To improve the fixation of bone and bone cement in THA we used Interface Bioactive Bone Cement technique (IBBC) since 1982. For prostheses 28 mm-alumina head with polyethylene socket (Kyocera Co.) were used. In IBBC not-resorbable pure crystalline HA granules (0.3– 0.5 mm) were smeared on the bone surface in one to three layers just before packing bone cement. Group 1: IBBC was used in six joints in 1982. Group 2: In 25 patients, conventional bone cement technique (non-IBBC) in the one hip (in 1985 to 1986) and IBBC technique in the other hip (in 1986) were performed in the same patients. Group 3: In 65 patients (71 joints) IBBC were performed. Neither radiolucent line nor osteolysis appeared in all case in group 1. In group 2 in Non-IBBC the appearance rate of radiolucent line and osteolysis were rather high, however in IBBC they were extremely low. In Non-IBBC loosening were seen in 5 joints in the acetabulum and in 2 joints in the femur. However, in IBBC there was no loosening. In group 3 the radiolucent line and the osteolysis appeared extremely low. There was no loosening. In Non-IBBC the appearance rate of radiolucent line and the osteolysis was very high and the rate of loosening was 20%. However, in IBBC, radiolucent line and osteolysis will be prevented eternally.

OBJECTIVE: This study evaluated the use of plasma, bone surface (periosteal) and whole bone as biomarkers of chronic fluoride (F) exposure. METHODS: Forty male Wistar rats were assigned to 4 groups (n=10/gr) that differed according to the F concentration they received in the drinking water. Groups 1, 2, 3 and 4 received water containing 0 (control), 5, 15, and 50 mg F/L, respectively. The rats were killed at 120 days of age. Plasma and femur were collected and analyzed for fluoride with the ion specific electrode by the direct method or after hexamethyldisiloxane-facilitated diffusion. Data were tested for statistically significant differences by ANOVA and linear regression (p<0.05). RESULTS: Mean (± SE) plasma F concentrations ranged from 0.030 ± 0.002 to 0.187 ± 0.013 (mg/mL). The concentrations in surface and whole bone ranged from 610 ± 32 to 4,693 222; and 647 ± 22 to 3,439 ± 134 µg/g, respectively. The surface/whole F concentration ratios were 0.941, 1.414, 1.173 and 1.377, for groups 1, 2, 3 and 4 respectively. For plasma and whole bone, the difference among all groups was statistically significant, except for group 2 compared to group 1. For bone surface, all groups differed from each other except for group 2 compared to group 3. A significant positive correlation was found between bone surface and whole bone F (r²=0.94), as well as between plasma and bone surface (r²=0.71) and plasma and whole bone (r²=0.74). CONCLUSIONS: Data suggest that both bone surface and whole bone are suitable biomarkers of chronic F exposure in rats and plasma may be used as indicator of bone fluoride levels.

This study assessed the effect of low (10 mg ⋅ kg−1 ⋅ h−1) and very high (18 mg ⋅ kg−1 ⋅ h−1) doses of lidocaine on defibrillation energy requirements (DER) to relate changes in indexes of sodium-channel blockade with changes in DER values using a dose-response study design. In group 1 (control; n = 6 pigs), DER values were determined at baseline and during treatment with 5% dextrose in water (D5W) and with D5W added to D5W. In group 2 ( n = 7), DER values were determined at baseline and during treatment with low-dose lidocaine followed by high-dose lidocaine. In group 3 ( n = 3), DER values were determined at baseline and high-dose lidocaine. Group 3 controlled for the order of lidocaine treatment with the addition of high-dose lidocaine after baseline. DER values in group 1 did not change during D5W. In group 2, low-dose lidocaine increased DER values by 51% ( P = 0.01), whereas high-dose lidocaine added to low-dose lidocaine reduced DER values back to within 6% of baseline values ( P = 0.02, low dose vs. high dose). DER values during high-dose lidocaine in group 3 also remained near baseline values (16.2 ± 2.7 to 12.9 ± 2.7 J), demonstrating that treatment order had no impact on group 2. Progressive sodium-channel blockade was evident as incremental reduction in ventricular conduction velocity as the lidocaine dose increased. Lidocaine also significantly increased ventricular fibrillation cycle length as the lidocaine dose increased. However, the greatest increase in DER occurred when ventricular fibrillation cycle length was minimally affected, demonstrating a negative correlation ( P = 0.04). In summary, lidocaine has an inverted U-shaped DER dose-response curve. At very high lidocaine doses, DER values are similar to baseline and tend to decrease rather than increase. Increased refractoriness during ventricular fibrillation may be the electrophysiological mechanism by which high-dose lidocaine limits the adverse effects that low-dose lidocaine has on DER values. However, there is a possibility that an unidentified action of lidocaine is responsible for these effects.

Hg release from amalgam restoration is continuos as long as an amalgam in the mouth. The difference in saliva pH is one factor that influences Hg releasing from amalgam restoration. The purpose of this research was to find data the effect of the difference in saliva pH against Hg release from amalgam restoration. This research was a true experimental in vitro research. This research used 40 samples of premolar teeth of the maxilla, prepared in occlusal Class I, restored with amalgam and then divided into 4 Groups. Teeth belonging to Group 1 were immersed in saliva artificial with a pH content 5, Group 2 with a pH content of 6, Group 3 with a pH content of 7 as a control, and Group 4 with a pH content of 8. All tooth Groups were immersed for one week after condensation. Research data results analyzed using the One-Way Analysis of Variance (ANOVA). Research results measured using the Atomic Absorption Spectrophotometry (AAS) indicated that Group 1 with a pH content of 5 had the highest average release, namely, 19,276 ppb, followed by Group 4 with a pH content of 8, with a Hg release of 17,691 ppb, followed by Group 3 with a pH content of 7 as a control, with a Hg release of 13,702 ppb, and Group 2 with a pH content of 6 the lowest Hg release, namely 12,377 ppb. Summary of this research showed that there was no effect of saliva pH difference against Hg release from amalgam restoration.

The aim of this study was to compare the failure rate and canal preparation times of the Primary WaveOne Gold file (Dentsply Sirona, Ballaigues, Switzerland) with the One Curve file (Micro Méga, Besançon, France). The influence of glide path preparation on failure rate and final preparation times were also evaluated. Endo training blocks (Dentsply Sirona) with simulated canals were separated into four groups: Group 1: Primary WaveOne Gold with WaveOne Gold Glider; Group 2: Primary WaveOne Gold without glide path; Group 3: One Curve with One G; Group 4: One Curve without glide path. The number of training blocks that were shaped before instrument fracture occurred was recorded. Glide path and final preparation times were also recorded. Where no glide path was prepared, One Curve file prepared a significantly higher number of canals (14.33± 0.58) than the Primary WaveOne Gold (4.6 ± 1.34) before instrument fracture occurred (p<0.001). The One Curve with One G Glide Path file prepared significantly higher number of simulated canals (28±1.41) than the Primary WaveOne Gold with WaveOne Gold Glider (15 ± 1.41) before instrument fracture (p<0.001). Glide path preparation times with WaveOne Gold Glider (4.8s) were significantly faster compared to the One G Glide Path file (7.29s)(p<0 .001). Significantly faster final canal preparation times were achieved in groups where glide path were prepared (p<0.001). One Curve files exhibits a greater fracture resistance than Primary WaveOne Gold files. Glide path preparation increases the longevity of preparation files and results in faster final canal preparation.

Aim. A retrospective evaluation of the results of treatment of myopic choroidal neovascularization (mCNV) with intravitreal injections of ranibizumab in a pro re nata (PRN) regimen in three groups of patients distributed according to axial length. Methods. The paper presents a retrospective multicenter study carried out with the cooperation of several Departments of Ophthalmology in the Czech Republic. The study included 60 eyes of 60 patients suffering from mCNV, divided according to axial length into three groups. The first group consisted of 20 patients with an axial length of the eyes shorter than 28 mm (Group 1), the second group included 27 patients with axial lengths ranging from 28 mm to 29.81 mm (Group 2), and 13 patients had axial lengths longer than 30 mm (Group 3). All patients were first administered 3 initial intravitreal ranibizumab injections at monthly intervals (loading phase), and other injections were administered according to a PRN treatment regimen. Patients were evaluated before treatment and then at intervals of 3, 6, 9, and 12 months. The effect of ranibizumab treatment on the functional and morphological parameters of the affected eye was evaluated. Results. The average baseline BCVA ± SD in Group 1 was 52.6 ± 12.5 letters of ETDRS optotypes, and at the end of the one-year follow-up, it was 63.3 ± 11.8 letters. The average baseline of CRT ± SD in this group was 377.4 ± 80.0 μm, and in the 12th month, it was 311.1 ± 63.7 μm. The average baseline BCVA ± SD in Group 2 was 50.2 ± 9.0 ETDRS letters, and at the end of the follow-up, it was 60 ± 12.4 letters. The average baseline of CRT ± SD in Group 2 was 391.2 ± 85.2 μm, and in the 12th month, it was 323.9 ± 91.2 μm. In Group 3, the average baseline of BCVA was 48.5 ± 14.5 ETDRS letters, and at the end of the one-year follow-up, it was 55.7 ± 16.1 letters. The average baseline CRT ± SD for Group 3 was 342.1 ± 94.9 μm, and after 12 months, it was 287.8 ± 88.4 μm. An improvement of BCVA by ≥15 letters of ETDRS optotypes was achieved by 3 patients of 20 (15%) in Group 1, by 5 patients of 27 (18.5%) in Group 2, and by 3 patients of 13 (23.1%) in Group 3. All these changes were statistically significant in comparison with the input values p<0.05. Conclusion. Ranibizumab treatment in patients with mCNV in our study resulted in statistically significant improvement in BCVA and a decrease in CRT in all groups of patients. Our results from a routine clinical practice correspond with the results of large clinical studies; we confirm a particularly good effect of treatment in patients with axial lengths of the eye smaller than 28 mm.

247 Background: We demonstrated that DC Bead (Biocompatibles UK, Ltd) could be loaded with sunitinib and injected intra-arterially in the rabbit without unexpected toxicity. The purpose of this study is to evaluate the antitumoral effect of sunitinib eluting beads in the VX2 tumor model of liver cancer. Methods: VX2 tumors were implanted in the left liver lobe of New-Zealand white rabbits. Animals were assigned to 3 groups: Group 1 (n=6) received 1.5mg of sunitinib loaded in 0.05ml of 100-300um DC Bead, group 2 (n=5) received 0.05ml of 100-300um DC Bead, group 3 (n=5) received 0.05ml NaCl 0.9% in the left hepatic artery. One animal in each group was sacrificed at 24 hours and the others were followed for survival until day 15. Liver enzymes were measured daily. In group 1, plasmatic sunitinib concentration were measured daily by LC MS/MS tandem mass spectroscopy. At day 15 all living animals were sacrificed. After sacrifice, the livers were harvested for determination of the VEGF receptor tyrosine kinase activity by western blot and histopathological examination. Results: In group 1, no animals died during follow-up. In group 2, 2 animals died during follow-up on day x. In control group 3, 3 animals died during follow up on day x. In group 1 plasmatic sunitinib levels remained under therapeutic concentration throughout the experiment. Very high concentrations of sunitinib were measured in the liver tissue 24 and 15 days after embolization. Inhibition of the phosphorylation of the RTK was demonstrated at 24h and 15 days in groups 1. Sunitinib eluting beads seemed to penetrate in the tumor more effectively and there was more necrosis around the beads than their bland counterparts. Conclusions: Administration of sunitinib eluting beads in VX2 carrying rabbits resulted invery high drug concentrations at the site of embolization with minimal systemic passage. Despite the very high tissular sunitinib concentration we did not observe any additional toxicity with loaded beads. Sunitinib eluting beads inhibit the activation of RTK’s triggered by ischemia and seem to prolong survival of the treated animals. Therefore we consider that local treatment with sunitinib may provide a promising approach for the treatment of liver cancer.

AbstractBackground and objective: During root canal instrumentation irrigants, necroticpulp tissue, microorganism and dentin debris may be extruded beyond the apexinto the periradicular tissue. These extrusions may cause flare-up. The purpose ofthis study was to compare the amount of apically extruded debris with three rotaryNickel titanium instruments.Methods: Sixty single rooted, single canalled premolars were selected. The rootswere divided randomly into three groups; (n = 10) according to the type ofinstrumentation system used, group 1“ProTaper Next” system, group 2 “OneShape” system and group 3 “Mani Silk” system. The Debris extruded during theinstrumentation was collected and dried in preweighed vials and the amount ofextruded debris was assessed with an electronic balance.Result: results showed that the Mani silk system extruded significantly less debristhan the ProTaper Next and One shape systems (P < .05), and there is nosignificant difference in apically extruded debris between ProTaper Next groupand One Shape group but the One Shape produced more debris than ProTaperNext.Conclusion: the Mani silk file instrument was behaved well during instrumentationand extruded less debris than the other groups.

Abstract   The concept of evolutionary accumulation of candidate gene polymorphisms in the realization of the hypertensive phenotype becomes promising in understanding of arterial hypertension (AH). A large amount of data about genetic polymorphisms in population have been accumulated and perspective approaches to their integrative assessment are being formulated for practical management. The purpose of the research was to analyze and compare the proportion of modified candidate genes (CG) in the group of hypertensive patients with and without obesity with the group of non-hypertensive patients by calculating the gene modification index (GMI) for the assessment of possible usage of genetic stratification in the management of AH. Methods 106 patients with AH and obesity (age 46.2 [24–72], m/f 79/27), (group 1), 96 hypertensive patients without obesity (age 48.3 [26–75], m/f 72/24), (group 2) and 98 non-hypertensive patients (age 41.6 [26–69], m/f 39/59), (group 3) (ESC/ISH 2018, WHO 1997) were examined. The analysis of polymorphisms of the following CG by PCR was perfomed: ADD1: 1378, AGT: 704, AGT: 521, AGTR1: 1166, AGTR2: 1675, CYP11B2: −344, GNB3: 825, NOS3: −786, NOS3: 894. The GMI which represents the percentage of “pathological” genotypes was formed and calculated by the formula: GMI = (N/13.5) × 100, where N is the sum of points of present GP (“pathological” homozygous was 1,5 points, heterozygous – 1 point, “normal” genotype – 0 points); 13.5 – maximum number of points. The GMI from 0 to 20% was considered as low genetic risk (GR), from 21 to 40% – moderate GR, from 41 to 70% – high GR, from 71 to 100% – very high GR. Results In patients with AH and obesity, a low GR was in 4 (3.8%), in the group of hypertensive patients without obesity – 13 (13.5%), in normotensive patients – in 81 (82.7%) (p1–3 = 0.0001, p2–3 = 0.0001, p1–2 = 0.02). Moderate GR was observed in 21 (19.8%) patients of the 1st group, in 25 (26%) patients of the 2nd group and in 12 (12.2%) patients of the 3rd group (p1–3 = 0.07, p2–3 = 0.01, p1–2 = 0.02). 54 (50.9%) patients of group 1, 41 (42.7%) patients of group 2 and 5 (5.1%) of patients of group 3 had a high GR (p1–3 = 0.0001, p2–3 = 0.0001, p1–2 = 0.08); a very high GR was in 27 (25.5%) patients of the 1st group, in 17 (17.8%) of the 2nd group and was absent in the patients of the 3rd group (p1–3 = 0.0001, p2–3 = 0.0001, p1–2 = 0.02). The average GMI in group 1 was 66.2% [CI 95%, 20–78], in group 2 – 56.4% [CI 95%, 18–72], in group 3 – 22% [CI 95% 5–41], (p1–3 = 0.0001, p2–3 = 0.0001, p1–2 = 0.04). Conclusions It was revealed a strong significant predominance of the proportion of modified CG in hypertensive patients with and without obesity compared with non-hypertensive patients. There was a moderate significant difference between hypertensive patients, depending on the presence of obesity. Evaluation of GMI is a perspective direction in the diagnosis and prevention of AH and can be used in practical management. Funding Acknowledgement Type of funding source: None

A mineralogical study was made of some drill cores of Bearpaw shales from the site of the proposed power and irrigation dam on the South Saskatchewan River. Mechanically, the samples fell into three distinct groups, viz., Group 1: low sand, high silt, and high clay; Group 2: high sand, moderate silt, and high clay; Group 3: high sand, low silt, and very high clay. A sample which showed lesser shear strength when compared with the others fell into Group 3 which differed, mineralogically, from the other groups by the presence of biotite in the sand and more illite in the silt and clay fractions. Otherwise all samples were quite similar in mineral composition, with montmorillonite the predominant clay mineral.The source of some of the soils of Southern Saskatchewan and Southern Alberta has generally been attributed to the Bearpaw formation. This view is supported by the present study.

Aim. To compare the efficiency of removable mini-implant retained dentures using different types of attachment.Materials and methods. The study included 51 edentulous patients. The patients were divided into three groups according to the attachment type and abutment shape: 1) ball-and-socket attachment, ball abutment (n = 26); 2) locator attachment, equator abutment, XiVE LOCATOR system (n = 14); 3) bar attachment (n = 11). Removable dentures were made of ordinary acrylic plastic. The placement of intraosseous dental implants with their sublingual engraftment was performed according to the traditional two-stage delayed-load procedure. The efficiency of implants was evaluated using the method of M.Z. Mirgazizov and the eponymous index. The dynamics of vertical bone resorption around implants was determined using dental panoramic radiography (6 months, 1 and 2 years after the treatment). A periotest was performed 6 months after the end of the orthopaedic treatment.Results. The one-year survival rate of implants reached 96.2% in group 1, amounting to 100% in groups 2 and 3. The two-year survival rate came to 92.3% in group 1, reaching 92.9% and 100% in groups 2 and 3, respectively. Inflammatory phenomena characteristic of peri-implantitis were noted one year after the implant placement in 7.7% (n = 2) of patients in group 1 and in 7.1% of patients in group 2, with no inflammatory phenomena being observed in group 3. Two years after the implant placement, peri-implantitis was observed in 19.2% and 21.4% of patients in groups 1 and 2, respectively, coming to 9.1% in group 3. The periotest revealed that six months after the placement, implant stability was higher in group 3 for both the lower and upper jaw (in comparison with groups 1 and 2), as evidenced by the shift in periotest values towards the negative part of the scale.Conclusion. The study revealed that bar attachment, as well as locator attachment (equator abutments), of removable dentures supported by mini implants is more efficient as compared to the ball-and-socket attachment.
