Background: Venous thromboembolism (VTE) is a frequent and potentially lethal condition. Venous thrombi are mainly constituted of fibrin and red blood cells, but platelets also play an important role in VTE formation. Information about VTE in patients with thrombocytopenia is, however, missing. Objectives: To identify VTE risk factors and describe treatment and outcome (bleeding episodes and mortality) in patients with thrombocytopenia. Patients/Methods: Patients with thrombocytopenia (platelet count <100 × 109/L) admitted to Odense University Hospital, Denmark, between April 2000 and April 2012 were included. Fifty cases had experienced VTE. Controls without VTE were matched 3:1 with cases on sex and hospital department. Medical records were examined, and data were analyzed using conditional logistic regression. Results: In multivariate analysis, platelet count <50 × 109/L (odds ratio [OR] 0.22, P < .05) and chronic liver disease (OR 0.05, 95% confidence interval [CI] 0.01-0.58) reduced the risk of VTE. Surgery (OR 6.44, 95% CI 1.37-30.20) and previous thromboembolism (OR 6.16, 95% CI 1.21-31.41) were associated with an increased VTE risk. Ninety-two percent of cases were treated with anticoagulants. There was no difference in bleeding incidence between cases and controls. Conclusions: Several known VTE risk factors also seems to apply in patients with thrombocytopenia. Also, patients with thrombocytopenia may be VTE risk stratified based on platelet count and comorbidities. Finally, patients having thrombocytopenia with VTE seem to be safely treated with anticoagulants without increased occurrence of bleeding.

Remdamiesi ligos istorijomis ir asmenine patirtimi, retrospektyviai išanalizavome per 35 metus gydytas pūslines makšties fistules. Kartu pateikiame trumpą istorinės bei šiuolaikinės literatūros apžvalgą. Ligoniai ir metodai Į Vilniaus miesto universitetinę ligoninę 1969–2004 m. pateko 51 moteris, kuriai susidarė pūslinė makšties fistulė. Iš jų 46 (90% moterims) fistulė atsivėrė po histerektomijos 1–14 pooperacinę dieną. Visos ligonės skundėsi daliniu ar visišku šlapimo nelaikymu. Šlapimo pūslė drenuota Foley kateteriu. Atlikti įprasti urologiniai, ginekologiniai ir klinikiniai tyrimai. Skirti antibiotikai. Fistulės kraštų elektrokoaguliaciją atlikome 13, o atvirąsias transvaginalines, transvezikines ir transabdominalines operacijas (65 kartus) – 47 ligonėms, iš jų devynioms – 18 pakartotinių antrinių. Rezultatai Nė viena fistulė neužgijo savaime; 4 (iš 13) užgijo po transvezikinės elektrokoaguliacijos ir 45 (iš 47) – po atvirųjų operacijų. Pasveiko (išgijo) 96% ligonių. Dviejų (4%) ligonių gydymas buvo neveiksmingas. Išvados Makšties pūslinė fistulė – retai pasitaikanti komplikacija; per 35 metus gydėme tik 51 tokią ligonę, iš jų 34 buvo atsiųstos iš kitų ligoninių. Histerektomija – dažniausia (90%) šios fistulės priežastis. Dalį mažų fistulių užgydo endovezikinė jų kraštų elektrokoaguliacija. Dažniausiai pasirinktinas vaginalinis operacinis fistulės užgydymo būdas, dalyvaujant urologui ir ginekologui. Based on case histories and mostly on our experience, we have made a retrospective analysis with a brief summary of historical and modern literature. Patients and methods There were 51 patients with VVF admitted to Vilnius Cty University Hospital Urological Department in 1969–2004. In 46 (90%) VVF opened after hysterectomy on 1–14 postoperative day. All (51) suffered from partial or complete urine leakage. Urinary blader was drained with a Foley catheter. Routine urological, gynaecological and common clinical analyses were made. Antibiotics were prescribed. Electrocoagulation of small and fresh fistula edges was performed in 13 cases, open transvaginal and transabdominal operations were made for 47 patients (65 times), in 9 of them 18 were repeated operations. Results No one of the VVF closed after conservative treatment. Four from 13 closed after transvesical electrocoagulation and 45 from 47 after open operations. Totally VVF closed in 96% of patients. In 2 cases the treatment was unsuccessful. Conclusions VVF is a rare complication: during 35 years we treated only 51 patients, in 17 of them VVF opened after procedures in our hospital and 34 elsewhere. In 90% the reason for VVF was hysterectomy. Part of small fistulas can be closed by endovesical electrocoagulation of fistula edges. In most cases vaginal approach for closuring VVF must be chosen with both the urologist and gynaecologist participating in the operation. A polyolefin with certified biocompatibility according to USP class VI was used by our group as feedstock for filament-based 3D printing to meet the highest medical standards in order to print personal protective equipment for our university hospital during the ongoing pandemic. Besides the chemical resistance and durability, as well as the ability to withstand steam sterilization, this polypropylene (PP) copolymer is characterized by its high purity, as achieved by highly efficient and selective catalytic polymerization. As the PP copolymer is suited to be printed with all common printers in fused filament fabrication (FFF), it offers an eco-friendly cost–benefit ratio, even for large-scale production. In addition, a digital workflow was established focusing on common desktop FFF printers in the medical sector. It comprises the simulation-based optimization of personalized print objects, considering the inherent material properties such as warping tendency, through to validation of the process chain by 3D scanning, sterilization, and biocompatibility analysis of the printed part. This combination of digital data processing and 3D printing with a sustainable and medically certified material showed great promise in establishing decentralized additive manufacturing in everyday hospital life to meet peaks in demand, supply bottlenecks, and enhanced personalized patient treatment.

Between 1984 and 1994, 23 diabetic patients were admitted to a university hospital/Department of Veterans Affairs (DVA) Medical Centre with a primary or secondary diagnosis of hand infection. We also identified 726 patients admitted to all DVA Medical Centres during the years 1989 through 1994 with co-diagnoses of diabetes mellitus and hand infection. Contrary to previously published literature, most diabetic hand infections are non-specific, confined to the soft tissues, and respond to broad spectrum parenteral antibiotic therapy. When surgery is necessary, most patients do not require amputation.

Introduction: Motherese, or emotional infant directed speech (IDS), is the specific form of speech used by parents to address their infants. The prosody of IDS has affective properties, expresses caregiver involvement, is a marker of caregiver-infant interaction quality. IDS prosodic characteristics can be detected with automatic analysis. We aimed to explore whether pregnant women “speak” to their unborn baby, whether they use motherese while speaking and whether anxio-depressive or obstetrical status impacts speaking to the fetus.Participants and Methods: We conducted an observational study of pregnant women with gestational ages from 26 to 38 weeks. Women were recruited in a university hospital department of obstetrics. Thirty-five women agreed to participate in the study, and 26 audio records were exploitable. We collected obstetrical and sociodemographic data, pregnancy outcomes, anxiety and depressive status using the Covy and Raskin Scales, and life events using the Sensations During Pregnancy and Life Event Questionnaire. Each participant was left alone with an audio recorder with a recommendation to feel free to speak to her fetus as she would have done at home. The recording was stopped after 3 min. Audio recordings were analyzed by two methods: psycholinguist experts' annotation and computational objective automatic analyses.Results: Most mothers (89%) reported speaking to their fetuses. We found a correlation between maternal first perceptions of fetal movements and the start of mother's speaking to fetus. Motherese prosody was detected with both annotation and automatic analysis with a significant correlation between the two methods. In this exploratory study, motherese use was not associated with maternal anxiodepressive or obstetrical status. However, the more future mothers were depressed, the less they spoke with their fetuses during the recording.Conclusion: Fetal directed speech (FDS) can be detected during pregnancy, and it contains a period of prosody that shares the same characteristics of motherese that can be described as prenatal motherese or emotional fetal-directed speech (e-FDS). This means that pregnant women start using motherese much earlier than expected. FDS seems to be correlated with maternal first perceptions of fetal movements and depression scores. However, more research is needed to confirm these exploratory results.
