Summary. The purpose of the research was to explore the appropriateness of combined usage of enterosorption amplification and intratissular electrophoresis (ITE) of antibiotics in injured with the substantial thermal burns. Clinical effect of the enterosorbtion and ITE antibacterial drugs in burn disease cases was studied in 26 patients with superficial and deep dermal burns (IIb-III deg.) from 27 to 60 % of their body surface. Enterosgel was prescribed from 3rd day after injury for 15–20 g 3–4 times a day for 7–14 days. ITE of antibiotics conducted from the 3–4 day by the «Поток-1» device, applying the direct current with density 0,03-0,05 mAh/cm2 for 60 min. Materials and methods. The authors studied the leukocyte reaction by a general blood test. The degree of intoxication was determined by the level of molecules of average mass and blood plasma electrical conductivity. They evaluated the dynamics of the wound process, conducted bacteriological and cytological researches of the burn wounds in dynamics on 1–3, 6–7, 13–14, and 19-21 days after traumatization. Results and discussion. This treatment tactic allows to reduce the level of endogenous intoxication, to optimize the course of the wound process in the area of thermal damage, to intensify demarcation-cleansing processes and reparative regeneration, which increases the clinical effectiveness of treatment. Conclusions. As a result of the research, it was found that the combined usage of enterosorption detoxification by enterosgel and ITE of antibacterial drugs in the complex treatment of victims with widespread burns is appropriate and justified.

Summary. The purpose of our research was to explore the appropriateness of combined usage of enterosorption amplification and intratissular electrophoresis (ITE) of antibiotics in cases of substantial thermal burns. The effect of intratissular electrophoresis (ITE) of antibacterial drugs and enterosorption was studied in 22 patients with dermal thermal burns from 28 to 54 % of their body surface. ITE of antibiotics started from the 3–4 day applying the direct current with density 0,03-0,05 mAh/cm² for 60 min. by the «POTOK-1» device. The authors determined the intoxication degree based on the leukocyte index, the number of average mass molecules and blood plasma electrical conductivity. Also, they observed the course of wound process, conducted bacteriological and cytological researches of the burn wounds in dynamics on 1–3, 6–7, 13–14 and 19–21 days of the burn disease progress. It was found that enterosorption usage in the comprehensive treatment of burn disease allows not only to reduce the level of endogenous intoxication but also to provide an indirect optimizing impact on the progressing of the wound process in the zone of thermal damage, stimulate the reparative regeneration. In the cases with extensive burns, combined usage of the enterosorption detoxification by Enterosgel with the ITE of antibacterial drugs contribute more qualitative positive influence on the wound process and burn disease in general.

The clinical effectiveness o f complex enzymotherapy with preparation Vobenzim in treatment o f gestosis was studied in comparison with traditional therapy. The author analyzes the course оf gestation and delivery, and the state оf neonates in women with nephropathy o f I degree, who got common treatment (28 patients) and complex treatment including Vobenzim (17patients). Positive clinical effect o f complex enzymotherapy in case o f nephropathy is displayed in decreasing of edematic syndrome, eliminating or decreasing o f proteinuria, lowering o f blood pressure and impairment o f fetalhypoxia in labors. It goes along with the reduction o f immune complexes fixed in placenta.

Introduction. Degenerative-dystrophic changes in the spine are the most common chronic human diseases. Pronounced clinical manifestations of vertebrogenic diseases are observed during active labor activity and represent one of the most frequent causes of temporary disability. It is also known that any pain syndromes are accompanied by the development of psychovegetative disorders, which reduces the effectiveness of treatment. In recent years, there has been a reasonable increase in the interest of the medical community to the non-drug treatment methods. It is caused by the polypharmacotherapy side effects, an increased numbers of allergic reactions, problems with polypragmasia, and a low level of compliance. At the same time, the question of the possibility of various non-drug treatment methods combining is quite acute.The goal of research is to substantiate the clinical effectiveness of the combined application of osteopathic correction and reflexotherapy in the complex treatment of patients with dorsopathy at the cervical-thoracic level.Materials and methods. A prospective controlled randomized study was conducted on the basis of the medical clinics of LLC «Family Health» (Kazan) and ANO «Center for SEB assistance» (Kazan) from April 2018 to May 2019. The study involved 52 people with dorsopathy at the cervical-thoracic level, aged from 25 to 45 years. In accordance with the exclusion criteria, 7 people dropped out of the study. The remaining patients (45 people), depending on the used treatment method, were divided by a simple randomization method using a random number generator into three groups of 15 people. The first group (main group) received osteopathic correction and reflexotherapyon the background of standard drug therapy; the second and the third groups (control groups) received only osteopathic correction orreflexotherapy, respectively, on the background of standard drug therapy. The osteopathic examination was performed for all patients regardless of the group, before and after the course of therapy, with the formation of an osteopathic conclusion.Also there was the estimation of the degree of pain syndrome severity,the asthenia, and the degree of accumulated emotional and energy charge that does not get out in a person.Results. It was found in the course of the study that the inclusion of osteopathic correction and reflexotherapy in the complex therapy of patients with dorsopathy at the cervical-thoracic level is clinically more effective than the isolated use of these methods. Such complex therapy allows to achieve a more significant reduction in the severity of the pain syndrome by VAS (p<0,05), relief of internal emotional tension (p<0,05), and increase the effectiveness of correction of dominant somatic dysfunction. Based on the obtained data, it could be assumed that reflexotherapy potentiates the effects of osteopathic correction.Conclusion. Based on the study, it can be concluded that the combination of osteopathic correction and reflexotherapy in the treatment of patients with dorsopathy at the cervical-thoracic level increases the clinical effectiveness of the standard drug therapy. At the same time, it is worth noting the potentiating effect of the combined use of two non-medicinal methods. The question of combining of various non-drug treatment methods is quite acute today, so this study will be continued.

In order to include innovative technologies in clinical recommendations, confirmation of their clinical effectiveness in comprehensive treatment of burned patients is necessary. 1,696 case histories of patients with burns were audited, which are divided into two groups depending on peculiarities of treatment. The use of innovative treatment technologies for burned patients has reduced the incidence of burn disease complications and mortality. Introduction of innovative technologies in treating burned patients into broad clinical practice improves results of provision of specialized, high-tech medical care for victims of burns.

A complex treatment of a patient with acute amitriptyline and cyclodol poisoning using enterosorption, intestinal lavage and hemosorption on a new column with a synthetic sorbent is presented. A two-layer synthetic polymer column developed for the selective sorption of cytokines by direct hemoperfusion was used for hemosorption. Quantitative determinations of amitriptyline and cyclodol levels before and after the column, as well as before and after hemosorption have showed high efficiency of the sorbent to remove the toxicant from the blood. The use of 6-hour hemosorption allowed to reduce the level of amitriptyline from the initial level by more than 4 times and the level of cyclodol - by more than 3 times to therapeutic levels and to obtain a pronounced positive clinical effect in the complex treatment of a patient with severe poisoning.

Purpose: Irreversible electroporation is a physical process which is used for killing the cancer cells. The process that leads to cell death in this method is a unique process. Thermal damage does not exist in this process. However, the temperature of the tissue also increases during the electroporation. In this study, we aim to investigate the effect of conductivity changes on tissue temperature increase during the irreversible electroporation process. Materials and Methods: To perform simulations and solve equations, COMSOL MultiPhysics has been used. Standard electroporation pulse sequence (8 pulses with different electric field intensities) was used as a pulse sequence in the simulation. Results: During the electroporation process, the electrical conductivity and the temperature of the tissue were increased. Changes in the tissue temperature in the simulation with variable electrical conductivity are more than in the simulation with constant electrical conductivity during the electroporation process. This difference for pulses with more vigorous electric field intensity and points closer to the electrodes has been achieved more. Conclusion: To more accurately estimate and calculate the temperature and thermal damage inside the tissue during the irreversible electroporation process, it is suggested to consider the effect of conductivity changes during this process.

Objective. To determine the possibility of using automatic plasmapheresis in patients with COVID-19- associated bilateral polysegmental pneumonia. Methods. The treatment of three patients with COVID-19-associated pneumonia with application of the Autopheresis-C™ automatic plasmapheresis machine (the USA) has been analyzed. The patients’ age was 47. 49 and 55 years. The patients’ case histories included factors aggravating the course of pneumonia (diabetes mellitus, chronic cardiovascular pathology with heart failure, obesity). The condition of all patients was severe. The effectiveness estimation of the given technique was carried out 6 hours after the manipulation and included a general clinical blood test, a biochemical blood test, hemodynamic parameters, and objective data. Results. After the first application of automatic plasmapheresis, all patients occurred a decrease in temperature and the level of respiratory failure, which was accompanied by an increase in hemoglobin saturation; a decrease in the severity of dyspnea was also reported, which was the reason for the changing patient position from prone to supine. All patients had a blood pressure reduction. Positive changes in the hemodynamic situation were the basis for reducing the dosage of antihypertensive drugs. Along with this, in the course of manipulation, a gradual decrease in the severity of tachycardia was noted in all patients: a reduction of heart rate was recorded. On the second day after the manipulation, the main blood parameters (leukocytosis, c-reactive protein) decreased. The observed positive effects contributed to the early transfer (by 10-15 days) of patients from the intensive care unit to the general somatic departments. Conclusion. The early inclusion of automatic plasmapheresis in the complex therapy of patients with COVID-19- associated bilateral polysegmental pneumonia leads to the stabilization of hemodynamic parameters, decline of the severity of respiratory failure, which made it possible to avoid the mechanical ventilation. What this paper adds For the first time, the clinical effectiveness of the method of automatic plasmapheresis in the framework of the complex therapy of COVID-19- associated polysegmental pneumonia is shown, which is expressed in a decrease in the manifestations of functional and metabolic disorders of vital systems for maintaining homeostasis. It was found that the inclusion of this technology in the complex therapy contributed to the stabilization of respiratory and hemodynamic parameters, which made it possible to avoid the mechanical ventilation.

Aim. Evaluation of epidemiologic effectiveness of polyvalent dysentery bacteriophage use for management of infection outbreak caused by Shigella flexneri. Methods. Investigation was performed in Tula psychoneurological hospital where the persistent focus of shigellosis was formed by Sh. flexneri 2a. As part of the study, evaluation of efficacy of antibacterial treatment (ciprofloxacin 250 mg per day for 7 days - group 1) was performed by evaluating duration of bacterial excretion in 18 patients with mild, 21 with moderate and 2 with severe Shigella infection. At the same time efficacy of Shigella bacteriophage (group 2) was assessed during treatment of 19 patients with mild and 17 with moderate form of the infection. Prevention of hospital-acquired shigellosis was provided for all patients in the departments where the infection was diagnosed. In this case bacteriophage was administered as following: first 5 days - 2 pills of bacteriophage 4 times a day, then 2 pills once a day during the whole period of hospitalization. This scheme was administered also to all patients newly admitted to the involved departments from day 1 of hospital stay. Results. Treatment with antibiotics led to fast and full recovery not in all cases. Insufficient clinical effect (persistence of disease signs and bacterial excretion for more than 7 days) in the treatment of mild, moderate and severe forms of Shigella infection was 16.7, 61.9 and 50.0%, respectively. High clinical effectiveness was noted in the treatment of patients with the use of Shigella bacteriophage. Ratio of insufficient clinical recovery from mild and moderate forms of the disease was 5.3 and 17.6%, respectively. In the course of anti-epidemiological measures aimed at localization and elimination of the focus, about 1000 patients were exposed to mentioned regimen of prevention. No patients had manifested infection or bacterial excretion registered. Conclusion. The study confirmed clinical and epidemiological effectiveness of polyvalent dysentery bacteriophage use for management of outbreak caused by Sh. flexneri 2a.
