A STUDY OF RESIDUAL BREAST TISSUE IN POST MASTECTOMY SKIN FLAP

Background: The objective of any mastectomy is to rid the chest wall of any breast tissue but since the breast being unencapsulated, there is every chance that some amount of breast tissue may be left behind in the skin aps after resection. As the remnant breast tissue is likely to be exposed to the same carcinogenic risk factors as the tissue removed. Hence, there is always a possibility that the residual tissue may become the seat of a recurrence. To get rid the chest wall so far the residual breast tissue will Objective: be minimal and the recurrence of breast carcinoma will be less. Patents Data source: of proven case of CA breast who underwent MRM at RG KAR MEDICAL COLLEGE AND HOSPITAL from July 2016 to June 2018 are included in the study group. This is a prospective study Study design: of total 45 patients are selected. Materials and methods: Prospective analysis of 45 post MRM women whose mean age is 49.5 years (mean ± SD age, 49.5 ± 2.4 years). Tissue specimens taken from central part of four quadrants of two skin aps for histo-pathological examination. Skin ap thickness measured for more than 8mm. All patients followed for any recurrence of breast carcinoma. Results: Of all histo-pathological specimens left sided breast cancer cases were more common than right side. 48-49 years age group had the most number of cases and more than 53 years the least. The lower medial quadrant the most 5(45.45%), upper and lower lateral quadrant 3(27.27%) each for positive and upper medial quadrant negative for breast tissue. Breast carcinoma negative in all specimens. Skin ap necrosis of 2(4.44%) cases. No recurrences noted during my study period. My study has demonstrated presence of residual breast tis Conclusion: sue in a signicant percentage of post mastectomy skin aps. No residual cancer tissue was found. During the duration of the study, no recurrence of breast carcinoma was detected in patients with residual breast tissue in skin aps.

Lateral calcaneal flap is frequently used for coverage of lateral calcaneum and posterior heel defects. We conducted a prospective study using lateral calcaneal artery flap to cover posterior heel defects from July 2011 to June 2015 in the Department of Burn and Plastic Surgery, Khulna Medical College to observe the clinical outcome of use of lateral calcaneal artery flap. A total number of ten soft tissue defects in posterior heel with and without exposed Achilles tendon or calcaneus due to different causes were reconstructed with lateral calcaneal artery flap. All ten flaps survived completely with no subsequent breakdown of the skin, even after regular wearing of normal shoes. The flap donor sites were grafted with split-thickness skin grafts in all cases. Partial graft loss was noted in two cases which were eventually healed spontaneously with dressing without the need for a secondary graft. Epidermal necrosis developed in one case which was ultimately healed with conservative treatment. There was no loss of sensation in flap area but sensory disturbance at the lateral part of the dorsum of foot was found in all cases. Our results were satisfactory in terms of coverage adequacy, perioperative morbidity, and functional and aesthetical outcomes. The lateral calcaneal artery skin flap can be used safely to provide sensory skin coverage in posterior heel defects.Mediscope Vol. 4, No. 1: Jan 2017, Page 5-10

Background: Breast carcinoma is the most common malignant tumor and leading cause of cancer related death in women worldwide. Apart from traditional markers, estrogen receptor, progesterone receptor and Her-2neu, which are important for prognostication and staging purposes, a novel marker cyclooxygenase-2 (COX-2) is being studied extensively. We intend to study the spectrum of COX-2 expression in normal breast tissue, ductal carcinoma in situ (DCIS) adjacent to invasive cancer, and in invasive cancer and compare COX-2 expression with histological prognostic parameters and hormone receptor status.Methods: The present study is a prospective study that was conducted in the department of Pathology, SGT Medical College and Hospital, Gurugram (2019-2020). Fifty patients, aged between 21 and 70, suffering from primary breast cancer constituted the study group. Various histological prognostic parameters were assessed. Immunohistochemical profile of the tumor was assessed. COX-2 score was correlated with various clinicopathologic parameters.Results: Among the total of 50 patients suffering from invasive breast carcinoma, 94 percent (47/50) of cases showed the same COX-2 expression level in normal breast epithelium and corresponding tumor areas and this correlation was statistically significant. The correlation between the level of COX-2 expression in tumor and DCIS was highly significant.Conclusion: Inhibition of COX-2 may represent a potential target for preventing breast cancer oncogenesis and as an adjuvant treatment following surgery to reduce local recurrence.

BACKGROUND Benign breast diseases (BBDs) are a common cause of breast problems in up to 30 % of women. The pathogenesis of BBD involves disturbances in the breast physiology. There is a necessity for thorough comprehensive, prompt diagnosis and management of the benign conditions to reduce the physical and psychological illness of the patients who are anxious about the possibility of the disease being a malignant one. METHODS This was a prospective study conducted in the Department of Surgical Oncology, GSL medical college from October 2016 to July 2018. Females presenting with breast lump, confirmed with fine needle aspiration cytology (FNAC), aged between 12 – 55 years and fit for surgery were included in the study. Those who did not submit the informed consent were excluded. Statistical analysis was performed by SPSS version-21 and MS Excel 2013. RESULTS Most (74 %) of the patients were between 20 and 40 years of age, 56 % were diagnosed to be fibroadenoma. In the study group, 82 % (41) participants with BBD were married. The most common presenting complaint in the study group was lump (50 %) in the breast followed by lump associated with pain (28 %). Patients with fibroadenoma mass had well defined features. CONCLUSIONS Lump was the most common clinical presentation followed by lump with pain. Based on the FNAC report, decision is made for management. Comprehensive and prompt preoperative diagnosis and management of the benign conditions is necessary to reduce the physical and psychological burden on the patients who are anxious about the possibility of malignancy. Abstract Background Breast cancer, although the most frequently diagnosed malignant tumor in humans, has a less clear etiology compared to other frequent cancer types. Mouse-mammary tumor virus (MMTV) is involved in breast cancer in mice and dogs and might play a role in the etiology of some breast cancers in humans, since it has been identified in 20-40% of breast cancer samples in Western Europe, USA, Australia and some other parts of the world’s population. The purpose of our study was to identify MMTV DNA sequences in breast tissue samples from breast cancer patients who underwent curative surgery in our regional center in Romania, EU. MethodsWe selected 75 patients with non-metastatic breast cancer treated surgically with curative intent, which did not undergo any neoadjuvant treatment. Out of these patients, 50 underwent radical lumpectomy and 25 modified radical mastectomy. We searched using PCR the MMTV-like DNA env sequence in the breast cancer tissue and normal breast tissue obtained from the same patients. ResultsNone of the examined samples was positive for MMTV-like target sequences on PCR.ConclusionsWe could not prove that MMTV plays a role in the etiology of breast cancer in our patient group. This finding is similar to publications of other geographically related research groups and might be due to the fact that only the Mus musculus domesticus mouse species was proven to carry infectious MMTV, but not the Mus musculus musculus species, which is specific to South-Eastern Europe (including Romania) and some parts of Asia.

Abstract Introduction We aimed to assess the accuracy of ultrasound elastography in detecting pediatric malignant cervical lymph nodes, and if this modality can obviate the need for surgical biopsies. Material and Methods A prospective study from September 2017 to September 2020 included 64 children with persistent cervical lymphadenopathy. Patients were evaluated by meticulous history and physical assessment. B-mode ultrasound, color Doppler, and sonoelastography were conducted thereafter. Elastography scans were classified into five patterns, and patterns from 3 to 5 were considered as malignancies. All children underwent open biopsies followed by pathological examination. Results of tissue diagnosis were compared with patterns of elastography to determine its accuracy. Results Twenty-eight patients (43.8%) had malignant nodes and the remaining 36 (56.2%) were due to benign causes. Elastography patterns of 1 and 2 were documented in 30 patients, and all of them were diagnosed as benign lesions. Patterns of 3 to 5 were demonstrated in 34 patients. Out of them, 28 were confirmed as malignancies, while 6 children were of benign nature (false positive). Ultrasound elastography achieved sensitivity and specificity of 100 and 85.7%, respectively, and an overall accuracy of 90.6% in the differentiation between malignant and benign entities. The overall accuracy of B-mode and color Doppler were 75 and 82.2%, respectively. Conclusion Elastography is a useful tool that should be added to ultrasound modalities during the diagnosis of pediatric cervical lymphadenopathy. Surgical biopsy in eligible patients is imperative to commence proper therapy or to discharge the child. Despite favorable results of elastography, it cannot replace surgical biopsy or change its indications.

Background: Congenital talipes equinovarus is the most common congenital foot disorder. neglected of clubfoot deformity occurs after the treatment by Ponseti method .we evaluate the relapse pattern of clubfoot basis of bhasker et al then treat the patient by Ponseti technique. Methods: This is a Prospective study .We are conducting a clubfoot clinic since 2013 in Department of Orthopaedics Gandhi medical college Bhopal . We register all clubfoot patient at our clubfoot clinic on his/her rst visit. All the clubfeet assessed with Pirani scoring system on the initial presentation to our institution and treat by Ponseti method. During this study there were a total of 558 children with 713 idiopathic clubfeet registered at our clinic. A total of 80 neglected clubfeet in 56 children presenting with neglected clubfeet were included in our study. Results: We found that The neglected feet in our study is (80 feet out of 713 feet. The male patient is 38 (67.86%) and female patient is 18 (32.14%). Male were predominantly involved as compared to female.The post treatment Pirani score after relapse treatment is 0.40 Conclusion: Ponseti technique is a simple, effective, inexpensive method for treatment of clubfeet there is, no requirement of special setup with limited resources and less rate of recurrence/complication than the surgical treatment It has been concluded that Ponseti method is a effective technique to treat congenital idiopathic clubfeet with success rate is 94.65% in our study

Objectives: To see the (a) type of congenital heart disease, (b) Clinical presentation of the cases, (c) association with extra-cardiac anomalies and disease, (d) complications of different CHD, (e) outcome of patients during hospital stay. Methodology: it was a prospective study conducted in the department of pediatrics of Rajshahi Medical College & Hospital over a period of one year.115 children from birth to 12 years of age who had congenital heart disease confirmed by echocardiography were included. All patients were treated conservatively and observed for immediate out come during the hospital stay. Result: major types of CHD were VSD (42.6%), TOF (18.3%), ASD (14.8%), PDA (7.8%). Male outnumbers female child. Common symptoms were breathlessness (60%), fatigue (54.8%), cough (43.5%), poor weight gain (41.7%), recurrent chest infection (34.8%), fever (28.7%), feeding problems (26.1%), palpitation (21.7%) and bluish discoloration of lips and fingertips (20%). Murmur with or without thrill and cardiomegaly was the most important cardiac finding. Lymphadenopathy is one of the common clinical presentation of patients suffering from different disease processes. Lymphnode enlargement may be due to reactive changes, tuberculosis, sarcoidosis, Hodgkin lymphoma, Non Hodgkin lymphoma, Metastatic deposits etc. Histopathological examination of the enlarged lymphnode is the gold standard method to ascertain the cause of the enlargement. The objective of our study was to explore the causes of lymphadenopathy in the population of Chittagong. The study was conducted in the department of Pathology, Chattagram Maa-O-Shishu Hospital Medical College, Chittagong during January 2012 to July 2014. A total of 90 (Ninety) lymphoid tissue specimens were received and Hematoxilin Eosin stained histopathological slides were prepared and microscopic examination was done. The most common morphologic changes of lymphadenopathy noted was reactive hyperplasia (36.66%) and other causes included were tuberculosis (27.77%), metastasis (20%), Non-Hodgkin lymphoma (7.77%), Hodgkin lymphoma (4.44%) and suppurative lymphadenitis (3.33%). Background: Anal fistula is an abnormal communicative small channel that has an internal opening and an external opening and connected by the primary track. Our study evaluate the effectiveness of seton in high variety anal fistula.Objectives: Evaluation the efficacy and safety of seton as surgical management of high anal fistula. Methods: This is a prospective study done on 57 patients in BIRDEM hospital and some other clinic of Dhaka city in 3 years period with high variety of anal fistula which is above the dentate line and were treated with seton .Outcome measured during follow up period were- successfully healed, recurrence, incontinence, percentage of complications and patients satisfaction.Result: Among 57 patients mean age was 38.2+_6.8 years. Overall outcome of the patients showed-fistula completely healed in 51 patients, incontinence occurred in 2 patients and recurrence occurred in 4 patients.Conclusion: Seton is relatively safe, effective and low cost for the management of high anal fistula with low rate of incontinence.
