Predicting Success with Hearing Aids in Everyday Living

Persons with impaired hearing who are candidates for amplification are not all equally successful with hearing aids in daily living. Having the ability to predict success with amplification in everyday life from measures that can be obtained during an initial evaluation of the patient's candidacy would result in greater patient satisfaction with hearing aids and more efficient use of clinical resources. This study investigated the relationship between various demographic and audiometric measures, and two measures of hearing aid success in 50 hearing aid wearers. Audiometric predictors included measures of audibility and suprathreshold distortion. The unaided and aided signal-to-noise ratio (SNR) loss on the QuickSIN test provided the best predictors of hearing aid success in daily living. However, much of this predictive relationship appeared attributable to the patient's age.

Following the Harvard master hearing aid study in 1947 there was little research on linear amplification. Recently, however, there have been a number of studies designed to determine the relationship between the frequency-gain characteristic of a hearing aid and speech intelligibility for persons with sensorineural hearing loss. These studies have demonstrated that a frequency-gain characteristic that rises at a rate of 6 dB/octave, as suggested by the Harvard study, is not optimal. They have also demonstrated that high-frequency emphasis of 10–40 dB above 500–1000 Hz is beneficial. Most importantly, they have demonstrated that hearing aids as they are presently being fit do not provide maximum speech intelligibility. Percent word correct scores obtained with the best frequency-gain characteristics tested in various studies have been found to be 9 to 19 percentage points higher than scores obtained with commercial aids owned by subjects. This increase in scores is equivalent to an increase in signal-to-noise ratio of 10 to 20 dB. This is a significant increase which could allow impaired listeners to communicate in many situations where they presently cannot. These results demonstrate the need for further research on linear amplification aimed at developing practical suggestions for fitting hearing aids.

Purpose Audiology clinics have a need for a nonlinguistic test for assessing speech scores for patients using hearing aids or cochlear implants. One such test, the Spectral-Temporally Modulated Ripple Test Lite for computeRless Measurement (SLRM), has been developed for use in clinics, but it, as well as the related Spectral-Temporally Modulated Ripple Test, has primarily been assessed with cochlear implant users. The main goal of this study was to examine the relationship between SLRM and the Arizona Biomedical Institute Sentence Test (AzBio) for a mixed group of hearing aid and cochlear implant users. Method Adult hearing aid users and cochlear implant users were tested with SLRM, AzBio in quiet, and AzBio in multitalker babble with a +8 dB signal-to-noise ratio. Results SLRM scores correlated with both AzBio recognition scores in quiet and in noise. Conclusions The results indicated that there is a significant relationship between SLRM and AzBio scores when testing a mixed group of cochlear implant and hearing aid users. This suggests that SLRM may be a useful nonlinguistic test for use with individuals with a variety of hearing devices.

Background: Although the benefits of amplification for persons with impaired hearing are well established, many potential candidates do not obtain and use hearing aids. In some cases, this is because the individual is not convinced that amplification will be of sufficient benefit in those everyday listening situations where he or she is experiencing difficulties. Purpose: To describe the development of a naturalistic approach to assessing hearing aid candidacy and motivating hearing aid use based on patient preferences for unamplified and amplified sound samples typical of those encountered in everyday living and to assess the validity of these preference ratings to predict hearing aid candidacy. Research Design: Prospective experimental study comparing preference ratings for unamplified and amplified sound samples of patients with a clinical recommendation for hearing aid use and patients for whom amplification was not prescribed. Study Sample: Forty-eight adults self-referred to the Army Audiology and Speech Center for a hearing evaluation. Data Collection and Analysis: Unamplified and amplified sound samples were presented to potential hearing aid candidates using a three-alternative forced-choice paradigm. Participants were free to switch at will among the three processing options (no gain, mild gain, moderate gain) until the preferred option was determined. Following this task, each participant was seen for a diagnostic hearing evaluation by one of eight staff audiologists with no knowledge of the preference data. Patient preferences for the three processing options were used to predict the attending audiologists' recommendations for amplification based on traditional audiometric measures. Results: Hearing aid candidacy was predicted with moderate accuracy from the patients' preferences for amplified sounds typical of those encountered in everyday living, although the predictive validity of the various sound samples varied widely. Conclusions: Preferences for amplified sounds were generally predictive of hearing aid candidacy. However, the predictive validity of the preference ratings was not sufficient to replace traditional clinical determinations of hearing aid candidacy in individual patients. Because the sound samples are common to patients' everyday listening experiences, they provide a quick and intuitive method of demonstrating the potential benefit of amplification to patients who might otherwise not accept a prescription for hearing aids.

Four hearing aid arrangements (monaural-omnidirectional, monaural-directional, binaural-omnidirectional, binaural-directional) and a number of FM system-personal hearing aid combinations (including direct input, neck loop, and silhouette inductor—monaural and binaural—and environmental microphone on and off) were evaluated in a school classroom on nine children with mild-to-moderate sensorineural hearing losses. Two measures of speech recognition in noise were employed. First, the signal-to-noise ratio (S/N) yielding 50% identification of spondees was determined using a simple up-down adaptive procedure. Second, word recognition scores were obtained for three amplification arrangements at two different S/Ns (+6 and + 15 dB). The average FM advantage over a personal hearing aid was equivalent to a 15-dB improvement in S/N. Activation of the hearing aid microphone caused most of the FM advantage to disappear. The benefit offered by the FM system decreased as the environmental S/N increased but remained significant even at +15 dB. Significant improvement also was found with the use of directional as compared to omnidirectional microphones, both in the hearing aids and FM teacher microphone.

Purpose Hearing loss self-management refers to the knowledge and skills people use to manage the effects of hearing loss on all aspects of their daily lives. The purpose of this study was to investigate the relationship between self-reported hearing loss self-management and hearing aid benefit and satisfaction. Method Thirty-seven adults with hearing loss, all of whom were current users of bilateral hearing aids, participated in this observational study. The participants completed self-report inventories probing their hearing loss self-management and hearing aid benefit and satisfaction. Correlation analysis was used to investigate the relationship between individual domains of hearing loss self-management and hearing aid benefit and satisfaction. Results Participants who reported better self-management of the effects of their hearing loss on their emotional well-being and social participation were more likely to report less aided listening difficulty in noisy and reverberant environments and greater satisfaction with the effect of their hearing aids on their self-image. Participants who reported better self-management in the areas of adhering to treatment, participating in shared decision making, accessing services and resources, attending appointments, and monitoring for changes in their hearing and functional status were more likely to report greater satisfaction with the sound quality and performance of their hearing aids. Conclusion Study findings highlight the potential for using information about a patient's hearing loss self-management in different domains as part of clinical decision making and management planning.

A custom in-the-ear (ITE) hearing aid fitting was compared to two over-the-ear (OTE) hearing aid fittings for each of 9 subjects with mild to moderately severe hearing losses. Speech intelligibility via the three instruments was compared using the Speech Intelligibility Rating (SIR) test. The relationship between functional gain and coupler gain was compared for the ITE and the higher rated OTE instruments. The difference in input received at the microphone locations of the two types of hearing aids was measured for 10 different subjects and compared to the functional gain data. It was concluded that (a) for persons with mild to moderately severe hearing losses, appropriately adjusted custom ITE fittings typically yield speech intelligibility that is equal to the better OTE fitting identified in a comparative evaluation; and (b) gain prescriptions for ITE hearing aids should be adjusted to account for the high-frequency emphasis associated with in-the-concha microphone placement.

The aims of this study were to investigate hearing aid satisfaction for a group of older Australians fitted with government-funded hearing aids using the Satisfaction with Amplification in Daily Life (SADL) questionnaire; to compare the Australian data gathered with the provisional normative data reported by Cox and Alexander (1999); and to investigate the relationship between SADL satisfaction and several participant variables, hearing aid variables, and other outcome measures.The SADL questionnaire and a Client Satisfaction Survey (CSS) were distributed by mail to 1284 adults fitted with government-funded hearing aids three to six months previously. 1014 surveys were returned. The mean age of participants was 75.32 years; 54.4% of participants were male, and 54.8% were fitted binaurally. Participants were fitted primarily with digitally programmable hearing aids of various styles (22.5% BTEs, 34.8% ITEs, 41.8% ITCs, 0.9% nonstandard [NS] devices).Overall, participants reported a considerable level of satisfaction with their devices. SADL Global and subscale scores were significantly higher for the Australian sample than the U.S. norms described by Cox and Alexander (1999).

Improving the signal-to-noise ratio (SNR) for individuals with hearing loss who are listening to speech in noise provides an obvious benefit. Although binaural hearing provides the greatest advantage over monaural hearing in noise, some individuals with symmetrical hearing loss choose to wear only one hearing aid. The present study tested the hypothesis that individuals with symmetrical hearing loss fit with one hearing aid would demonstrate improved speech recognition in background noise with increases in head turn. Fourteen individuals were fit monaurally with a Starkey Gemini in-the-ear (ITE) hearing aid with directional and omnidirectional microphone modes. Speech recognition performance in noise was tested using the audiovisual version of the Connected Speech Test (CST v.3). The test was administered in auditory-only conditions as well as with the addition of visual cues for each of three head angles: 0°, 20°, and 40°. Results indicated improvement in speech recognition performance with changes in head angle for the auditory-only presentation mode at the 20° and 40° head angles when compared to 0°. Improvement in speech recognition performance for the auditory + visual mode was noted for the 20° head angle when compared to 0°. Additionally, a decrement in speech recognition performance for the auditory + visual mode was noted for the 40° head angle when compared to 0°. These results support a speech recognition advantage for listeners fit with one ITE hearing aid listening in a close listener-to-speaker distance when they turn their head slightly in order to increase signal intensity.

Impaired hearing and the use of hearing aids are often perceived negatively. Many adults deny hearing loss and reject amplification, in part due to such stigma. Women and men differ in how they age and adjust to impaired hearing, yet little is known specifically about women’s perceptions of stigma related to hearing loss and hearing aid use. The purpose of this study was to examine the degree of stigma associated with hearing loss and hearing aid use among women in three age groups (35–45 years, 55–65 years, and 75–85 years). Participants were 191 women with hearing within normal limits based on age-related norms. Using pairs of descriptors (i.e., semantic differentials), participants completed statements related to hearing loss and hearing aid use. Results suggest that negative perceptions associated with hearing loss and hearing aid use are affected by age. Younger women perceive greater stigma than older women. Less stigma is associated with hearing aid use than hearing loss, suggesting a positive effect of hearing loss management. Implications for clinical practice and marketing of hearing instruments are discussed.

Hearing aids are small electronic devices designed to improve hearing for persons with impaired hearing, using sophisticated audio signal processing algorithms and technologies. In general, the speech enhancement algorithms in hearing aids remove the environmental noise and enhance speech while still giving consideration to hearing characteristics and the environmental surroundings. In this study, a speech enhancement algorithm was proposed to improve speech quality in a hearing aid environment by applying noise reduction algorithms with deep neural network learning based on noise classification. In order to evaluate the speech enhancement in an actual hearing aid environment, ten types of noise were self-recorded and classified using convolutional neural networks. In addition, noise reduction for speech enhancement in the hearing aid were applied by deep neural networks based on the noise classification. As a result, the speech quality based on the speech enhancements removed using the deep neural networks—and associated environmental noise classification—exhibited a significant improvement over that of the conventional hearing aid algorithm. The improved speech quality was also evaluated by objective measure through the perceptual evaluation of speech quality score, the short-time objective intelligibility score, the overall quality composite measure, and the log likelihood ratio score.
