Aims: This study evaluated the effect of magnification on the quality of preclinical class I cavity preparations according to operator’s experience. Methodology: This was an experimental laboratory study. The response variable was the quality of Class I cavity preparations evaluated by the Class I Cavity Preparation Assessment - COCA. Class I cavity preparations were performed in all artificial first molars under four conditions: unaided visualization, using a simple loupe, using a Galilean loupe, and using a Keplerian loupe (N=640). The two-factor analysis of variance was performed (α=0.05). Results: No interactions between operator’s experience and magnification device were found (p>0.05). Although, higher quality scores were given to cavity preparations in tooth #36 using the Galilean loupe compared to naked eye (p<0.01). The more experienced operator received higher quality scores for tooth #46 regardless of the magnification system used (µexperienced=7.0±2.3 and µinexperienced=5.4±2.4). Conclusion: The use of magnification devices did not influence the quality of class I cavity preparations performed on tooth #16, 26, 46. Galilean loupe was associated with improved quality of the cavity preparation performed on tooth #36, and the operator’s experience influenced the quality of the preparation on tooth #46.

This study sought to develop and validate two methods of quality assessment, one for Class I cavity preparation and another for composite resin restoration. This was an experimental laboratory study. The methods are named Class I Cavity Preparation Assessment – COCA and Class I Cavity Restoration Assessment – COCRA. During the development of the methods, 5 items were elaborated for COCA and 10 items for COCRA. Each item should be classified as appropriate, partially appropriate, or inappropriate. For each method, after evaluation, all item values should be added with a maximum possible score of ten points. The reliability of the COCA and COCRA was estimated through intra-observer reproducibility. For the methods application, 80 Class I cavity preparation and restoration in first molars were evaluated. A descriptive statistical analysis was performed, and the intra-observer concordance was estimated using the intraclass correlation coefficient (ρ). As a result, it was possible to observe that the reproducibility for COCA of evaluator 1 (ρ=0.76) and evaluator 2 (ρ=1.00) was classified as good and excellent, respectively. The reproducibility for COCRA of evaluator 1 (ρ=0.99) and evaluator 2 (ρ=0.77) was classified as excellent and good, respectively. It was concluded that COCA and COCRA were valid and reliable for the assessment of quality of Class I cavity preparation and restoration.

Background: Epitendinous sutures not only join the 2 severed tendon edges but also supply strength and support to the repair. Multiple techniques have been described, but none of them include another thread of suture. This could potentially increase the strength of the repair without affecting gliding. Methods: Caprine tendons were harvested, transected, and sutured with 6-0 Prolene. Four groups were created: single thread running (SR), single thread locking (SL), double thread running (DR), and double thread locking (DL). An Instron 5542 was used to pull the repaired tendons apart, and the energy at the break was calculated (gf/mm). For gliding resistance, we harvested a human A2 pulley. A pre- and postrepair value was obtained, and a ratio was elaborated. A single-factor analysis of variance and independent sample t tests were performed. Results: The SR group had a mean energy at break of 9339.71 ± 1630.212 gf/mm; the SL group, 9629.96 ± 1476.45 gf/mm; and the DR group, 9600.221 ± 976.087 gf/mm, with no statistical significance. The DL group was significantly higher at 14 740.664 ± 2596.586 gf/mm ( P < .05). When comparing SR with DL for gliding, SR had less than half of the resistance than DL (0.018 ± 0.004 and 0.049 ± 0.015 N/mm, respectively), with statistical significance ( P < .05). Conclusion: Using a single suture thread for running epitendinous repair is no different than locking with a single thread or using an extra thread in a running fashion. Performing a double suture thread with a locking technique is significantly stronger than the previously mentioned repairs, with the disadvantage of more resistance at the pulley.

Techniques for three-dimensional reconstruction of macromolecular complexes from electron micrographs have been successfully used for many years. These include methods which take advantage of the natural symmetry properties of the structure (for example helical or icosahedral) as well as those that use single axis or other tilting geometries to reconstruct from a set of projection images. These techniques have traditionally relied on a very experienced operator to manually perform the often numerous and time consuming steps required to obtain the final reconstruction. While the guidance and oversight of an experienced and critical operator will always be an essential component of these techniques, recent advances in computer technology, microprocessor controlled microscopes and the availability of high quality CCD cameras have provided the means to automate many of the individual steps.During the acquisition of data automation provides benefits not only in terms of convenience and time saving but also in circumstances where manual procedures limit the quality of the final reconstruction.

Abstract Background Home postpartum care is a major part of midwifery care in Germany. The user perspective plays an increasingly important role in the evaluation of health services, but there is a lack of valid and theoretically based measuring instruments, especially in midwifery care. The aim of this study was to develop and validate an instrument for measuring quality of midwifery care in the postpartum period from the perspective of women. Methods The following steps were taken to achieve this: (1) definition of the goals of midwifery work; (2) literature-based item development; (3) item selection based on a pre-test (n = 16); (4) item reduction and investigation of factor structure by means of explorative factor analysis (EFA; n = 133);(5) second EFA (n = 741) and confirmatory factor analysis (CFA; n = 744) based on a split representative sample survey; (6) hypothesis-based testing of correlations to sociodemographic characteristics of women and to characteristics of care. Results Measurement of Midwifery quality postpartum (MMAYpostpartum) consists of three scales with a total of 17 items which were found to have acceptable internal consistency: Personal Control (Cronbach’s alpha = .80), Trusting Relationship (Cronbach’s alpha = .87) and Orientation and Security (Cronbach’s alpha = .78). CFA verified and confirmed three factors: CFI = .928, TLI = .914, RMSEA = 0.073. Conclusion MMAYpostpartum is a predominantly valid, reliable short tool for evaluating the quality of midwifery care postpartum. It can be used to evaluate midwifery care, to compare different care models and in intervention research. It thus supports the orientation of midwives’ work towards the needs of women and their families.

Abstract Background Person-centred care is the preferred model for caring for people with dementia. Knowledge of the level of person-centred care is essential for improving the quality of care for patients with dementia. The person-centred care of older people with cognitive impairment in acute care (POPAC) scale is a tool to determine the level of person-centred care. This study aimed to translate and validate the Dutch POPAC scale and evaluate its psychometric properties to enable international comparison of data and outcomes. Methods After double-blinded forward and backward translations, a total of 159 nurses recruited from six hospitals (n=114) and via social media (n=45) completed the POPAC scale. By performing confirmatory factor analysis, construct validity was tested. Cronbach’s alpha scale was utilized to establish internal consistency. Results The confirmatory factor analysis showed that the comparative fit index (0.89) was slightly lower than 0.9. The root mean square error of approximation (0.075, p=0.012, CI 0.057–0.092) and the standardized root mean square residual (0.063) were acceptable, with values less than 0.08. The findings revealed a three-dimensional structure. The factor loadings (0.69–0.77) indicated the items to be strongly associated with their respective factors. The results also indicated that deleting Item 5 improved the Cronbach’s alpha of the instrument as well as of the subscale ‘using cognitive assessments and care interventions’. Instead of deleting this item, we suggest rephrasing it into a positively worded item. Conclusions Our findings suggest that the Dutch POPAC scale is sufficiently valid and reliable and can be utilized for assessing person-centred care in acute care hospitals. The study enables nurses to interpret and compare person-centred care levels in wards and hospital levels nationally and internationally. The results form an important basis for improving the quality of care and nurse-sensitive outcomes, such as preventing complications and hospital stay length.
