Objetivo: analisar os saberes dos enfermeiros durante o processo de alta hospitalar do recém nascido prematuro.Método: estudo descritivo, exploratório, qualitativo, realizado entre janeiro e maio de 2019 em uma unidade neonatal do Hospital Municipal Dra. Naelma Monteiro da Silva, vinculado ao Município de Rio das Ostras, Estado do Rio de Janeiro. Participaram do estudo onze  enfermeiros atuantes na Unidade, sendo os dados coletados por meio de entrevista semiestruturada submetidos à análise de conteúdo na modalidade temática. Resultados: constatou-se que os enfermeiros demonstraram conhecimento dos saberes acerca dos cuidados ao recém nascido na alta hospitalar, destacando-se a amamentação, a higiene corporal, o coto umbilical, a criação de vínculo e o plano de alta hospitalar com o propósito de evitar as reinternações. Conclusão: há necessidade de uma assistência qualificada, segura e humanizada, com acolhimento e respeito às subjetividades, tendo como propósito conhecer as necessidades do recém nascido, e promover estratégias para a educação em saúde, em especial no que tange aos cuidados do recém-nascido prematuro.

Sporotrichosis is the subcutaneous mycosis caused by several species of the Sporothrix genus. With worldwide occurrence, the State of Rio de Janeiro is presently undergoing a zoonotic sporotrichosis epidemic. The form of lymphocutaneous nocardiosis is rare, being caused especially by Nocardia brasiliensis. It appears as a nodular or ulcerated lesion, with multiple painful erythematous nodules or satellite pustules distributed along the lymphatic tract, similar to the lymphocutaneous variant of sporotrichosis. We present a 61-year-old man who, after an insect bite in the left leg, developed an ulcerated lesion associated with ascending lymphangitis, nonresponsive to previous antibiotic therapies. The patient was admitted for investigation, based on the main diagnostic hypothesis of lymphatic cutaneous sporotrichosis entailed by the highly suggestive morphology, associated with the epidemiologic information that he is a resident of the city of Rio de Janeiro. While culture results were being awaited, the patient was medicated with sulfamethoxazole-trimethoprim to cover CA-MRSA and evolved with total healing of the lesions. After hospital discharge, using an ulcer fragment, an Actinomyces sp. was cultivated and N. brasiliensis was identified by molecular biology. The objective of this report is to demonstrate a case of lymphocutaneous nocardiosis caused by N. brasiliensis after a probable insect bite. Despite the patient being a resident of the State of Rio de Janeiro (endemic region for sporotrichosis), it is highlighted that it is necessary to be aware of the differential diagnoses of an ulcerated lesion with lymphangitis, favoring an early diagnosis and appropriate treatment of the illness.

Abstract We sought to know the spectra of mothers of premature children regarding their experience with circle of culture of educational character and identifying the learning provided by the circle of culture about newborn care after hospital discharge. A descriptive study was performed in a hospital located in Fortaleza, Brazil. Three meetings of a circle of culture with 17 mothers of premature newborns were performed. The interpretation of the corpus was performed using thematic analysis. Emerged from the categories: Maternal experience in a circle of culture; Promoted social support among mothers through the circle of culture; and Learning provided by the circle of culture. It was concluded that teaching parents during the hospitalization of the child should be held in a way to involve parents in the care of the newborn, provide moments of health education, opportunities for support and dialogue between professionals and family.

A necessidade de alcançar uma maior eficiência na utilização dos recursos hospitalares torna necessária a sincronização dos processos internos. Para tanto, é essencial a aproximação entre os fluxos físicos e de informação envolvidos nos processos, como, por exemplo, a gestão da alta hospitalar. Essa aproximação é apoiada pela difusão de Tecnologias da Informação e pela contínua adoção dos conceitos de Sistemas Ciber-Físicos. Neste contexto, o presente artigo tem por objetivo propor e analisar políticas de sincronização dos processos internos de gestão da alta hospitalar, por meio da adoção de Sistemas Ciber-Físicos inteligentes. Para isto, após realizar uma proposição do estado futuro almejado, um modelo de simulação foi construído para apoiar sua comparação com o estado atual do processo. Assim, foi possível evidenciar possíveis ganhos de eficiência na utilização dos recursos hospitalares, bem como redução do tempo de permanência dos pacientes.Palavras-chave: Alta hospitalar. To this, a simulation model was elaborated to support its comparison with the current state of the process. Readmissions to the hospital are frequent after hospital discharge. Pharmacist-led interventions have been shown to reduce readmissions. The objective of this study was to describe pharmacist-led interventions to support patients’ medication management at hospital discharge in Switzerland and to compare them to international guidelines. We conducted a national online survey among chief hospital pharmacists focusing on medication management at hospital discharge. To put our findings in perspective, Cochrane reviews and guidelines were searched for summarised evidence and recommendations on interventions. Based on answers in the survey, hospitals with implemented models to support patients at discharge were selected for in-depth interviews. In semi-structured interviews, they were asked to describe pharmacists’ involvement in the patients’ pathway throughout the hospital stay. In Swiss hospitals (n = 44 survey participants), interventions to support patients at discharge were frequently implemented, mostly “patient education” (n = 40) and “communication to primary care provider” (n = 34). These interventions were commonly recommended in guidelines. Overall, pharmacists were rarely involved in the interventions on a regular basis. When pharmacists were involved, the services were provided by hospital pharmacies or collaborating community pharmacies. In conclusion, interventions recommended in guidelines were frequently implemented in Swiss hospitals, however pharmacists were rarely involved.

Purpose Feeding challenges are common for infants in the neonatal intensive care unit (NICU). While sufficient oral feeding is typically a goal during NICU admission, this can be a long and complicated process for both the infant and the family. Many of the stressors related to feeding persist long after hospital discharge, which results in the parents taking the primary role of navigating the infant's course to ensure continued feeding success. This is in addition to dealing with the psychological impact of having a child requiring increased medical attention and the need to continue to fulfill the demands at home. In this clinical focus article, we examine 3 main areas that impact psychosocial stress among parents with infants in the NICU and following discharge: parenting, feeding, and supports. Implications for speech-language pathologists working with these infants and their families are discussed. A case example is also included to describe the treatment course of an infant and her parents in the NICU and after graduation to demonstrate these points further. Conclusion Speech-language pathologists working with infants in the NICU and following hospital discharge must realize the family context and psychosocial considerations that impact feeding progression. Understanding these factors may improve parental engagement to more effectively tailor treatment approaches to meet the needs of the child and family.
