THE BASICS OF ENSURING SANITARY AND EPIDEMIOLOGICAL WELFARE OF THE POPULATION DURING PERIODS OF FLOODING THE TERRITORY OF THE DISTRICT

The unfavorable sanitary-epidemiological situation in the conditions of natural disasters and man-made disasters testifies to the need to improve the activities of the sanitary-epidemiological service as one of the most important links in ensuring the safety of the environment for public health. In an emergency situation related to the unfavorable flood situation in the Tuapse district of Krasnodar region in 2012 and 2014, an increase in the incidence of the sum of acute intestinal infections (AII) in the post-flood periods was registered, which exceeded the average long-term morbidity rate by 33.3 % and 20.9 %, respectively. Mostly recorded AII bacterial etiology to 53.8 %, caused by conditionally pathogenic microflora. The peculiarity of the situation was an increase in the population of intestinal infections of viral etiology by 20 times and caused by pathogenic microorganisms by 2 times compared to the preplant period. The share of water factor in the transmission of AII cases was 34.8 % in 2012, 14.7 % in 2014, the leading pathway of infection - contact and household. The implementation of sanitary-hygienic and anti-epidemic measures provided the operational management of the system of life support of the population during the natural disaster.

How do parents cope when their child is ill or dying, when he or she experiences constant pain or suffering? What do parents think of the contributions that medical professionals make to the care of their chronically or terminally ill child? Is it possible for a parent to love a child so much that the child is wished dead? The purpose of this paper is to explore those questions and aspects of the care of chronically or terminally ill children using Mourning Dove’s portrayal of one family’s attempt to care for their ill daughter. A play written by Canadian playwright Emil Sher, Mourning Dove is based on the case of Saskatchewan wheat farmer Robert Latimer who killed his 12 year old daughter Tracy who suffered with cerebral palsy and lived in tremendous pain. Rather than focusing on the medical or legal aspects of the care of a chronically ill child, the play offers a glimpse into how a family copes with the care of such a child and the effects the child’s illness has on a family. Reading and examination of non-medical literature, such as Mourning Dove, therefore serve as a useful means for medical professionals to better understand how illness affects and is responded to by patients and their families. Abstract Me & My Wishes are person-centered videos of long term care residents (ages 65-95) living with dementia discussing their preferences for care including end-of-life (EOL) medical intervention. We evaluated the congruence of six EOL treatment preferences between the residents’ personal videos, medical records (e.g. advance directive), and surveys of family (n= 49) and staff (n=37; 118 responses) knowledge of their preferences. Results were highly discordant. Treatments with the most discordance when comparing videos to comparison groups were IV fluids (medical record, 57.1%) and life support (family, 69.4%; staff, 82.2%). Residents reported EOL treatments were considered acceptable if they were temporary, would relieve suffering, or enabled a return to baseline health. These caveats may lead to discordance if they are not conveyed to family or staff. Our findings highlight the need for conversations among residents living with dementia and their caregivers to improve understanding, congruence and adherence of resident EOL preferences.

Purpose: To evaluate the tolerance of peritonectomy procedures (PP) combined with intraperitoneal chemohyperthermia (IPCH) in patients with peritoneal carcinomatosis (PC), a phase II study was carried out from January 1998 to September 2001. Patients and Methods: Fifty-six patients (35 females, mean age 49.3) were included for PC from colorectal cancer (26 patients), ovarian cancer (seven patients), gastric cancer (six patients), peritoneal mesothelioma (five patients), pseudomyxoma peritonei (seven patients), and miscellaneous reasons (five patients). Surgeries were performed mainly on advanced patients (40 patients stages 3 and 4 and 16 patients stages 2 and 1) and were synchronous in 36 patients. All patients underwent surgical resection of their primary tumor with PP and IPCH (with mitomycin C, cisplatinum, or both) with a closed sterile circuit and inflow temperatures ranging from 46° to 48°C. Three patients were included twice. Results: A macroscopic complete resection was performed in 27 cases. The mortality and morbidity rates were one of 56 and 16 of 56, respectively. The 2-year survival rate was 79.0% for patients with macroscopic complete resection and 44.7% for patients without macroscopic complete resection (P = .001). For the patients included twice, two are alive without evidence of disease, 54 and 47 months after the first procedure. Conclusion: IPCH and PP are able to achieve unexpected long-term survival in patients with bulky PC. However, one must be careful when selecting the patients for such an aggressive treatment, as morbidity rate remains high even for an experienced team.

The high transmissibility, mortality, and morbidity rate of the SARS-CoV-2 Delta (B.1.617.2) variant have raised concerns regarding vaccine effectiveness (VE). To address this issue, all publications relevant to the effectiveness of vaccines against the Delta variant were searched in the Web of Science, Scopus, EMBASE, and Medline (via PubMed) databases up to 15 October 2021. A total of 15 studies (36 datasets) were included in the meta-analysis. After the first dose, the VE against the Delta variant for each vaccine was 0.567 (95% CI 0.520–0.613) for Pfizer-BioNTech, 0.72 (95% CI 0.589–0.822) for Moderna, 0.44 (95% CI 0.301–0.588) for AstraZeneca, and 0.138 (95% CI 0.076–0.237) for CoronaVac. Meta-analysis of 2,375,957 vaccinated cases showed that the Pfizer-BioNTech vaccine had the highest VE against the infection after the second dose, at 0.837 (95% CI 0.672–0.928), and third dose, at 0.972 (95% CI 0.96–0.978), as well as the highest VE for the prevention of severe infection or death, at 0.985 (95% CI 0.95–0.99), amongst all COVID-19 vaccines. The short-term effectiveness of vaccines, especially mRNA-based vaccines, for the prevention of the Delta variant infection, hospitalization, severe infection, and death is supported by this study. Limitations include a lack of long-term efficacy data, and under-reporting of COVID-19 infection cases in observational studies, which has the potential to falsely skew VE rates. Overall, this study supports the decisions by public health decision makers to promote the population vaccination rate to control the Delta variant infection and the emergence of further variants.

Objective: Epidemiological data analysis concerning the spread of brucellosis in Russia and Primorsky Region, study of the manifestations of the epidemic process and the current state of brucellosis issue.Methods: Descriptive epidemiological analysis of materials on infectious morbidity based on state statistical reporting forms No. 1, No. 2 “Information on infectious and parasitic morbidity”.Results: Brucellosis affects workers in the main occupational groups of livestock breeders, workers involved in the processing of livestock, who contact sick animals and other sources of brucellosis infection. Primorsky Region is a territory with an unstable epidemiological situation on brucellosis. A different morbidity rates were registered over the years. However usually it does not exceed the national average rate. In 2009, the morbidity rate counted 27.5% less than in Russian Federation (0.21 and 0.29 per 100,000 respectively), compared to previous years, the average long-term morbidity rate in the region increased by 3.5 times. The disease rate for the period from 2009 to 2019 ranged from 0.05 to 0.21%. In 2012, it exceeded the index in the Far Eastern Federal District (0.1 per 100,000 population). Brucellosis cases were detected among livestock breeders during an extraordinary medical examination due to the bad situation on brucellosis among animals.Conclusions: In recent decades, the epizootic and epidemiological situation on brucellosis remains tense due to the decrease in the number of serological and bacteriological studies among animals and humans, the weakening of veterinary-sanitary control and the formation of new private farms.

Landscapes of the Tobol-Ishim interfluve were not stable in the Holocene and varied from forests and drowned floodplains at the beginning of the V and III millennia BC to steppificated territories with a lowered water level at the beginning of the Atlantic Period and in the middle of the Subboreal Period, which determined the main types of economic activities, one of them was fishing. Changes in hydrological regime of water bodies influenced the methods of fishing, including the use of different traps. Thus, in the Neolithic, when the water level decreased, the location of settlements in the system river-creek-lake (for example, Mergen 6), a large number of fish bones, bone harpoons, fishing spears, fishing tackles for catching pike and a total absence of plummets were indicative of individual fishing for large fish and, perhaps, of stop net fishery, which was facilitated by a decrease in the width of watercourses and tombolos. Stop net (stake net) fishery led to a settled lifestyle of the population, collective activities and the emergence of long-term settlements with deep foundation pits of dwellings. When the water level in rivers and lakes increased and floods became more frequent, the life support system changed, the population began to develop coasts more widely, its mobility increased, and they started to build framed above-ground dwellings. Following those changes, biconic, cigar-shaped, and corniculate plummets emerged in the Tobol River Basin and on the adjacent western and north-western territories in the III and early II millennium BC. When the water level was high, it was efficient to fish using traps, seines and, probably, nets, although the latter could also be used in drive hunting for shedding geese and ducks. Subrectangular plummets with one or two ties for fastening, and disk-shaped plummets with a tie in the center had been prevailing since the beginning of the II millennium BC; they existed until the first third of the I millennium BC. This period, the transition time from the Bronze Age to the Early Iron Age, is characterized by the absence of clay plummets, while there are large accumulations of fish scales and bones in the settlement layers. We can suppose that the population of that time (local Late Bronze Age population, mixed with northern migrants who made utensils with cross ornamentation) switched from net fishing to stop net fishing.
