The landscape of mental health recovery is changing; there have been calls for a shift from the clinical expertise being the dominant voice within mental healthcare towards a more personalised and collaborative service that supports those in need of mental healthcare to define what recovery is for the individual. Within this new recovery movement, there has been a recognition of the importance of the social environment in which individuals are situated and the relationship of this to mental health and wellbeing. Included in this is the importance of an individual’s role within society and the ways in which knowledge, such as experts by experience, can hold an important value. The argument then, is that social connectedness forms part of the recovery journey and that relationships can help us develop or re-connect with who we are in powerful ways. Such a view has only been strengthened by the recent and ongoing global COVID-19 pandemic. Within the UK, discussions of the importance of our wellbeing have become commonplace within the context of restricted social contact. With this heightened awareness of how the social contributes to wellbeing, it is important to consider the environments in which those in receipt of mental healthcare are situated. One of which is institutionalised care, where it is commonplace to restrict social contact. For example, by virtue of being within a locked environment, individuals’ freedom of movement is often non-existent and thus contacts with those not residing or working within the institution is restricted. While such restrictions may be deemed necessary to protect the individual’s mental health, such environments can be unintentionally toxic. Data are presented from an ethnography that was conducted within an inpatient forensic mental health hospital in the UK to highlight the problematic social environment which some individuals experience. Key interpersonal issues are presented, such as, trust, racism, the threat of physical violence and bullying that was experienced by staff and residents at the hospital. Consideration is given to the coping strategies enacted by residents and the pathologising of such behaviour. The consequences on interpersonal wellbeing are explored.

Variation in the social environment can have profound effects on survival and reproduction in wild social mammals. However, we know little about the degree to which these effects are influenced by genetic differences among individuals, and conversely, the degree to which social environmental variation mediates genetic reaction norms. To better understand these relationships, we investigated the potential for dominance rank, social connectedness and group size to modify the effects of genetic variation on gene expression in the wild baboons of the Amboseli basin. We found evidence for a number of gene–environment interactions (GEIs) associated with variation in the social environment, encompassing social environments experienced in adulthood as well as persistent effects of early life social environment. Social connectedness, maternal dominance rank and group size all interacted with genotype to influence gene expression in at least one sex, and either in early life or in adulthood. These results suggest that social and behavioural variation, akin to other factors such as age and sex, can impact the genotype–phenotype relationship. We conclude that GEIs mediated by the social environment are important in the evolution and maintenance of individual differences in wild social mammals, including individual differences in responses to social stressors.

Abstract Background: The social environment, including social support, social burden, and quality of interactions, influences a range of health outcomes, including mental health. Passive audio data collection on mobile phones (e.g., episodic recording of the auditory environment without requiring any active input from the phone user) enables new opportunities to understand the social environment. We evaluated the use of passive audio collection on mobile phones as a window onto the relationship between the social environment within a study of mental health among adolescent mothers in Nepal.Methods: We enrolled 23 adolescent mothers who first participated in qualitative interviews to describe their social support and identify sounds potentially associated with that support. Then episodic recordings were collected for two weeks from the same women using an app to capture 30 seconds of audio every 15 minutes from 4am to 9pm. Audio data were processed and classified using a pretrained model. Each classification category was accompanied by a predicted accuracy score. Manual validation of the machine-predicted speech and non-speech categories (10%) was done for accuracy.Results: In qualitative interviews, mothers described a range of positive and negative social interactions and the sounds that accompanied these. Potential positive sounds included adult speech and laughter, baby babbling and laughter, and sounds from baby toys. Sounds characterizing negative stimuli included yelling, crying, screaming by adults and crying by babies. Sounds associated with social isolation included silence and TV or radio noises. Speech comprised of 43% of all passively recorded audio clips (n=7725). Manual validation showed a 23% false positive rate and 62% false-negative rate for speech, demonstrating potential underestimation of speech exposure. Other common sounds included music and vehicular noises.Conclusions: Passively capturing audio has the potential to improve understanding of the social environment. However, the limited accuracy of the pre-trained model used in this study did not adequately distinguish between positive and negative social interactions. To improve the contribution of passive audio collection to understanding the social environment, future work should improve the accuracy of audio categorization, code for constellations of sounds, and combine audio with other smartphone data collection such as location and activity.

Abstract Background: Social contact in the early suckling period as an enriched social environment can reduce the aggressive behaviors of piglets at weaning regrouping, and improve their production performance and welfare. We speculated that the social environment could modulate the maturation of piglet intestinal microbiota at later growth stages. Therefore, we performed microbial 16S rRNA gene sequencing from fecal samples, collected at 16, 35, 42, and 63 days of age, to investigate the structure and function of intestinal microbiota in piglets that experienced early social contact. Results: The results showed that the litter weight of the piglets was not significantly different between the control (CON) and social contact (SC) groups at 35 days of age (P > 0.05), but the weight of piglets in the SC group significantly decreased at 63 days compared to the CON group (P < 0.05). While the proportion of Firmicutes were the most abundant bacterial phylum detected in both groups at all times, the proportion of Firmicutes and the Firmicutes-to-Bacteroides ratio were significantly higher in the SC group compared to the CON group and the proportion of Bacteroides was lower in the SC group compared to the CON group at 35, 42, and 63 days of age (P < 0.05). At the genus level, early social contact had a significant positive effect on the level of Lactobacillus at 35 and 42 days of age (P < 0.05), but a negative effect on Prevotella at 35, 42, and 63 days of age (P < 0.05). Furthermore, functional analysis of the microbial composition showed that the changes induced by early social contact mainly altered the relative abundance of metabolic and related pathways. From 35 days of age, the social contact notably had a negative effect on the abundance microbial pathways for protein digestion and absorption and lipid metabolism (P < 0.05). Conclusions: Early social contact truly changed the taxonomy of fecal microbiota in piglets, which in turn, impacted the potential for microbial function within the piglet intestine. At present, we speculate that providing continuous social contact negatively influences the nutrient metabolism for the growing piglets.

Critics have alleged that in attempting to adapt to the individual-centric environment of contemporary health provision, mentalization-based therapy itself has been complicit with the atomization of society. Conversations with his colleague Peter Fuggle and Dickon Bevington at the Anna Freud Centre have also had a profound role in highlighting to Fonagy the importance of the wider social system around the individual. Pursuing these questions, this chapter begins by examining the growing attention to the social environment shown by Fonagy and colleagues, and especially their exploration of the role of friends and friendships for mentalization and epistemic trust. It will then examine the reflections and research by Fonagy and collaborators on public mental health. The researchers’ hopes regarding school-based prevention will be given particular attention, and the chapter will also show how this work has shaped Fonagy’s efforts as a policy influencer. Finally, the chapter will appraise the considerations offered by Fonagy and colleagues of the role of culture, in particular the issue of whether attention to cultural processes should be regarded as mentalizing, non-mentalizing or as not mentalizing, and whether organizations and societies can themselves be said to institutionalize cultures of mentalizing or non-mentalizing.

Home treatment as an alternative to acute adult in-patient care is part of the National Health Service's Plan for mental health services in the UK. As a form of crisis intervention, it benefits from an understanding of, and ways of working with, the social systems relevant to the patient in crisis. This article describes the social systems intervention process as an alternative to admission and also considers its application in achieving early in-patient discharge.

Men’s mental health has remained undertheorized, particularly in terms of the gendered nature of men’s social relations. While the importance of social connections and strong supportive networks for improving mental health and well-being is well documented, we know little about men’s social support networks or how men go about seeking or mobilizing social support. An in-depth understanding of the gendered nature of men’s social connections and the ways in which the interplay between masculinity and men’s social connections can impact men’s mental health is needed. Fifteen life history interviews were undertaken with men in the community. A theoretical framework of gender relations was used to analyze the men’s interviews. The findings provide rich insights into men’s diverse patterns of practice in regards to seeking or mobilizing social support. While some men differentiated between their social connections with men and women, others experienced difficulties in mobilizing support from existing connections. Some men maintained a desire to be independent, rejecting the need for social support, whereas others established support networks from which they could actively seek support. Overall, the findings suggest that patterns of social connectedness among men are diverse, challenging the social science literature that frames all men’s social relationships as being largely instrumental, and men as less able and less interested than women in building emotional and supportive relationships with others. The implications of these findings for promoting men’s social connectedness and mental health are discussed.

SummaryThe UK Mental Health Act 1983 does not apply in prison. The legal framework for the care and treatment of people with mental illness in prison is provided by the Mental Capacity Act 2005. We raise dilemmas about its use. We highlight how assessing best interests and defining harm involves making challenging judgements. How best interests and harm are interpreted has a potentially significant impact on clinical practice within a prison context.

Abstract In this paper, we consider changes to memorial practices for mental health service users during the asylum period of the mid-nineteenth up to the end of the twentieth century and into the twenty-first century. The closing of large asylums in the UK has been largely welcomed by professionals and service-users alike, but their closure has led to a decrease in continuous and consistent care for those with enduring mental health challenges. Temporary and time-limited mental health services, largely dedicated to crisis management and risk reduction have failed to enable memory practices outside the therapy room. This is an unusual case of privatised memories being favoured over collective memorial activity. We argue that the collectivisation of service user memories, especially in institutions containing large numbers of long-stay patients, would benefit both staff and patients. The benefit would be in the development of awareness of how service users make sense of their past in relation to their present stay in hospital, how they might connect with others in similar positions and how they may connect with the world and others upon future release. This seems to us central to a project of recovery and yet is rarely practised in any mental health institution in the UK, despite being central to other forms of care provision, such as elderly and children's care services. We offer some suggestions on how collective models of memory in mental health might assist in this project of recovery and create greater visibility between past, present and future imaginings.

On a recent visit to the UK, Dr Guiseppe Dell Acqua, a leading exponent of the Italian psychiatric reforms, heard the ‘Med 3’ band play and extended an invitation to visit Trieste and give a concert. ‘Med 3’, the name being an allusion to the medical certificate, were formed in 1992. A Mental Health Week was held in the City of Portsmouth that year. In association with the Arts Connection, a local organisation promoting the arts to a wide audience, and the Portsmouth Care Consortium who were organising the event, artists in many fields ran workshops for mental health service users. Guy and Emma Heape, session musicians, ran a workshop at the Social Service Mental Health Day Centre and it was so successful it led to a band being formed. The band achieved recognition by winning a Mental Health Task Force award in 1993. An invitation to accompany the band to Trieste was readily accepted.

As a result of these judgments, the case law has extended significantly the responsibilities of mental healthcare providers. In this article we discuss the repercussions of these landmark decisions which are likely to have significant consequences for mental health service providers in the UK.
