Non-mandatory training is seen as an important facilitator in developing individual employees' competencies and the organization's performance. However, previous studies of non-mandatory training in the context of the work environment show a lack of conceptual clarity. A universal definition in work environment settings is missing from published literature. Therefore, this chapter analyzes the concept called non-mandatory training in the work environment setting. To attain this objective, the researcher applied the concept analysis proposed by Walker and Avant. Based on the analysis, a definition of non-mandatory training was established. That definition was used in this study to reconcile the discrepancies between the understanding of the concept by academics and practitioners, and by allied HR professionals, who tended to view it from different perspectives. Proper understanding of basic assumptions can facilitate communication between different educational and work environment settings and enhance the concept clarity and validity for future research.

Background: Ethical relationships are important among many participants in healthcare, including the ethical relationship between nurse and employer. One aspect of organizational behavior that can impact ethical culture and moral well-being is institutional betrayal. Research aim: The purpose of this concept analysis is to develop a conceptual understanding of institutional betrayal in nursing by defining the concept and differentiating it from other forms of betrayal. Design: This analysis uses the method developed by Walker and Avant. Research context: Studies were reviewed using health literature databases with no date restrictions. Ethical considerations: Analysis was conducted using established guidelines for ethical research. Findings: Although institutional betrayal is a concept applied in the literature, there was a paucity of studies exploring the concept within nursing. Examples of the concept in the literature include violation of trust between organization (i.e. employer) and nurse, such as provision of inadequate workplace protections, ineffective or hostile management, and gaslighting of those who experience negative events. Examples of institutional betrayal have become more visible during the COVID-19 pandemic. Discussion: A conceptual definition of institutional betrayal is a deep violation of trust or confidence or violation of moral standards committed by an institution toward a nurse. This definition incorporates experiences and issues suggested by the literature. Outcomes are likely negative, including impacts on nurse psychological and workplace well-being. This concept likely fits within a framework of ethical workplaces and has conceptual relationships with moral distress and moral resilience. Further studies can help qualitatively explore and empirically measure this concept. Conclusion: In the pursuit of improving the ethical culture of healthcare workplaces, this concept can provide meaningful insight into organizational behavior and its consequences. Naming and describing the concept can promote conceptual clarity and equip researchers, nurses, and leaders to identify and mitigate the issue.

Nursing research using concept analysis plays a critical role for knowledge development, particularly when concerning to broad and foundational concepts for nursing practice, such as dignity. This study aimed to synthesize research concerning concept analysis of dignity in nursing care. Based on a literature review, electronic databases were searched (MEDLINE, CINAHL, COCHRANE, SCOPUS, EBSCOhost, and PubMed) using the terms “dignity,” “human dignity,” “concept analysis,” and nurs*. Papers in Portuguese or English were included. The research synthesis was conducted independently by two reviewers. A total of 35 citations were identified and seven papers were included. Six studies were elected using Walker and Avant concept analysis methodology while one applied the Beth Rodgers evolutionary model. The concept of dignity has been studied by nurses, and its attributes, antecedents, consequences, and similar concepts were synthesized into a definition. Dignity emerged as a fundamental concept in nursing ethics and the main attributes synthesized were personhood (intrinsic), sociability (relational/behavioral), respect, and autonomy. The antecedents identified were: facilitators—patient focus care, recognition (of the attributes of dignity), education, and ethical competence; threats—vulnerability and organizational environment. The consequences were positive coping, empowerment, and dignity preservation. The synthesis of these seven studies using concept analysis provided a clear definition of dignity. These findings challenge future research and education, particularly for the study of undergraduate and postgraduate nursing education programs to enhance skills for preserving patient dignity in clinical practice.

Assuming a continuum between psychotic experiences and psychotic symptoms aligned between healthy individuals and patients with non-psychotic and psychotic disorders, recent research has focused on subclinical psychotic experiences. The wide variety of definitions, assessment tools, and concepts of psychotic-like experiences (PLEs) might contribute to the mixed findings concerning prevalence and persistence rates and clinical impact. In this narrative review, we address the panoply of terminology, definitions, and assessment tools of PLEs and associated concerns with this multitude. Moreover, the ambiguous results of previous studies regarding the clinical relevance of PLEs are described. In conclusion, we address clinical implications and highly suggest conceptual clarity and consensus concerning the terminology and definition of PLEs. The development of an agreed upon use of a “gold standard” assessment tool seems essential for more comparable findings in future research.

Sport is increasingly used as a tool for development and peacebuilding to reach an array of populations (Hayhurst, 2009), including girls and women in the Two-Thirds World (Brady, 2005; Hayhurst, 2014; Saavedra, 2009). However, scholars have cautioned against a universal definition of sport considering its historical link to colonization (Darnell & Hayhurst, 2011; Saavedra, 2009) as well as the promotion of universal benefits of sport for girls (Brady, 2005; Larkin, Razack, & Moola, 2007). Therefore, a postcolonial feminist framework was employed to qualitatively explore how 12 secondary school girls in northern Uganda define sport. In addition, participants in this study identified the benefits that they and other girls and women receive from participating in sport. Semistructured interviews were conducted face-to-face and were transcribed, coded, and thematized by the researchers. Trustworthiness was established by engaging a peer debriefer from Uganda and critical awareness of researcher positionality through reflexivity. Results include how the participants defined sport and physical activity, some as a singular and others as a binary concept, and how girls benefit from participating in sport in northern Uganda. The identified benefits include aspects of health, social life, engagement, opportunities, socioemotional development, and competition. Many of these benefits are congruent with literature from within and outside of Uganda; however, the results also indicate a need for a deeper understanding of how communities define and benefit from sport where sport for development programs are delivered. Connections between the results and the postcolonial feminist framework, study limitations and future research directions are also discussed.

Background:Gut microbes influence the development several chronic conditions marking them as targets for holistic care, prevention strategies, and potential treatments. Microbiome studies are relatively new to health research and present unfamiliar terms to clinicians and researchers. “Dysbiosis” often refers to an alteration in the gut microbiome, but conceptual clarification is rarely provided. Purpose: The purpose of this study is to refine a conceptual definition of dysbiosis based on a review of nursing literature. Method: A Rodgerian approach to concept analysis was used. CINAHL, PubMed, and Web of Science were queried using “dysbiosis” through December 2018. Each article was analyzed with regard to the antecedents, attributes, and consequences of dysbiosis. Essential elements were tabulated and compared across studies to determine recurring themes and notable outliers. Findings: Analysis revealed several important antecedences, attributes, and consequences of dysbiosis. The findings also elucidated notable gaps and highlighted the co-evolving nature of the proposed definition with advances in microbiome research. Conclusion: This article adds a proposed definition of dysbiosis, offering a contribution of conceptual clarity upon which to enhance dialogue and build research. The definition emphasizes risk factors and consequences of dysbiosis as implications for holistic nursing practice.

Abstract Background and Objectives Elderspeak is an inappropriate simplified speech register that sounds like baby talk and is used with older adults, especially in healthcare settings. Understanding the concept of elderspeak is challenging due to varying views about which communicative components constitute elderspeak and whether elderspeak is beneficial or harmful for older adults. Research Design and Methods Rodgers’ evolutionary concept analysis method was used to evaluate the concept of elderspeak through identification of elderspeak’s attributes, antecedents, and consequences. A systematic search using the PubMed, CINAHL, PsycINFO, and Embase databases was completed. Results Eighty-three theoretical or research articles from 1981 to 2020 were identified. Elderspeak characteristics were categorized by semantic, syntactic, pragmatic, paralinguistic, and nonverbal attributes. The primary antecedent to elderspeak is implicit ageism, in which old age cues and signs of functional or cognitive impairment led to simplified communication, usually from a younger caregiver. Research studies varied in reporting whether elderspeak facilitated or interfered with comprehension by older adults, in part depending on the operational definition of elderspeak and experimental manipulations. Exaggerated prosody, a key feature of elderspeak, was found to reduce comprehension. Elderspeak was generally perceived as patronizing by older adults and speakers were perceived as less respectful. In persons with dementia, elderspeak also increases the probability of resistiveness to care, which is an important correlate of behavioral and psychological symptoms of dementia. Discussion and Implications Based on this concept analysis, a new definition of elderspeak is proposed, in which attributes that have been found to enhance comprehension are differentiated from those that do not. Recommendations for consistent operationalization of elderspeak in future research are made.

Abstract With an ageing society, the demand for health and social care is increasing. Traditionally, staff provide care for their clients rather than with them. In contrast, reablement aims to support people to maximise their competences to manage their everyday life as independently as possible. There is considerable variation between and within countries regarding the conceptual understanding of the approach. This variation affects the ability to evaluate reablement approaches systematically, compare and aggregate findings from different studies, and hinders the development of a robust evidence. Therefore, a Delphi study was conducted in 2018/9 with the aim of reaching agreement on the characteristics, components, aims and target groups of reablement, leading towards an internationally accepted definition of reablement. The study consisted of four Web-based survey rounds. In total, 82 reablement experts from 11 countries participated, reaching agreement on five characteristics (e.g. person-centred), seven components (e.g. goal-oriented treatment plan) and five aims (e.g. increase clients’ independency). Furthermore, most experts agreed that reablement is an inclusive approach irrespective of the person's age, capacity, diagnosis or setting. Based on these features, a definition of reablement was developed, which was accepted by 79 per cent of participating experts. This study is a significant step towards providing conceptual clarity about reablement. Future research should focus on evaluating the implementation of agreed reablement components to inform practice, education and policy.

Abstract Background High rates of attrition undermine the success of weight management interventions (WMIs), but a comprehensive understanding of the factors that increase dropout risk remains absent. This is partly explained by heterogeneity of intervention design, and the absence of a universal definition of attrition. This systematic review aimed to identify the factors related to- and predictive of attrition and retention in multidisciplinary WMIs for adults with obesity. Methods The systematic literature search, conducted in Cochrane, Medline, PsycInfo, and Scopus, aimed to identify original research articles published between February 2008 and December 2019. Articles investigating attrition or retention in multidisciplinary WMIs were eligible for inclusion if interventions were for adults (≥18) with obesity identified by body mass index ≥30kg/M2 and lasted ≥6 months. Multidisciplinary was defined as ≥2 interventionist disciplines or professions, for the purpose of this review. Data was synthesised narratively. Results The literature search resulted in seventeen studies which satisfied the inclusion criteria. Attrition rates ranged from 10% at 3-months to 81% at 3-years. The sociodemographic factors associated with reduced risk of attrition included older age, living in less deprived areas, higher levels of education, and female gender. Poor mental health, low social support, high weight loss goals and poor or unsatisfactory results may increase the likelihood of participant dropout, but evidence was limited and inconclusive because of different methodologies, and only a small number of studies investigating some of the variables. Conclusions The scope for targeted retention strategies is limited because few variables were consistently associated with attrition. Until a comprehensive understanding of attrition emerges, WMIs should seek to reduce social inequities in the benefit of WMI provision. Future research should consider factors reported qualitatively, such as intervention expectations and satisfaction, social support, patient-clinician relationships, and logistical barriers. Adopting a universal definition of attrition and de-homogenising participant dropouts would advance future research. As qualitative evidence is limited, exploring participant experiences of WMIs would help understand how attrition rates can be reduced, and in-turn improve WMI effectiveness.

Malaysia has recorded the second highest involuntary turnover rate at 6% and third highest voluntary rate at 6.5% in South East Asia. Employee loyalty becomes critical towards employer. Thus, the purpose of this study was to examine the relationship between career development, compensation, job security, work environment and employee loyalty. In conducting this study, convenience sampling technique has been employed and respondents have been chosen in the area of Kuala Lumpur. Hypothesis were tested using regression analysis by using Smart-PLS. Based on the result, career development, compensation and job security were found to be significant with employee loyalty. However, work environment was found to be not significant with employee loyalty. It is suggested for future research to explore other variables in order to find the factors contributed in employee loyalty.
