BACKGROUND: The paper describes the case of a drowned diver in the Adriatic Sea near Pula, Croatia. CASE PRESENTATION: The deceased diver had not been trained at any Health and Safety at Work programs nor had he been examined by the occupational medicine (OM) specialist. The pathologist in Pula concluded that this accident was caused by drowning and that diver’s blood was saturated with 38% carboxyhemoglobin (COHb). The second pathologist in Zagreb concluded that the diver drowned due to atherosclerosis sickness but that COHb was not present in his blood. The third expert witness from Forensics in Rijeka evaluated that the diver died due to increased exertion while diving, due to atherosclerosis and weakened heart. The fourth, the OM specialist expert witness from Rijeka, explained that the diver was a healthy athlete who died due to faulty diving equipment. CONCLUSION: Persons who work under the special working conditions should be examined at an Occupational medicine department as well as trained for work in a safe way. Periodical examinations and permanent training courses should be provided. That way the risk of fatal accidents would be reduced to a minimum.

ABSTRACT BACKGROUND: Endoscopic submucosal dissection (ESD) is a complex endoscopic procedure, with high rates of adverse events and technical difficulties. To overcome that problem, many training centers published the importance of animal models for skill acquirement in ESD. However, no study has used the submucosal dissection depth (DSUB) as a parameter to evaluate the learning curve in ESD, which might be a relevant factor since an optimal resection plane is important to achieve a curative resection and avoid intraoperative complications. OBJECTIVE: This study aimed to assess ESD skill acquirement after short-term training sessions by evaluating the submucosal dissection depth (DSUB) and the association with adverse events. METHODS: This experimental study included 25 experienced endoscopists in therapeuthic procedures (>5years) and 75 specimens resected by ESD (three resections / endoscopist). Learning parameters (resection time, size, en bloc resection rate, bleeding, perforation and submucosal dissection depth) were prospectively evaluated. The percentages of DSUB of all specimens resected were calculated. RESULTS: All specimens were resected from the gastric body (n=75). The mean size of the resected specimens was 23.97±7.2 mm. The number of adverse events, including bleeding, perforation, and death, were 17 (22.67%), 3 (4%), and 0 cases, respectively. The average mean time by the third dissection decreased from 28.44±9.73 to 18.72±8.81 min (P<0.001). The proportion of DSUB in the bleeding and non-bleeding group were respectively 37.97%±21.13% and 68.66%±23.99%, indicating a significant association between DSUB and bleeding incidence (P<0.001). The ROC curve analysis indicated a cut-off point of 61% (sensitivity, 64%; specificity, 94%) of submucosal dissection depth associated with bleeding. Therefore, when ESD was performed at a depth of >61% of the submucosal layer, the risk for bleeding during the procedure decreased (PPV, 0.97; 95% CI, 0.85-0.99). CONCLUSION: Improvement in the learning curve in ESD and a better cognitive ability were seen by the third dissection in these short term training courses. And a significant association between DSUB and the risk of bleeding.

Abstract Background This is a case report of an asymptomatic SARS-CoV-2 infection associated with new-onset nephrotic syndrome in a pediatric patient. This is the third case of new-onset nephrotic syndrome in children associated with SARS-CoV-2 infection, but is the first case report describing a new-onset nephrotic syndrome presentation in a patient who had asymptomatic COVID-19 infection. Case presentation This is a case of a previously healthy 5 year old female who presented with new-onset nephrotic syndrome in the setting of an asymptomatic COVID-19 infection. She presented with progressive edema, and laboratory findings were significant for proteinuria and hypercholesterolemia. She was treated with albumin, diuretics, and corticosteroid therapy, and achieved clinical remission of her nephrotic syndrome within 3 weeks of treatment. Though she was at risk of hypercoagulability due to her COVID-19 infection and nephrotic syndrome, she was not treated with anticoagulation, and did not develop any thrombotic events. Conclusions Our case report indicates that SARS-CoV-2 infection could be a trigger for nephrotic syndrome, even in the absence of overt COVID-19 symptoms.

It gives me great pleasure and enthusiasm to welcome you to the special issue of Acta Medica Philippina - Workplace and Environment Safety and Health Issue. The occupational medicine and occupational health and safety activities primarily focusing on preventing diseases, injuries, and deaths due to working conditions. While exposure to harmful substances and or activities can happen at any time, I believe there are always some measures to ensure that no one has to suffer a work-related injury or illness because of their job. In this special issue, we published several articles including papers from the 13th Indonesia Occupational Medicine Updates, covering research on important aspects of occupational health and safety in the Philippines and Indonesia. The impacts of psychosocial hazards and shift work, the potential biomarker of effect on cardiovascular risk, pesticide exposure among farmers; and key statistics and trends in occupational injuries and traffic accidents in the Philippines are featured in this issue. I sincerely hope this work will be of interest to our readers and meet the real needs of the scientific community, stimulates thought, and eventually open up new research ideas. BACKGROUND: Psychiatric disorders are not compatible with carrying firearms or with driving a car. Persons with such disorders are often not employed and are persistent in demanding invalidity pensions, but some of them also insist on holding on to the mentioned licenses. In such cases, where persons are already in possession of firearms and driving licences, it never occurs to them, that they should surrender their permits back. AIM Pointing to the importance of OM controlling firearm/car driving licenses. CASE REPORTS: This paper discusses the problem of three cases that should be widely recognised as it is potentially life-threatening to other people. The first is the case of a war veteran in retirement with PTSD that had his application for firearms licence rejected by the authorities. The second is the case of a labourer who suffers from a depressive disorder, temporarily incapable of work. The third is the case of a war veteran, a chronic alcoholic with toxic epilepsy, who is applying for invalidity retirement but wants to keep his driving license. CONCLUSION: Occupational medicine assess every single worker by applying advanced methods and psycho tests that enable a thorough assessment of work capacity and fitness for carriage of firearms, driving as well as the assessment of psychiatric disorders, which are the most delicate to assess.

In his article in this issue, Tee Guidotti casts recent works addressing corporate influence on occupational medicine as “collective act[s] of disparagement … undertaken … for political reasons.” We move beyond the question of reputation to address key conflicts in the history of occupational medicine, including the American Occupational Medical Association's historical role in weakening the beryllium standard and the American College of Occupational and Environmental Medicine's recent efforts to limit the extent of the Family Medical Leave Act. The corporate practice of externalizing health and safety costs makes industry influence an important ongoing topic of debate in occupational and environmental medicine.
