Introduction: Specific management schemes of the exceedingly rare primitive mesenchymal leiomyosarcomas do not exist. Reporting their behavior and management is necessary to be able to construct evidence-based directives. Case Report: We report a case of a 42-year-old Caucasian woman that presented with a large indolent abdominal mass evolving over 3 months. Two differential diagnosis were looked upon, either GIST or leiomyosarcoma. The abdomino-pelvic ct-scan proved that this tumor could be completely resected, so the patient was successfully operated within 2 weeks of the preliminary diagnosis. A monobloc negative margin resection was performed and the patient recovered on the surgical floor. The anatomopathological studies and immunohistochemistry tests confirmed the presence of a primitive mesenchymal leiomyosarcoma. The patient was recommended a follow up every 4 months with a thoraco-abdomino-pelvic ct-scan due to her economic status. Conclusion: The management of rare tumors is always challenging due to the lack of knowledge concerning its behavior and response to the various treatment modalities. Reporting this rare case of mesenchymal leiomyosarcoma and similar cases will enable a better understanding of this disease and its treatment.

Abstract Background Small intestine duplication cysts (SIDCs) are rare congenital anatomical abnormalities of the digestive tract and a rare cause of hematochezia. Case presentation We describe an adult female presented with recurrent hematochezia. The routine gastric endoscope and colonic endoscope showed no positive findings. Abdominal CT scan indicated intussusception due to the "doughnut" sign, but the patient had no typical symptoms. Two subsequent capsule endoscopes revealed a protruding lesion with bleeding in the distal ileum. Surgical resection was performed and revealed a case of SIDC measuring 6 * 2 cm located inside the ileum cavity. The patient remained symptom-free throughout a 7-year follow-up period. Conclusion SIDCs located inside the enteric cavity can easily be misdiagnosed as intussusception by routine radiologic examinations.

UNSTRUCTURED A case report is a detailed narrative that describes the symptoms, signs, diagnosis, treatment, and follow-up of a medical problem of an individual patient. They usually describe an unusual or novel occurrence and although they lie at the bottom of the hierarchy of the Evidence Based Medicine Pyramid, they still remain one of the cornerstones of medical progress and provide key additions to the existing medical literature. Unfortunately, abysmally few physicians-in-training receive a formal education regarding what constitutes a publishable case report. The article throws light on this aspect of medical education

Introduction: Dentigerous cyst is the most common odontogenic cysts that are associated with the crowns of permanent teeth. Treatment modalities normally include enucleation or marsupialization of the cyst. However, currently there are no standard assessment criteria to dictate which kind of treatment for certain cases. The purpose of this report is to describe the successful outcome of conservative surgical management of a large dentigerous cyst associated with an unerupted right maxillary permanent canine in an 8-year-old boy. The cyst was enucleated partially but leaving the cystic lining surrounding the unerupted canine in order to preserve the tooth. 3-year follow up revealed good healing with significant root formation and tooth eruption.

SummaryThis case report describes an acute traumatic fracture of the medial coronoid process (MCP) in an immature male Rottweiler. On routine radiographs of the left elbow joint, clear abnormalities could not be detected. The definitive diagnosis was based on craniolaterocaudomedial oblique radiographs and a CT scan. The fragment was removed arthroscopically via a medial approach. Histologically the fragment did not show any signs of degeneration nor demineralisation, as seen in classical fragments of the medial coronoid process. One week after treatment, complete limb function had been regained. Follow-up radiographs after nine month showed a minimal increase of osteoarthrosis.

Abstract Aim The aim of the documentation of this clinical case is to make clinicians aware of “meth mouth” and the medical risks associated with this serious condition. Background Methamphetamine is a very addictive, powerful stimulant that increases wakefulness and physical activity and can produce other effects such as cardiac dysrhythmias, hypertension, hallucinations, and violent behavior. Dental patients abusing methamphetamine can present with poor oral hygiene, xerostomia, rampant caries (“meth mouth”), and excessive tooth wear. Oral rehabilitation of patients using methamphetamine can be challenging. Case Description A 30-year-old Caucasian woman presented with dental pain, bad breath, and self-reported poor esthetics. A comprehensive examination including her medical history, panoramic radiograph, and intraoral examination revealed 19 carious lesions, which is not very common for a healthy adult. She reported her use of methamphetamine for five years and had not experienced any major carious episodes before she started using the drug. Summary The patient's medical and dental histories along with radiographic and clinical findings lead to a diagnosis of “meth mouth.” Although three different dental treatment modalities (either conventional or implantsupported) have been offered to the patient since August 2007, the patient has yet to initiate any treatment. Clinical Significance This clinical case showing oral manifestations of meth mouth was presented to help dental practitioners recognize and manage patients who may be abusing methamphetamines. Dental practitioners also may be skeptical about the reliability of appointment keeping by these patients, as they frequently miss their appointments without reasonable justification. Introduction Quality of life (QOL) is significantly impaired among individuals with post-traumatic stress disorder (PTSD); however, few treatment outcome studies examine QOL following treatment. Furthermore, the use of videoconferencing to deliver evidence-based treatments for PTSD is increasing dramatically. Although videoconferencing has demonstrated non-inferiority to in-person treatment modalities for improving PTSD symptom severity, no studies to date have directly compared QOL outcomes of an evidence-based intervention delivered via videoconferencing to one delivered in-person. Methods This study presents a secondary data analysis of two randomized controlled trials comparing cognitive processing therapy (CPT) delivered via videoconferencing or a traditional in-person modality. The Men’s study delivered group CPT to 125 male veterans with PTSD, whereas the Women’s study delivered individuals CPT to 126 female civilians and veterans. Multigroup latent growth curve models were used to model changes in QOL Inventory (QOLI) scores over time. Results There was no effect of treatment modality on changes in QOLI scores over time (modality effect on slope estimate = 0.004 (–0.60, 0.61) and on quadratic estimate = 0.001 (–0.18, 0.20); all ps > 0.33). Model fit was the same for both genders (Δ χ2 (2) = 2.28, p = 0.32) and for the gender × treatment modality interaction (Δ χ2 (2) = 2.87, p = 0.24). QOLI scores improved at post-treatment and three-month follow-up assessments, but declined at the six-month follow-up assessment. Discussion This secondary analysis extends the findings of the parent studies by establishing the efficacy of the videoconferencing platform in improving QOL. Clinical implications of findings are discussed.

For patients who require removal of anterior teeth and their replacement various treatment modalities are available. With advancement in technology and availability of glass/polyethylene fibres, use of natural tooth as pontic with fibre reinforced composite restorations offers the promising results. The present case report describes management of periodontally compromised mandibular anterior tooth using natural tooth pontic with fibre reinforcement. A 1-year follow-up showed that the bridge was intact with good esthetics and no problem was reported.

Introduction. Treatment of cerebral arteriovenous malformations is very challenging and controversial in spite of current recommendations. Surgery is recommended in patients with hemorrhagic stroke, but in patients with good neurological status, when symptoms improve rapidly, the risk of surgical morbidity may be much higher than the risk of rebleeding. Case report. We report a case of a patient with an intracranial hemorrhage due to a ruptured arteriovenous malformation located in the right temporal region of the brain. Because of angiographic and anatomical features of the arteriovenous malformation (deep location and deep venous drainage, but also small arteriovenous malformation nidus size), radiosurgery was the preferred treatment modality. The patient was treated conservatively in the acute stage, and the arteriovenous malformation was subsequently completely eradicated with gamma knife radiosurgery. During the 3-year imaging follow-up, no sings of rebleeding were found. Also, angiography demonstrated that the arteriovenous malformation was completely excluded from the cerebral circulation. The patient was in a good condition and presented without neurological deficits or seizures during the follow-up period. Conclusion. All treatment modalities carry a risk of neurological compromise, but gamma knife radiosurgery may be a good option, even in cases with hemorrhagic presentation. It needs to be mentioned that complete obliteration takes approximately 1 to 3 years after the treatment, and in some cases it cannot be obtained.

A 10-year-old boy known case of Gaucher disease about 2 years ago was admitted with nausea, vomiting and epigastric pain from 3 days before hospitalization to Mofid Children’s hospital, Tehran, Iran. On the initial examination, no abnormalities were found except for splenomegaly and brief tenderness on the spleen. In secondary paraclinical evaluations (with Ultrasound and Ct-scan), we noticed mesenteric lymphadenopathy in the patient. The lesions remained unchanged and no significant changes were observed in the clinical and laboratory evaluations of the patient during one-year follow-up period. This is a rare and benign manifestation of Gaucher disease, which is usually monitored and does not require any intervention without signs of malignancy or severe space occupying effects

The authors describe a rare complication of ureteral stenting is the case study of a patient admitted to the hospital for congestion in the outlet system of both kidneys due to external ureteral compression by tumorous mass in the retroperitoneum. Histology confirmed the B-lymphoma of the retroperitoneum as a cause of the patient‘s problems. The ureteral stent was perforated in the course of inserting the stent into the ureter and the end of the splint was introduced into the inferior vena cava. The patient was asymptomatic, and this complication was detected as late as on day 12 on the follow-up CT scan. Stent extraction was without complications and without bleeding.
