IMMEDIATE IMPLANTATION AND AESTHETIC COMPONENT AS A RESULT OF SUCCESSFUL FORECAST TREATMENT

The aim: Improving the method of immediate implantation in the aesthetic zone in case of bone deficiency to obtain the highest aesthetic and predictable treatment result. Materials and methods: Under clinical observation were 32 patients with different clinical diagnoses in the anterior part of the upper jaw aged 30 to 55 years. In the course of recent advances, the following methods have been used: clinical protocol of immediate implantation with passive exceptional loads by temporary orthopedic constructions, X-ray method using cone-beam computed tomography, statistical analysis. Results: After surgical treatment of patients 1 year after surgery, the distribution of biotypes was as follows: in group 1 – thick biotype 12.87%, medium – 87.13%; in group 2 – thick biotype 27.04%, medium – 72.96%, with p <0.05. According to the results of CT, the distance between the implant and the vestibular in the first group was after 6 months – 1.67 ± 0.04 mm (p <0.05); in the second group of the study we obtained the following results after 6 months – 1.59 ± 0.06 mm (p <0.05). Conclusions: The advanced method of immediate implantation in the anterior part of the upper jaw allows to change the biotype of soft tissues, improve the color spectrum of the gums, increase the thickness of soft tissues with connective tissue autograft, and increase gum density and fixation of osteoplastic material in the presence of defect ), as well as reduce the risk of recession.

Relevance. Staged orthopedic treatment was used to improve the quality of life of patients who underwent radical maxillofacial surgeries for cancer.Patients and methods. 197 patients receiving treatment for maxillofacial cancer were observed at the Department of head and neck tumors, National Medical Research Centre for Oncology of the Ministry of Health of Russia, in 1998- 2018. All patients underwent radical surgical treatment resulting in postoperative defects of the upper jaw, soft tissues of the zygomatic- buccal-orbital region, nose, or auricle.Results. Removable obturator prostheses with various supporting and retaining elements were made for 159 (80.7 %) patients. Individual facial prostheses were made for 38 (19.3 %) patients: 17 (44.7 %) – external orbital prostheses, 14 (36.8 %) – external nasal prostheses, 6 (15.8 %) – external zygomatic- buccal-orbital prostheses, 1 (2.7 %) – external auricle prosthesis. Combined prostheses were made for 4 patients– removable upper jaw obturator and nose prosthe[1]sis; removable upper jaw obturator and eye prosthesis. Combined prostheses were fixed to each other using magnets. The results of maxillofacial prosthetics were evaluated according to the aesthetic requirements of the patients and their quality of life. Maxillofacial prostheses allowed a complete restoration of chewing, swallowing, and speaking, restored facial deformation, and improved the appearance of patients.Conclusions. Timely and comprehensive orthopedic treatment of patients with postoperative maxillofacial defects after radical surgeries for malignant tumors takes the main place in the complex of rehabilitation measures. Early elimination of extensive defects is aimed at maximum restoration of oral dysfunctions and appearance preservation. The apparent advantages of maxillofacial prostheses involve improvement of social adaptation and the quality of life of patients, which promotes complete rehabilitation and a return to socially useful activities.

Relevance. Skeletal Class III malocclusion is one of the most difficult to correct. Genetics, environmental factors, and postembryonic development influence its etiology. Sagittal expansion of the upper jaw will ensure the correct position of the lower jaw, which in turn will improve the patient's aesthetic parameters and health. The study examines a treatment method using the Fixed anterior growth guidance appliance (FAGGA).Materials and methods. This clinical case presents a 21-year-old male with skeletal class III due to maxillary bone deficiency. The patient refused surgery to increase the size of the upper jaw and opted for orthodontic treatment. The latter was performed using a Fixed anterior growth guidance appliance (FAGGA), followed by a rapid palatal (maxillary) expansion (RME) and brackets. We removed the FAGGA after eight months. The profile and occlusion improved.Results. The change in the inclination and protrusion of the maxillary incisors improved the profile. We received 2mm of space behind the upper right canine and 1.5mm of space behind the left one and the SNA angle increased by 2 degrees. The treatment continues with RME and brackets.Conclusions. An increase in inclination and protrusion of the maxillary incisors and a slight skeletal change improved the aesthetic parameters of the facial area.

Introduction: In the scenario of extraction associated with immediate implantation in the aesthetic area, tooth extraction is common in the daily clinical practice of the dentist, with the main causes of caries, periodontal disease, and coronal-radicular fractures. All extractions must be performed with precise indication, given defined prosthetic planning, thus avoiding an imbalance in the occlusion, swallowing, and aesthetics of the patient. Objective: To carry out a systematic review on the main approaches to extraction associated with immediate implantation in the aesthetic area. Methods: Followed a systematic review model (PRISMA). The search strategy was carried out in the PubMed, Embase, Ovid, Cochrane Library, Web Of Science, and Scopus databases. The quality of the studies was based on the GRADE instrument and the risk of bias was analyzed according to the Cochrane instrument. Results: A total of 134 studies were found that were submitted to the eligibility analysis and, after that, 41 studies of high to medium quality and with risks of bias were selected that do not compromise the scientific basis of the studies. According to the main literary findings, the use of the technique of installing immediate implants, after extraction, requires planning and care in the management of soft tissues, to correct aesthetic sequelae. The use of precise and minimally traumatic techniques enabled satisfactory and functional aesthetic results, as well as improving the patient's aesthetics and self-esteem. Conclusion: The immediate implant placement procedure after tooth extraction preserves bone height and thickness, reduces treatment time and cost, in addition to maintaining the gingival architecture, being important for the aesthetic success of future prosthetic rehabilitation. Also, it presents success rates comparable to implants in fully healed edges, the extraction must be done in a minimally traumatic way, to preserve the maximum bone tissue. The horizontal defects present after the installation of the immediate implant (gaps), if they are less than or equal to 3mm, will heal with complete bone filling. However, if they are larger than 3mm, bone graft material and/or membrane should be used so that there is bone healing.

Aim. To assess the geriatric and somatic status of older patients with falls.Materials and methods. The study included 655 patients (81% of them women) with the age of 75.1±8.2 years hospitalized in geriatric departments. Depending on whether they experienced falls, the patients were divided into two groups: group 1 (332 people, 50.7%) consisting of patients who experienced at least one fall in the current year and group 2 (323 people, 49.3%) consisting of patients who did not experience any falls. The patients underwent general clinical tests and comprehensive geriatric assessment.Results. Half of the hospitalized patients over the age of 60 experienced falls, and among patients without a fall experience 50.8% had a high risk of falls. As a result of falls 246 (74.1%) patients suffered from bruises and abrasions of soft tissues, 36 (10.8%) patients had fractures of various localizations, 6 (1.8%) patients had traumatic brain injury (TBI). Patients of both groups were diagnosed with an average of 5 diseases while there were no differences in the nosological structure among patients of both groups. As a result of the performed comprehensive geriatric assessment patients with falls were significantly more often diagnosed with frailty, dementia, depression, visual impairment, gait and balance disorders as well as orthostatic reactions.

The paper presents the results of the study of pathomorphological changes in organs in ornithobacteriosis in decorative pigeons with the establishment of the underlying disease, including the pathological signs associated and pathognomonic for the disease. The objects of the study were the corpses of fallen pigeons of the English postal and peacock breeds, at the age of 2-3 years, kept in a private farm on the territory of the Yemelyanovskiy district of the Krasnoyarsk Territory. Bacteriological examination of the material revealed the causative agent of ornithobacteriosis (Ornithobacterium rhinotracheale), and the causative agent of colibacillosis (Escherichia coli) was also found in 70,0 % of cases. In 70 % of the individuals, a complication was observed in the form of colibacillosis, characterized by serous-catarrhal gastroenterocolitis, spleen hyperplasia, and serous polyarthritis, to the underlying disease. The features of ornithobacterial infection in pigeons include the absence of pronounced serous-inflammatory edema of soft tissues in the head area, signs of serous-fibrinous aerosacculitis and the onset of the disease in birds of the older age group (2-5 years).

As postoperative edema is more relevant in such surgery due to its localization, soft tissue healing can take longer and pain can restrict function. There is no universal approach to surgery aftercare among foot and ankle surgeons. Although infections following foot and ankle surgery are rare, soft tissue healing can be jeopardized after extensive and multiple approaches. Postoperative edema, however, is common and contributes to unfavorable conditions for wound healing. Our study addresses the results of avoiding postoperative edema with the use of the Jones dressing cast. Methods: We defined a precise fixation technique of the foot and ankle in the immediate post-operative phase using what we call ‘the Jones dressing cast’. This technique is a modification of the Jones dressing bandage. In a previous study we compared two groups of patients (N = 20/23) who underwent similar reconstructive surgery with the application of the described cast for one week and without. At the twomonth follow-up we observed that the group treated with the cast required less pain relief, spent less time in hospital and achieved faster autonomy using crutches (Gottlieb and Klaue, 2013). In this study we considered a group of 45 patients who underwent similar reconstructive procedures to those in the first study and who were treated with the cast for two weeks post-operatively. There was no visual check of the soft tissues before removal of the cast. Results: Although patients in Group 2 had more complex surgery, mobility was achieved earlier and hospitalization time was shorter. The incidence of infection was lower in the group wearing the cast for 2 weeks. The patients treated with the cast for 2 weeks regained autonomous mobility significantly earlier than those in Group 1 (cast 1 week or less). Conclusion: The Jones dressing cast is a useful tool that can help minimize postoperative complication rates. Wearing the cast for two weeks is preferable. Our study suggests its effectiveness in reducing soft tissue complications and facilitating independent mobility.

In practice, dental surgeon often performs the operation on removal of impacted third lower molar. Oppression of general and local immunity and violation of regeneration are always observed in a patient with gastric ulcer and duodenal ulcer. The purpose of this work was to determine by the use of X-ray method the efficiency of application of proposed combination of preventive and curative bandages, including silver osteoplastic material (BIOMIN Gtls) and platelet-rich plasma autologous blood (РRР) on the post-extraction wound in patients with peptic ulcer. The basic material of research is the data obtained during the examination and treatment of 55 patients of both sexes who were on treatment at the departments of dentistry and gastroenterology of Crimean Republican clinical hospital with the diagnosis of gastric and 12 duodenal ulcers associated with H. Pylori. These patients needed the extraction of low third impacted molars. Radiographic evaluation processes of the organization of the alveolus tooth and bone tissue restoration was carried out on the basis of the analysis of indicators: the timing of forming osteogenic structures in apical part and along the walls of the hole, the rate of creation of large alveolar spongy bone, time of resorption contours of the walls of the alveoli. The X-ray examination was performed before the operation of tooth extraction, and on the 1st, 3rd, 6th and 12th months after treatment procedure as well. In comparison with the traditional method of surgical wounds, the use of silver osteoplastic material (Biomin HTSA) and platelet-rich plasma autologous blood (РRР) allows to optimizing the conditions for the repair processes in the bone, reduce the time of formation of full-fledged regenerate.

Opportunistic diseases in HIV-infected patients have changed since the introduction of highly active anti-retroviral therapy (HAART). This study aims at evaluating the frequency of associated diseases in patients with AIDS admitted to an university hospital of Brazil, before and after HAART. The medical records of 342 HIV-infected patients were reviewed and divided into two groups: group 1 comprised 247 patients before HAART and, group 2, 95 patients after HAART. The male-to-female rate dropped from 5:1 to 2:1for HIV infection. There was an increase in the prevalence of tuberculosis and toxoplasmosis, with a decrease in Kaposi's sarcoma, histoplasmosis and cryptococcosis. A reduction of in-hospital mortality (42.0% vs. 16.9%; p = 0.00002) has also occurred. An agreement between the main clinical diagnoses and autopsy findings was observed in 10 out of 20 cases (50%). Two patients with disseminated schistosomiasis and 2 with paracoccidioidomycosis are reported. Overall, except for cerebral toxoplasmosis, it has been noticed a smaller proportion of opportunistic conditions related to severe immunosuppression in the post HAART group. There was also a significant reduction in the in-hospital mortality, possibly reflecting improvement in the treatment of the HIV infection.
