Introduction: The Size and shape of the teeth are genetic characteristics. Anomaly in tooth size and shape is due to disturbances in a sequence of morph differentiation and his to differentiation in tooth bud formation periods and is more common in permanent dentition than deciduous teeth. The most common tooth size discrepancy is in the upper lateral incisor and upper and lowers 2nd premolar teeth. About 5% of malocclusions are due to tooth size discrepancy. For having the best occlusion, we must have the appropriate size and shape of teeth. As there is a relation between deciduous and permanent teeth, proper evaluation and timely intervention are essential for achieving a good occlusion in anterior and posterior segments of permanent teeth. Thus we need interceptive orthodontic in mixed dentition period with a good treatment plan for removing the malformed tooth at the proper time and appliance therapy for eruption guidance of other teeth. Case Report: A rare tooth anomaly (double teeth) in deciduous teeth: The case reported here is a 9- years old boy with a delayed eruption of the right lower permanent lateral incisor and gemination of lower right deciduous canine and crowded upper incisors that refer to the orthodontic department of dental school of Ahvaz Jundishapur University of Medical Sciences on7th October of 2020. Conclusion: After creating patient’s file, the further steps of molding from the patient and gathering all diagnostic records such as panoramic radiography, intraoral photography was carried out and ordered to remove the malformed baby tooth afterward. In the next stage, on the patient’s treatment, we installed a strap onto the patient’s first permanent molars, remolded them, then created a lingual arch to maintain the vegetative space in the lower permanent canine and prevent its collapse, and increased the patient’s overbite.

Arch space/ Tooth size discrepancy occurs whenever there is a early loss of the deciduous molars which eventually leads to malocclusion. Prevention of this discrepancy has always been a challenge for a pediatric dentist. This case report describes modified bilateral distal shoe appliance fabricated due to inadequate abutment support.

ABSTRACT Objectives To explore whether variations in odontogenesis-related genes are associated with tooth-size discrepancies. Materials and Methods Measurements of the width of permanent teeth were obtained from dental casts of 62 orthodontic patients (age 15.65 ± 6.82 years; 29 males and 33 females). Participants were classified according to the anterior and overall Bolton ratios as without tooth-size discrepancy or with maxillary or mandibular tooth-size excess. Genomic DNA extracted from buccal cells was used, and 13 single nucleotide polymorphisms (SNPs) across nine genes were genotyped by polymerase chain reaction using TaqMan chemistry. χ2 or Fisher exact tests were applied to determine the overrepresentation of genotypes/alleles depending on the type of tooth-size discrepancy (α = .05; corrected P value: P < 5.556 × 10−3). Odds ratios (ORs) and their correspondent 95% confidence intervals (CIs) were also calculated to investigate the risk of this phenotype for the SNPs having significant association. Results Individuals carrying the FGF10 rs900379 T allele were more likely to have larger mandibular teeth (OR = 3.74; 95% CI: 1.65–8.47; P = .002). This effect appeared to be stronger when two copies of the risk allele (TT) were found (recessive model, OR = 6.16; 95% CI: 1.71–22.16; P = .006). On the other hand, FGF13 rs5931572 rare homozygotes (AA, or male A hemizygotes) had increased risk of displaying tooth-size discrepancies when compared with the common homozygotes (GG, or male G hemizygotes; OR = 10.32; 95% CI: 2.20–48.26; P = .003). Conclusions The results suggest that FGF10 and FGF13 may contribute to the presence of tooth-size discrepancies.

There are several methods of analizing tooth size discrepancy in orthodontics include prediction methods for mixed dentition. Prediction method of Moyers and Sitepu most commonly used although both were obtained from 2 different races, Caucasian and Deutromelayu. Yemeni ethnic is one of the ethnic groups settled in Indonesia which descendants of the Caucasian race. The aim of the study was to observed the suitable prediction table for Yemeni ethnic. It was an observasional analitics study consist of 40 samples with cross sectional design. The results showed that slight difference for prediction of Moyers on the maxilla (1.02) and prediction of Sitepu on the mandibula (0.11). As conclusion, the most suitable predicition method for Yemeni ethnic is Moyers’s method for maxila and sitepu’s method for mandibula.   Key words: mesiodistal width permanen teeth, Moyers method, Sitepu method, Yemeni Etnic

Pendahuluan: Diskrepansi ukuran gigi dan profil wajah merupakan dua faktor yang membantu dalam proses penegakan diagnosis dan penentuan rencana perawatan ortodonti. Pengukuran diskrepansi ukuran gigi dapat dilakukan dengan analisis Bolton, analisis ini dapat menunjukkan rasio lebar mesiodistal antara gigi pada maksila dan gigi pada mandibula. Profil wajah dapat digunakan sebagai guideline dalam menentukan estetik wajah yang ingin dicapai setelah perawatan. Tipe profil wajah dapat diinterpretasi dari hasil fotografi ekstraoral, pada penelitian ini analisis Bolton yang dilakukan hanya meliputi rasio anterior saja. Penelitian ini bertujuan untuk menganalisis hubungan  antara diskrepansi ukuran gigi rahang atas dengan ukuran gigi rahang bawah berdasarkan analisis Bolton terhadap profil jaringan lunak wajah pada mahasiswa Fakultas Kedokteran Gigi Universitas Padjadjaran. Metode: Penelitian analitik observasional diawali dengan pengambilan sampel dengan teknik purposive sampling dan didapatkan 50 orang sampel mahasiswa Fakultas Kedokteran Gigi angkatan 2016 kemudian dilakukan analisis rasio anterior Bolton pada model studi dan interpretasi hasil foto profil ekstraoral. Uji korelasi Pearson dilakukan untuk mengetahui korelasi dari kedua variabel tersebut. Hasil: Rata-rata anterior ratio dari 50 orang sampel sebesar 79,53% ±2,61 dengan profil wajah lurus sebanyak 33 orang (66%) dan profil wajah cembung sebanyak 17 orang (34%) dengan rata-rata derajat konveksitas 172,11°. Uji korelasi Pearson menghasilkan nilai p-value (0,896) yang lebih besar dari α=0,05. Tidak ada korelasi yang signifikan antara diskrepansi ukuran gigi dan profil wajah. Simpulan: Tidak terdapat hubungan antara diskrepansi ukuran gigi dan profil wajah.Kata kunci: Analisis Bolton, diskrepansi ukuran gigi, profil wajah. ABSTRACTIntroduction: Tooth size discrepancy and facial profile are two crucial factors for helping diagnosis and treatment planning of orthodontic treatment. Bolton analysis could measure tooth size discrepancy; and the analysis result shows the ratio of mesiodistal width between maxillary and mandibular teeth. The facial profile could be used as a guideline for aesthetic result in orthodontic treatment. The facial profile can be analysed from an extraoral profile photograph. In this research, we only calculate the anterior ratio of the sample. The purpose of this study was to determine the correlation between maxillary and mandibular anterior size discrepancy and soft tissue facial profile of the students of the Faculty of Dentistry Universitas Padjadjaran based on Bolton analysis. Methods: The research was conducted with observational analytic methods. The sampling method was conducted using the purposive sampling method, which resulted in 50 samples taken from students of the Faculty of Dentistry Universitas Padjadjaran batch 2016. Analysis of the sample’s anterior ratio was measured on the study cast with Bolton analysis and interpretation of the extraoral profile photograph. Both variables (tooth size discrepancy and facial profile type) were then analysed with Pearson’s correlation test to determine the correlation between the two variables. Results: The average anterior ratio from 50 samples was 79.53% ± 2.61, and the most common facial profile type was straight/orthognathic, which was found in 33 samples (66%) followed by convex in 17 samples (34%) with average facial convexity was 172.11°. The result of Pearson’s correlation test showed that the p-value was 0.896, which was higher than α = 0.05. There was no significant correlation between tooth size discrepancy and soft tissue facial profile. Conclusion: There is no correlation between tooth size discrepancy and soft tissue facial profile.Keywords: Bolton analysis, tooth size discrepancy, facial profile.

Pendahuluan: Pasien yang telah melewati masa tumbuh kembang dapat dirawat dengan perawatan ortodonti kamuflase atau bedah ortognati. Perawatan ortodonti kamuflase dengan hasil yang cukup baik dapat dilakukan jika diskrepansi skeletal tidak terlalu berat. Laporan kasus ini bertujuan untuk mengetahui keefektifan penatalaksanaan perawatan maloklusi skeletal kelas III dengan perawatan ortodonti kamuflase. Laporan kasus: Pasien perempuan, berusia 16 tahun 7 bulan, datang ke klinik PPDGS Ortodonti RSGM UNPAD dengan keluhan gigi depan tidak rapi dan gigi bawah terlihat lebih maju dibandingkan gigi rahang atas. Hasil diagnosis menunjukkan maloklusi dentoskeletal kelas III disertai asimetri wajah, palatum tinggi, pergeseran garis median rahang bawah, overjet terbalik, crossbite anterior, geligi berjejal, dan kurva Spee dalam. Pasien dirawat dengan straight wire appliance selama 1 tahun 8 bulan. Breket rahang atas dipasang terlebih dulu sampai overjet terkoreksi. Breket rahang bawah dipasang diikuti reduksi interproksimal. Pasien menggunakan elastik kelas III untuk koreksi relasi kaninus. Reduksi interproksimal menggunakan strip abrasif metal merupakan salah satu cara untuk mendapatkan ruangan pada kasus crowding ringan. Kombinasi reduksi interproksimal gigi anterior rahang bawah dan flaring gigi rahang atas, diikuti penggunaan elastik kelas III efektif mengoreksi overjet terbalik pada kasus maloklusi kelas III. Simpulan: Reduksi interproksimal dan elastik kelas III berhasil mengoreksi kasus maloklusi kelas III disertai crowding ringan dan masalah TSD.Kata kunci: Maloklusi, skeletal kelas III, crowding ringan, tooth size discrepancy (TSD), reduksi interproksimal. ABSTRACTIntroduction: Patient with dentoskeletal class III malocclusion, post-pubertal growth spurt, may be treated with orthodontic camouflage or orthognathic surgery. Camouflage treatment with good prognosis can have acceptable results if there is an only mild skeletal discrepancy. This report was aimed to investigate the effectiveness of camouflage treatment in dentoskeletal class III patient. Case report: A female patient, 16 y.o 7 months, came to Orthodontics Clinic of Universitas Padjadjaran Dental Hospital, complaining her anterior teeth looks crowded and her protrusive mandible. She was diagnosed with dentoskeletal class III malocclusion with asymmetrical face, high palate, midline shifting mandible, reverse overjet, crossbite anterior, crowding, and the deep curve of Spee. The patient was treated with straight wire appliance for one year and eight months. A lower bracket was bonded after anterior crossbite corrected, followed with interproximal reduction. Elastic class III was used to correct canine relationship. Interproximal reduction with metal abrasive is a common practice in orthodontic practice to gain spaces in mild crowding cases. Interproximal reduction in mandibular anterior teeth combined with anterior flaring of maxillary teeth, followed with elastic class III, effectively correct anterior crossbite in class III skeletal patient. Conclusion: Interproximal reduction and elastic class III effective to correct class III malocclusion with mild crowding and tooth size discrepancy problem.Keywords: Maloccusion, dentoskeletal class III, mild crowding, tooth size discrepancy (TSD), interproximal reduction.

ABSTRACT Tooth crowding and protrusions demand rigorous attention during orthodontic planning that includes the extraction of first and second premolars. Some characteristics, such as dentoalveolar bone discrepancies, maxillomandibular relations, facial profile, skeletal maturation, dental asymmetries and patient cooperation, are important elements of an orthodontic diagnosis. This study discusses the options of treatments with extractions and describes the correction of a Class I malocclusion, bimaxillary protrusion, severe anterior crowding in both dental arches and tooth-size discrepancy, using first premolar extractions.
