Vaccination against mumps virus (MuV) (mostly measles-mumps-rubella) is routinely performed in more than 120 countries and has resulted in a distinct decrease of mumps incidence. However, alteration of mumps epidemiology has been observed in several countries after implementation of the vaccine but is sparsely documented. Moreover, outbreaks have occurred after starting vaccination, even in highly vaccinated populations. In the former German Democratic Republic (DDR) mumps was a notifiable disease but vaccination against mumps was not implemented. In the five eastern German states forming the DDR until 1990, mumps was not notifiable until 2001. Except for the lack of reporting between 1990–2000, data from Eastern Germany allow analysis of mumps epidemiology after initiating the vaccination campaign. For the period from 2001 to 2016 the data show that the incidence of mumps dropped notably after initiating vaccines, and was accompanied by an increase of the median age of patients with mumps. In Eastern Germany, no outbreaks were noted, while several outbreaks occurred in Western Germany, possibly due to a lower vaccination rate. Further literature analysis revealed that outbreaks were facilitated by waning immunity and crowding. Nevertheless, although vaccination prevented infection, the course of illness, once infected, was sometimes more complicated. In comparison to non-vaccinated populations, high rates of complicated courses occurred and were marked by orchitis, due to higher age of mumps patients. Therefore, refusing vaccination against mumps increases the risk of severe courses when living in a vaccinated population.

Abstract Background At the moment we have more than 177 million cases and 3.8 million deaths (as of June 2021) around the world and vaccination represents the only hope to control the pandemic. Imperfections in planning vaccine acquisition and difficulties in implementing distribution among the population, however, have hampered the control of the virus so far. Methods We propose a new mathematical model to estimate the impact of vaccination delay against the 2019 coronavirus disease (COVID-19) on the number of cases and deaths due to the disease in Brazil. We apply the model to Brazil as a whole and to the State of Sao Paulo, the most affected by COVID-19 in Brazil. We simulated the model for the populations of the State of Sao Paulo and Brazil as a whole, varying the scenarios related to vaccine efficacy and compliance from the populations. Results The model projects that, in the absence of vaccination, almost 170 thousand deaths and more than 350 thousand deaths will occur by the end of 2021 for Sao Paulo and Brazil, respectively. If in contrast, Sao Paulo and Brazil had enough vaccine supply and so started a vaccination campaign in January with the maximum vaccination rate, compliance and efficacy, they could have averted more than 112 thousand deaths and 127 thousand deaths, respectively. In addition, for each month of delay the number of deaths increases monotonically in a logarithmic fashion, for both the State of Sao Paulo and Brazil as a whole. Conclusions Our model shows that the current delay in the vaccination schedules that is observed in many countries has serious consequences in terms of mortality by the disease and should serve as an alert to health authorities to speed the process up such that the highest number of people to be immunized is reached in the shortest period of time.

Objective.To describe practices for influenza vaccination of healthcare personnel (HCP) with emphasis on correlates of increased vaccination rates.Design.Survey.Participants.Volunteer sample of hospitals in Louisiana.Methods.All hospitals in Louisiana were invited to participate. A 17-item questionnaire inquired about the hospital type, patients served, characteristics of the vaccination campaign, and the resulting vaccination rate.Results.Of 254 hospitals, 153 (60%) participated and were included in the 124 responses that were received. Most programs (64%) required that HCP either receive the vaccine or sign a declination form, and the rest were exclusively voluntary (36%); no program made vaccination a condition of employment. The median vaccination rate was 67%, and the vaccination rate was higher among hospitals that were accredited by the Joint Commission; provided acute care; served children, pregnant women, oncology patients, or intensive care unit patients; required a signed declination form; or imposed consequences for unvaccinated HCP (the most common of which was to require that a mask be worn on patient contact). Hospitals that provided free vaccine, made vaccine widely available, advertised the program extensively, required a declination form, and imposed consequences had the highest vaccination rates (median, 86%; range, 81%–91%).Conclusions.The rate of influenza vaccination of HCP remains low among the hospitals surveyed. Recommended practices may not be enough to reach 90% vaccination rates unless a signed declination requirement and consequences are implemented. Wearing a mask is a strong consequence. Demanding influenza vaccination as a condition of employment was not reported as a practice by the participating hospitals.

The aim of this article is to present the principles underlying the political system in force in the German Democratic Republic between 1949 and 1990, with a particular emphasis on the issue of the state of emergency law. The article describes the two Constitutions from 1949 and 1968 and the state institutions established under them, including the GDR People's Chamber, the Council of Ministers, the GDR State Council and the National Defence Council. It also discusses the constitutional solutions within the scope of the emergency law. The legal basis for the protection of the border between the two then existing German states was also considered. This article is based on the two East German Constitutions, other legal acts and on the principles of East German and Polish doctrine. The article consists of an introduction, three parts and a summary.

Abstract Background Vaccination is the most effective prevention of seasonal influenza. Despite its recommendation and active promotion, vaccination coverage remains low among healthcare staff. The goal of the study was to test if a pre-employment health check is a good opportunity to promote future vaccination against influenza among healthcare workers newly hired by a university hospital. Methods All new hospital employees active at the bedside who underwent a pre-employment health check between the end of 2016’s influenza epidemic and the start of the next influenza vaccination campaign were randomly allocated to a control group or an intervention group. The intervention consisted of a semi-structured dialog and the release of an information leaflet about influenza and influenza vaccination during the check-up, and the shipment of a postcard reminder 2 weeks before the next vaccination campaign. Vaccination rates during the campaign were compared among the two groups. Results Three hundred fifty-seven employees were included. Vaccination rates were similar in both groups: 79/172 (46%) in the control and 92/185 (50%) in the intervention group. A significantly higher rate of vaccination was noted among physicians (70/117, 60%) than among other employees (101/240, 42%, p = 0.001). In a pre-defined exploratory analysis among physicians, the vaccination rate was higher in the intervention group (36/51, 71%) than in the control group (34/65, 52%, p = 0.046). Conclusions Promotion of the influenza vaccine during pre-employment health check did not improve the vaccination rate of newly hired hospital healthcare workers overall during the next influenza vaccination campaign. Results suggest a favourable impact on the vaccination rate of physicians. Thus, there may be an interest in using communication strategies tailored to the different categories of healthcare workers to promote the influenza vaccine during pre-employment health check. Objectives.To describe the procedures used during an influenza immunization program and the use of a randomized survey to quantify the vaccination rate among healthcare workers with and without patient contact.Design.Influenza immunization vaccination program and a randomized survey.Setting.Johns Hopkins University and Health System.Methods.The 2008/2009 Johns Hopkins Influenza Immunization Program was administered to 40,000 employees, including 10,763 healthcare workers. A 10% randomized sample (1,084) of individuals were interviewed to evaluate the vaccination rate among healthcare workers with direct patient contact.Results.Between September 23, 2008, and April 30, 2009, a total of 16,079 vaccinations were administered. Ninety-four percent (94.5%) of persons who were vaccinated received the vaccine in the first 7 weeks of the campaign. The randomized survey demonstrated an overall vaccination rate of 71.3% (95% confidence interval, 68.6%-74.0%) and a vaccination rate for employees with direct patient contact of 82.8% (95% confidence interval, 80.1%-85.5%). The main reason (25.3%) for declining the program vaccine was because the employee had received documented vaccination elsewhere.Conclusions.The methods used to increase participation in the recent immunization program were successful, and a randomized survey to assess participation was found to be an efficient means of evaluating the workforce's level of potential immunity to the influenza virus.

Thirty years on from the peaceful revolution in the former communist German Democratic Republic (GDR) Germany remains profoundly divided between the perspectives of Germans living in the eastern and the western parts of the country, which is becoming ever more obvious by the polarization of domestic politics. Hence, Germany today resembles a nation which is formally unified but deeply divided internally in cultural and political terms. This article examines the background to the growing cleavages between eastern and western regions, which have their roots in the mistakes that were made as part of the management of the domestic aspects of German reunification. From a historic-institutionalist perspective the merger of the pathways of the two German states has not taken place. Instead, unified Germany is characterized by the dominance of the institutional pathway of the former West German Federal Republic, which has substantially contributed to the self-perception of East Germans as dislocated, second-class citizens.

On October 3, 1990 the territory of the German Democratic Republic was incorporated into the Federal Republic of Germany, thereby ending forty-five years of German division. At the time, assessments varied widely about whether the wholesale introduction of the West German political, legal, and socioeconomic systems into the formerly communist east would be a success, and what the implications of success or failure would be for the new united Germany. Ten years later, opinions on these fundamental questions remain divided. One group of optimistic observers maintains that the full integration of the east into an enlarged Federal Republic is well underway, though these observers acknowledge that progress has been slower and more uneven than first anticipated. A more pessimistic assessment is provided by those who claim that, if the present pattern of development continues, the east will remain in a position of permanent structural weakness vis-à-vis the west in a way analogous to that of Italy’s Mezzogiorno.

This article analyses Danish relations with the two German states. After 1949 Denmark found itself in a special position as the only West European country that was neighbour to both Germanys, having a land border with the Federal Republic and a sea border and important communications links with the German Democratic Republic. But Denmark recognised only the Federal Republic as the legitimate representative of Germany. Germany had historically constituted a serious problem for Denmark, and even in the after-war period Danish relations with its big neighbour were beset with problems. After 1955, when the minority question was settled and Denmark and the FRG were both members of NATO, relations with West Germany improved. Relations with the GDR were much more troubled because Denmark was to an extent forced to bow to West German interests, but could not ignore the existence of the East German neighbour state.

ONE CONTINUING CHARACTERISTIC OF GERMAN POLITICAL STYLE IS A ‘politics of fear’ which is propagated by both Left and Right. The Extra Parliamentary Opposition's anxieties about the neo-fascist potential in the political system are complemented by the campaign of the Springer press against totalitarian extremists of the Left. Fear is of course not necessarily unfounded in a regime which at its foundation was regarded as only ‘provisional’ and which has had every reason to fear its enemies after its experience of reactionary conservatism and Nazism and now of developments in the German Democratic Republic. The Federal Republic has been under considerable pressure since its inception to protect itself more successfully than its Weimar predecessor against its enemies. Unfortunately a ‘militant democracy’ can easily become an illiberal democracy, more concerned with its own stability than with political development.

The desire to control Covid 19 pandemic has continued to exist in the Mbala district of Zambia, with the latest trend showing a significant increase in a number of people testing positive, with a corresponding increase in vaccines (AZ, JJ) hesitancy resulting in a low (2.8 %) vaccination rate in the district. Thus, the need to probe further on covert factors under acceptability (myths, AEFs) and accessibility (vaccines availability, adequacy of vaccination sites) that could be reducing Covid 19 vaccine uptake in Mbala district. The study used a cross-sectional survey, a mixed (quantitative & qualitative) method in eliciting information from data sources covering a period of six months (April – September 2021). In all, 341 research respondents were interviewed through self-administered questionnaires. Data were analysed using descriptive statistics and binary logistic regression under SPSS v16. Study findings provide sufficient evidence that high myth (89.8%) reduced acceptability levels, while a low number of vaccination sites (59%) reduced accessibility, resulting in a low uptake rate in Mbala district. Therefore, the study recommended; building up of well-financed District Covid 19 task forces with educational aims on acceptability and accessibility, Governments to introduce specific funding lines for Covid 19 vaccination campaign and enshrine it into monthly grants for routine-outreach Covid 19 vaccination services, and Local Governments through the directorate of Public Health to introduce by-laws on mandatory Covid 19 vaccination passports for the public. With proper implementation of all these study recommendations, Covid 19 vaccination coverage rates can increase drastically across all districts of northern Zambia.
