Orthodontic management in a female patient with an impacted incisor in the mandible – case report

Traumas to deciduous teeth, such as intrusion and complete avulsion, lead to the most severe injuries to tooth buds of permanent teeth. The most common post-traumatic complications of permanent teeth include: enamel hypoplasia or hypocalcification, crown or root dilaceration, partial or complete inhibition of root growth, and eruption disorders.<b> Aim</b>. The paper aimed to present orthodontic treatment in a female patient with an impacted permanent lower incisor with significantly impaired morphology due to a trauma to its deciduous predecessor. <b>Material</b>. Material included: patient’s orthodontic records, gypsum models, photographs and radiological records. <b>Results</b>. Orthodontic treatment was successful despite abnormal morphology of the impacted tooth 41. After 4-year follow-up the tooth remains in the arch. <b>Conclusions</b>. A tooth can be preserved in the socket when the crown-to-root ratio (CRR) is at least 1 : 1. In this case, CRR for the tooth 41 was 2 : 1, and therefore its extraction would be recommended. Despite this a decision was made to introduce an impacted tooth into the arch. Thanks to its presence in the dental arch it was possible to preserve appropriate bone quality for implantation of a prosthetic implant in the future. Prosthetic treatment (if necessary) may be performed after completion of vertical growth of the face that is observed in women after the age of 16 years. <b>Summary</b>. This case of a 15-year female patient shows that as a result of orthodontic and surgical intervention and despite significant root shortening the tooth may remain in the dental arch and play its functional and aesthetic role.<b> (Świątkowska A, Dunin-Wilczyńska I, Świątkowski W. Introduction: Impacted teeth is complex anomaly of teeth eruption that requires a balanced approach not only in the differential diagnosis of its forms, but choice of rational methods of treatment. The aim: Optimization of the tactics of orthodontic-surgical treatment of patients with impacted teeth based on the development and implementation of computed tomographic indices (KT) and photometric indices (FM) of opening of surgical access (OSA) to crowns of impacted teeth Materials and methods: The results of treatment of 48 patients with delay of permanent teeth eruption have been analyzed. For an objective assessment of treatment results, a group of 24 (aged from 9 to 19 years old ) was formed. All 24 patients had typical clinical situation. Results: Orthodontic correction of patients envisaged, first of all, the elimination of obstacles in the way of teeth eruption, if necessary to provide space in dental arch and simultaneous treatment of associated bite malocclusions. Surgical exposure of impacted tooth crown was carried out and at the same time a triangular shaped guiding channel was formed, base of channel was at the impacted tooth and its angle finished into the dental arch. Precise dimensions and depth of the channel were preliminary planed on computed tomography slices with 3D reconstruction. Mean values of CT width (7,13±0,54mm), and length (6,42±0,78mm) of OSA and CT index (130,79±8,19%) of OSA to impacted teeth crowns were determined. Conclusion: To improve the quality of diagnosis and optimization of methodological approaches to treatment of patients with teeth impaction, we have proposed CT and FM OSA indices to the crowns of impacted teeth. The developed indices serve as specific reference points for optimization of diagnostic process, for reducing of probability of repeated surgical interventions and choosing the optimal path for instrumental orthodontic treatment of patients with impacted teeth

ABSTRACT Objective: This article describes the orthodontic treatment performed on an adult patient with multiple dental losses. Case report: A female patient, 20 years and 4 months old, presented with the following conditions: absence of teeth #26, #35, #36 and #46; semi-impacted tooth #48; inclined molars adjacent to an edentulous space; canines and premolars in a Class II relationship; a convex profile; biprotrusion; and forced lip sealing. Results: Space in the region of tooth #26 was closed, as well the space of tooth #46; tooth #48 erupted and followed mesial movement passively; space of the region of tooth #35 was maintained for the placement of a dental implant; uprighting of tooth #37 was obtained. Aesthetic and functional goals of the treatment were achieved. Results remained stable 10 years after the end of the treatment. Conclusion: The modified helical loop could be effectively used in orthodontic mechanics to close edentulous spaces. Passive semi-impacted mandibular third molar eruption and mesialization can occur in adults when proper space is provided.

Bruxism merupakan suatu kebiasaan parafungsional berupa gerakan menggertakan dan menggerus gigi. Tidak jarang pasien yang mempunyai kebiasaan bruxism memerlukan perawatan ortodontik. Perawatan bruxism dapat dilakukan bersamaan dengan perawatan ortodontik cekat. Studi kasus ini bertujuan untuk menganalisa efek Trainer for Braces (T4B) pada pasien bruxism yang memerlukan perawatan ortodontik cekat. Pasien perempuan usia 21 tahun, mengeluhkan gigi berjejal dan tidak rapi. Pemeriksaan objektif menunjukkan bidental protrusi, crowding rahang atas dan rahang bawah, deep overbite, konstriksi berat pada regio gigi premolar dan molar rahang atas dan rahang bawah, 47 linguoversi dan disertai bruxism. Maloklusi Angle Kelas I dengan hubungan skeletal Kelas I, bidental protrusi, overjet 3,7 mm, overbite 4 mm, crowding, edge to edge bite, cross bite dan bruxism. Pasien dirawat menggunakan alat ortodontik cekat teknik Edgewise dengan alat tambahan Lingual Arch Bar untuk ekspansi rahang dan koreksi 47 yang linguoversi dan Trainer for Braces (T4B) untuk bruxism. Setelah 8 bulan perawatan, crowding rahang atas dan rahang bawah terkoreksi, ekspansi rahang dapat tercapai, 47 yang linguoversi terkoreksi, overjet dan overbite berkurang menjadi 3,5 mm, perawatan pada pasien masih berlangsung hingga saat ini. Kombinasi perawatan ortodontik cekat dengan penggunaan alat tambahan seperti Trainer for Braces (T4B) efektif untuk membantu koreksi maloklusi pada pasien bruxism. ABSTRACT: Edgewise Technique Combined with Trainer for Braces for Bruxism Patient. Bruxism is a parafunctional habit of grinding and clenching the teeth. It is common for patients with fixed orthodontic treatment to experience bruxism. When dealing with these patients, clinicians could initiate the bruxism treatment in conjunction with the orthodontic treatment. This case report will analyze the effects of Trainer for Braces (T4B) in a patient with malocclusion and bruxism habit. A 21 year old female patient complained of her crowding in upper and lower anterior teeth. The objective examination shows protrusion and crowding in upper and lower teeth, deep overbite, severe maxillary and mandibulary constriction, 47 linguoversion and bruxism habit. Angle Class I with skeletal Class I malocclusion, bidental protrusion, overjet 3.7 mm, overbite 4 mm, crowding, edge to edge bite, crossbite and bruxism habit. The orthodontic treatment used fixed preadjusted Edgewise appliance with Lingual Arch Bar for expansion and lingoversion molar correction. Trainer for Braces (T4B) was also prescribed to treat her bruxism. After 8 months of treatment, the crowding in upper and lower teeth was corrected, dental arch expansion was achieved, linguoversion molar was corrected, and overjet and overbite became 3.5 mm and the treatment was still on going. The combination of fixed preadjusted Edgewise appliance with Trainer for Braces (T4B) can be considered as an effective therapy for correcting malocclusion in bruxism patient.

The purpose of this article was to present a case report of 11–year old female patient with a large osteolytic mandibular lesion which healed after endodontic treatment. The patient was referred for radio diagnostics due to an incidental finding of a large osteolytic lesion of the area of the left lower first and second premolars in the panoramic radiograph taken before orthodontic treatment. CBCT was performed and the patient asked to have teeth 33-35 treated by endodontics before surgery. The patient missed the surgical appointment and when she reappeared several months later, the lesion showed signs of healing thus surgery were aborted. The presented case testifies to the observation that even large osteolytic lesions can heal after endodontic treatment without surgical approach.

Abstract An impacted or missing permanent tooth can add significant complications to an otherwise straightforward case. When multiple impacted teeth are present, the case complexity increases further. Developing a treatment sequence, determining appropriate anchorage, and planning and executing sound biomechanics can be a challenge. The following case report illustrates a patient reportedly diagnosed with mild scleroderma as an adolescent. He presented for orthodontic treatment as an adult with multiple retained primary teeth and multiple impacted teeth. Diagnosis, treatment planning, and various methods of managing guided eruption of impacted teeth will be discussed. Following orthodontic treatment that required extraction of multiple primary and permanent teeth as well as exposure and ligation of multiple permanent teeth by an oral surgeon, the patient finished with a significantly improved functional and esthetic result.

Introduction: The Size and shape of the teeth are genetic characteristics. Anomaly in tooth size and shape is due to disturbances in a sequence of morph differentiation and his to differentiation in tooth bud formation periods and is more common in permanent dentition than deciduous teeth. The most common tooth size discrepancy is in the upper lateral incisor and upper and lowers 2nd premolar teeth. About 5% of malocclusions are due to tooth size discrepancy. For having the best occlusion, we must have the appropriate size and shape of teeth. As there is a relation between deciduous and permanent teeth, proper evaluation and timely intervention are essential for achieving a good occlusion in anterior and posterior segments of permanent teeth. Thus we need interceptive orthodontic in mixed dentition period with a good treatment plan for removing the malformed tooth at the proper time and appliance therapy for eruption guidance of other teeth. Case Report: A rare tooth anomaly (double teeth) in deciduous teeth: The case reported here is a 9- years old boy with a delayed eruption of the right lower permanent lateral incisor and gemination of lower right deciduous canine and crowded upper incisors that refer to the orthodontic department of dental school of Ahvaz Jundishapur University of Medical Sciences on7th October of 2020. Conclusion: After creating patient’s file, the further steps of molding from the patient and gathering all diagnostic records such as panoramic radiography, intraoral photography was carried out and ordered to remove the malformed baby tooth afterward. In the next stage, on the patient’s treatment, we installed a strap onto the patient’s first permanent molars, remolded them, then created a lingual arch to maintain the vegetative space in the lower permanent canine and prevent its collapse, and increased the patient’s overbite.

Introduction. Orthodontic treatment of Class II Division 1 (II/1) malocclusions in adults can be challenging since skeletal effects are limited. Possible treatment options are orthodontic camouflage or orthognatic surgery, in severe cases. The aim of this paper was to present a successful management of Class II malocclusion in an adult patient using temporary anchorage devices (TADs). Case report. After detailed clinical examination, study models and cephalometric analysis, a 26 years old patient was diagnosed with Class II malocclusion, an overjet of 12 mm, congenitally missing tooth 41 and midline shifted to the right in upper dental arch. In prior orthodontic treatment, patient had upper premolars extracted. Posterior teeth in upper left quadrant were shifted mesially. The camouflage treatment was considered, using temporary anchorage devices (TADs) to distalize posterior teeth on the left side, and gain space for incisor retraction and midline correction in upper dental arch. Results. Using TADs as additional anchorage in anterior region and coil spring for molar distalization, the space was made for tooth 23, midline correction and incisor retraction. After 40 months, a satisfactory result was achieved, overjet and midline correction, class I canines occlusion and class II molar occlusion. Conclusion. Class II/1 malocclusion in adults can be successfully treated using TADs. The success depends on the severity of malocclusion and patient cooperation.

Facial asymmetry, following early childhood condylar trauma, is a common complaint among patients who seek surgical treatment. G.D.M., a 27-year-old male patient, sought professional help to correct his cosmetic flaw, caused by a condylar fracture when he was 8-years-old. After the proper orthodontic treatment, he underwent a double jaw orthognathic surgery and, 9 months later, a second one to correct the remaining asymmetry. Two years after this second procedure, the patient is still under surveillance and has no complaints.

Introduction. An appliance according to Veltri is a kind of palatal construction which is rarely used in our region. It appeared as a logical consequence of the evolution of appliances for rapid palatal separation. It is primarily indicated for upper molars distalization. However, its good qualities allow the use it in case of some other orthodontic problems. Case report. The aim of this report is overview of the therapy of 14-year-old boy with asymmetry of the upper dental arch and unilateral open bite and crossbite, using the appliance according to Veltri. The plan of therapy was primarily aimed at correction of upper arch asymmetry by using appliance according to Veltri. Extraction of the lower first premolars was done and then upper and lower fixed appliance for leveling the upper and lower dental arch, normalization overjet and the depth of overlap was placed. The total duration of the therapy was two years. Conclusion. Presented combined camouflage therapy achieved a satisfactory result, regardless of the poor prognosis due to hyperdivergent growth.
