Persuasive technologies have been identified as a potential motivational tool to tackle the rising problem of physical inactivity worldwide, with research showing they are more likely to be successful if tailored to the target audience. However, in the physical activity domain, there is limited research on how culture moderates users’ susceptibility to the various persuasive features employed in mobile health applications aimed to motivate behavior change. To bridge this gap, we conducted an empirical study among 256 participants from collectivist (n = 67) and individualist (n = 189) cultures to determine their culture-specific persuasion profiles with respect to six persuasive features commonly employed in fitness applications on the market. The persuasive features include two personal features (goal-setting/self-monitoring and reward) and four social features (competition, cooperation, social learning and social comparison). We based our study on the rating of storyboards (on which each of the six persuasive features is illustrated) and the ranking of the six persuasive features in terms of perceived persuasiveness. The results of our analysis showed that users from individualist and collectivist cultures significantly differ in their persuasion profiles. Based on our rating measure, collectivist users are more likely to be susceptible to all six persuasive features (personal and social) than individualist users, who are only likely to be susceptible to personal features. However, based on our ranking measure, individualist users are more likely to be susceptible to personal features (goal-setting/self-monitoring and reward) than collectivist users. In contrast, collectivist users are more likely to be susceptible to social features (cooperation and social learning) than individualist users. Based on these findings, we provide culture-specific persuasive technology design guidelines. Our study is the first to uncover the moderating effect of culture on users’ susceptibility to commonly employed persuasive features in fitness applications.

Fitness apps equipped with various persuasive features have become popular worldwide due to the physical inactivity crisis. However, there is a limited understanding of the most important persuasive features that drive their adoption and the moderating effect of age and gender. To bridge this gap, we designed storyboards illustrating six of the commonly employed persuasive strategies in persuasive health applications: Goal-Setting/Self-Monitoring, Reward, Social Learning, Social Comparison, Competition and Cooperation. We conducted an empirical study in which we asked the participants to evaluate their receptiveness to the six persuasive features and their intention to use a fitness app that features them. The result of our Partial Least Square Path Modeling (PLSPM) shows that, overall, Goal-Setting/Self-Monitoring is the strongest predictor of the intention to use a fitness app, followed by Reward and Competition, both of which are in second place. However, Social Learning and Social Comparison turn out to be non-predictors of intention to use. Based on these findings, we recommend that a minimally viable (one-size-fits-all) fitness app, in a personal setting, should support a Goal-Setting/Self-Monitoring feature, coupled with a Reward feature, to increase its appeal to a wide audience. Moreover, in a social setting, it should support a Competition feature to increase its appeal to a wide audience. We discuss these findings and the gender and age differences in the relationships between users’ receptiveness to the six persuasive features and their intention to use a fitness app that support them.

BACKGROUND Currently, the use of smartphones to deliver health-related content has experienced a rapid growth, with more than 165,000 mobile health (mHealth) applications currently available in the digital marketplace such as iOS store and Google Play. Among these, there are several mobile applications (mobile apps) that offer tools for disease prevention and management among older generations. These mobile apps could potentially promote health behaviors which will reduce or delay the onset of disease. However, no review to date that has focused on the app marketplace specific for older adults and little is known regarding its evidence-based quality towards the health of older adults. OBJECTIVE The aim of this review was to characterize and critically appraise the content and functionality of mobile apps that focuses on health management and/or healthy lifestyle among older adults. METHODS An electronic search was conducted between May 2019 to December 2019 of the official app store for two major smartphone operating systems: iPhone operating system (iTunes App Store) and Android (Google Play Store). Stores were searched separately using predetermined search terms. Two authors screened apps based on information provided in the app description. Metadata from all included apps were abstracted into a standard assessment criteria form. Evidenced based strategies and health care expert involvement of included apps was assessed. Evidenced based strategies included: self-monitoring, goal setting, physical activity support, healthy eating support, weight and/or health assessment, personalized feedback, motivational strategies, cognitive training and social support. Two authors verified the data with reference to the apps and downloaded app themselves. RESULTS A total of 16 apps met the inclusion criteria. Six out of 16 (37.5%) apps were designed exclusively for the iOS platform while ten out of 16 (62.5%) were designed for Android platform exclusively. Physical activity component was the most common feature offered in all the apps (9/16, 56.3%) and followed by cognitive training (8/16, 50.0%). Diet/nutrition (0/16, 0%) feature, however, was not offered on all reviewed mobile apps. Of reviewed apps, 56.3% (9/16) provide education, 37.5% (6/16) provide self-monitoring features, 18.8% (3/16) provide goal setting features, 18.5% (3/16) provide personalized feedback, 6.3% (1/16) provide social support and none of the reviewed apps offers heart rate monitoring and reminder features to the users. CONCLUSIONS All reviewed mobile apps for older adults in managing health did not focused on diet/nutrition component, lack of functional components and lack of health care professional involvement in their development process. There is also a need to carry out scientific testing prior to the development of the app to ensure cost effective and its health benefits to older adults. Collaborative efforts between developers, researchers, health professionals and patients are needed in developing evidence-based, high quality mobile apps in managing health prior they are made available in the app store.

BACKGROUND Gestational diabetes mellitus (GDM) emerges worldwide and is closely associated with short- and long-term health issues in women and their offspring, such as pregnancy and birth complications respectively comorbidities, Type 2 Diabetes (T2D), Metabolic Syndrome (MetS) as well as cardiovascular disease (CD). Against this background mobile health applications (mHealth-Apps) do open up new possibilities to improve the management of GDM clearly. OBJECTIVE Since there is – to our knowledge – no systematic literature review published, which focusses on the effectiveness of specific mHealth-Apps on clinical health-related short and long-term outcomes of mother and child, we conducted these much-needed analyses. METHODS Data sources: A systematic literature search in Medline (Pubmed), Cochrane Library, Embase, CINAHL and Web of Science was performed including full text publications since 2008 up to date. An additional manual search in references and Google Scholar was conducted subsequently. Study Eligibility Criteria: Women diagnosed with GDM using specific mHealth-Apps during pregnancy compared to control groups, which met main clinical parameters and outcomes in GDM management as well as maternity and offspring care. Study appraisal and synthesis methods: Study quality was assessed and rated “strong”, “moderate” or “weak” by using the Effective Public Health Practice Project (EPHPP) tool. Study results were strongly categorized by outcomes; an additional qualitative summary was assessed. Study selection: Overall, n= 114 studies were analyzed, n= 46 duplicates were removed, n=5 studies met the eligible criteria and n=1 study was assessed by manual search subsequently. In total, n=6 publications, analyzing n=408 GDM patients in the interventional and n=405 women diagnosed with GDM in the control groups, were included. These studies were divided into n=5 two-arm randomized controlled trials (RCT) and n=1 controlled clinical trial (CCT). RESULTS Distinct improvements in clinical parameters and outcomes, such as fasting blood glucoses (FBG), 2-hour postprandial blood glucoses (PBG), off target blood glucose measurements (OTBG), delivery modes and patient compliance were analyzed in GDM patients using specific mHealth-Apps compared to matched control groups. CONCLUSIONS mHealth-Apps clearly improve clinical outcomes in management of GDM effectively. More studies need to be done more in detail.

AbstractObjectivesThere are many mobile health applications (apps) now available and some that use in some way laboratory medicine data. Among them, patient-oriented are of the lowest content quality. The aim of this study was to compare the opinions of non-laboratory medicine professionals (NLMP) with those of laboratory medicine specialists (LMS) and define the benchmarks for quality assessment of laboratory medicine apps.MethodsTwenty-five volunteers from six European countries evaluated 16 selected patient-oriented apps. Participants were 20–60 years old, 44% were females, with different educational degrees, and no professional involvement in laboratory medicine. Each participant completed a questionnaire based on the Mobile Application Rating Scale (MARS) and the System Usability Scale, as previously used for rating the app quality by LMS. The responses from the two groups were compared using the Mann-Whitney U test and Spearman correlation.ResultsThe median total score of NLMP app evaluation was 2.73 out of 5 (IQR 0.95) compared to 3.78 (IQR 1.05) by the LMS. All scores were statistically significantly lower in the NLMP group (p<0.05), except for the item Information quality (p=0.1631). The suggested benchmarks for a useful appear: increasing awareness of the importance and delivering an understanding of persons’ own laboratory test results; understandable terminology; easy to use; appropriate graphic design, and trustworthy information.ConclusionsNLMP’ evaluation confirmed the low utility of currently available laboratory medicine apps. A reliable app should contain trustworthy and understandable information. The appearance of an app should be fit for purpose and easy to use.

Chronic disease is challenging to treat because treatment often requires lifestyle behavior changes. In recent years the use of health and wellness coaches (HWC) has emerged as a way to support patients making behavioral changes. The use of HWCs has resulted in improved management of chronic disease for many patients. The success of HWCs is often thought to be due to the extended care they provide and the behavioral modification techniques they promote such as goal setting and self-monitoring. This article describes how HWC’s conformity to the current health care zeitgeist of personalized, holistic care may be another reason for their success.
