Focal thyroid lesions are common ultrasound findings with the estimated prevalence up to 67% of the population. They form characteristically enveloped regions with individual encapsulated microenvironment that may involve the specific distribution of immune system compounds—especially antigen presenting cells (APC). We analyzed and compared the most potent APC—plasmacytoid and conventional dendritic cells (DCs) subpopulations and three monocyte subpopulations as well as other immune cells—in peripheral blood and local blood of thyroid gland obtained parallelly in patients with focal thyroid lesions using flow cytometry. The analysis revealed significant differences in the distribution of main subsets of assessed cells between peripheral blood and biopsy material. The results support the existence of local, organ-specific immune reaction control networks within thyroid nodules.

Background: Thyroid abnormalities are the most common endocrine abnormalities in Saudi Arabia (KSA). Ultrasonography (USG) is the commonest method of thyroid gland imaging. Thyroid lesions prevalence depends on factors such as sex, age, iodine intake, and other metabolic parameters. Objectives: Investigating different thyroid lesions and their associated clinical parameters. Subjects and Methods: This is a cross-sectional study of adult residents who visited Internal Medicine Clinic in Riyadh, KSA. 206 participants were subjected to thyroid USG, weight and height measurement, laboratory investigations including FBG, HbA1C, ALT, AST, creatinine, uric acid, lipid profile, thyroid stimulating hormone (TSH), and FT4. Results: Thyroid nodules were found in 27.2%, 30.1% had thyroiditis, and 6.8% had pure cysts. There was a significant prevalence of thyroid ultrasound findings within different age groups, (P = 0.002). The highest BMI was found in subjects with nodular size ≤1cm. Subjects with thyroid nodules had higher level of FBG, HbA1c, ALT, AST, uric acid, total cholesterol, HDL-C, LDL-C and TSH. The most frequent thyroid status was euthyroid, followed by subclinical hypothyroidism which had higher frequency in subjects with nodular size ≤1cm (30.8%). Conclusion: Imaging, particularly USG, plays an important role in classifying thyroid gland abnormalities. We found that thyroid lesions, including nodules, pure cysts, and thyroiditis, were non-significantly positively associated with disturbed metabolic profile such as high FBS, HBA1C, ALT, AST, total cholesterol, and LDL levels. Our results for biochemical parameters do not allow inferences on whether they are independent factors associated with thyroid lesions or not.

ObjectiveSjögren's syndrome (SS) is a lymphoproliferative autoimmune disease, characterised by dryness of the mouth and eyes. Dendritic cells (DC) are potent antigen-presenting cells crucial for initiating and maintaining primary immune responses. This study quantified interferon-producing plasmacytoid DC (pDC) and two myeloid DC subsets (mDC1 and mDC2) in peripheral blood (PB) from primary SS (pSS) patients and healthy controls.MethodsBlood samples from 31 pSS patients and 28 gender and age-matched healthy controls were analysed by flow cytometry using the Miltenyi Blood DC enumeration kit. The presence of pDC in salivary glands (SG) from pSS patients was analysed by immunohistochemistry.ResultsPatients with pSS had significantly less pDC and mDC2 in PB compared with healthy controls. Moreover, pDC are present in SG from patients with pSS.ConclusionPatients with pSS have alterations among DC populations in PB, and pDC are present in the SG, suggesting a potential role of these cells in SS.

Mononuclear phagocytes play a major role in the development of vascular lesions in atherogenesis. The goal of our study was to characterize circulating blood monocyte subpopulations as potential cellular markers of systemic immunological abnormalities in hypercholesterolemia. In normal subjects, three-parameter immunophenotyping of whole blood revealed that 61.3±6.0% of monocytes showed “bright” expression of the lipopolysaccharide receptor (LPSR: CD14) and Fcγ receptor I (RI: CD64) without expression of Fcγ-RIII (CD16). Other monocyte subsets (populations 2, 3, 4, and 5) were characterized by the simultaneous expression of both Fcγ-R's (25.6±5.0%), isolated expression of Fcγ-RIII (9.4±1.7%), or high expression of CD33 (3.7±1.1%) with only dim expression of CD14, respectively. The smallest subset of monocytes (population 5: 2.1±0.8%) differed from the predominant population of CD14 bright CD64 + CD16 − monocytes by additional expression of neural cell adhesion molecule (N-CAM: CD56). In a group of hypercholesterolemic patients (n=19), high density lipoprotein cholesterol levels were negatively correlated to the population size of CD64 − CD16 + monocytes. In both healthy subjects (n=55) and hypercholesterolemic patients, the rare apolipoprotein E3/E4 and E4/E4 phenotypes were associated with a tendency toward a larger population of CD64 − CD16 + monocytes. Expression of the variant activation antigen CD45RA by peripheral blood mononuclear phagocytes showed a positive correlation to plasma levels of the atherogenic lipoproteins low density lipoprotein and lipoprotein(a). These data suggest that systemic abnormalities in mononuclear phagocyte subpopulations may play a role in the pathogenesis of atherosclerosis.
