AbstractBackground and objective: During root canal instrumentation irrigants, necroticpulp tissue, microorganism and dentin debris may be extruded beyond the apexinto the periradicular tissue. These extrusions may cause flare-up. The purpose ofthis study was to compare the amount of apically extruded debris with three rotaryNickel titanium instruments.Methods: Sixty single rooted, single canalled premolars were selected. The rootswere divided randomly into three groups; (n = 10) according to the type ofinstrumentation system used, group 1“ProTaper Next” system, group 2 “OneShape” system and group 3 “Mani Silk” system. The Debris extruded during theinstrumentation was collected and dried in preweighed vials and the amount ofextruded debris was assessed with an electronic balance.Result: results showed that the Mani silk system extruded significantly less debristhan the ProTaper Next and One shape systems (P < .05), and there is nosignificant difference in apically extruded debris between ProTaper Next groupand One Shape group but the One Shape produced more debris than ProTaperNext.Conclusion: the Mani silk file instrument was behaved well during instrumentationand extruded less debris than the other groups.

Introduction. Root canal instrumentation is performed to achieve cleaning and shaping with maximum preservation of its original anatomy. However, in curved canals this approach may cause excessive damage or canal transportation. The present study aimed to evaluate changes in dentin wall caused by instrumentation of curved canals using two nickel titanium rotary systems, ProTaper and Twisted File. Material and Methods. Twenty five extracted human mandibular first molars with two separated mesial root canals were selected. Distal roots were amputated and 50 canals (25 mesiobuccal and 25 mesiolingual) were randomly assigned into two groups: group 1 - ProTaper, and group 2 - Twisted File. All files were used with torque control engine, following the manufacturer instructions. The final instruments were F3 (group 1) and #30/.06 (group 2). Pre and postoperative cross sections were obtained from coronal, middle, and apical portions. Changes in dentin wall were measured and evaluated using Bramante method and digital image software. Statistical analysis was performed using the Mann Whitney U test (p<0.05). Results. The ammount of dentin removed by ProTaper in cervical and middle canal thirds was significantly greater than with Twisted File (p=0.040 and p=0.043). ProTaper eliminated significantly greater ammount of dentin than Twisted File from mesial wall in coronal sections (p=0.039). Mean differences in distance from the center to other walls were not statistically significant in the middle and apical thirds of both groups (p>0.05). Conclusion. ProTaper system produced greater changes in cross sectional area of the root canal compared to Twisted File system.

Abstract This study evaluated the influence of irrigation tip, root canal curvature and final apical diameter on periapical debris and irrigant extrusion during root canal instrumentation. After being classified as mildly or moderately curved, the mesiobuccal root canals of 90 human molars were instrumented. The teeth were allocated into three groups according to the used needle type: a flat open-end needle (Group 1), a side-vented needle (Group 2) or a double side-vented needle (Group 3). The amount of extruded material after instrumentation was calculated for each group. The differences in irrigant and debris extrusion between the experimental groups were analyzed with the Mann-Whitney U test. Irrigation tips and final instrument diameter differences were analyzed with the Kruskal-Wallis test. Multiple groups were compared using the Mann-Whitney U-test with a Bonferroni correction at α=0.05. The amount of extruded material from the double side-vented needle was significantly less than the open-ended flat needle (p=0.008), but there was no difference compared to the side-vented needle (p=0.929). The results showed no effect of root curvature and final apical diameter on the amount of irrigant and debris extrusion (p>0.05). The double side-vented needles generated the least material extrusion to the periapical tissues. Irrigation needle type had the greatest influence on periapical irrigant and debris extrusion and the double side-vented needles may be safely used during endodontic therapy.

We have determined whether changes in lung hyaluronan content affect extravascular water in lungs of unanesthetized rabbits. Three groups of experiments were performed. In group 1 (n = 12), no infusions were given; in group 2, nine pairs of rabbits received either intravenous hyaluronidase (750 U.kg-1.min-1) or an equivalent volume of saline; in group 3, nine pairs of rabbits received either hyaluronidase or saline, followed by intravenous saline infusion amounting to 24% of body weight. At the end of each experiment, one lung was analyzed for extravascular lung water by the wet-dry method. Except for group 3, in all animals the other lung was analyzed for hyaluronan content by a method that involved hydrolyzing lung hyaluronan with fungal hyaluronidase to release reducing N-acetyl glucosamine groups, which were quantified. In group 1, lung hyaluronan, which varied from 50 to 159 micrograms/g dry wt (mean 106 +/- 35 micrograms/g dry wt), significantly correlated with variation in extravascular lung water (mean 4.2 +/- 0.3 g/g dry wt). In group 2 rabbits given hyaluronidase, lung hyaluronan was 40% lower and extravascular lung water was 14.6% lower than in paired controls (P less than 0.01). In group 3, volume expansion did not affect lung water, except after hyaluronidase when lung water was 47% higher than paired controls. We conclude that in the lung the content of hyaluronan is one of the determinants of extravascular water content.

Objectives: The aim of this study was to evaluate the effect of the precipitate that was formed by combining Sodium Hypochlorite (NaOCl) and Chlorhexidine Gluconate (CHX) on the sealing ability of root canal obturation materials. Materials and Methods: The fluid filtration method was conducted on a total of 100 roots. Samples were randomly divided into two control (n=5) and three experimental groups (n=30). The samples in group 1 were irrigated with 1.5 mL of 2.5% NaOCl, and then the smear layers of the teeth were removed by 17% EDTA, while the specimens of group 2 were irrigated by 1.5 mL of 2.5% NaOCl and 1.5 mL of 2% CHX; after the smear layer removal, a final flush with 1.5 mL of 2.5% NaOCl was performed. The samples of group 3 were irrigated the same as group 1 but after the smear layer removal canals were irrigated again with 1.5 mL of 2.5% NaOCl and then a final flush with 1.5 mL of 2% CHX was performed. Teeth were obturated with gutta-percha and AH26 sealer and after seven days, microleakage was evaluated by the fluid filtration technique. The results were analyzed by the ANOVA and Tukey's test. Results: The samples in group 3 had significantly greater microleakage compared to teeth in group 1, 2 (p<0.05), and the specimens in group 1 showed significantly less amount of microleakage than samples in group 2, 3 (p<0.05). Conclusion: The presence of the precipitate that is formed due to interaction between NaOCl and CHX has negative effect on the sealing ability of gutta-percha and AH26 sealer.

In this retrospective study employing chart reviews, 75 open heart surgery patients (OHSPs) were divided into 3 groups of 25 patients. Group 1 received no intravenous (IV) norepinephrine (NE) after surgery. Group 2 and group 3 received a minimum of 0.028 mcg/kg/min of IV NE for 6-24 h and greater than 24 h, respectively. In the 3 groups, preoperative lymphocyte counts were compared to counts obtained on postoperative days 1 and 2. The results showed lower lymphocyte counts on postoperative day 2 in group 3 subjects, who received NE for 24 h or more, compared to subjects of the other groups who received no NE or 6-24 h of NE (p < 0.05). There was also evidence that preoperative use of beta-blocking agents significantly affected the change in lymphocyte counts from day 1 to day 2 in both groups receiving NE. Furthermore, postoperative infections were more prevalent in group 3 than the other 2 groups (p < 0.05). The lower lymphocyte counts and higher infection rate, however, may be linked to lower postoperative blood pressure and increased number of intensive care unit days in group 3. Further investigation is warranted to elucidate the effects of IV NE administration on the lymphocyte counts of OHSPs and to reduce infections in those receiving NE.

AIM: To evaluate the effect of different irrigant solutions employed during removal and replacement of calcium hydroxide paste on the diffusion of hydroxyl ions through root canal dentine in vitro. METHODOLOGY: Thirty-five maxillary and mandibular human canines with straight and fully developed roots were used. After mechanical preparation up to 1mm short of tooth length, 30 canals were filled with calcium hydroxide paste and 5 canals were left empty; all teeth had their coronal accesses properly sealed. Teeth were placed in plastic containers with distilled water, and pH was read after 30 days when the paste from 20 teeth was renewed. After removal of the paste by endodontic instrumentation and irrigation with distilled water, canals were replenished with newly mixed paste in Group 1 and 2. In these groups, final irrigation was conducted with 5 mL of EDTA followed by 5 mL of NaOCl in specimens in Group 1, and 5 mL of NaOCl only in specimens in Group 2. In 10 teeth the paste was not replenished at 30 days (Group 3). All specimens were returned to the containers with fresh distilled water, and the pH was recorded after another 30 days. The differences between the first (30d) and second (60d) pH readings were calculated and submitted to analysis of variance and individual comparisons using the Scheffeé's test. RESULTS: Results of mean analysis on differences of pH readings showed that greater diffusion had occurred on specimens in Group 3. Individual comparisons using Scheffeé's test showed statistical significance between Groups 2 and 3, and equivalence between all other groups. CONCLUSION: It was concluded that the use of EDTA did not enhance diffusion of hydroxyl ions through root canal dentine.

Abstract This study aimed to evaluate sodium hypochlorite (NaOCl), limewater (LW), and Polymyxin B (PMB) as irrigants over MMP-3, MMP-8 and MMP-9. Thirty-three patients with apical periodontitis of single-rooted teeth were treated according to three-experimental groups (n=11): group-1: 2.5% NaOCl was used as irrigant; group-2: 2.5% NaOCl for the first two files and LW: [0.14% Ca(OH)2] for the last two files; group-3: 2.5% NaOCl for the first two files and PMB for the last two files. The association of Ca(OH)2 and CHX was used as an intracanal medication in all groups. Four root canal samplings (S) were collected: S1) immediately after access cavity; S2) after biomechanical preparation; S3) after EDTA application; and S4) after removal of the intracanal medication. After quantification of MMP-3, MMP-8, and MMP-9, the data were analyzed by Friedman and Kruskal-Wallis tests and completed by Dunn test (5%). Regardless the used irrigant, there was no difference in reducing MMP-3 or MMP-8 (P=0,5273, P=0,7048 respectively). However, in reducing MMP-9 (P=0,0246) the NaOCl group was the most effective followed by NaOCl+LW group and NaOCl+PMB group respectively. The intracanal medication [Ca(OH)2 + CHX] with the NaOCl and NaOCl+LW was effective in reducing MMP-8 (P<0,0001, P=0,0025) and MMP-9 (P=0,0007, P=0,0047) respectively, but not for the group of NaOCl+PMB which was not effective in reducing MMP-8 or MMP-9 (P=0,1718, P=0,1953) respectively. NaOCl and NaOCl+LW were effective in reducing MMP-9 levels, and this effectivity could be improved by the use of the intracanal medication [Ca(OH)2 + CHX] in reducing MMP-8 and MMP-9 levels.

Abstract Objective The purpose of this study was to evaluate and compare the axial inclination of the mandibular first molars and their respective bone morphology among individuals with different facial patterns. Materials and Methods The sample comprised the cone beam computed tomographies (CBCTs) of 58 subjects divided into three groups according to the facial patterns: 18 brachyfacial (Group 1), with a mean age of 21.58 years; 23 mesofacial (Group 2), with a mean age of 19.14 years; and 17 dolichofacial subjects (Group 3), with a mean age of 19.09 years. Eight variables were evaluated on CBCT scans of each subject: buccal and lingual mandibular height, cervical and middle mandibular width, inclination of mandibular body, inclination of the mandibular molar buccal surface, molar width, molar angulation and tooth/bone angle. Intergroup comparisons were performed with one-way analysis of variance followed by Tukey tests. Results Buccal mandibular height presented statistically significant difference in the three facial patterns. Lingual mandibular height and mandibular inclination showed to be statistically and significantly smaller in brachyfacial subjects than in the other two groups. Mandibular width presented a statistically significant difference between brachyfacial and mesofacial groups. Negative correlations could be observed between the facial pattern and the buccal and lingual mandibular heights and inclination of the mandibular body. Conclusion Buccal mandibular height was significantly and progressively larger in brachyfacial, mesofacial, and dolichofacial subjects. Lingual mandibular height was significantly smaller in brachyfacial than in mesofacial and dolichofacial subjects. Mandibular width was significantly thicker in brachyfacial than in mesofacial subjects. Brachyfacial subjects had smaller mandibular inclination than mesofacial and dolichofacial subjects.

Different procedures of treatment of large follicular ovarian cysts in 177 sows using GnRH, hCG and PGF2α are evaluated in this study. Ovarian cysts were diagnosed by transcutaneous ultrasonography, which was a part of routine pregnancy diagnosis. No treatment was performed in the control group (Group 1, <i>n</i> = 29); the method of treatment used in the other groups immediately after the diagnosis was intramuscular administration of lecirelin in doses 50 µg (Group 2, <i>n</i> = 28), 100 µg (Group 3, <i>n</i> = 27) and 200 µg divided into 2 equal doses administered at a 12-hour interval (Group 4, <i>n<i> = 25) and of hCG in doses 1 500 IU (Group 5, <i>n</i> = 23), 3 000 IU (Group 6, <i>n</i> = 21), and 250 µg of cloprostenol (Group 7, <i>n</i> = 24). Insemination rate (IR), conception rate (CR) in inseminated sows, pregnancy rate (PR = recovery rate), treatment-insemination interval (TII) and treatment-pregnancy interval (TPI) within 42 days after the initial examination were evaluated. In addition PR in groups of sows divided according to parity (1–3, 4–6 and ≥ 7) were also evaluated. IR and PR were higher in Group 4 (84.0% and 44.0%) and lower in Group 1 (17.2% and 6.9%) in comparison with the other groups (<i>P</i> < 0.001 and <i>P</i> < 0.05). CR, TII and TPI did not differ between the experimental groups. PR were similar in sows with different parity. The study proved a positive response in sows with large follicular ovarian cysts to the treatment consisting of 2 administrations of 100 µg GnRH at a 12-hour interval.

The objective of this study was to evaluate the bacterial seal at the implant-abutment interface using two morse taper implant models, by means of an in vitro microbiological analysis. For that were used 15 implants with mini-abutments tightened by friction, no screws (Group 1); and 30 implants with screw-tightened abutments, of which 15 received 20 N.cm of closing torque (Group 2) and the other 15 received 30 N.cm (Group 3). Microbiological analysis was carried out using colonies of Escherichia coli transported directly from a culture dish to the prosthetic component. Friction implants (Group 1) were activated by tapping and a torque wrench was used for screw-tightened implants (Groups 2 and 3). Each abutment/implant set was immersed in test tubes containing 5 mL of brain-heart infusion broth and incubated at 37 °C for 14 days, observed daily for the presence of contamination. A statistically significant difference was observed regarding the number of contaminated implants. There was greater contamination in Group 2 implants (p<0.05), with no statistically significant difference between the other groups (Group 1 = 20% and Group 3 = 0%). It was concluded that there was no significant difference in in vitro bacterial sealing between implants with mini-abutments tightened by friction without screws and implants with screw-tightened abutments with 30 N.cm of closing torque. The difference in closing torque altered the in vitro sealing ability of the tested abutments, with a greater contamination for components that received a closing torque of 20 N.cm.
