The study analyzed the promotion of infant food and nutrition for children under two years in the Primary Health Care in a city located in Minas Gerais. An exploratory and descriptive study interviewing managers, coordinating nurse, community health workers and mothers of registered children. The data were analyzed using the Shapiro-Wilk normality test and Fisher's exact test, Pearson's chi-square test and simple Student t test. The city did not implement the Strategy Amamenta e Alimenta Brasil. Strategy for Family Health performed childcare, but in an irregular or incomplete way. The main difficulties were low adherence by mothers and work overload by nurses. SISVAN's nutritional diagnosis reports are not used.55.6% of mothers participate in the childcare. 33.3% of mothers received guidance on breastfeeding before and after the baby was born. 22.2% received it during prenatal care and 5.6% received it only after the baby was born. Half of the mothers received guidance on healthy complementary feeding. No significant associations were found for the participation of mothers and the variables maternal education, maternal age, having an only child, being an adolescent mother. Mothers should be advised on the approach of the units to monitor the child's growth and development, preventing health problems.

In this commentary paper, we highlight the key role that community health workers and family health professionals can perform for the identification and care for women experiencing domestic violence in communities. These workers are part of the primary health-care strategy in the Brazilian public health system, who are available in every municipalities and neighborhoods of the country. Based on our ethnographic research, we argue that identification and care of abused women by these workers and professionals follow a pattern which we described and named “the Chinese whispers model.” We also point gaps in training these workers to deal with complex issues, such as domestic violence, arguing for the need of formal qualification for both community health workers and family health professionals by, for example, incorporating such themes into curricula, further education, and continuing professional development.

RESUMOObjetivo: identificar de que forma os agentes comunitários de saúde contribuem para o desenvolvimento das ações de combate ao tabagismo na atenção primária à saúde.  Método: trata-se de um estudo qualitativo, descritivo e exploratório, realizado com 20 agentes comunitários de saúde. Utilizou-se uma entrevista semiestruturada para a obtenção dos depoimentos. Realizou-se a análise de conteúdo temática das entrevistas, identificando as palavras-chave, e discutiram-se os resultados em categorias. Resultados: evidenciaram-se duas categorias de análise: as estratégias utilizadas pelos agentes comunitários de saúde para a divulgação das ações de controle do tabagismo e as ações dos agentes comunitários de saúde voltadas aos usuários que não cessaram com o tabagismo. Conclusão: inferiu-se que o profissional capacitado é capaz de estabelecer um vínculo com o usuário da atenção primária à saúde, o que favorece a resolubilidade das ações, contribuindo para a qualidade de vida dos usuários que procuram por atendimento no serviço de saúde. We practise an integrated approach to the management of skin diseases, leprosy and sexually transmitted diseases because the latter diseases are still stigmatized and patients are unwilling to attend clinics so labelled. When approached by a patient with skin disease it is advisable to see the patient promptly, because any further delay encourages the use of assorted remedies which may lead to undesirable complications, physically, emotionally and financially. Since there is no health insurance scheme, it is also prudent to manage the patient as much as possible without admission to hospital and with minimal laboratory investigations, to save cost, so that the patient still has sufficient money to buy the required drugs. Family health workers treat patients at the primary health care (PHC) level. At this level, patients are managed by the use of specially prepared standing orders (SO), where checklists and flow charts are grouped by problems or complaints, to facilitate usage by an individual with minimal training in morphology. The SO presents, so far as is possible, the best treatment available for each condition, but which cannot be misused by the primary health care personnel. Before discussing the management of individual diseases, it is important for the reader to appreciate the milieu in which we practise which determines our approach to the patient with skin disease. Some of its features will be highlighted before the management of individual disorders is discussed.

O conhecimento de como o acolhimento é entendido e praticado no serviço de Atenção Primária em Saúde (APS) pode trazer melhorias para o cuidado em saúde e o trabalho em rede, uma vez que acolher é uma técnica leve que motiva o cuidado significativo para profissional e usuário. Este estudo teve como objetivo identificar, conhecer e descrever as concepções de acolhimento em saúde de trabalhadores de uma unidade de Estratégia de Saúde da Família (ESF), bem como discutir as diferenças entre as referidas concepções e as diretrizes da Política Nacional de Humanização (PNH). Trata-se de pesquisa qualitativa com coleta de dados por meio de entrevista semi-estruturada com seis trabalhadores inseridos na Atenção Básica em Saúde (ABS). A análise dos dados, refinada por meio de análise do discurso na modalidade temática, apontou as seguintes regularidades discursivas: concepção de acolhimento; método de acolhimento; acolhimento no cotidiano de trabalho.  Os resultados salientam a falta de articulação teórico-prático entre a PNH e trabalhadores, os parâmetros de qualidade e os fatores prejudiciais ao acolhimento. Constata-se o acolhimento como prática no cotidiano do serviço possibilita o estabelecimento de cuidado com o usuário e legitima boa relação de trabalho entre os trabalhadores de saúde desta unidade ESF.Abstract:The knowledge of how the host is understood and practiced in the Primary Health Care (PHC) service can bring improvements to health care and networking. The purpose of this study was to identify and describe the conceptions of health reception of workers from a Family Health Strategy (FHS) unit, as well as to discuss these concepts in the light of the National Humanization Politics (NHP) guidelines. This is qualitative research with data collection through a semi-structured interview with six workers enrolled in PHC. The analysis of the data was made through discourse analysis in the thematic modality, pointed to the following discursive regularities: reception conception; host method; host in the daily work. The results relate the professionals' conceptions to the importance of welcoming SUS as a quality tool, not only in the health production of users, but also in the production of professional valorization and healthy work environments. It is important to highlight the challenges of placing the care in accordance with the NHP, as a practice in the day-to-day of the service in establishing care for the user and in the relationships among the health workers of this FHS unit.Keywords: User Embracement, Primary Health Care; Humanization; Health professionals. ResumenEl conocimiento de cómo la acogida es entendido y practicado en el servicio de Atención Primaria en Salud (APS) puede traer mejoras para el cuidado en salud y el trabajo en red. Esta es una investigación cualitativa con recolección de datos a través de una entrevista semiestructurada con seis trabajadores inscritos en Atención Primaria de Salud (ABS). El análisis de los datos se realizó a través del análisis del discurso en la modalidad temática, apuntando a las siguientes regularidades discursivas: concepción de recepción; método de acogida; Anfitrión en el trabajo diario. Los resultados relacionan las concepciones de los profesionales con la importancia de recibir al SUS como una herramienta de calidad, no solo en la producción de salud de los usuarios, sino también en la producción de valorización profesional y entornos laborales saludables. O Sistema Único de Saúde enfrenta desafios em relação à doença de Chagas devido ao elevado número de portadores crônicos no Brasil. Os enfermeiros e auxiliares/técnicos de enfermagem são os profissionais que prestam acolhimento e encaminhamento dos portadores nas Unidades Básicas de Saúde. Objetivou-se analisar a percepção da equipe de enfermagem sobre a doença de Chagas e serviços de saúde relacionados nos municípios da microrregional de saúde de Itaúna, Minas Gerais. Foram aplicados questionários semiestruturados aos profissionais de enfermagem. Os profissionais dominam aspectos relacionados aos vetores, sintomas, exames diagnósticos e fluxo de exames. Há ausência de ações de promoção à saúde voltadas a doença, problemas no rastreamento dos portadores na Atenção Primária à Saúde, e ausência de capacitações para os profissionais abordando o manejo clínico da doença. Sugere-se a realização de capacitações com foco técnico/clínico voltadas à equipe de enfermagem, e implantação de ações de promoção a saúde nos municípios.Descritores: Atenção Primária à Saúde, Estratégia de Saúde da Família, Serviços de Saúde. Nursing team: perception of Chagas diseaseAbstract: The primany health care in Brazil, faces challenges in relation to Chagas disease due to the high number of chronic carriers in Brazil. ABSTRACT Objective To develop, validate and analyse intra- and inter-raters’ agreement and reliability of a scale for the evaluation of food and nutrition education practices in primary health care. Methods Three theoretical references were used for the development of items with answers on a 4-point scale (not observed, partially/satisfactorily/fully observed): the Brazilian Dietary Guidelines, the Interprofessional Collaborative Practice and the Family Health Strategy as models for organizing primary health care. The scale was submitted for content and face panel validation and it was further reevaluated online. Thirty specialists used the scale for the rating of 4 videos produced for the study showing food and nutrition education practices in primary health care settings. Intra- and inter-raters agreement and reliability were analyzed by test at one time point and retest 30 days later. Results The scale initially included 72 items but was reduced to 17 items after content and face validation, together with a fill-out manual that was reviewed by experts. In the re-evaluation, all 17 items were maintained, and content adjustments were made in the manual. Thirty raters applied the scale to the videos in the test, and 28 in the retest. All videos obtained combined kappa values of >0.4. In the test-retest, 19 raters showed >80% agreement; in the inter-rater evaluation, 14 raters generated kappa values >0.75. Conclusion The scale underwent content and face validation and showed to be useful to diagnose the absence and the full addressing of the Brazilian Dietary Guidelines content with good intra- and inter-rater agreement and reliability in different settings of Primary Health Care.

A Atenção Básica (AB) é considerada o primeiro nível de atenção em saúde e tem na Saúde da Família sua estratégia prioritária para expansão e consolidação. O objetivo do estudo foi descrever o perfil e a classificação de risco familiar dos usuários de uma Unidade Básica de Saúde da Família em Uberlândia, Minas Gerais. Trata-se de um estudo transversal, com dados coletados dos Sistemas de Informações, nos anos 2017 e 2018. Foram analisadas variáveis sociodemográficas, classificação dos grupos populacionais e risco familiar. Para a classificação de risco familiar foram considerados fatores socioeconômicos (grau de alfabetização do chefe da família, renda e abastecimento de água) e fatores clínicos (grau de risco dos seguintes grupos/condições: gestante, criança, hipertensão, diabetes, doença renal crônica ou outras condições crônicas de alto risco). Foram avaliados 1553 usuários, sendo 70,6% do sexo feminino. A faixa etária predominante foi entre 40 e 59 anos (33,23%) e a menos atendida foi a de adolescentes entre 14 e 17 anos (1,55%). Em relação à classificação de risco, observou-se maiores prevalências de hipertensão com médio risco, diabetes de alto risco e tuberculose de baixo risco. O estudo evidencia a importância do fortalecimento de políticas públicas, que visem à promoção e monitoramento das condições de saúde na AB. Palavras-chave: atenção básica, fator de risco, doença crônica.   Abstract Primary Health Care (PHC) is considered the first level of health care and has in Family Health its priority strategy for expansion and consolidation. The objective of the study was to describe the profile and family risk classification of users of the Family Health Unit in Uberlândia, Minas Gerais. This is a cross-sectional study, with data collected from the municipality's Information Systems, during the years 2017 and 2018. Sociodemographic variables, classification of population groups and family risk were analyzed. For the classification of family risk, socioeconomic factors (degree of literacy of the head of the household, income and water supply) and clinical factors (degree of risk of the following groups/conditions: pregnant woman, child, hypertension, diabetes, chronic kidney disease or other high-risk conditions). A total of 1553 users were evaluated, 70.6% of whom were female. The predominant age group was that of adults between 40 and 59 years old (33.23%) and the least attended was that of adolescents between 14 and 17 years old (1.55%). Regarding the risk classification, there was a higher prevalence of medium-risk hypertension, high-risk diabetes and low-risk tuberculosis. The study highlights the importance of strengthening public policies, aimed at promoting and monitoring health conditions in PHC. Keywords: primary health care, risk factors, chronic disease.

ABSTRACT Objective To describe the profile and performance of nutritionists in Primary Health Care. Methods A cross-sectional study was carried out, and all nutritionists in two municipalities of Paraíba, Brazil, were interviewed. Information was collected through structured interviews on demographic characteristics, professional qualification, development of food and nutrition activities, knowledge and use of essential bibliography for the work in Primary Care. Results In one municipality there were 28 teams of the Family Health Strategy and in the other, nineteen teams. In all, nineteen nutritionists were interviewed, fourteen of whom were working in the health teams and five were working exclusively in the Family Health Support Centers. All but one were women and the majority were between 20 and 39 years; the majority (n=10) had no graduate training. Nutritionists from the basic health teams developed more public health nutrition actions, such as defining nutritional care protocols and vitamin A and iron supplementation than those from the Family Health Support Centers (11 versus 1; and 13 versus 1, respectively). About half were satisfied with work in general, and dissatisfaction was related to deficiencies in the availability and quality of anthropometric equipment, physical structure and material. Conclusion Nutritionists work in food and nutrition actions in collective health, emphasizing the importance of qualification and practices that better combine the programmatic agenda of this area with Primary Care.
