Seasonal variations in food availability and access contributes to inadequate nutrient intakes, particularly in low income countries. This study assessed the effect of seasonality on dietary diversity (DD) and nutrient intakes of women and children aged 6–23 months in a rural setting in Western Kenya. A longitudinal study was conducted among 426 mother-child pairs during the harvest and post-harvest seasons in 2012. Dietary intakes were assessed using 24-h dietary recalls and dietary diversity scores (DDS) and nutrient intakes calculated for both seasons. Effect of seasonality on women dietary diversity scores (WDDS) and children's dietary diversity scores (CDDS) were assessed using generalised linear mixed models (GLMM). The proportion of women consuming diets with high DDS (>4 out of 9 food groups) increased from 36.4 to 52.4% between the two seasons, with mean WDDS being significantly higher in November compared to July/August (4.62 ± 1.43 vs. 4.16 ± 1.14, P < 0.001). A significantly higher proportion of children consumed foods from ≥4 out of 7 food groups in November compared to July/August (62.4 vs. 52.6%, P = 0.004). Mean CDDS (3.91 vs. 3.61, P = 0.004) was low but significantly higher in November compared to July/August. Estimated marginal mean WDDS increased from 4.17 to 4.38, and decreased for CDDS from 3.73 to 3.60 between the seasons. Seasonality had a small but significant effect on WDDS, P = 0.008 but not on CDDS, P = 0.293. Increase in CDDS in November was due to age and not seasonal effect. Higher women education and household food security were associated with higher WDDS and CDDS. Intakes of iron, calcium and vitamin E were higher among women in November and significantly different between the seasons. Agro-ecological zone, ethnic group and home gardening influenced nutrient intakes of the women. Seasonality had an effect on the DD of women but not of children, thus other factors apart from food availability influence the quality of children's diets during the complementary feeding period. With increasing age and transition to family foods, children's DD is expected to be affected by seasonality. Integrated interventions to alleviate seasonal food insecurity and strengthen rural households' resilience against seasonal deterioration in diet quality are recommended.

Introduction: There is a high prevalence of malnutrition in Cambodia during the past 10 years. One of the programmes implemented to secure household food security in order to alleviate malnutrition was home gardening. This study aimed to compare the nutritional status of 2 to 5 years old children in households with and without home gardens. Methods: Twenty-five households with home gardens and 60 households without home gardens were sampled. Results: The households sampled had a mixed type of home garden with three to five crop varieties occupying 20 to 200 square meters area. The purpose of the garden was to increase vegetable consumption and produce additional income. Results showed that 72% and 65% of the children resided in households with and without home gardens, respectively. Majority had medium dietary diversity score, i.e. consumed four to five different food groups per day. Except for stunting, higher prevalences of undernutrition (32%) and wasting (8%) were observed among children residing in households with home gardens compared to children in households without home gardens. Conclusion: There were no associations for dietary diversity score and nutritional status of children from households with and without home gardens. Among the sociodemographic and economic factors, age, sex, family size, and vegetable consumption were significantly associated with dietary diversity score and nutritional status of the children.

AbstractObjectiveIn Ethiopia, women’s dietary diversity is low, primarily due to poor food availability and access, both at home and market level. The present study aimed to describe market access using a new definition called market food diversity (MFD) and estimate the impact of MFD, crop and livestock diversity on dietary diversity among women enrolled in the Agriculture to Nutrition (ATONU) trial.DesignBaseline cross-sectional data collected from November 2016 to January 2017 were used for the analysis. Availability of foods in markets was assessed at the village level and categorized into nine food groups similar to the dietary diversity index for women. Bivariate and multivariate mixed-effects regression analyses were conducted, adjusted for clustering at the village level.SettingChicken-producing farmers in rural Ethiopia.ParticipantsWomen (n 2117) aged 15–49 years.ResultsOverall, less than 6 % of women met the minimum dietary diversity (≥5 food groups) and the most commonly consumed food groups were staples and legumes. Median MFD was 4 food groups (interquartile range: 2–8). Multivariate models indicated that women’s dietary diversity differed by livestock diversity, food crop diversity and agroecology, with significant interaction effects between agroecology and MFD.ConclusionsWomen’s dietary diversity is poor in Ethiopia. Local markets are variable in food availability across seasons and agroecological zones. The MFD indicator captures this variability, and women who have access to higher MFD in the highland agroecological zone have better dietary diversity. Thus, MFD has the potential to mitigate the effects of environment on women’s dietary diversity.

Abstract This study investigates the effect of participation into a microcredit program on household food security parameters of female borrowers in a rural setting in Uganda. We explore the modes of food acquisition, dietary diversity, caloric and protein intake, and qualitative food insecurity measures for different categories of respondents. We conduct a cross-sectional analysis comparing old clients to newly registered first time borrowers. Next, we compare first time borrowers and non-borrowers using a panel design. While the cross-sectional analysis allows the comparison of women that similarly self-selected into borrowing, the panel analysis complements by providing insights into changes of food security parameters. In both cases, we use Kernel matching, or difference-in-difference with Kernel matching, to control for potential bias in observables, and perform a sensitivity analysis with respect to unobservables using Rosenbaum bounds as well as an individual fixed effects panel analysis. Results show a decline in food security following the uptake of microcredit. In particular, the analysis reveals robustly lower dietary diversity among long-time borrowers than new borrowers, and larger reductions in dietary diversity scores among new borrowers, after 1 year, compared to controls. The reduction in dietary diversity was traced to a reduction in animal-source food, fruit and sugar intake. We find indicative evidence that this is partly explained by a shift from own production to reliance on food purchase by households. Other household members relegating the burden of food provision to women after borrowing may also help explain the observed result.

Abstract Objectives In Ethiopia, where 40% of the population is under 15 years, evidence on adolescent nutrition is limited. We examined the dietary diversity and its determinants among in-school adolescent girls in 2 regions in southern Ethiopia. Methods We used household survey data from 162 in-school adolescent girls aged 10–14 y across 54 primary schools in one agrarian region (SNNP) and one pastoralist region (Somali), collected in October-November 2019. Multiple regression models were used to examine factors such as nutrition knowledge, food availability and food insecurity associated with the dietary diversity score, adjusted for age and geographic clustering. Results All of the adolescent girls were currently enrolled in school (grades 4–8), and most resided with their mothers (96.9%) and fathers (80.2%). Prevalence of thinness was 11.7% (body mass index (BMI)-for-age Z-score < −2SD), and 35.2% were mildly thin (BMI-for-age Z-score < -1SD). Dietary diversity was low, with 3.7 food groups (out of 10) consumed in the last 24 h. Also, 48.8% reported consuming sweets and sugar-sweetened beverages in the last 24 h. Nutrition knowledge among adolescents was moderate, with an average score of 4.8 out of 8 knowledge items. On occurrences of food insecurity in the past 30 days, they reported an average score of 1.1 (out of 9 items). However, household surveys revealed a high degree of food insecurity (56.2%). Adolescents also reported that only 3.6 food groups (out of 10) were available at home some/most/all of the time (that is, 3–7 days) over the past 7 days. Unsurprisingly, food availability at home (β = 0.30, P = 0.000) and food insecurity (β = −0.12, P = 0.013) followed by nutrition knowledge (β = 0.11, 0 = 0.031) were significantly associated with dietary diversity score. Conclusions Understanding the factors influencing poor diets among adolescents will help to enhance the design of educational interventions to improve adolescent nutrition outcomes, a critical priority in Ethiopia. However, household food insecurity and household food availability are serious concerns in this context. Abstract Background To identify individual and household characteristics associated with food security and dietary diversity in seven Haitian–Dominican bateyes. Methods A cross-sectional sample of 667 households were surveyed. Novel household food security scores were calculated from components of the Household Food Insecurity Assessment Scale, while the Food and Agricultural Organization’s Household Dietary Diversity Score was utilized to calculate individual dietary diversity scores. Multivariable analyses were performed using ordinal logistic regression models to estimate the association between these scores and the covariate variables. Secondary dietary diversity analyses were performed after removing non-nutritious food groups. Results Food security was significantly associated with being above the poverty line (OR 3.14, 95% CI 1.92 to 5.14), living in a rural batey (OR 1.44, 95% CI 1.02 to 2.03), receiving gifts and/or donations (OR 1.76, 95% CI 1.03 to 3.00) and having a salaried job (i.e., not being paid hourly; OR 1.67, 95% CI 1.05 to 2.64). Dietary diversity was significantly associated with living in a semi-urban batey (OR 1.70, 95% CI 1.26 to 2.30), living with a partner (OR 1.47, 95% CI 1.08 to 2.00), growing at least some of one’s own food (OR 1.62, 95% CI 1.17 to 2.23), and receiving gifts and/or donations (OR 1.72, 95% CI 1.08 to 2.73). Conclusions Food insecurity and low dietary diversity are highly prevalent in Haitian–Dominican bateyes. The inclusion of sweets and non-milk beverages in dietary diversity calculations appear to skew scores towards higher levels of diversity, despite limited nutritional gains.

Abstract Background Food insecurity is a global problem that requires a One Health approach. As many households in low- and middle-income nations rely on crops and livestock that they produce to meet their household’s needs, food security and nutrition are closely linked to the health of animals and the environment. Resources controlled by women are more often allocated to uses that benefit the entire household, such as food, health, and educating children, than men’s resources. However, studies of gender control of resources among pastoralist societies are scant. We examined the effect of female and male control of livestock resources on food security and women’s dietary diversity among households from one agro-pastoralist and two pastoralist tribes in Iringa Region in south-central Tanzania. Methods We conducted surveys with 196 households, which included questions on food availability and food consumption among women, livestock holdings, gender control of livestock and livestock product income, and household demographics, as well as open-ended questions on the use of income. Food availability and food consumption responses were used to construct food security and women’s dietary diversity indexes, respectively. We conducted mixed effects logistic regression to analyze how household food security and dietary diversity were associated with livestock and other household variables. We also examined qualitative responses for use of income controlled by women and how the household obtained income when needed. Results Female-controlled livestock generally supported better household nutrition outcomes. Greater chicken holdings increased the probability of being food secure in pastoralist households but decreased it in agro-pastoralist households, while increasing the probability of having medium-high dietary diversity among all tribes. Male-controlled livestock holdings were not related to food security status. Women used income to supplement food supplies and livestock they controlled as a primary response to unanticipated household needs. Conclusions Our results show that female-control of livestock is significantly related to household food security and dietary diversity in pastoralists and agro-pastoralists in rural Tanzania. Importantly, the relationship between food security and dietary diversity differs among tribes for both male and female-controlled livestock, which suggests that blanket policies regarding management of livestock holdings may have unintended consequences.

Background: Dietary diversity is generally considered as a good indicator of nutrient adequacy and is influenced by various factors at the national, household, and individual levels. Objective: The present study sought to determine the relationships between household food insecurity, primary caregivers’ nutrition knowledge, and dietary diversity of school-aged children in Ghana. Methods: This forms part of a longitudinal study conducted in the Ayawaso West Municipal district in Accra (urban setting) and the Upper Manya Krobo district (rural setting) in Ghana. Data were collected from a total of 116 caregiver-child dyads using 24-h dietary recall and a short version of the US 12-month Household Food Security Survey Module. Nutrition knowledge and sociodemographic data were obtained using a structured questionnaire. Multivariable logistic regression was used to check for factors associated with children’s dietary diversity. Results: Majority of households reported food insecurity, with a higher percentage of insecure households located in the rural area (88.9% vs. 46.5%, P ≤ 0.0001), compared to the urban setting. Diet diversity among the study children was low, with a mean (standard deviation [SD]) of 5.8 (2.1) out of 14 food groups. Children living in food insecure households were three times more likely to have received low diverse diet compared to those from food secure households (adjusted odds ratio [OR] =3.3, 95% confidence interval [CI]: 1.4–8.0). Caregivers’ nutrition knowledge was, however, not related to children’s dietary diversity. Discussion and conclusion: Household food insecurity was a main predictor of dietary diversity among school-age children in this study. Thus, caregiver knowledge in nutrition may not be enough, particularly in the presence of food insecurity to guarantee adequate nutrition for school-aged children.

Introduction: Adequate nutrition during pregnancy is a pre-requisite for good pregnancy outcomes as well as future wellbeing, development and quality of life of the unborn child. This analytical cross-sectional study evaluated the adequacy of nutrient intakes of pregnant women resident in northern Ghana. Methods: A total of 400 pregnant women in 25 communities in the Northern, Upper East and Upper West regions of Ghana were interviewed at the household level. The nutrient intakes were assessed using a structured 24-hour dietary recall questionnaire. A dietary diversity score (DDS) was measured as a count of food groups. A nutrient adequacy ratio (NAR) of 14 nutrients as well as mean adequacy ratio (MAR) were calculated based on the 24-hour dietary recall. Results: The average energy, protein and fat intakes were 2,770.8 ± 1,127.5 Kcal/day, 59.2 ±27.5 g/day, and 105.25±58.0 g/day, respectively. The proportion of women meeting the recommended dietary allowance (RDA) of these macro-nutrients were 58.8%, 27.0% and 50.3% respectively. The average MAR of 14 nutrients was calculated to be 68% as the overall measure of nutrient adequacy. MAR correlated positively with DDS (r = 0.24 P < 0.001). Over 50% pregnant women obtained less than 66% of the RDA for iron, calcium, riboflavin, folic acid and vitamin B12. Conclusion: Diets of the majority of these pregnant women were deficient in several nutrients. Dietary diversity scores served as a useful proxy indicator of nutrient adequacy in this sample. In order to meet the requirements for essential nutrients, more effort should be made to promote dietary diversity among pregnant women in northern Ghana.
