Romania has the highest incidence of cervical cancer morbidity and mortality in Europe. This study identifies the major clusters for genital cancers, observes the features of genital and cervical cancer, and determines the extent to which cancer is a contributor to total Disability-Adjusted Life Year (DALY). Spatial analysis used Besag and Newell’s method for genital cancer distribution, prevalence considered Arad County patients records (2008–2017), and DALY was determined according to WHO methodology and GLOBOCAN 2013 data. Diagnosis was established by histopathological examination of diagnostic biopsies or tissues obtained by surgical procedures, followed by clinical staging. 1695 women were recorded with genital cancer. Of these, 14.9% of lesions were in situ (n = 252) and 74.20% of cases were recorded in stage III or IV (n = 1258) (p < 0.0001). Over 90% of cervical cancers were squamous cell carcinomas (n = 728), 33.76% of endometrial cancers were adenocarcinomas in situ (n = 131), 32.42% of ovarian cancers were serous adenocarcinomas (n = 131), and 70.58% of vulvar cancers were squamous cell carcinomas (n = 48) (p < 0.0001). DALY/1000 was 67.2 for genital cancers and 33 for cervical cancers. From the point of view of Romanian women, cervical cancer remains one of the major problems that need to be dealt with and access to optimal treatment proves to be extremely limited.

Objective: Cervical cancer is the second common type of cancer among women worldwide. Infection by certain types of human papillomavirus infection (HPV) such as HPV 16/18 has been considered as the most significant risk factor for the development of cervical cancer. This study was conducted to determine the prevalence HPV 16/18 in women with and without cervical cancer in Aleppo city, Syria.Methods: Paraffin embedded cervical tissue samples were collected from 113 patients and 40 controls (normal cervix), DNA was extracted and analyzed by PCR for the presence of HPV16/18. Out of 113 cases of cervical carcinoma, 25 cases (22.1%) were carcinoma in situ (CIS), 72 cases (63.7%) were squamous cell carcinomas (SCC) and 16 cases (14.2%) were adenocarcinoma (ADC).Results: HPV 16 and/or 18 DNA was detected in 84 (74.3%) cases out of 113 of cervical cancers, while it was found in only two of the 40 samples in the control group. At the same time, 9 out of 25 (36%) carcinoma in situ, 61 out of 72(84.72%) squamous cell carcinomas, and 14 out of 16 (87.5%) adenocarcinomas, were positive for HPV 16 and/or 18.Conclusion: The frequency of infection with HPV 16 and 18 is high among Syrian women and a vaccine against these two types may be effective in about 74% of cervical cancers in Syria.

Human papillomaviruses (HPV) are responsible for the development of almost all cervical cancers. HPV is also found in 85% of anal cancer and in 50% of penile, vulvar, and vaginal cancers, and they are increasingly found in a subset of head and neck cancers, i.e., oropharyngeal squamous cell carcinomas (OPSCC). The model for how HPV causes cancer is derived from several decades of study on cervical cancer, and it is just presumed that this model is not only completely valid for cervical cancer but for all other HPV-driven cancers as well. Next-generation sequencing (NGS) has now provided the necessary tools to characterize genomic alterations in cancer cells and can precisely determine the physical status of HPV in those cells as well. We discuss recent discoveries from different applications of NGS in both cervical cancer and OPSCCs, including whole-genome sequencing and mate-pair NGS. We also discuss what NGS studies have revealed about the different ways that HPV can be involved in cancer formation, specifically comparing cervical cancer and OPSCC.

The objective of this study was to identify multiple plasma protein markers that might be characteristic of in situ and invasive cervical cancers. Plasma samples obtained from patients with in situ cervical cancer (carcinoma in situ [CIS], n = 32), from patients with early invasive cervical cancer without lymph node metastasis (squamous cell carcinoma [SCC], n = 60), and from age-matched disease-free controls (n = 37) were analyzed by cation-exchange protein chips and surface-enhanced laser desorption and ionization time-of-flight mass spectrometry. A classification tree defined by six protein peaks could discriminate 84 of the 92 cancers (CIS and SCC) and 36 of the 37 controls, with 91% sensitivity and 97% specificity. In comparing the CIS and SCC samples, two protein peaks with Mr values of 6586.41 and 3805.68 were able to classify 55 of the 60 SCC and 31 of the 32 CIS samples, with 92% sensitivity and 97% specificity. This study demonstrates for the first time the feasibility of differentiating in situ and invasive cervical cancers through plasma protein profiling. Identification of the proteins different in invasive and in situ cancer may be of great value in the understanding of cervical cancer invasion and in the development of novel therapeutic intervention.

Vulvar and vaginal cancers are rare and predominantly involve squamous cell carcinomas. Some studies combine these cancers, presumably because of their rarity, anatomic proximity, and shared risk factors. Major risk factors include human papillomavirus (HPV) and cigarette smoking. This chapter explores the similarities and important differences in etiology between these cancer sites. In addition to its focus on invasive cancer, the chapter also discusses high-grade precursor lesions, or in situ disease, that sometimes progress to cancer and must, therefore, be treated.
