The objective of the study was to evaluate the prevalence and factors associated with Burnout Syndrome (BS) in Primary Health Care (PHC) nursing professionals from the state of Bahia, Brazil. A multicentre, cross-sectional population-based study was conducted in a cluster sample among 1125 PHC Nursing professionals during the years 2017 and 2018. We used a questionnaire that included sociodemographic, labor and lifestyle variables and the Maslach Burnout Inventory scale to identify BS. The associations were evaluated using a robust Poisson regression with the hierarchical selection of the independent variables. The prevalence of BS was 18.3% and the associated factors were ethnicity (prevalence ratio (PR) = 0.62, confidence interval (CI) 95% = 0.47–0.83), residence (PR = 2.35, CI 95% = 1.79–3.09), economic situation (PR = 1.40, CI 95% = 1.06–1.86), satisfaction with current occupation (PR = 1.75, CI 95% = 1.31–2.33), (PR = 1.60, CI 95% = 1.23–2.08), rest (PR = 1.83, 95% CI = 1.41–2.37), technical resources and equipment (PR = 1.37, CI 95% = 1.06–1.77), night shift (PR = 1.49, CI 95% = 1.14–1.96), physical activity practice (PR = 1.72; CI 95% = 1.28–2.31), smoking (PR = 1.82, CI 95% = 1.35–2.45), and satisfaction with physical form (PR = 1.34, CI 95% = 1.01–179). Strategies are needed to prevent BS, with an emphasis on implementing worker health programs in the context of PHC.

This research aims at evaluating prevalence and factors associated with metabolic syndrome (MS) in primary health care (PHC) nursing professionals. A multicenter, population-based and cross-sectional study was conducted in a team-tested sample of 1125 PHC nurses in the state of Bahia, Brazil. Sociodemographic, labor, lifestyle and human biology variables were investigated by mean of anamnesis. MS was evaluated according to the criteria of the first Brazilian Guideline for Metabolic Syndrome, which fully adopts the criteria of the National Cholesterol Education Program’s Adult Treatment Panel III. MS-associated factors were tested by using robust Poisson Regression. The prevalence of MS found was 24.4%; low High Density Lipoprotein (HDL) cholesterol was the most prevalent component of the syndrome. In the multivariate analysis, physical inactivity (PR = 1.25, 95% CI = 1.02–1.53), alcohol use (PR = 1.84, 95% CI = 1.22–2.77), acanthosis nigricans (PR = 3.23, 95% CI = 2.65–3.92), burnout syndrome (PR = 1.45, 95% CI = 1.17–1.81), (PR = 1.37, 95% CI = 1.12–1.69), working as a nursing technician (PR = 1.43, 95% CI = 1.14–1.80), were associated to MS. It was found that the prevalence of MS was high, which evidences the need for interventions in the PHC environment, improvement of working conditions, monitoring of worker safety and health, diet programs and physical activity.

OBJECTIVE: to analyze the factors associated with the underreporting on the part of nurses within Primary Health Care of abuse against children and adolescents.METHOD: cross-sectional study with 616 nurses. A questionnaire addressed socio-demographic data, profession, instrumentation and knowledge on the topic, identification and reporting of abuse cases. Bivariate and multivariate logistic regression was used.RESULTS: female nurses, aged between 21 and 32 years old, not married, with five or more years since graduation, with graduate studies, and working for five or more years in PHC predominated. The final regression model showed that factors such as working for five or more years, having a reporting form within the PHC unit, and believing that reporting within Primary Health Care is an advantage, facilitate reporting.CONCLUSION: the study's results may, in addition to sensitizing nurses, support management professionals in establishing strategies intended to produce compliance with reporting as a legal device that ensures the rights of children and adolescents.

OBJECTIVE: To characterize prenatal care and verify possible factors associated with its adequacy. METHODS: This is a cross-sectional study based on interviews with health care professionals and consultations on official documents of women attending prenatal of the primary health care in the city of João Pessoa, capital of Paraíba, in the Northeast region of Brazil. Prenatal care was evaluated by an index with criteria referring to aspects of structure, process and outcome, denominated IPR/Prenatal. The multivariate logistic regression method revealed that demographic, socioeconomic, reproductive and maternal morbidity variables were possible determinants for prenatal adequacy. RESULTS: The survey involved 130 services and 1,625 primary health care patients. Prenatal care was adequate in approximately 23% of the cases. Low prevalence of referral to maternity, educational strategies and examinations were observed. The analysis showed that nonadolescent women (OR = 1,390), with a longer period of schooling (OR = 1.750), higher per capita income (OR = 1,870) and primiparous women (OR = 1,230) were more likely to have an adequate prenatal. CONCLUSIONS: Prenatal care, when evaluated by broader criteria, showed a low percentage of adequacy. Strategies should be developed to ensure the referral to the maternity where the birth will take place and health education activities and examinations to provide adequate prenatal care in the municipality under study. In addition, factors associated with adequacy must be considered by managers and health professionals.

Curing and eradicating Human Immunodeficiency Virus (HIV)/Acquired Immunodeficiency Syndrome (AIDS) are to the core principles of the United Nations’ Sustainable Development Goals (SDGs). The incidence of HIV in the world remains high. Although midwives play a pivotal role in PMTCT implementation, the factors associated with midwives’ role in its implementation are not well understood. The aim of this study was to determine factors associated with midwives’ role in implementation of PMTCT. This study used a cross-sectional design. The subjects were 80 midwives at 14 primary health care in Yogyakarta City, Indonesia. The study was conducted from April to August 2017. Data were analyzed through univariate, bivariate with chi-square and Fisher’s exact test, multivariate with logistic regression. The results showed that 47.5% of midwives were in the poor category regarding implementation of PMTCT. Information availability through socialization (p-value = 0.047) and knowledge level (p-value = 0.016) were found to be related to PMTCT implementation. There was no relationship between age, length of work, education level, marital status, availability of information, midwife’s attitude, perception of the availability of facilities and institutional support with midwife behavior in PMTCT implementation. Multivariate analysis showed that level of knowledge was the most dominant factor affecting PMTCT implementation (OR:6.2; CI 95% = 1.8-21.4). We recommend that efforts should be made to continuously improve the knowledge of midwives on PMTCT implementation through peer support and training in order to achieve sustainable development goals.

Nurses in primary health care (PHC) have multiple responsibilities but must often work with limited resources. The study’s aim was to estimate burnout levels among PHC nurses. A Quantitative, observational, cross-sectional, multicentre study of 338 nurses working in PHC in the Andalusian Public Health Service (Spain) is presented. A total of 40.24% of the nurses studied had high levels of burnout. The dimensions of emotional exhaustion and depersonalisation were significantly associated with anxiety, depression, neuroticism, on-call duty and seniority-profession and inversely related to agreeableness. In addition, depersonalisation was significantly associated with gender, and emotional exhaustion correlated inversely with age. Personal achievement was inversely associated with anxiety and depression and positively correlated with agreeableness, extraversion and responsibility. There is a high prevalence of burnout among nurses in PHC. Those most likely to suffer burnout syndrome are relatively young, suffer from anxiety and depression and present high scores for neuroticism and low ones for agreeableness, responsibility and extraversion.

Abstract: Introduction: Health assistance reorganization has been oriented, since the Unified Health System (SUS) creation, by a proposal of expansion and strengthening of Primary Health Care (PHC). In this context, the scarcity of trained professionals is considered one of the main challenges for PHC consolidation. Among the career options after graduation, most physicians choose to seek focal specializations to work in other health care levels. Objective: Therefore, this quantitative study aims to analyze the prevalence and associated factors of medical students’ interest in following a career in PHC. Method: A cross-sectional study was conducted with medical students attending the last two years in four medical schools in the state of Minas Gerais, Brazil. A total of 524 students participated by answering a self-administered questionnaire, which was prepared by the authors, including questions regarding sociodemographic and economic profile, interaction with PHC during undergraduate school, career intention and intention to attend the Family and Community Medicine Residency (FCMR) program. Results: The results showed that 26.3% of the respondents indicated interest in following a career in PHC, a higher percentage when compared to previous studies. Regarding PHC being a temporary work choice, 79,3% of the participants indicated that they intend to work in this field after graduation. Ten percent of the students informed they intend to attend the FCMR program. Among the factors associated with the intention to work in PHC, a successful PHC experience during undergraduate school stands out. Conclusion: The results should be taken into consideration by health and medical education managers, fostering strategies that promote successful PHC experiences, encouraging learning-service integration and including students in a consolidated PHC network during undergraduate school. Initiatives such as increasing the workload in PHC during undergraduate school, increasing the number of FMC specialized teachers and health network improvement in the municipalities where these Higher Education Institutions are located can be powerful actions toward increasing the graduating students’ interest to work in PHC.
