Media representations of destinations play a powerful role in tourism appeal. The narrator assumes a role infused with knowledge and power, employing discourse to describe and interpret places and people to entice armchair audiences to not only travel vicariously alongside them, but to follow in their footsteps. This review article uses the English actor and writer Michael Palin to examine this phenomenon through the lens of the flâneur and choraster. Palin's travels have traditionally been viewed based on their ability to create space from the perspective of a representational voice of authority. In the present article, we wish to ask whether the power of the travel narrator for tourism is perhaps better expressed in their ability to develop a counter (or chora discourse), one where we are able to see space as locally contested. Palin's narrator expresses appreciation of his reliance on the people (chora) that inhabit the spaces he visits. His narrations of travel evidence how the flâneur perspective is influenced (and/or disrupted) by a chora in two ways—that which influences the perspective before travel and directs the gaze, and those that occupy and inscribe meaning on the spaces that are traveled to, that influences and/or forms experience.

An enzyme-linked immunosorbent assay (ELISA) can be used for quantitative measurement of proteins, and improving the detection sensitivity to the ultrasensitive level would facilitate the diagnosis of various diseases. In the present review article, we first define the term ‘ultrasensitive’. We follow this with a survey and discussion of the current literature regarding modified ELISA methods with ultrasensitive detection and their application for diagnosis. Finally, we introduce our own newly devised system for ultrasensitive ELISA combined with thionicotinamide adenine dinucleotide cycling and its application for the diagnosis of infectious diseases and lifestyle-related diseases. The aim of the present article is to expand the application of ultrasensitive ELISAs in the medical and biological fields.

Abstract Background The aim of this study was to evaluate the impact of cervical lymph node dissection on acid and duodenogastroesophageal reflux (DGER) in patients undergoing transthoracic esophagectomy with gastric tube reconstruction and intrathoracic esophagogastrostomy. Methods Thirty one patients receiving transthoracic esophagectomy gastric tube reconstruction by intrathoracic esophagogastrostomy were subjected and divided into two groups: two field lymph node dissection group (the 2F group) and three field lymph node dissection group (the 3F group). All patients underwent 24h pH and bilirubin monitoring and gastrointestinal endoscopy one year after surgery. The results of 24h pH and bilirubin monitoring, endoscopic findings, and reflux symptoms, were compared between two groups. Results No acid reflux was observed in the 2F group, whereas it was observed in 6 (40%) of the 3F group (P = 0.023). DGER was observed in 2 patients (13%) of the 2F groups, whereas it was observed in 8 (53%) of the 3F group (P = 0.007). The percentage time of acid reflux in the 3F group was significant higher than that in the 2F group (median 0.8 vs 0%, P = 0.008). The percentage time of bile reflux in the 3F group was also significantly higher than that in the 2F group (median 2.600 vs 0%, P = 0.027). Four patients (25%) had reflux esophagitis in the 2F group, and nine patients (60%) had reflux esophagitis in the 3F group (P = 0.048). Conclusion Cervical lymph node dissection increases acid reflux and duodenogastroesophageal reflux, and can lead to the increase of the incidence of reflux esophagitis in patient with intrathoracic esophagogastrostomy.
