Abstract BackgroundThe growth and development of children is influenced not only by heredity but also by environmental factors, including nutrition and temperature. The aim of this study was to evaluate the growth and nutritional status of preschool children in Daxing’anling, the coldest region of China.MethodsA descriptive, cross-sectional survey was performed among preschool children aged 3 to 6 years by stratified cluster sampling in Daxing’anling. The children’s parents completed the questionnaires. Height, body weight and head circumference were measured, and Z scores for weight for height, weight for age, height for age and head circumference for age were evaluated. Anthropometric data were compared with World Health Organization standards and China’s growth references. The levels of vitamin A, E and 25-(OH)-D3 in serum were detected by high-performance liquid chromatography.ResultsA total of 305 children were recruited. The average height of the preschool children was lower than China’s growth reference but higher than the WHO standard. More than half of the preschool children ranged from -1 SD to +1 SD. Both the values of weight for height and of weight for age were positive and higher than the WHO standards (P<0.01), with a significant difference between boys and girls (P<0.01). The incidence of stunting, wasting, and underweight were 4.59%, 2.95% and 2.30%, respectively, although the prevalence of overweight and obesity was high (18.03% and 6.89%, respectively). The rates of vitamin A and D deficiency were 7.54% and 88.85%, respectively. Vitamin A was also positively associated with 25-(OH)-D3.ConclusionsThe growth and nutritional status of preschool children were generally good, although the double burden of malnutrition was observed. Vitamin D deficiency should be paid much attention to. It is necessary and urgent to carry out nutritional interventions for health.

ABSTRACT Background In some regions, multiple vitamin A (VA) interventions occur in the same target groups, which may lead to excessive stores. Retinol isotope dilution (RID) is a more sensitive technique than serum retinol to measure VA status. Objective We evaluated VA status before and after a high-dose supplement in preschool children living in a region in South Africa with habitual liver consumption and exposed to VA supplementation and fortification. Methods After baseline blood samples, subjects (46.7 ± 8.4 mo; n = 94) were administered 1.0 μmol [14,15]-13C2-retinyl acetate to estimate total liver retinol reserves by RID with a follow-up 14-d blood sample. Liver intake was assessed with a frequency questionnaire. In line with current practice, a routine 200,000 IU VA capsule was administered after the RID test. RID was repeated 1 mo later. Serum retinyl esters were evaluated using ultra-performance liquid chromatography. Results At baseline, 63.6% of these children had hypervitaminosis A defined as total liver retinol reserves ≥1.0 μmol/g liver, which increased to 71.6% after supplementation (1.13 ± 0.43 to 1.29 ± 0.46 μmol/g; P < 0.001). Total serum VA as retinyl esters was elevated in 4.8% and 6.1% of children before and after supplementation. The odds of having hypervitaminosis A at baseline were higher in children consuming liver ≥1/mo (ratio 3.70 [95% CI: 1.08, 12.6]) and in children receiving 2 (4.28 [1.03, 17.9]) or 3 (6.45 [0.64, 65.41]) supplements in the past 12 mo. Total body stores decreased after the supplement in children in the highest quartile at baseline compared with children with lower stores, who showed an increase (P = 0.007). Conclusions In children, such as this cohort in South Africa, with adequate VA intake through diet, and overlapping VA fortification and supplementation, preschool VA capsule distribution should be re-evaluated. A community-based cross-sectional study was undertaken to determine the prevalence of undernutrition using body mass index (BMI) among 2–6-year Santal preschool children of Purulia District, West Bengal, India. A total of 251 (116 boys and 135 girls) children from 12 villages were measured. Commonly used indicators, that is, weight, height, and BMI, were used to evaluate the nutritional status. More boys (59.5%) than girls (53.3%), based on BMI, were undernourished. Significant age differences in weight (F=44.29∗∗∗; df=3), height (F=58.48∗∗∗; df=3), and BMI (F=3.52∗∗∗; df=3) among boys were observed. Similarly, significant differences between ages in mean weight (F=56.27∗∗∗; df=3), height (F=64.76∗∗∗; df=3), and BMI (F=2.62∗∗∗; df=3) were observed among the girls. The present study revealed that the nutritional status of the preschool children of Santal tribal community of these villages was poor with very high rate of thinness in boys and girls (59.5% and 53.3%, resp.).

In the present study, we investigated nutritional status and health-related factors in a multistage cluster sample of 3119 Thai urban and rural children aged 0·5–12·9 years. In a subsample, blood samples were collected for the measurement of Hb, transferrin receptor, vitamin A and vitamin D concentrations. The prevalence of stunting and underweight was higher in rural children than in urban children, whereas the wasting rate was similar in both rural and urban areas. Among children aged 3·0–5·9 years, the prevalence of overweight was significantly higher in urban areas than in rural areas and so was the obesity rate in children aged 6·0–12·9 years. Protein intakes of all age groups were relatively high in both the areas. Intakes of Ca, Fe, Zn and vitamin C were significantly higher in urban areas than in rural areas. The prevalence of anaemia in rural areas was twice as high as that in urban areas, particularly in infants and young children. However, the prevalence of Fe-deficiency anaemia was similar in both urban and rural areas. While the prevalence of vitamin A deficiency (by serum retinol cut-off < 0·7 μmol/l) seemed to be very low, vitamin A insufficiency (by serum retinol cut-off < 1·05 μmol/l) was more prevalent (29·4–31·7 %) in both the areas. The prevalence of vitamin D insufficiency ranged between 27·7 and 45·6 % among the children. The present study indicates that the double burden of malnutrition is still a major public health problem in Thailand. Further studies need to explore the associated risk factors for these nutrient deficiencies. Effective strategies and actions are needed to tackle the nutritional problems in Thai children.
