Background:Listeria monocytogenes is a gram-positive bacteria that cause the invasive disease listeriosis. Human clinical syndromes are infrequent, mostly appearing in immunosuppressed individuals, newborns, the elderly, pregnant women, and occasionally healthy patients.Objectives:We describe and analyze Listeria-related demographics and clinical features to determine the predisposing conditions for severe infections in an immunodepressed population by rheumatic diseases.Methods:Descriptive Observational Study. A retrospective analysis of 143 patients were performed affected by listeriosis, with positive isolation of Listeria monocytogenes from blood, treated in the H.U. Virgen del Rocío (Seville- Spain) between 2003-2019. Of them 9 were rheumatic patients. The type of clinical manifestation was analyzed, paying special attention to the characteristics associated with patients with neurological complications or unfavorable outcome (death and / or abortion in pregnant women), immunosuppression (associated with cancer or rheumatic disease) was assessed as independent variables, chronic diseases (Hypertension, Diabetes Mellitus, dyslipidemia, COPD, Renal Insufficiency and Ischemic Heart Disease) as well as other baseline characteristics of the patient. (age, sex, pregnancy) and their toxic habits (tobacco and alcohol).Results:The sample includes a similar proportion of men (70 cases) and women (73 cases), of all ages. Of the total patients, most (85%) required hospital admission, with a duration median (non-parametric data) of 11 days. 78% of the cases admitted showed a favorable evolution. However, 15.4% resulted in death and 5.6% in abortion. This percentage of abortions represented 29% of the total pregnant women admitted Of all the patients admitted, a third (33%) were immunocompromised, including patiets with cancer (79%) and rheumatic diseases (21%). Include lupus (33%), RA (22%), APs (11%), polymyalgia rheumatica (11%), panuveitis (11%) and ANCA vasculitis MPO specificity (11%). All of them required admission although the majority showed a favorable evolution, except one of the patient. which resulted in death, in which case in addition to lupus he presented with prostate cancer. Regarding the baseline treatment of these patients, 7 underwent treatment with synthetic DMARDs and three with biological DMARDs (1 Adalimumab, 1 Infliximab and 1 Rituximab) As a result of the listeria infection, most of them had fever or digestive symptoms and two of they experienced neurological manifestations (meningoencephalitis) None of these last two (with lupus and RA) had biological DMARDs.Conclusion:Listeriosis is an uncommon but potentially serious infection usually in older people, pregnant women and immunocompromised patients. In our sample, 33% of the patients were immunocompromised. Of the 9 patients. affected by listeria with rheumatic disease we find a death for meningoencephalitis. Given the impact of this infection in immunosuppressed patients should pay attention in our patients with fever and neurological manifestations.Reference:[1]Eleftherios Mylonakis et al. Background: Somatoform disorders are common problem among children and adolescents. As children's verbal skills are relatively poorly developed to express their psychological problem, it is not surprising that they display somatic symptoms in response to life stresses.Objectives: The study aimed to explore the difference of somatoform disorders and symptoms between boys and girls the causal association of psychosocial factors.Materials & Methods: This is a cross-sectional descriptive study conducted at weekly Child Psychiatry Clinic of Psychiatry OPD and Pediatrics OPD in Bangabandhu Sheikh Mujib Medical University (BSMMU) of 6-16 years' age group from June to December' 2003. Total 450 respondents, 350 from Pediatric OPD & 100 from Child Psychiatry Clinic of Psychiatry OPD, were included in this study.Results: No significant gender difference was found in the diagnostic categories of somatoform disorders. Both boys and girls reported higher rates of undifferentiated somatoform disorders, 33.33% and 37.50% respectively. Abnormal psychosocial factors were found in majority of the cases and the most common was parental overprotection (26.42%). Overall, associated abnormal psychosocial situations were significantly higher among girls than that of boys (P<0.01). It was also revealed that higher rate of abnormal psychosocial factors was found to have causal relationship of higher rate of somatoform disorders among girls than that of boys.Conclusion: Findings suggested that somatoform disorders in children and adolescents were frequent in clinical settings and more in girls than that of boys due to more associated abnormal psychosocial situations among girls.KYAMC Journal Vol. 9, No.-2, July 2018, Page 69-72

Objectives: Discussions regarding the specific management and outcomes for laryngeal MEC are limited to very small, single-institution case series. To look further into the diagnosis and management of these uncommon non-squamous cell carcinomas of the larynx, we present 3 recent cases of laryngeal MEC treated at our institution. Methods: Patients at a tertiary hospital treated for MEC between October 2019 and December 2020 were retrospectively identified. Chart review, imaging analysis, and histologic slide creation were completed for all patients. Results: We identified and treated 2 patients with high-grade supraglottic and 1 patient with intermediate-grade glottic MEC. These patients presented to our clinic with a primary complaint of either gradual, worsening dysphonia, dysphagia, or both. All patients underwent laryngovideostroboscopy as well as panendoscopy with directed submucosal biopsy, which was consistent with MEC. MRI was performed in 2 of the cases further elucidating the extent of submucosal spread. PET-CT was performed in all 3 cases, and none demonstrated evidence of regional or distal metastases. Surgically, high-grade MEC lesions were treated with a total laryngectomy. The intermediate MEC lesion was managed with a supracricoid partial laryngectomy (SCPL). Surgical margins were free of tumor in all cases with no nodal metastases by modified radical neck dissection. Radiation therapy was offered to both high-grade MEC patients and declined by one. Radiation was not recommended to the patient with intermediate-grade MEC as we believed that the risk of additional treatment outweighed the benefit. Conclusion: We believe that MEC of the larynx should be considered in patients with atypical submucosal laryngeal masses. Laryngovideostroboscopy, MRI, and PET imaging may be valuable in determining the extent of the lesions and planning appropriate surgery. Postoperative radiation therapy should be considered a per tumor grade in other more studied sites, as there is no data on efficacy in laryngeal MEC.

94 children and 25 adolescents, 42% male and 58% female, aged predominantly between six and ten years old (40%) were studied at the Martagão Gesteira Childcare and Pediatrics Institute (Rio de Janeiro Federal University) between 2005 and 2011. The prevalent clinical form of vitiligo was the generalized type (34%). Although the clinical response was similar between the treatment modalities, we decided for low and high power topical steroids in our routine. Halo nevi were found in seven (5.9%) cases. Thirty (25%) patients underwent specialized evaluation and 18 (60%) reported some relevant psychological situation.

Objetivo: Compreender a ótica dos profissionais de enfermagem sobre a hospitalização de adolescentes. Metodologia: Pesquisa qualitativa, descritiva, utilizando entrevista com perguntas semi-estruturadas, realizada em uma enfermaria especializada em saúde do adolescente no Rio de Janeiro, no período de fevereiro a abril de 2016, com 15 profissionais de enfermagem. Resultados: A partir da análise de conteúdo de Bardin, definiram-se três categorias: a inexperiência no cuidado de enfermagem ao adolescente como um desafio, especificidades da adolescência, e sentimentos gerados pela hospitalização do adolescente no profissional de enfermagem. Conclusão: Conclui-se que os profissionais de enfermagem se colocaram inexperientes em cuidar desse público e declararam que o cuidado a essa população é um desafio para a equipe.Descritores: Adolescente, Enfermagem, Hospitalização.THE ADOLESCENT’S HOSPITALIZATION IN THE OPTICS OF NURSING PROFESSIONALSObjective: Analyze the contents of nursing notes in patients’ records in an intensive care unit (ICU) of a public tertiary hospital in Fortaleza, CE. It is a descriptive study with quantitative approach that analyzed 151 medical records of patients admitted to an ICU, from September 2014 to February 2015, whose 48 hour and discharge notes were evaluated. Data revealed notes with poor content, which did not express the patients’ situation, nor the nursing care provided. Data concerning date, time, and patient identification were adequate. However, the COREN number and the professional’s signature raised concern given the high percentage of non-compliance regarding ethical and legal aspects.
