Objective: Intravenous hydroxyethyl starch (HES) solutions are potentially nephrotoxic due to rapid renal tissue uptake, subsequent osmotic nephrosis, and long-lasting intracellular storage. This study aimed to investigate the severity of intracellular storage of HES in renal tissue samples from critically ill dogs receiving 6% HES 130/0.4.Materials and Methods: Fresh, post-mortem (<2 h after death) renal tissue samples were analyzed through histology, immunohistochemistry (HES 130/0.4-specific antibodies), and electron microscopy for the severity of renal tubular vacuolization (VAC), intravacuolar HES accumulation (ACC), and ultra-structure impairment. Moreover, we investigated the relationship between VAC or ACC grade and HES dose (mL/kg), duration of HES administration (h), and pre-HES plasma creatinine concentrations.Results: Histology revealed that 2/20 dogs (10%) had no, 11/20 dogs (55%) had mild, 5/20 dogs (25%) had moderate, and 2/20 dogs (10%) had severe VAC. Immunohistochemistry revealed that 5/20 dogs (25%) had no, 6/20 dogs (30%) had mild, 7/20 dogs (35%) had moderate, and 2/20 dogs (10%) had severe ACC. Both changes were predominantly found in the distal tubular epithelium of mild and moderate cases, and all tubular segments were affected in severe cases. Seven of 20 dogs (35%) had osmotic nephrosis (ON). On electron microscopy, large granules with an electron-dense content were repeatedly detected in individual cells, mainly in the distal tubules. No correlation was found between cumulative HES dose or duration of HES administration and VAC grade, ACC grade, or presence/absence of ON.Conclusion: A high percentage of dogs had renal tubular HES storage and one-third of dogs showed HES-induced ON. Short-term HES administration caused VAC and ACC, regardless of the dose or duration of administration. In contrast to previous studies, HES 130/0.4 deposits were mainly located in the renal distal tubule.

Mục tiêu: Đánh giá hiệu quả ổn định huyết áp của dung dịch 6% Hydroxyethyl starch 130/0.4 với dung dịch Natriclorua 0.9% truyền trước gây tê tủy sống để mổ lấy thai. Đối tượng và phương pháp nghiên cứu: Nghiên cứu tiến cứu can thiệp lâm sàng, ngẫu nhiên, mù đơn, có đối chứng trên sản phụ thai đủ tháng có chỉ định mổ lấy thai đơn thuần tại Khoa Phẫu thuật và gây mệ hồi sức tại Bệnh viện Phụ sản Trung ương từ tháng 5 năm 2020 đến tháng 11 năm 2020. Kết quả: Thời gian khởi phát tê mức T10 trung bình ở nhóm 1 là 2,88 ± 0,43 và ở nhóm 2 là 2,85 ± 0,70 (p>0,05). Thời gian khởi phát tê mức T6 trung bình ở nhóm 1 là 4,04 ± 0,57 và ở nhóm 2 là 3,80 ± 0,72 (p>0,05). Tỷ lệ vô cảm tốt cho cả 2 nhóm là 100%. Cả ba giá trị HATT, HATTr, HATB sau khi truyền dịch ở cả hai nhóm đều tăng cao hơn giá trị huyết áp ban đầu của bệnh nhân khi chưa truyền dịch (p<0,01). HATT min ở nhóm I thấp hơn nhóm II (p<0,05), nhóm I giữ được HATT ở mức cao hơn so với nhóm II. Tổng lượng dịch truyền ở nhóm II nhiều hơn nhóm I với p<0,01. Kết luận: Truyền dung dịch 6% HES 130/0.4 trước GTTS là một biện pháp phòng chống tụt HA hiệu quả, nên áp dụng rộng rãi trong GTTS, đặc biệt là GTTS để mổ lấy thai.

Abstract This study is the first report of the preparation of hydroxyethyl starch (HES) hydrogels rapidly crosslinked with divinyl sulfone in a single step and single pot. To develop the physical and chemical features of hydrogels, Graphene oxide (GO) nanoparticles were combined with the crosslinked HES. In addition to swelling studies, structural characterization of the samples was conducted with a scanning electron microscope (SEM) and transmission electron microscope (TEM), Fourier transform infrared (FTIR) spectroscopy, X-ray diffractometry (XRD), Brunauer–Emmett–Teller (BET) analysis and thermogravimetric analysis (TGA). For the removal of malachite green model dye by GO-HES, the effects of GO content, solution concentration, temperature, contact duration, dosage and pH on varying adsorption features were researched. Additionally, adsorption isotherms, kinetic and thermodynamic systematics were analyzed. The maximum adsorption capacity of GO-HES composite hydrogel was found to be 89.3 mg/g for Langmuir isotherm. The possible adsorption mechanism of the composite hydrogels for malachite green dye involved electrostatic, hydrogen bonding, and π–π interactions. In addition to reasonable cost and simple synthesis method, the prepared composite materials have potential use in wastewater treatment as adsorbents for the removal of dye from aqueous solutions due to efficient adsorption capacity.

Background: Hydroxyethyl starch solutions which have been introduced some three decades ago, have been extensively used for treatment of intraoperative hypovolaemia. Aims and Objectives: The present prospective study was conducted to study changes in post-operative renal function and coagulation profile in patients undergoing orthopedic hip surgery under spinal anesthesia after coloading with 6% HES 130/0.4. Materials and Methods: Seventy patients, aged between 50 and 80 years, of either sex, belonging to ASA physical Status I and II, scheduled for orthopedic hip surgery under spinal anesthesia were included in the study. As per the group allocation, coloading was done with 7.5 mlkg-1 of either Group RL or 6% 130/0.4 Group HES during the 1st h of surgery. Venous samples were withdrawn before the day of surgery, on the day of surgery, and on the 3rd day of surgery. Results: No statistically significant difference between renal and coagulation profile was found among groups. Conclusion: Thus, we recommend using tetrastarch (6% HES 130/0.4) for intraoperative hypovolemia in elderly patients with normal renal function and coagulation profile undergoing orthopedic hip surgery.
