this study was 646. Consanguineous marriage was classified by the degree of relationship between couples. First cousin marriages (14.2%) were the most common type of consanguineous marriages, followed by second cousin (7.0%), beyond second cousin (1.5%) and first cousin once removed (0.6%). The mean inbreeding coefficient (α) was calculated as 0.0104 for the population. The present study shows that the study population, as other Iranian populations, has a high level of consanguinity.

Background: The clinical characteristics of hepatocellular carcinoma are varied from person to person.Objectives: The purpose of the present study was to see the clinical profiles of hepatocellular carcinoma patients.Methodology: This cross sectional study was carried out in the Department of Radiology and Imaging in Dhaka Medical College, Dhaka and Banghabandhu Sheikh Mujib Medical University, Dhaka from January 2007 to May 2008 for a period of around one and half year. All the patients presented with hepatocellular carcinoma at the age group of more than 20 years with both sexes were selected as study population. The patients were undergone CT-scan examination and the confirmation was performed by histopathological examination. All the clinical features of HCC patients were recorded.Result: A total number of 50 patients were recruited in this study after fulfilling the inclusion and exclusion criteria. Mean age of my study was 48.78 and SD ± 12.07. Hepatocellular carcinoma commonly presented with hepatomegaly (96%). Upper abdominal pain (60%), weight loss (60%), anorexia (44%), nausea/vomiting (44%), bruit (21%), jaundice (40%) and ascitis (32%) was also present.Conclusion: In conclusion the hepatocellular carcinoma is most commonly occurred in middle age group with a predominance of male.Journal of Science Foundation 2016;14(2):36-39

Objective: To obtain informations regarding the profile of contraceptive users in Raden Saleh Clinic, Jakarta. Method: A cross-sectional study was conducted in June 2012, at Raden Saleh Clinic, Jakarta. The study population was the patient who came to Raden Saleh Clinic from 2008 until 2011. Results: The mean age of the contraception users was 34.06 and the mean of their husband’s age was 38.91 years old. Most of the patients were graduated from senior high school (43.2%) and university (37.9%). More than half of the patients (55.2%) were not working and more than a half of their husband (53.2%) were private employee. Almost all (92.6%) of the patient were married. Most of the patient (38.8%) came with no prior contraception. After consultation, all patients had chosen their preferred contraceptive methods, namely IUD (61.4%), injectable contraception (20.9%), pill (13.7%), sterilization (3.4%), and implant (0.6%). Conclusion: Family planning reduces maternal mortality and the best method is different for each patient because of the difference in their own condition and the consideration of the cost and benefit. Background: Proton pump inhibitors (PPIs) are in routine widely prescribed to hemodialysis patients. Recent studies have reported the association of PPIs use with hypomagnesemia in patients with long term hemodialysis. This study aims to determine the frequency of hypomagnesemia in patients of hemodialysis taking proton pump inhibitors. Patients and methods: This cross-sectional study was conducted in the Department of Nephrology of Sandman Provincial Hospital Quetta from 01-6-2019 till 01-9-2019. A total of 120 patients (52 PPI users and 68 non-PPI users) who were on HD for more than 06 months were included. Data regarding age, gender, duration of hemodialysis and taking PPIs were collected. Determination of serum magnesium was made by taking 3 different samples at 2 weeks’ interval and the mean value of serum magnesium was calculated. Serum Mg2+ levels <2.0 mg/dL was taken as hypomagnesemia. A Chi-square test was applied to determine the association of PPI use with hypomagnesemia. Results: Demographic variables such as age and gender were not significantly different between the groups. There was female dominance in both groups (73% in PPI groups and 66.1% in the non-PPI group (p-value 0.65). The mean duration of dialysis was 45.3±13.8 months in PPI users versus 48.9±12.9 months in non-PPI users (p-value 0.14). There was a significantly higher frequency of hypomagnesemia in PPI users; 36 (69.3%) versus 27 (39.7%) in non-PPI users (p-value 0.001). Conclusion: The use of PPI is associated with a significant reduction in serum magnesium levels. So serum magnesium levels should be advised as routine monitoring in patients of hemodialysis taking PPIs.
