ABSTRACT Context: The effective use of electronic records (ie, electronic health/medical records) is essential to professional initiatives and the overall advancement of the athletic training profession. However, evidence suggests comprehensive patient care documentation and wide-spread use of electronic records is still limited in athletic training. The lack of formal training and education for clinicians and students are often cited as primary barriers to electronic records use. Other healthcare disciplines have used academic electronic health records (AEHR) systems to address these barriers with promising results. Objectives: To identify common challenges associated with the effective use of electronic records in clinical practice, discuss how an AEHR can address these challenges and encourage more effective use of electronic records, and describe strategies for deploying AEHRs within the athletic training profession. Description: The AEHR is an electronic records system specifically designed for educational use to support simulation learning among all types of learners (eg, practicing clinicians, students). Mimicking the form and function of an EHR, the AEHR offers various educational tasks including patient care documentation projects, critical reviews of standardized patient cases, and assessments of patient care data for quality improvement efforts. Clinical and Research Advantages: Recent evidence suggests the use of an AEHR can improve knowledge and enhance skills. Specifically, AEHR use has been associated with enhanced attitudes toward EHR technology, enhanced informatics competencies, and improved documentation skills. Also, the use of an AEHR has been associated with improved critical thinking and decision-making skills. AEHRs appear to be valuable tools for health professions education and athletic training stands to benefit from AEHR use to better train and upskill clinicians and students alike for clinical practice. Although the implementation of an AEHR will require much time and large-scale coordinated efforts, it will be a worthy investment to address current challenges and advance the athletic training profession.

DNA modifications vary in form and function but generally do not alter Watson-Crick base pairing. Diaminopurine (Z) is an exception because it completely replaces adenine and forms three hydrogen bonds with thymine in cyanophage S-2L genomic DNA. However, the biosynthesis, prevalence, and importance of Z genomes remain unexplored. Here, we report a multienzyme system that supports Z-genome synthesis. We identified dozens of globally widespread phages harboring such enzymes, and we further verified the Z genome in one of these phages, Acinetobacter phage SH-Ab 15497, by using liquid chromatography with ultraviolet and mass spectrometry. The Z genome endows phages with evolutionary advantages for evading the attack of host restriction enzymes, and the characterization of its biosynthetic pathway enables Z-DNA production on a large scale for a diverse range of applications.

AbstractMechanical forces in the cardiovascular system occur over a wide range of length scales. At the whole organ level, large scale forces drive the beating heart as a synergistic unit. On the microscale, individual cells and their surrounding extracellular matrix (ECM) exhibit dynamic reciprocity, with mechanical feedback moving bidirectionally. Finally, in the nanometer regime, molecular features of cells and the ECM show remarkable sensitivity to mechanical cues. While small, these nanoscale properties are in many cases directly responsible for the mechanosensitive signaling processes that elicit cellular outcomes. Given the inherent challenges in observing, quantifying, and reconstituting this nanoscale environment, it is not surprising that this landscape has been understudied compared to larger length scales. Here, we aim to shine light upon the cardiac nanoenvironment, which plays a crucial role in maintaining physiological homeostasis while also underlying pathological processes. Thus, we will highlight strategies aimed at (1) elucidating the nanoscale components of the cardiac matrix, and (2) designing new materials and biosystems capable of mimicking these features in vitro.

Terrestrial laser scanning (TLS) is providing exciting new ways to quantify tree and forest structure, particularly above-ground biomass (AGB). We show how TLS can address some of the key uncertainties and limitations of current approaches to estimating AGB based on empirical allometric scaling equations (ASEs) that underpin all large-scale estimates of AGB. TLS provides extremely detailed non-destructive measurements of tree form independent of tree size and shape. We show examples of three-dimensional (3D) TLS measurements from various tropical and temperate forests and describe how the resulting TLS point clouds can be used to produce quantitative 3D models of branch and trunk size, shape and distribution. These models can drastically improve estimates of AGB, provide new, improved large-scale ASEs, and deliver insights into a range of fundamental tree properties related to structure. Large quantities of detailed measurements of individual 3D tree structure also have the potential to open new and exciting avenues of research in areas where difficulties of measurement have until now prevented statistical approaches to detecting and understanding underlying patterns of scaling, form and function. We discuss these opportunities and some of the challenges that remain to be overcome to enable wider adoption of TLS methods.

Architecture and urbanism have been of constant interest to Minoan archaeologists since the beginning of the twentieth century. While there is some scholarly bias to this, with the field deeply affected by Sir Arthur Evans’s focus on the monumental architecture of Knossos, Minoan Crete continues to yield abundant evidence for a substantial built environment. Focusing on urban and architectural remains creates a strong bias in favour of one block of time, the Neopalatial period, which produced the largest amount of wellpreserved settlements and buildings. Yet, in general, the evidence we now have on the Minoan built environment is an undeniable resource, one that continues to grow thanks to ongoing studies of pre-existing remains as well as new excavation and survey projects. As is clear in Evans’s magnum opus, The Palace of Minos at Knossos, the large-scale excavations typical of the dawn of the last century were heavily directed towards the urban cores of the largest Minoan sites (e.g. Boyd Hawes et al. 1908; Hutchinson 1950). The bulk of what we know about the Minoan built environment comes from the first half of the twentieth century, initially through the intensive work of the foreign schools at Malia, Phaistos, Palaikastro, Gournia, Mochlos, and Pseira, later joined by countless excavations by Greek archaeologists. Yet, synthetic treatments really only began with the work of James Walter Graham, in the form of numerous papers published in the American Journal of Archaeology (see Letesson 2009 for a detailed review), and especially his Palaces of Crete (Graham 1962). Nonetheless, his comparative analyses, which also dealt with non-palatial buildings, were largely focused on polite architecture. With a particular interest in form and function, he built on Evans’s insights to be the first to identify, across a large sample of buildings, recurring architectural patterns in the Minoan built environment (e.g. Piano Nobile, residential quarters, banquet halls). His studies also included an innovative quantitative component, emphasizing the existence of a unit of length that builders would have used to lay out the palaces and some of the so-called ‘villas’.

Context To improve patient care, athletic training clinicians and researchers should work together to translate research findings into clinical practice. Problems with patient care observed in clinical practice should be translated into research frameworks, where they can be studied. Practice-based research networks (PBRNs) provide a compelling model for linking clinicians and researchers so they can conduct translational research to improve patient care. Objective To describe (1) the translational research model, (2) practice-based research as a mechanism for translating research findings into clinical practice, (3) the PBRN model and infrastructure, (4) the research potential using the PBRN model, and (5) protection of human participants in PBRN research. Description Translational research is the process of transforming research findings into health behavior that ultimately serves the public and attempts to bridge the gap between research and clinical practice. Practice-based research represents the final step in the translational research continuum and describes research conducted by providers in clinical practices. The PBRNs are characterized by an organizational framework that transcends a single site or study and serves as the clinical research “laboratory” for conducting comparative-effectiveness studies using patient-oriented measures. The PBRN approach to research has many benefits, including enhanced generalizability of results, pooling of resources, rapid patient recruitment, and collaborative opportunities. However, multisite research also brings challenges related to the protection of human participants and institutional review board oversight. Clinical and Research Advantages Athletic training studies frequently include relatively few participants and, consequently, are able to detect only large effects. The incidence of injury at a single site is sufficiently low that gathering enough data to adequately power a treatment study may take many years. Collaborative efforts across diverse clinical practice environments can yield larger patient samples to overcome the limitations inherent in single-site research efforts.

AbstractMotivationTrait data are fundamental to quantitatively describe plant form and function. Although root traits capture key dimensions related to plant responses to changing environmental conditions and effects on ecosystem processes, they have rarely been included in large-scale comparative studies and global models. For instance, root traits remain absent from nearly all studies that define the global spectrum of plant form and function. Thus, to overcome conceptual and methodological roadblocks preventing a widespread integration of root trait data into large-scale analyses we created the Global Root Trait (GRooT) Database. GRooT provides ready-to-use data by combining the expertise of root ecologists with data mobilization and curation. Specifically, we (i) determined a set of core root traits relevant to the description of plant form and function based on an assessment by experts, (ii) maximized species coverage through data standardization within and among traits, and (iii) implemented data quality checks.Main types of variables containedGRooT contains 114,222 trait records on 38 continuous root traits.Spatial location and grainGlobal coverage with data from arid, continental, polar, temperate, and tropical biomes. Context Recently, calls to conduct comparative effectiveness research (CER) in athletic training to better support patient care decisions have been circulated. Traditional research methods (eg, randomized controlled trials [RCTs], observational studies) may be ill suited for CER. Thus, innovative research methods are needed to support CER efforts. Objectives To discuss the limitations of traditional research designs in CER studies, describe a novel methodologic approach called the point-of-care clinical trial (POC-CT), and highlight components of the POC-CT (eg, incorporation of an electronic medical record [EMR], Bayesian adaptive feature) that allow investigators to conduct scientifically rigorous studies at the point of care. Description Practical concerns (eg, high costs and limited generalizability of RCTs, the inability to control for bias in observational studies) may stall CER efforts in athletic training. In short, the aim of the POC-CT is to embed a randomized pragmatic trial into routine care; thus, patients are randomized to minimize potential bias, but the study is conducted at the point of care to limit cost and improve the generalizability of the findings. Furthermore, the POC-CT uses an EMR to replace much of the infrastructure associated with a traditional RCT (eg, research team, patient and clinician reminders) and a Bayesian adaptive feature to help limit the number of patients needed for the study. Together, the EMR and Bayesian adaptive feature can improve the overall feasibility of the study and preserve the typical clinical experiences of the patient and clinician. Clinical Advantages The POC-CT includes the basic tenets of practice-based research because studies are conducted at the point of care, in real-life settings, and during routine clinical practice. If implemented effectively, the POC-CT can be seamlessly integrated into daily clinical practice, allowing investigators to establish patient-reported evidence that may be quickly applied to patient care decisions. This design appears to be a promising approach for CER investigations and may help establish a “learning health care system” in the sports medicine community.

Conservation-development projects are increasingly enacted across large expanses of land where human livelihoods hang in the balance. Recent initiatives–often called ‘landscape approaches’ or ‘ecosystem-based’ conservation–aim to achieve economic development and conservation goals through managing hybrid spaces. I argue that the landscape/ecosystem approach is a socioecological fix: an effort to resolve social-environmental crises through sinking capital (financial, natural, and social) into an imagined ecosystem. Rwanda’s Gishwati Forest has been the locus of diverse crises and fixes over the past 40 years, including an industrial forestry and dairy project, a refugee settlement, a privately managed chimpanzee sanctuary, a carbon sequestration platform, and, most recently, an “integrated silvo-pastoral conservation landscape.” This paper considers how these governance schemes have intersected with broader processes of agrarian change to generate crises that subsequent conservation/development projects then attempt to resolve. I demonstrate how visions for ecosystems privilege certain forms of governance around which imagined socioecological histories are mobilized to frame problems and legitimize certain solutions, technologies, and actors. The Gishwati ecosystem and its fixes are repeatedly defined through an imaginary of crisis and degradation that engenders large-scale landscape modification while foreclosing reflection about root causes of crises or how these might be addressed. Thus, even while conservation/development paradigms have shifted over the past 40 years (from separating people and nature to integrating them in conservation landscapes), this crisis-fix metabolism has consistently generated livelihood insecurity for the tens of thousands of people living in and around Gishwati. Imagining and enacting more just and inclusive social-environmental landscapes will require making space for diverse voices to define ecosystem form and function as well as addressing deeply rooted power imbalances that are at the heart of recurrent crises.
