OBJECTIVE: To assess whether palatal mini-implants are effective as direct anchorage for distal movement of the upper molars. METHODS: It was used an acrylic model of the upper dental arch. After making a groove in the region corresponding to dental alveolus, acrylic teeth were fixed in groove with #7 wax, with the roots being previously immersed in adhesive wax. The orthodontic appliance was placed according to the Edgewise technique and then a mini-implant (SIN, São Paulo, Brazil) was inserted at the site corresponding to the palatal raphe. A 0.019 x 0.025-in stainless steel archwire was made and attached to the upper arch with elastics. A transpalatal arch bar (0.019 x 0.025in) was mounted and two hooks were soldered to it in order to retain chain elastics (Unitek, Brazil) to be connected to the mini-implant under a force of 1.5 N on each side. The maxillary model was immersed in water 40 times and photographed after each immersion, for observation of dental movements. Analysis of variance (ANOVA) and Tukey's test were employed for analyzing the obtained data. RESULTS: Molars displaced distally 3.1 mm, in average, with distal inclination ranging from 3 to 5 mm. CONCLUSIONS: Molar movements occurred due to distal inclination, with a slight rotation and no extrusive effect.

Pencabutan dini pada gigi desidui seringkali menyebabkan perpendekan lengkung sehingga ruang yang dibutuhkan untuk erupsi gigi permanen kurang. Kaninus ektopik, impaksi premolar dan asimetri rahang adalah kasus yang sering terjadi akibat kurangnya ruang untuk erupsi gigi. Pasien perempuan usia 20 tahun, mengeluhkan gigi gingsul dan tidak rapi. Pemeriksaan objektif menunjukkan crowding rahang atas dan rahang bawah, kaninus maksila kanan atas ektopik, premolar mandibula kanan impaksi, disertai pergeseran midline. Maloklusi Angle kelas I dengan hubungan skeletal kelas I, overbite 1 mm, overjet  1,3 mm, crowding mandibula dan maksila, ektopik kaninus unilateral, impaksi premolar unilateral, asimetri rahang dan pergeseran midline. Pencabutan dilakukan pada 2 gigi premolar pertama rahang atas dan 2 gigi molar pertama rahang bawah yang mengalami nekrosis. Perawatan menggunakan alat cekat teknik Edgewise dengan multiloop. L loop digunakan untuk ekstrusi gigi kaninus ektopik dan menarik impaksi premolar. Asimetri rahang dikoreksi dengan ekspansi lengkung gigi menggunakan mainarchwire. Setelah 7 bulan perawatan, gigi kaninus yang ektopik, impaksi premolar dan crowding terkoreksi. Overjet dan overbite menjadi 2,5 mm. Perawatan masih berlangsung hingga saat ini. Penggunaan multiloop pada teknik Edgewise efektif untuk mengoreksi crowding gigi dan L loop efektif untuk mengekstrusi gigi kaninus ektopik dan impaksi premolar. ABSTRACT: Management of Impacted Premolar, Ectopic Canine and Jaw Asymmetry Case Using Edgewise Technique. The premature loss of primary teeth may reduce the arch length required for permanent teeth eruption. Ectopic canine, premolar impaction and jaw asymmetry are often encountered due to the lack of space for permanent teeth eruption. A twenty-year-old female patient complained an ectopic canine and crowding of upper and lower teeth. The objective examination found maxilla and mandibula crowding, ectopic right maxilla canine, mandibular premolar impaction, and midline shift of maxilla and mandibula. Angle class I malocclusion, overbite 1 mm, overjet 1.3 mm, maxilla and mandibula crowding, unilateral ectopic canine, unilateral premolar impaction, jaw asymmetry and midline shift occurred. Extraction of 2 maxillary first premolars and 2 necrose mandibular first molars was conducted followed by a treatment using fixed orthodontic appliance Edgewise technique with multiloop. The ectopic canine and premolar impaction were extruded using L loop. Jaw asymmetry was corrected by expanding the dental arch using the expanded main archwire. Ectopic canine, premolar impaction, maxilla and mandibula crowding were corrected after 7 months of treatment. The overjet and overbite reached 2.5 mm. The treatment is still ongoing. The multiloop edgewise archwire technique is found effective to overcome dental crowding, and L loop is effective to extrude the ectopic canine and premolar impaction.

Impacted and transposed teeth cause serious difficulties in tooth eruption and movement as well as esthetic and functional outcomes. Proper treatment planning including good biomechanical control is essential in order to avoid side effects during traction and aligning of the impacted and/or transposed teeth. The purpose of the present study was to present a successfully treated female patient having transposed and impacted lower canines by means of a modified lingual arch and fixed orthodontic appliance. A female patient aged 13 years and 9 months presented to the orthodontic department with a chief compliant of bilateral spacing and missing teeth in mandibular dentition. After leveling and creating sufficient space in the mandibular arch for the canines, a modified lingual arch was cemented to the mandibular first molars. The lingual arch had two hooks extending to the distobuccal areas of the canine spaces. Elastic chains were applied between the hooks on the lingual arch and the ligatures tied to the attachments on the canine crowns. The light forces generated by elastic materials caused impacted canines to erupt and tend towards their own spaces in the dental arch. As a result, impacted and transposed lower canines were properly positioned in their spaces, and the treatment results were stable during the retention period.

The correction of rotated malpositioned tooth/teeth into the dental arch alignment in the mixed-dentition is often a challenging task for paediatric dentists. Failure in addressing this issue can bring about detrimental effect to the developing dentition and increases the probability of a complex orthodontic treatment in later years. Factors such as severity of the malpositioned teeth, patient’s treatment compliance, limitation in specific functions of the selected appliance, availability of bone and space may dictate the success of the treatment. The combined use of a simple sectional orthodontic wire appliance and a 2×4 orthodontic appliance has been shown to produce a positive effect. The appliances resulted in derotation of the rotated malpositioned teeth and bringing them into arch alignment in two cases. This treatment option had eliminated the detrimental effects to the developing dentition and helped patients to enhance their smile and dental aesthetics.

ABSTRACTIntroduction: Children with bad habit and the persistence of deciduous teeth can caused malposition of anterior permanent teeth or Angle Class I Type 1 malocclusion. One of the curative treatment is removable orthodontic appliance. The purpose of this study was to determine the difference of dental arch width, overjet and overbite in patients with Angle Class I Type 1 before and after treated with removable orthodontic appliance in Dental Student Clinic in Faculty of Dentistry Padjadjaran University. Method: The research was conducted using 33 patient study models of Angle Class I Type 1 who were treated with removable appliance from year 2014 to 2015 in accordance with the criteria. Research method statistically analyzed with paired t-test sample. The dental arch width, overjet and overbite were measured with caliper of patient study models before and after treatment with removable orthodontic appliance. Results: The difference value before and after treatment using removable orthodontic appliance were 2.8% for anterior dental arch width, 2.4% for posterior dental arch width, 10.7% for overjet and 11% for overbite. Conclusion: The differences value of dental arch width, overjet and overbite in patients with Angle Class I Type 1 before and after treated with removable orthodontic appliance showed only a little progress of the treatment caused by multiple factors. Keywords: Angle class I type 1, Dental arch width, Overjet, Overbite

Introduction. Functional maxillary orthodontics has a large number of different mobile devices with different effects on craniomandibular system and great capabilities in solving many orthodontic problems. The aim of this article was to show the effects of 9-month treatment of malocclusion class II, division 1 in a 14-year-old female patient using pre-fabricated functional appliance Trainer T4CII. Case Outline. Skeletal distal relation, deep bite, increased overjet, narrowness and irregular position of upper and lower frontal teeth are indicated for orthodontic treatment with fixed appliance. After refusal of fixed appliance therapy, a female patient was proposed treatment with mobile orthodontic appliance. A pre-fabricated functional appliance Trainer T4CII was delivered to the patient. She was motivated and she was wearing appliance at night and 2-3 hours during the day. After 9 months of treatment there was a significant improvement in the position of upper and lower frontal teeth and reshaping of upper and lower dental arch, yet overbite and overjet were corrected. Conclusion. Surprisingly good and fast improvement of all problems within class II, division 1 in a 14-year-old patient was achieved with prefabricated functional appliance Trainer T4CII.

Complete tooth eruption, especially canine, will affect facial esthetic and the harmony. Surgical procedure will helpto expose an impacted tooth determining orthodontically into dental arch. Clinical and radiographic examinationshould be taken for locating the position of the tooth. The flap was designed differently between labially or palatallyimpacted canine. A 16 years old female came to clinic with Class III Angle dental and skeletal malocclusion with animpacted of upper left canine. She was treated with fixed orthodontic appliance using edgewise technique andsurgical exposure using closed-eruption technique. The proper diagnosis and management between oral andmaxillofacial surgery, and orthodontics will guarantee good result.

Introduction: The number of adult patients looking for orthodontic treatment has markedly increased in recent years. However, fake orthodontic appliance use, performed without minimum expertise or knowledge of the damage that it may cause to oral health, became very common in Brazil. Objective: To report the case of a male patient, 42 years old, who used an orthodontic appliance of uncertain origin. The patient had poor oral hygiene and grade III mobility in several units, and was referred to perform multiple extractions, with subsequent rehabilitation. Discussion: Orthodontic appliance used by orthodontists are strictly supervised by ANVISA. They are manufactured from biocompatible materials and their wires have specifically calibrated forces and appropriate formats to the dental arch, in order to move the teeth. Pirate accessories, without invoice and manufacturing origin, are usually found with street salesman. Materials used to make these devices, such as elastic bands, rubbers and dental yarns, are sold on the streets or in social networks. Often, application includes broom wires and superglue, in addition to twisted wires and elastic bands. These materials do not have any type of control and may cause poisoning, severe allergies and periodontal or dental changes. Final considerations: It is vital to dentists to be aware of this new fashion that has been adopted by young people, in order to make them aware of the risks they are subjected when adopting unnecessary use of dental appliance, without due supervision of specialized personnel.

ABSTRAK Pendahuluan: Penggunaan alat ortodonti lepasan dengan sekrup ekspansi bertujuan untuk memperoleh ruangan pada kasus gigi berjejal dengan cara melebarkan lengkung gigi. Pergerakan gigi yang dihasilkan dapat berpengaruh pada salah satu komponen estetika wajah yaitu profil jaringan lunak. Tujuan penelitian ini untuk mengevaluasi perubahan profil jaringan lunak bibir sebelum dan setelah  perawatan ekspansi lengkung gigi menggunakan alat ortodonti lepasan. Metode: Jenis penelitian ini adalah deskriptif analitik menggunakan metode purposive sampling pada subjek yang merupakan pasien ortodonti di Klinik Profesi Dokter Gigi RSGM Unpad yang dirawat menggunakan alat lepasan akrilik dengan sekrup ekspansi lateral maksila dan mandibula. Terdapat 4 subjek yang memenuhi kriteria inklusi yaitu sepuluh kali aktivasi yang dilakukan setiap minggu dalam waktu tiga bulan.  Subjek dilakukan foto lateral wajah dengan posisi standar sebelum dan setelah aktivasi. Pengukuran dilakukan menggunakan jangka sorong digital dari bibir atas (Ls) terhadap E-line (Ns-Pog), bibir bawah (Li) terhadap E-line (Ns-Pog), dan busur derajat digital untuk mengukur sudut nasolabial(Ns-Sn-Ls). Hasil dan Pembahasan: Nilai rerata perubahan jarak bibir atas terhadap E-line yaitu 0,59 mm, nilai rerata perubahan jarak bibir bawah terhadap E-line yaitu 0,63 mm dan nilai rerata perubahan sudut nasolabial yaitu -2,03o. Penelitian ini menunjukkan adanya perubahan posisi bibir  menjadi lebih ke anterior setelah dilakukan aktivasi sekrup ekspansi lateral rahang atas dan rahang bawah sebanyak 10 kali. Simpulan: Hasil penelitian menunjukkan adanya perbedaan yaitu  posisi bibir atas dan bibir bawah yang menjadi lebih ke anterior dan tidak terdapat perbedaan pada sudut nasolabial setelah dilakukan 10 kali aktivasi sekrup ekspansi lateral pada alat ortodonti lepasan.Kata kunci: perbedaan profil jaringan lunak; alat ortodonti lepasan; ekspansi lengkung gigi; sekrup ekspansi. ABSTRACT Introduction: The use of removable orthodontic appliance with expansion screws aim to gain a space in crowding teeth by expanding the dental arch. The resulting tooth movement can affect one of the aesthetic components of the face, the  soft tissue profile. The aim of this study to evaluate the change of lip soft tissue profile before and after treatment of dental arch expansion using removable orthodontic appliance. Method: The research method used is descriptive analysis and purposive sampling in patients of Rumah Sakit Gigi dan Mulut Universitas Padjadjaran’s Orthodontic Clinic treated by using removable appliance with lateral expansion screw in maxilla and mandible. There are 4 patients who meet the inclusion, namely the srew is activated 10 times, once every week in 3 months. Subjects photographed in lateral position before and after activation. Measurement is done by using a calipers from upper lip to E-line, lower lip to E-line, and nasolabial angle using a digital protractor. Results and Discussion: The average of the changes in upper lip to E-line distance are 0,59 mm, the average of the changes in lower lip to E-line distance are 0,63 mm, and the average of the changes in nasolabial angle are -2,03o . This study showed a lip position that became more anterior after 10 times the activation of the maxillary and mandibular expansion screws. Conclusions: The results showed a difference in the position of the upper lip and lower lip which became more anterior and there is no difference in nasolabial angle after 10 times the activation of lateral screws in removable appliances.Keywords: soft tissue profile differences; removable orthodontic appliances; dental arch expansion;  expansion screws.
