Objective. To analyze the provision of the population of the Republic of Sakha (Yakutia) with medical personnel. Materials and methods. The article presents an analysis of the human resources of the healthcare of the Republic of Sakha (Yakutia) for 20002020. The main trends in the medical personnel provision of the Republic of Sakha (Yakutia) are determined according to the data of the Yakut Republican Medical Information and Analytical Center. Results. The personnel resource of healthcare is its fundamental component. The problem of providing the population with medical personnel is one of the key factors in the socio-economic development of the regions, especially the regions of the Arctic zone of Russia. The national project "Healthcare" includes the federal project "Providing Medical Organizations of the Healthcare system with Qualified Personnel". The key goal of the national project on staffing the medical service is to achieve almost full staffing of doctors and nurses in healthcare institutions. The article analyzes the provision of the population with medical personnel in the Republic of Sakha (Yakutia) for the period 2017-2019. In dynamics, there is an increase in the indicator of providing the population with doctors and nurses in general; provision with therapeutists and pediatricians in the primary healthcare sector is growing. The article describes the qualification characteristics of medical personnel in the Republic of Sakha (Yakutia). Conclusions. The indicators of the provision of the population of the Republic of Sakha (Yakutia) with medical personnel according to official reports of the Ministry of Health of the RS (Y) are higher than the average in Russia. This is due to the territorial, logistical characteristics, features of the network and healthcare system of the region.

This chapter presents an overview of an integrated system for eMedicine that the authors propose and implement in the Republic of Macedonia. The system contains advanced medical information systems, various telemedicine services supported by modern telecommunication technologies, and decision support modules. The authors describe their telemedicine services that use wireless broadband technologies (WiMAX, 3G, Wi-Fi). A significant part of the chapter presents a web based medical expert system that performs self training using a heuristic rule induction algorithm. The data inserted by medical personnel while using the e-medicine system is subsequently used for additional learning. The system is trained using a hybrid heuristic algorithm for induction of classification rules that we developed. The SA Tabu Miner algorithm (Simulated Annealing and Tabu Search based Data Miner) is inspired by both research on heuristic optimization algorithms and rule induction data mining concepts and principles.

Despite the importance of issues related to the implementation of the national project «Healthcare» and the federal projects it includes, in particular the Federal Project «Providing medical organizations of the healthcare system with qualified personnel», the scientific literature does not fully cover the issues related to the implementation of project activities in this area, both in terms of the procedure for developing and approving projects of the appropriate level and in terms of making changes to existing projects of national, federal, regional and departmental significance. This article provides an overview of the basic regulatory acts of the legislation regulating project activities in the Russian Federation and examines the structure of project activities in general, as well as the implementation of the Federal Project «Providing medical organizations of the healthcare system with qualified personnel», which is part of the national project «Healthcare». Special attention is paid to the monitoring of the Federal Project and the structure of its indicators, including target, main and additional ones.

The issue of reforming not only the healthcare system as a whole, but also the system of training medical and pharmaceutical personnel is relevant today. The main direction of the National Project "Healthcare" is the solution of the issue of providing healthcare institutions with qualified personnel, the introduction of a system of continuing medical education (the National Project "Healthcare", the federal project "Providing medical organizations of the healthcare system with qualified personnel"). In this regard, the requirements for persons who have mastered educational programs and are engaged in medical activities on the territory of Russia have changed. Admission to medical activity, according to modern trends, should be objective, uniform and universal throughout the country. The aim of this article is familiarization of specialists with higher medical and pharmaceutical education with the legislative framework and methodology for conducting the accreditation procedure for specialists. Primary, primary specialized and periodic accreditation is a complex organizational, methodological and engineering process that requires a competent approach in terms of planning, organization and implementation. The training of employees of accreditation centers according to the programs of training specialists in medical simulation training, as well as close interaction with the accreditation commission, certainly allows us to solve these problems and flawlessly ensure that all stages of accreditation of specialists are carried out.

The strategy for the development of healthcare in the Russian Federation improves the staffing of the healthcare system, identified as a priority. The state programs set target indicators for the provision of the population with medical personnel, measures to equip medical organizations with medical workers, and measures of social support for medical workers. The aim of the work was to study the factors relevant for graduates that determine the choice of place of work and the development, based on the data obtained, of measures aimed at improving the availability of qualified medical personnel for medical organizations. The results of a sociological study of graduates of a medical university on the problems of employment in medical organizations are studied. Analyzed: indicators of the population of the region with doctors; the need for medical organizations of the region in medical personnel; events implemented in the region in order to provide medical organizations with qualified personnel. The measures, the implementation of which is aimed at increasing the availability of medical organizations with qualified personnel and achieving the effectiveness of employment of graduates of the university, are determined.

Healthcare systems worldwide are facing problems in providing health care to patients in a pandemic caused by the SARS-CoV-2 virus (COVID-19). The pandemic causes an extreme disease to spread with fluctuating needs among patients, which significantly affect the capacity and overall performance of healthcare systems. In addition, its impact on the sustainability of the entire economic and social system is enormous and certain sustainable management strategies need to be selected. To meet the challenges of the COVID-19 pandemic and ensure sustainable performance, national healthcare systems must adapt to new circumstances. This paper proposes an original multi-criteria methodology for the sustainable selection of strategic guidelines for the reorganization of a healthcare system under the conditions of the COVID-19 pandemic. The selection of an appropriate strategic guideline is made on the basis of defined criteria and depending on infection capacity and pandemic spread risk. The criteria for the evaluation of strategic guidelines were defined on the basis of a survey in which the medical personnel engaged in the crisis response team during the COVID-19 pandemic in the Republic of Serbia participated. The Level-Based Weight Assessment (LBWA) model and Measuring Attractiveness by a Categorical-Based Evaluation Technique (MACBETH) method were used to determine the weight coefficient criteria, while a novel fuzzy Ranking of Alternatives through Functional Mapping of Criterion Subintervals into a Single Interval (RAFSI) model was used to evaluate the strategic guidelines. The proposed multi-criteria methodology was tested in a case study in the Republic of Serbia. The validity of the proposed methodology is shown through the simulation of changes in input parameters of Bonferroni aggregation functions and through a comparison with other multi-criteria methodologies.

The aim: evaluate the effectiveness of the implemented state programs in the field of healthcare in Kazakhstan. Methods. The paper analyzes 4 state programs for the development of the healthcare system implemented in the years of independence of Kazakhstan and describes preliminary data on the program - "State Program for the Development of Healthcare of the Republic of Kazakhstan for 2020-2025". Results. As a result of the implementation of the state programs "Healthy Nation", the state programs for 2005-2010, "Salamatty Kazakhstan", "Densaulyk", maternal mortality decreased by 3,3 times, infant mortality decreased by 1,7 times, mortality from diseases of the circulatory system almost 3 times, mortality from malignant neoplasms by 1,4 times, mortality from tuberculosis by 8,4 times, as a result, life expectancy increased by 6,96 years and reached 73,18 years. Conclusion. Over the years of implementation of health development programs of the Republic of Kazakhstan, there has been a positive trend in indicators that characterize the health of the population, which in turn contributes to improving the quality and increasing the life expectancy of the country's population. Basic health indicators have been improved and health funding has been increased. However, despite these results, life expectancy and funding remain low compared to other countries. The article discusses the problems of staffing the healthcare sector in Ukraine, since today it is growing due to the need for sustainable work of the healthcare system, namely the provision of qualified personnel and the availability of personnel, their qualifications and distribution, motivation, working conditions, remuneration, social well-being, financially, technical and technological support of the labor process determine the appropriate level of medical care to the population. It is noted that the implementation of effective staffing in the healthcare system is an important tool for reforming the medical sphere. It was established that the quality and availability of medical care for the population of the country depends on the level of provision of health care facilities with medical workers and their professional level. The rapid growth in information technology (IT) has reshaped traditional infrastructure in several application domains including education, business, and health. Access to patient information, specifically, is vital for effective functioning of a healthcare system; the unavailability of such medical information leads to management issues. IT is no longer perceived as just a supporting tool; it is a tactical necessity for establishing an integrated healthcare infrastructure. Developed nations benefit from the use of modern technology within healthcare to maximize life expectancy and decrease disease. This chapter presents an overview of healthcare conditions of south Asia region. A framework is presented that embeds IT within the healthcare system. The proposed framework has potential to not only restructure but also remove many errors that plague healthcare system. In addition, key problems within the healthcare sector of south Asian countries—Bangladesh, India, Pakistan, and Sri-Lanka—have been highlighted, and recommendations are proposed to overcome the challenges.

Any economic stress in itself carries a particular risk, since it creates problems in different areas, influencing individual incomes/expenditures, as well as incomes/ expenditures in the healthcare sector. The imbalance in the healthcare system can be understood as an unexpected phenomenon that occurs outside of the healthcare system but leads to negative consequences of access to resources in the healthcare system; in terms of access to medical services. The healthcare system in the Republic of Moldova is perceived as unproductive, which diminishes its prioritization. Therefore, the healthcare system is poorly funded, with few efficient regulations, which made it extremely sensitive during times of crisis, being unpredictable for new challenges. The authors consider that a method of anticipating the crisis in the healthcare system could be the application of economic and financial expertise.

В статье отражены имеющиеся в системе здравоохранения проблемы, в частности - низкое качество медицинских услуг, дефицит бюджетных средств, рост платности медицинских услуг, низкий уровень материально-технической базы сферы здравоохранения, и отмечено, что одной из перспективных мер решения указанных проблем является внедрение механизмов государственно-частного партнерства и муниципально-частного партнерства. Авторы рассмотрели реализацию проектов государственно-частного партнерства и муниципально-частного партнерства в Республике Башкортостан, отметили успехи и существующие проблемы, на основании чего сделали вывод о том, что модернизация государственного сектора системы здравоохранения в области управления и администрирования инфраструктуры возможна лишь при условии участия бизнеса. В статье авторы предложили пути и способы повышения инвестиционной привлекательности медицинской сферы, а также возможные способы повышения эффективности реализации проектов государственно-частного партнерства в системе здравоохранения, отметив при этом, что потребуется активизация усилий и государства, и участников рынка. The article reflects the problems existing in the health care system, in particular, the poor quality of medical services, budget deficits, the increase in the cost of medical services, the low level of material and technical base of the healthcare sector, and it is noted that one of the promising measures to solve these problems is the introduction of mechanisms of state private partnerships and municipal-private partnerships. The authors reviewed the implementation of public-private partnership and municipal- private partnership projects in the Republic of Bashkortostan, noted the successes and existing problems, on the basis of which they concluded that the modernization of the public sector of the healthcare system in the field of infrastructure management and administration is possible only with the participation of business. In the article, the authors proposed ways and means to increase the investment attractiveness of the medical sector, as well as possible ways to increase the efficiency of public-private partnership projects in the healthcare system, while noting that intensification of efforts by both the state and market participants will be required.
