As an exponential increase in road corridors, there is also an increase in injuries, within them, to Medial Tibial Stress Syndrome (SETM), which is described as a lesion in the external layer of the tibia, causing the posteromedial region to have a size of approximately ≥ 5cm. The objective of this study is to identify the factors of the cliff, how it has been carried out or the diagnosis and physiotherapeutic treatment for the SETM. Methodology: it is about a literature review, using a qualitative research method, of a descriptive nature. A bibliographic survey was carried out in PubMed, Google Scholar and SciELO data banks. Result: Due to the exact cause of SETM is unknown, being more oily than it is a multifatorial condition, identified cliff factors: o increased BMI; navicular left above 10mm; increase in plantar flexion ADM; inappropriate treinamento plane; Previous injuries no 2/3 distal to the leg and imbalance of muscular forces. Its diagnosis was made through a clinical evaluation, containing a historical examination, followed by a physical examination. Despite the clinical evaluation being certified, image exams may be necessary in some specific cases. Conclusion: Despite the great incidence of SETM, it was not identified during the literature review of this study, a standard protocol considered effective, therefore its treatment is concentrated on the clinical presentation of the patient, being generalized interventions, lacking further scientific evidence of good quality. to verify the effectiveness of a specific physiotherapeutic intervention, not specific treatment of SETM.

: Infective Endocarditis (IE), a known complication of hemodialysis (HD), has recently been categorized as Healthcare-Associated Infective Endocarditis (HAIE). Single pathogen bacteremia is common, polymicrobial endocardial infection is rare in this cohort of the patients. We report a case of endocarditis caused by Enterococcus faecalis (E. faecalis) and Burkholderia cepacia (B. cepacia), a first ever reported combination of a usual and an unusual organism, respectively, in a patient on HD. Clinical presentation of the patient, its complicated course ,medical and surgical management ,along with microbial and echocardiographic findings is presented herein. The authors believe that presentation of this case of HAIE may benefit and contribute positively to cardiac science owing to the rare encounter of this organism as a pathogen in infective endocarditis and the difficulties in treating it.

Partial physeal bars may develop after injury to the growth plate in children, eventually leading to disturbance of normal growth. Clinical presentation, age of the patient, and the anticipated growth will dictate the best treatment strategy. The ideal treatment for a partial physeal bar is complete excision to allow growth resumption by the remaining healthy physis. There are countless surgical options, some technically challenging, that must be weighted according to each case’s particularities. We reviewed the current literature on physeal bars while reporting the challenging case of a short stature child submitted to a femoral physeal bar endoscopic-assisted resection with successful growth resumption. This case dares surgeons to consider all options when treating limb length discrepancy, such as the endoscopic-assisted resection which might offer successful results.

Abstract Background Patients with pulmonary hypertension (PH) have progressive and disabling symptoms, as well as a burden of treatments and a difficult clinical evaluation that make health-related quality of life a particularly relevant endpoint in this disease. The objective of the study was to evaluate patient-reported outcomes of patients receiving specific treatment for PH in a tertiary hospital using a specific questionnaire (Cambridge Pulmonary Hypertension Outcome Review-CAMPHOR) in the pharmacy consultation. Methods A cross-sectional, observational, descriptive study was conducted. It included all patients receiving specific treatment for PH in a tertiary hospital in Madrid, Spain. The inclusion period comprised between August to December 2019. CAMPHOR questionnaires containing three domains: symptoms, activities and quality of life were completed by the patients at the pharmacy consultation. Demographic and clinical variables, including WHO Functional Class (WHO FC), PH-specific tests and hemodynamic parameters, were recorded. Non-parametric analyses to assess relations between variables and CAMPHOR domains were performed. Results Thirty-six patients consented to participate in the study and completed the questionnaire. Median scores for symptoms, activities, and quality of life domains were 5.5 (2.5–10), 8.0 (4.5–10.5) and 3.5 (1–7.5), respectively. Statistically significant differences were found in the three domains when comparing by WHO FC, in the activities domain for 6-m walking test and in the quality of life domain for patients who had emergency visits or hospitalizations in the last year. Conclusions The CAMPHOR questionnaire could be useful as a complementary test to achieve an integrated evaluation of PH patients, who could complete it easily during their routine pharmacy visits.

Bipolar dislocation of the clavicle is a rare injury that is defined as a concomitant dislocation of the ipsilateral acromioclavicular joint and sternoclavicular joint. This injury is also described as a floating clavicle. Although this injury has been known for nearly two centuries, knowledge about it is limited and the treatment strategy remains controversial. Bipolar dislocation includes several combinations of both joints’ injury types. We reported two patients with bipolar dislocation of the clavicle: one with an anterior dislocation and the other with a posterior dislocation of the sternoclavicular joint. After reviewing the currently available literature, we discussed these cases to highlight the necessity of a specific treatment approach that is modified based on the pattern of each joint’s lesion.

AbstractLipomas are common benign connective tissue tumours composed of adult adipose tissue. They are relatively rare in the upper aerodigestive tract, although they occur with considerable frequency in other areas, particularly in the subcutaneous tissues of the neck. Although there are several reports of this tumour occurring in the oropharynx, there is no recorded case of a lipoma of the tonsillar fossa. An 83-year-old man with a left tonsillar fossa lipoma is presented. Clinical presentation, management and a literature review are discussed.

Sarcoidosis is a systemic granulomatous disease of unknown etiology and pathogenesis with a heterogeneous clinical presentation. In the appropriate clinical and radiological context and with the exclusion of other diagnoses, the disease is characterized by the pathological presence of non-caseating epithelioid cell granulomas. Sarcoidosis is postulated to be a multifactorial disease caused by chronic antigenic stimulation. The immunopathogenesis of sarcoidosis encompasses a complex interaction between the host, genetic factors and postulated environmental and infectious triggers, which result in granuloma development.The exact pathogenesis of the disease has yet to be elucidated, but some of the inflammatory pathways that play a key role in disease progression and outcomes are becoming apparent, and these may form the logical basis for selecting potential biomarkers.Biomarkers are biological molecules that are altered pathologically. To date, there exists no single reliable biomarker for the evaluation of sarcoidosis, either diagnostically or prognostically but new candidates are emerging. A diagnosis of sarcoidosis ideally requires a biopsy confirming non-caseating granulomas, but the likelihood of progression that requires intervention remains unpredictable. These challenging aspects could be potentially resolved by incorporating biomarkers into clinical practice for both diagnosis and monitoring disease activity.This review outlines the current knowledge on sarcoidosis with an emphasis on pulmonary sarcoidosis, and delineates the understanding surrounding the implication of biomarkers for the clinical evaluation of sarcoidosis.

The objective of the present study was to identify scientific evidence in the literature regarding management and prevention of skin tears by nurses over the last ten years. This is an integrative review of articles found in the LILACS, SciELO, BDENF, MEDLINE, Scopus, ScienceDirect and PubMed databases, identified with the Portuguese, English and Spanish descriptors for "skin," "friction," or with the keyword "skin tears". After inclusion and exclusion criteria were applied, fifteen texts were selected. Scientific evidence demonstrated that the best management results are obtained by covering the tear with the skin flap itself, using octylcyanoacrylate or silicone-based products. Prevention consists of promoting a safe environment, through multidisciplinary work and health education. We conclude that it is the nursing staff's responsibility to avoid infection, trauma of any intensity, pain and bleeding.

The authors proposed a systematic review on the current concepts of primary neural leprosy by consulting the following online databases: MEDLINE, Lilacs/SciELO, and Embase. Selected studies were classified based on the degree of recommendation and levels of scientific evidence according to the “Oxford Centre for Evidence-based Medicine”. The following aspects were reviewed: cutaneous clinical and laboratorial investigations, i.e. skin clinical exam, smears, and biopsy, and Mitsuda's reaction; neurological investigation (anamnesis, electromyography and nerve biopsy); serological investigation and molecular testing, i.e. serological testing for the detection of the phenolic glycolipid 1 (PGL-I) and the polymerase chain reaction (PCR); and treatment (classification criteria for the definition of specific treatment, steroid treatment, and cure criteria).

A Hiperbilirrubinemia neonatal é uma condição clínica caraterizada pelo acúmulo de bilirrubina no organismo do neonato podendo acarretar graves sequelas quando não tratada. Um dos meios terapêuticos consiste fototerapia, um tratamento que demanda cuidados específicos para prevenir iatrogenias. A enfermagem desempenha relevante papel nesses cuidados, pois permanece 24 horas no seguimento dos casos prevenindo complicações e contribuindo para a eficácia da terapêutica. Portanto, nesse estudo o objetivo foi investigar, através de revisão integrativa da literatura, as evidências científicas sobre os cuidados de enfermagem ao recém nascido em fototerapia. Para tal, foram pesquisadas as bases de dados: BDENF, LILACS, MEDLINE e SCIELO. Na formulação da estratégia de busca foram utilizados os descritores em saúde combinados entre si através dos operadores booleanos AND e OR. Foram encontrados quarenta artigos que após uso de critérios de inclusão e exclusão pré-definidos, permaneceram quatro que compuseram a amostra. A revisão revelou que os cuidados de enfermagem influenciam positivamente na adesão e resultado do tratamento.Descritores: Assistência de Enfermagem, Hiperbilirrubinemia Neonatal, Fototerapia. Nursing care for newborn in phototherapy, what the evidence reveals: integrative reviewAbstract: Neonatal hyperbilirubinemia is a clinical condition characterized by the accumulation of bilirubin in the neonate's organism and can cause serious sequelae when not treated. One of the therapeutic means consists of phototherapy, a treatment that requires specific care to prevent iatrogenesis. Nursing plays an important role in this care, as it remains 24 hours in the follow-up of cases, preventing complications and contributing to the effectiveness of therapy. Therefore, in this study the objective was to investigate, through an integrative literature review, the scientific evidence on nursing care for newborns undergoing phototherapy.
