Abstract Objective.—To describe a comprehensive integrated laboratory quality management plan for gynecologic cytology. Design and Setting.—Cytopathology laboratory performance monitors with interlaboratory comparison. Results.—Utilizing College of American Pathologists Q-Probes studies, the College of American Pathologists Interlaboratory Comparison Program in Cervicovaginal Cytology, and other published data, a quality management program for gynecologic cytology involving diagnostic statistics, screening limits and competency assessment, retrospective rescreening, real-time rescreening, cytology-biopsy correlation, follow-up of patients with abnormal cytology results, turnaround time, examination of unknown slides (survey programs), and new technology is described. Conclusion.—Regular coordinated monitoring of performance, with longitudinal and interlaboratory comparison utilizing the methods described, provides an opportunity to optimize gynecologic cytology service.

In South Africa the ultimate goal in water quality management is to keep the water resources suitable for all “beneficial uses”. Beneficial uses provides a basis for the derivation of water quality guidelines, which, for South Africa, are defined in Water quality guidelines for the South African coastal zone (DWAF, 1991). The CSIR has developed a practical approach to marine water quality management, taking into account international trends and local experience, which can be applied to any coastal development with potential influence on water quality. The management plan is divided into three logical components, i.e. • site-specific statutory requirements and environmental objectives; • system design with specific reference to influences on water quality; and • monitoring programmes. Within this management approach water quality issues are addressed in a holistic manner, through focused procedures and clear identification of information requirements. This paper describes the procedures and information requirements within each component of the water quality management plan, with specific reference to marine disposal systems. Ideally, the management plan should be implemented from the feasibility and conceptual design phase of a development and the timing of the different procedures within the development process are therefore also highlighted. However, the logical lay-out of procedures allows for easy initiation (even to existing disposal system) at any stage of development.

Context Immunohistochemistry has become an indispensable ancillary technique in anatomic pathology laboratories. Standardization of every step in preanalytic, analytic, and postanalytic phases is crucial to achieve reproducible and reliable immunohistochemistry test results. Objective To standardize immunohistochemistry tests from preanalytic, analytic, to postanalytic phases. Data Sources Literature review and Geisinger (Geisinger Medical Center, Danville, Pennsylvania) experience. Conclusions This review article delineates some critical points in preanalytic, analytic, and postanalytic phases; reiterates some important questions, which may or may not have a consensus at this time; and updates the newly proposed guidelines on antibody validation from the College of American Pathologists Pathology and Laboratory Quality Center. Additionally, the article intends to share Geisinger's experience with (1) testing/optimizing a new antibody and troubleshooting; (2) interpreting and reporting immunohistochemistry assay results; (3) improving and implementing a total immunohistochemistry quality management program; and (4) developing best practices in immunohistochemistry.

Enbridge Pipelines Inc. operates the world’s longest and most complex liquids pipeline network. As part of Enbridge’s Integrity Management Program In-Line Inspections have been and will continue to be conducted on more than 15,000 km of pipeline. The Inspection Programs have included using the most technologically advanced geometry tools in the world to detect geometrical discontinuities such as ovality, dents, and buckles. During the past number of years, Enbridge Pipelines Inc. has been involved in developing a method of evaluating the suitability of dents in pipelines for continued service. The majority of the work involved the development of a method of modeling the stresses within a dent using Finite Element Analysis (FEA). The development and validation of this model was completed by Fleet Technology Limited (FTL) through several projects sponsored by Enbridge, which included field trials and comparisons to previously published data. This model combined with proven fracture mechanics theory provides a method of determining a predicted life of a dent based on either the past or future operating conditions of the pipeline. CSA Standard Z662 – Oil and Gas Pipeline Systems provides criteria for the acceptability of dents for continued service. There have been occurrences, however, where dents that meet the CSA acceptability criteria have experienced failure. The dent model is being used to help define shape characteristics in addition to dent depth, the only shape factor considered by CSA, which contribute to dent failure. The dent model has also been utilized to validate the accuracy of current In-Line Inspection techniques. Typically a dent will lose some of its shape as the overburden is lifted from the pipeline and after the indentor is removed. Often there can be a dramatic “re-rounding” that will occur. The work included comparing the re-rounded dent shapes from a Finite Element model simulating the removal of the constraint on the pipe to the measured dent profile from a mold of the dent taken in the field after it has been excavated. This provided a measure of the accuracy of the tool. This paper will provide an overview of Enbridge’s dent management program, a description of the dent selection process for the excavation program, and a detailed review of the ILI validation work.

One of the best ways for an intramural sports program to ensure that an ordinary and reasonable standard of care is adhered to, as well as guarding against litigation, is communication of a risk management program. While having a risk management plan has been widely stressed, no previous research has been conducted from a participant's viewpoint. Thus, the purpose of this study was to determine the effectiveness of university intramural risk management plans from the participant's perception. The primary results of this study indicate that the majority of the intramural sport participants responded that they had never: a) noticed an intramural supervisor being present while the activity was taking place; b) been informed about the potential for participant injury; c) noticed signage relating to emergency procedures at the area of the activity; d) knew of a risk management plan for intramural sports; d) noticed emergency equipment at the site of the activity; and e) been informed about the possession of First Aid/CPR certification or equivalent by the supervisor.

The mountain pine beetle, Dendroctonus ponderosae Hopk., causes extensive mortality of lodgepole pine, Pinus contorta var. latifolia Engelm., throughout western North America (Van Sickle 1982). The Prince Rupert Forest Region, in the northwest of British Columbia, initiated an aggressive beetle management program in 1981. Logging of infested stands, and winter felling and burning of individual infested trees are the most common direct control techniques.The “Bristol Lake” infestation developed in the Bulkley Forest District, approximately 55 km northwest of Smithers, B.C., on a steep rocky ridge within the valley of Harold Price Creek. The area contained large volumes of mature lodgepole pine, and control of the infestation was therefore considered critical to the local beetle management plan, but the size (50 ha) and rough topography of the infested area precluded normal direct control measures.

Many studies on the implementation of quality management programs were carried out by previous researchers. However, the study on the soft or human factors in the implementation of quality management programs is still poorly explored. Therefore, this article aims to analyze soft factors in the implementation of quality management programs at Selangor Islamic Religious Council or Majlis Agama Islam Selangor (MAIS). This study uses a qualitative method involving MAIS as a case study. Semi-structured interviews were used as a data collection method and supported by relevant documents collected from MAIS. A total of fifteen informants comprising top management and MAIS officers were interviewed to obtain in-depth information on the subjects studied. The findings reveal that there are six soft factors emerged in the implementation of quality management programs at MAIS namely leadership, training and development, teamwork, customer focus, Syura and rewards. This study demonstrates that soft factors has helped facilitate the implementation of quality management programs in MAIS. The results of this study can be used as guidance by managers in public or corporate organizatons in their efforts to implement quality management programs effectively.
