An Integrated System for E-Medicine (E-Health, Telemedicine and Medical Expert Systems)

This chapter presents an overview of an integrated system for eMedicine that the authors propose and implement in the Republic of Macedonia. The system contains advanced medical information systems, various telemedicine services supported by modern telecommunication technologies, and decision support modules. The authors describe their telemedicine services that use wireless broadband technologies (WiMAX, 3G, Wi-Fi). A significant part of the chapter presents a web based medical expert system that performs self training using a heuristic rule induction algorithm. The data inserted by medical personnel while using the e-medicine system is subsequently used for additional learning. The system is trained using a hybrid heuristic algorithm for induction of classification rules that we developed. The SA Tabu Miner algorithm (Simulated Annealing and Tabu Search based Data Miner) is inspired by both research on heuristic optimization algorithms and rule induction data mining concepts and principles.

Diabetes Mellitus (DM) is a degenerative disease that has many causative factors. DM patients cannot recover completely but they can maintain the stability of their glucose level by following a healthy lifestyle. The goal of this research is to make an integrated system for people to promote a healthy lifestyle. This system also allows people to monitor the changes in their daily health conditions regularly. Main features in this system diagnose, treatment, guidelines for a healthy lifestyle, including a feature for counting and saving daily calorie consumption and physical activity. The system using the backward inference method to get the conclusion from a set of rules in the knowledge base. The verification using a query, validation with using recall and precision, and usability testing using a questionnaire, was conducted to test the performance of the system. The result of the testing shows that the system gives a good response to all queries given and the user also satisfied with the system. The recall value is 1 with the precision is 0.658. This precision value also promotes the inference process used in the system, which every user will suspect has DM when they start to use the diagnose feature.

Objective. To analyze the provision of the population of the Republic of Sakha (Yakutia) with medical personnel. Materials and methods. The article presents an analysis of the human resources of the healthcare of the Republic of Sakha (Yakutia) for 20002020. The main trends in the medical personnel provision of the Republic of Sakha (Yakutia) are determined according to the data of the Yakut Republican Medical Information and Analytical Center. Results. The personnel resource of healthcare is its fundamental component. The problem of providing the population with medical personnel is one of the key factors in the socio-economic development of the regions, especially the regions of the Arctic zone of Russia. The national project "Healthcare" includes the federal project "Providing Medical Organizations of the Healthcare system with Qualified Personnel". The key goal of the national project on staffing the medical service is to achieve almost full staffing of doctors and nurses in healthcare institutions. The article analyzes the provision of the population with medical personnel in the Republic of Sakha (Yakutia) for the period 2017-2019. In dynamics, there is an increase in the indicator of providing the population with doctors and nurses in general; provision with therapeutists and pediatricians in the primary healthcare sector is growing. The article describes the qualification characteristics of medical personnel in the Republic of Sakha (Yakutia). Conclusions. The indicators of the provision of the population of the Republic of Sakha (Yakutia) with medical personnel according to official reports of the Ministry of Health of the RS (Y) are higher than the average in Russia. This is due to the territorial, logistical characteristics, features of the network and healthcare system of the region.

Expert systems aim to employ expert skills for non-expert person. These designs simulate intellectual and practical performance of human making the performance of expert systems close to that of an expert human. Various expert systems have been proposed in medicine, thus this area is attracting attention. Most problems in modern medicine are very complicated and there is no logic reason for accurate decision making. To this end, doctors decide arbitrarily and variably. On the other hand, large volume of medical information makes decision making more difficult while modern technologies add to volume of information and make problems more difficult. Considering these problems, there is a great challenge in medical diagnosis which requires decision-making support systems. In this paper, an expert system is presented for diagnosis and treatment of nephrolithiasis in which knowledge required for diagnosis and treatment is stored as rules in the system knowledge base. If experts are absent, diagnosis and treatment can be done reliably. Expert systems can be used as decision support by the users. However, currently, they cannot replace experts.

AbstractAn object-oriented medical database management system is presented for a typical cardiologic center, facilitating epidemiological trials. Object-oriented analysis and design were used for the system design, offering advantages for the integrity and extendibility of medical information systems. The system was developed using object-oriented design and programming methodology, the C++ language and the Borland Paradox Relational Data Base Management System on an MS-Windows NT environment. Particular attention was paid to system compatibility, portability, the ease of use, and the suitable design of the patient record so as to support the decisions of medical personnel in cardiovascular centers. The system was designed to accept complex, heterogeneous, distributed data in various formats and from different kinds of examinations such as Holter, Doppler and electrocardiography.

The public service administration system at the government offices of the Kemiri village office in the Kemiri District currently does not have a computerized and integrated system. Because the system runs, residents who submit letters for administrative completeness, still use the Ms.Word / Ms.Excel application. of course has many weaknesses including human error, not neat in file storage, resulting in the lengthy process of searching and making a cover letter and required reports. The analytical method used in this study is to use PIECES (Performance, Informance, Economy, Control, Efficiency, Service) analysis, the design of the model uses UML (Unified Modeling Language). The results of this study are web-based letter information systems at the Office of the Village Chief of Kemiri that can accessed using a local computer browser. Thus the information letter needed by the Kemiri Village community and more effective and efficient in making the letter.
