AbstractCoronavirus disease 2019 (COVID-19) threatened human lives worldwide since first reported. The current challenge for global intensivists is to establish an effective treatment for severe COVID-19. Blood purification has been applied to the treatment of various critical illnesses. Theoretically, its technique also has an enormous possibility of treating severe COVID-19 in managing inflammatory cytokines and coagulopathy. Recent clinical studies have revealed the positive clinical effect of therapeutic plasma exchange. Other studies have also indicated the considerable potential of other blood purification techniques, such as Cytosorb, AN69 surface-treated membrane, and polymyxin b hemoperfusion. Further research is needed to elucidate the actual effects of these applications.

Objective — to increase the effectiveness of treatment of psoriasis patients with degenerative changes in the nails, by including in the complex therapy of the topical drug Onychocid® Emtrix, aimed at restoring the integrity of the nail plates. Materials and methods. 38 patients with psoriasis and various forms of onychodystrophy (OD) were examined and treated. The effectiveness of therapy was evaluated based on the results of treatment of patients who were divided into two clinical groups — the main and the comparison ones. Patients in both groups received basic therapy. In addition, patients of the main group were prescribed a combined drug Onychocid® Emtrix, which was applied on the nail plates and under the tips of damaged nails once a day for 3 – 6 months. Results and discussion. Patients with psoriasis are diagnosed with dystrophic changes of nail plates of varying severity. OD of the fingers only was detected in 14 (36.8 %) patients; toes — in 5 (13.2 %); fingers and toes at the same time — in 19 (50.0 %) patients. As a result of complex treatment, in patients of the main group there was an early (already on the 2—4th week of use of the drug Onychocid® Emtrix) improvement of the appearance of deformed nails. After the course of treatment, these patients showed a positive clinical effect, i. e. decrease in thickness, normalization of density, color and structure of nail plates. At the same time, «significant improvement» and «improvement» were noted in 34 (89.5 %), «slight improvement» — in 3 (7.9 %), «no improvement» — in 1 (2.6 %) patients. Conclusions. Treatment of psoriasis patients with dystrophic nail lesions should be complex and focused, including the restoration of the integrity of the nail plates. The inclusion in the complex therapy of patients with psoriatic OD of the combined drug Onychocid® Emtrix increases the effectiveness of treatment, in particular, helps to restore the structure, shape, integrity, thickness and color of the nail plates.

AIM: This study aimed to investigate the effectiveness of the application of a combination of the preparation of micronized purified flavonoid fraction (MPFF) and elastic compression in patients with acquired lymphostasis. MATERIALS AND METHODS: Sixty patients with stage II secondary lower limb lymphedema according to М. Foeldi were included. The patients were divided into two groups through randomization with the envelope method. The first group (n = 30) was subjected to a conservative treatment (MPFF, 1000 mg/day) coupled with elastic compression (3rd class compression stockings). The second group was given compressive therapy (third-class compression stockings). The patients were physically examined through the measurement of the circumference of the limb at different levels. RESULTS: In the first group, the circumference of the lower third of the shin decreased by 8.15% (p = 0.005) after 1 month and by the end of treatment by 10.6% (p 0.001), of the middle third of shin by 3.15% (p = 0.001) and 4.78% (p 0.001), and of the upper thirdby 4.08% (p 0.001) and 5.99% (p 0.001). By the end of the observation period (3 months), the circumference of the lower third of the shin in the second group (29.68 4.67 cm) was significantly greater than that in the first group (26.65 2.92 cm, p = 0.035). No adverse reactions were observed in the MPFF group. CONCLUSIONS: The volume of the lower limbs of patients with acquired lymphedema decreased after using a combination of MPFF and elastic compression to a larger extent than after the isolated use of elastic compression. Patients taking MPFF had a positive clinical effect without adverse reactions. Therefore, MPFF could be used in the pharmacotherapy of secondary lymphedema of the lower limbs.

The effectiveness of using «Asepta plus remineralization» special toothpaste, combination of the same toothpaste and «Asepta parodontal fresh» rinse or «Asepta remineralizing gel» (Vertex CJSC, St. Petersburg, Russia) using analysis technique of the structure and severity of the interior disease sight based on psychosensorial-anatomy-functional desadaptation syndrome. It was established that patients who used «Asepta plus remineralization» toothpaste and «Asepta remineralizing gel» on day 3 showed adaptation to the conditions of their existence. Among the patients using «Asepta plus remineralization» toothpaste and «Asepta parodontal fresh» rinse, despite the positive clinical effect of their use on day 3, there was a violation of adaptability to living conditions. In case of the isolated using of «Asepta plus remineralization» toothpaste for oral hygiene, adaptation to their conditions of existence occurred by 7 days of patient observation. The obtained information is important for planning secondary prevention events for dental hyperesthesia, which allows recommending oral care products to patients taking into account the peculiarities of the clinical course of this pathology.

Presented is a complex treatment of a patient with acute amitriptyline and cyclodole poisoning using enterosorption, intestinal lavage and hemosorption on a new column with a synthetic sorbent. For hemosorption, a column with a two-layer synthetic polymer was used, developed for selective sorption of cytokines by direct hemoperfusion. Quantitative measurements of levels of amitriptyline and cyclodole before and after the column, as well as before haemosorption and after haemosorption, showed a high effectiveness of the sorbent in removing toxicant from the blood. The use of 6-hour hemosorption allowed to reduce the level of amitriptyline from the initial by more than 4 times and the level of cyclodole by more than 3 times to therapeutic levels and obtain a pronounced positive clinical effect in the complex treatment of a patient with severe poisoning.

Introduction: The irrational use of medicines leads to a decrease in the quality of care, an increase in treatment costs and side effects. In the case of antibacterial drugs, in addition to all the above-mentioned consequences, their improper use can lead to an aggravation of the existing and quite challenging problem of our time – the growth of antibiotic resistance among pathogenic microorganisms. The aim of the study: to determine the level of basic knowledge of medical specialists in the field of a rational use of antimicrobial drugs (AMD). Materials and methods: The study was based on an analysis of an anonymous multicenter survey in the framework of the KANT project (the full name of the project is “Physicians’ (Students’) Knowledge of Antimicrobials Usage”). It was conducted in 2018–2019 in 10 major centers of Russia. Results and iscussion: According to the results of the study, the respondents showed a low level of knowledge of the rational use of antibacterial drugs. The best results are obtained for questions No.1 (time interval for evaluating the effectiveness of the initial antimicrobial therapy (AMT)), No.2 (rationality and period of AMD change with a positive clinical effect), and No.9 (determining the mode of using the proposed drugs), whereas the worst results were obtained for questions No.3 (determining irrational combinations of AMD), No.4 (determining a situation requiring a long course of AMT), and No.7 (choosing auxiliary drugs for bacterial respiratory infections). Conclusion: The results obtained in the study indicate the need for additional educational activities among health professionals.
