La articulación de la atención médica con los objetivos estratégicos en instituciones de salud requiere del pensamiento sistémico. El objetivo de esta investigación es determinar la influencia del pensamiento sistémico en el logro de objetivos estratégicos, el método empleado es cuasiexperimental y el estudio tuvo lugar en el Instituto Mexicano del Seguro Social (imss), Campeche, en donde se analizó la intervención con la metodología del pensamiento sistémico en el proceso de atención médica de diabetes mellitus tipo 2 (DM2) y el logro de objetivos estratégicos (control metabólico). Entre los resultados obtenidos se observó que los profesionales de la salud con pensamiento sistémico se asocian con el control metabólico de los pacientes con DM2. Por lo tanto, la organización de salud comprende el funcionamiento como un sistema que integra el pensamiento creativo, el estratégico y el control de líneas de acción. AbstractThe structural link of medical care with strategic objectives in healthcare institutes require systematic thinking. This research aims to determine the influence systematic thinking has in achieving strategic objectives. A quasi-experimental method was used, and research was performed in the Mexican Social Security Institute (Instituto Mexicano del Seguro Social – IMSS) in Campeche, where systematic thinking methodology intervened with the process of medical attention of Type 2 diabetes mellitus and the achievement of strategic objectives (metabolic control). Among the results obtained we observed that health professionals which present systematic thinking are associated to the metabolic control of patients with DM2. Consequently, the health organization comprehends its functionality as a system that integrates creative, strategic thinking and the control of strategic lines of action.

Objective: To evaluate the effect of sitagliptin on somatosensory-evoked potentials (SEPs) and metabolic control in patients with type 2 diabetes mellitus without clinical diabetic neuropathy. Materials and methods: Interventional, prospective, and open study. Patients with less than six months from the diagnosis were included. Examinations of SEPs and laboratory tests at fasting and after food stimulation were performed before and after three months of treatment with sitagliptin (100 mg/day). Results: There was a reduction in the mean levels of HbA1c (P < 0.0001), fasting glucose (P = 0.001), total cholesterol (P = 0.019), and ALT (P = 0.022). An increase in active GLP-1 was found at the end of the study (P = 0.0025). Several SEPs showed statistically significant differences when analyzed before and after treatment with sitagliptin. Conclusion: The results give a glimpse of the possible use of sitagliptin in the treatment of some neurodegenerative conditions of the peripheral nervous system, in addition to its already established role in glycemic control.

BACKGROUND: Diabetes is a type of chronic disease with exceptional medical care for a patient's lifetime, which ultimately requires lifestyle and behavioural adjustments to prevent complications to death. Patients with good self-care behaviour will cause diabetes to be controlled to avoid complications to death and make patients have a better quality of life. AIM: This study aims (1) to determine the model of self-care behaviour in Type 2 diabetes patients in Binjai City (2) to analyse the effect of self-care behaviour on quality of life, metabolic control and lipid control of Type 2 diabetes patients in Binjai City. METHODS: This type of research is survey-based and explanatory using a cross-sectional approach. The study population was Type 2 Diabetes Mellitus (T2DM) patients who remained patients in 8 primary health centres in Binjai City. The consecutive sampling yielded a sample size of 115 people. Data analysis method uses descriptive statistics and Structural Equation Modeling (SEM) using SPSS and Amos 16.0. RESULTS: The results showed that all factors that build T2DM patient self-care behaviour were able to be predictors that shape the patient's self-care behaviour. The self-care behaviour model consists of knowledge, attitudes, communication, financing, family support, motivation, and self-efficacy. Motivation is the most significant predictor of its contribution to the self-care behaviour of Type 2 diabetes patients. Self-care behaviour was also known to be significantly related to the quality of life, metabolic control and lipid control of T2DM patients (p < 0.05). CONCLUSION: Self-care behaviour in T2DM patients can have a substantial and significant impact on quality of life, metabolic control and lipid control possessed by Type 2 Diabetes patients.

Background: Diabetes Mellitus (DM) is a chronic and non-communicable disease that has serious impacts. Previous studies have focused on a single intervention in the management of DM. Therefore, EMAS (education, nutrition management, physical activities, and stress management) interventions are proposed to convey the pillars of diabetes mellitus as endorsed by the Indonesian Ministry of Health.Purpose: This study aimed to analyze the effects of EMAS interventions on the knowledge, attitudes, skills, and glucose control in patients with type-2 DM.Methods: This study used a pretest-posttest quasi-experimental design without control groups. The samples were 86 diabetic patients recruited using a purposive sampling technique. The EMAS interventions were conducted for six months and eight sessions (October 2018 to March 2019). The EuroQoLfive-dimensional (EQ-5D) questionnaire was used to collect the data, and the paired t-test was used for data analysis.  Results: The results showed that there were significant differences in the knowledge, attitudes, skills (p=0.001), and glucose control (p=0.04) of type 2 diabetes mellitus after the implementation of EMAS interventions. Conclusion: EMAS interventions significantly increased the knowledge, attitudes, and skills in patients with type 2 diabetes to behave healthier to control their blood sugar. Community nurses can use EMAS intervention for the management of DM among diabetic patients.
